Abstract

The main modality in autoimmune disease is a long-term immunosuppressant treatment aiming to control disease progression and increase patient life expectancy. This scoping review aims to evaluate the effect of immunosuppressant treatment in autoimmune patients with COVID-19 on clinical outcomes and disease progression. This scoping review was conducted following the PRISMA extension for scoping review (PRISMA-ScR) guidelines. The Pubmed and Science Direct databases are used to find articles that match the study objectives. Thirteen articles met the inclusion criteria, and all of them were classified as observational studies. Most immunosuppressant treatments are the disease-modifying anti-rheumatic drugs (DMARD) and glucocorticoids. The highest number of autoimmune patients with rheumatoid arthritis (RA) was 43.4%, systemic lupus erythematosus (SLE) 13.6%, and others was 43%. Autoimmune patients with COVID-19 taking immunosuppressant medications, particularly glucocorticoids, significantly increased the risk of hospitalization and the use of ventilators.  However, there was no mention of the dosage and duration of immunosuppressant therapy in most of the studies.  In general, the use of immunosuppressant drugs was not associated with an increased risk of COVID-19 infection and mortality compared with the general population. Increasing age and comorbidities were associated with poor clinical outcomes. In conclusion, autoimmune patients with COVID-19 who are taking immunosuppressant therapy particularly glucocorticoid exacerbate clinical outcomes.  Periodic clinical monitoring and appropriate pharmacological interventions are required in autoimmune patients with COVID-19 to improve clinical outcomes and prevent death.Keywords: Autoimmune, COVID-19, Immunosuppressant, Clinical outcome.

Abstract

Fetal breeching presentation is position of fetus with his bottom-first in the maternal’s uterus. It will increase risk of perinatal or neonatal mortality and morbidity, if breech presentation persisted at term. During labor, a woman could experienced perineal tears wound due to the impact of contraction pressure on intact perineum – especially for primiparous woman and heavier birth weight – as well as to episiotomy. The degree of perineal tears wound are classified as the first and second grade which are being considered as at low risk of infection and there is no indication of prophylactic antiobiotics; while the third and fourth degree perineal trauma are compulsory to be given antibiotics due to the higher risk of infection. This is cases of two multiparous women whose maternal status during antenatal care presented at Puskesmas – a pirmary healthcare facility in Indonesia – one patient with breech position at weeks 29- 34 and the other one was on chonic energy deficiency who were constantly in low blood pressure (BP) of 100/60 mmHg, MAP 73,3 mmHg, and poor nutritional state. The patients came from low social-economic and education. Healthcare team at Puskesmas provided childbirth education including perineal tears wound care, and pharmacist served medication literacy assistance due to lack of knowledge about their medication administration. This pharmacist’s assistance was delivered on hybrid system due to Covid-19 pandemic. The patient managed to deliver the baby on vaginal birth with second degree perineal tears and started on antibiotics, analgesics, vitamin A, and iron tablets.  One of the patient presented with fever, BP of 90/60 mmHg, and wound infection on day-3 postpartum; thus received another cure of antibotics and analgesic. The patients’ knowledge were improved, drug compliance were 100%, perineal tears wound care had been done properly, so that the wound completely dried and healed on day-42 postpartum.  Keywords: antibiotics, high risk, perineal wound, knowledge, behaviour.

Abstract

Penelitian ini bertujuan mengetahui pengaruh pemberian fraksi air lingzi (G. lucidum) terhadap penurunan kadar glukosa darah dan gambaran histopatologi di jaringan hepar pada tikus yang mengalami diabetes akibat induksi aloksan monohidrat. Sejumlah 36 ekor tikus dibagi dalam 6 kelompok, yaitu kelompok normal, kelompok diabetes, kelompok pembanding glibenklamid, dan kelompok fraksi air G. lucidum dengan dosis 225 mg/KgBB. Pada hari ke-0 diberikan injeksi aloksan monohidrat 150 mg/KgBB secara i.p. Pemberian glibenklamid dan fraksi air G. lucidum selama 9 hari diberikan setelah timbul hiperglikemia yang persisten. Hari ke-10 setelah treatment tikus dikorbankan dan diambil jaringan heparnya untuk dilakukan pengujian histokimia menggunakan hematoxylin eosin. Hasil penelitian menunjukkan bahwa pemberian fraksi air G. lucidum dosis 225 mg/kgBB Lingzhi segar mampu menurunkan kadar glukosa darah yang hampir sama dengan kelompok tikus DM yang diberikan glibenklamid 0,45 mg/KgBB serta mampu melindungi jaringan hepar dari kerusakan karena kondisi DM. Ditunjukkan oleh perubahan morfologi sel hepatosit mengalami perbaikan. Kata kunci  : Fraksi air Lingzi, G. lucidum, Diabetes Melitus, Resistensi insulin dan Hepatoprotektif

Abstract

This study aims to compare conventional heating with microwave irradiation in order to obtain the optimal method for 3-amino-2-(2-chlorophenyl)quinazolin-4(3H)-one synthesis. For the compound to be completely synthesised, one needs to carry out much heating for a long period of time. 10 hours are required for the reaction using conventional heating; yet it takes only 5 minutes using microwave with 800 watts of power. It was found in this study that there was an improvement in percent yields of 3-amino-2-(2-chlorophenyl)quinazolin-4(3H)-one, 87 %  using microwave irradiation compared to 79 % using reflux. Therefore, microwave-assisted method is a quicker, higher yielding method for 3-amino-2-(2-chlorophenyl)quinazolin-4(3H)-one synthesis.

Abstract

Senna leaf (Cassia acutifolia) is a plant that is widely known by the public to treat constipation. Senna leaf products have weaknesses in the form of an unpleasant aroma and taste and the use of inappropriate doses. This study aims to determine the effect of variations in the levels of lactose and mannitol as filler on the physical properties of the effervescent granules of senna leaf extract. Effervescent granules were made using the melting method with a ratio of lactose and mannitol 25% : 75%, 50% : 50%, and 75% : 25%, while the extraction method for senna leaves used the infundation method with distilled water as a solvent. The effervescent granules obtained were tested for flow time, angle of repose, setting and moisture content. The data obtained were analyzed by theoretical approach and statistical test using analysis of variance (ANOVA) with 95% confidence level. The results showed that the use of variations in the filler levels of lactose and mannitol with groups of 25% : 75%, 50% : 50%, and 75% : 25%,  could affect the moisture content and angle of repose of the granules but had no effect on the compressibility and flow time of the granules. Keywords : Cassia acutifolia, sennoside, laxative, effervescent granules, mannitol, lactose

Abstract

Temugiring (Curcuma heyneana) mengandung senyawa flavonoid dan kurkumin yang berpotensi sebagai tabir surya, sehingga konsentrasi ekstrak yang ditambahkan dalam formulasi sediaan krim harus ditentukan secara tepat karena akan berpengaruh terhadap mutu fisik dan nilai Sun Protection Factor (SPF). Tujuan penelitian ini adalah mengetahui pengaruh variasi konsentrasi ekstrak temugiring terhadap mutu fisik dan nilai SPF krim. Dalam penelitian ini, ekstrak temugiring diperoleh dari metode maserasi menggunakan pelarut etanol 96%, kemudian diformulasikan dalam sediaan krim dengan variasi konsentrasi ekstrak 5%; 7,5%; dan 10%. Selanjutnya, penentuan nilai SPF dilakukan dengan metode spektrofotometri UV-Vis. Hasil penelitian menunjukkan variasi konsentrasi ekstrak temugiring menyebabkan nilai SPF yang berbeda pada setiap formula. Nilai SPF formula 1, 2, dan 3 adalah 3,307±0,11; 3,497±0,18; 4,966±0,24. Uji One Way Anova menghasilkan p-value 0,05.  Dengan demikian, dapat disimpulkan bahwa konsentrasi fraksi etil asetat bangle berpengaruh terhadap nilai SPFnya.Kata Kunci: Curcuma heyneana, krim, sun protection factor, temugiring

Abstract

Keputihan merupakan salah satu permasalahan kesehatan reproduksi yang sering dialami oleh wanita, dalam mengatasi hal ini maka perlu menggunakan sabun pembersih kewanitaan (feminine hygiene). Candida albicans dapat dikatakan sebagai agen utama penyebab keputihan. Penelitian ini bertujuan untuk mendapatkan formula sabun cair pembersih kewanitaan (feminine hygiene) ekstrak etanol daun ceremai (Phyllanthus acidus (L.) Skeels) terbaik, ditinjau dari segi kestabilan, kenyamanan, dan efektivitas antifungi terhadap Candida albicans, dengan variasi konsentrasi ekstrak etanol daun ceremai (Phyllanthus acidus (L.) Skeels) sebesar 42,8%, 50% dan 57,1%. Pada penelitian ini serbuk daun ceremai (Phyllanthus acidus (L.) Skeels) diekstraksi dengan metode maserasi menggunakan etanol 96% dan diperoleh ekstrak kental sebesar 86,2 gram. Ekstrak etanol daun ceremai (Phyllanthus acidus (L.) Skeels) mempunyai aktivitas antifungi diduga dari senyawa metabolit sekunder yaitu golongan senyawa flavonoid, tanin, saponin, dan alkaloid. Sabun cair pembersih kewanitaan (feminine hygiene) ekstrak etanol daun ceremai (Phyllanthus acidus (L.) Skeels) di uji stabilitas fisik menggunakan metode cycling test, hasil yang didapatkan secara organoleptis tidak stabil karena adanya perubahan bentuk dan bau pada siklus-1 sampai siklus-6, lalu uji pH, homogenitas, tinggi busa, iritasi, viskositas, dan kadar air sesuai dengan literatur, adapun beberapa yang tidak sesuai literatur yaitu uji  tinggi busa F1 siklus-1, F2 siklus-0, F3 siklus-2, siklus-4, dan siklus-5, serta uji viskositas F1 siklus-6, dan F2 siklus-6. Uji efektivitas terhadap fungi Candida albicans menggunakan metode difusi sumuran, hasil yang didapatkan yaitu F1 sebesar 5,24 mm, F2 sebesar  9,18 mm, dan  F3 sebesar 9,24 mm. Sehingga dapat disimpulkan bahwa F1, F2, dan F3 kurang efektif dalam menghambat Candida albicans. Kata kunci : Keputihan, Candida albicans, feminine hygiene, ekstrak etanol daun ceremai (Phyllanthus acidus (L.) Abstract

ABSTRACT  In Indonesia, there are various kinds of health supplement products that have obtained a distribution permit number by the Food and Drug Supervisory Agency of the Republic of Indonesia (Badan POM RI), both those that do not have halal certification and those that are already certified halal. Halal labels will become important information and become an attraction to the public, so that it will affect the decision to buy health supplement products. Besides to halal labels, brand image or brand image also plays an important role in product marketing to the public. So, in this study, the influence of the MUI halal label and brand image on the decision to buy health supplement products by the Muslim community, especially in the city of Malang, will be examined. The sampling technique in this study was using non-probability sampling with incidental sampling in the Muslim community of the city of Malang who entered the inclusion criteria. The results of this study found that the MUI halal logo, brand image, as well as the MUI halal logo and brand image had a significant positive influence on purchasing decisions for halal supplement products. Keywords: MUI halal label; brand image; health supplements; Malang city ABSTRAK  Indonesia terdapat berbagai macam produk suplemen kesehatan yang sudah mendapatkan nomor izin edar oleh Badan Pengawas Obat dan Makanan Republik Indonesia (Badan POM RI), baik yang belum memiliki sertifikasi halal maupun yang sudah tersertifikasi halal. Label halal akan menjadi informasi penting dan menjadi daya tarik kepada para masyarakat, sehingga akan berpengaruh pada keputusan pembelian produk suplemen kesehatan. Selain label halal, brand image atau citra merek juga berperan penting dalam pemasaran produk kepada masyarakat. Oleh karena itu dalam penelitian ini akan dikaji pengaruh label halal MUI dan Brand Image terhadap keputusan pembelian produk suplemen kesehatan oleh masyarakat muslim terutama di Kota Malang. Teknik pengambilan sampel pada penelitian ini dengan menggunakan nonprobability sampling dengan sampling incidental pada masyarakat muslim kota malang yang masuk kriteria inklusi. Hasil penelitian ini didapatkan bahwa logo halal MUI, brand image, serta logo halal MUI dan brand image memiliki pengaruh positif yang signifikan terhadap keputusan pembelian produk suplemen halal. Kata Kunci: label halal MUI; brand image; suplemen kesehatan; kota malang

Abstract

Aloe vera contains flavonol compounds, saponins such as kaempeferol, quercetin and merycetin as much as 257.7, 94.80 and 1283.50 mg/kg, respectively. Saponins contained in aloe vera gel are able to clean the dirt from skin. Vitamin E in aloe vera is also able to soften, moisturize and increase smoothness of the skin. Mangosteen peel waste contains higher xanthones  compared to the mangosteen fruit itself. In 100 grams of mangosteen peel, there are 107.76 mg of xanthones which are an extraordinary nutritions for the skin because they act as antioxidants and antiaging. This study aims to formulate a combination of aloe vera flesh extract and mangosteen peel extract into a cream dosage form and observe the effect in moisturizing the skin of volunteers within 3 months of treatment.The research was initiated by making extracts of aloe vera and mangosteen peel by using the maceration method. Then the extract was formulated into cream preparations with varies concentrations of each cream A (0%:20%), cream B (5%:15%), cream C (10%:10%), cream D (15%:5% ), cream E (20%:0%). The preparations were evaluated for their physical characteristics, namely homogeneity test, pH test, organoleptic test, dispersibility test, stability test, irritation test and its effectiveness in moisturizing volunteers skin using a moisture checker.The results showed that the combination extract of aloe vera flesh and mangosteen peel can be formulated in cream preparation that is homogeneous, does not change color, and remains stable in 12 weeks of storage with a pH between 6.0-6.6. The best cream formula in increasing moisture of skin are found in cream with a combination of 15% aloe vera flesh extract with 5% mangosteen peel extract by an uplifting until 92.5%.Lidah buaya atau aloe vera mengandung senyawa flavonol, saponin seperti kaempeferol, quercetin dan merycetin masing-masing sebanyak 257,7, 94,80 dan 1283,50 mg/kg. Kandungan saponin yang terdapat dalam gel lidah buaya dapat membersihkan kotoran dari kulit. Vitamin E pada aloe vera juga mampu melembutkan, melembabkan dan menambah kehalusan kulit. Limbah kulit manggis mengandung xanthone yang lebih tinggi daripada dalam daging buah manggis. Dalam 100 gram kulit manggis terdapat  xanthone sebesar 107,76 mg dan memiliki khasiat yang luar biasa untuk kulit karena berperan sebagai antioksidan dan antiaging. Penelitian ini bertujuan untuk memformulasikan kombinasi ekstrak daging lidah buaya dan ekstrak kulit buah manggis ke dalam bentuk sediaan krim dan mengamati efek kelembaban pada kulit sukarelawan selama 3 bulan perawatan.Penelitian diawali dengan membuat ekstrak lidah buaya dan kulit manggis yang diperoleh dengan menggunakan metode maserasi. Kemudian ekstrak diformulasikan menjadi sediaan krim dengan variasi konsentrasi masing-masing krim A (0%:20%), krim B (5%:15%), krim C (10%:10%), krim D (15%:5%), krim E (20%:0%). Sediaan dievaluasi untuk karakteristik fisiknya yaitu uji homogenitas, uji pH, uji organoleptis, uji daya sebar, uji stabilitas, uji iritasi dan uji kelembaban pada sukarelawan dengan menggunakan moisture checker.Hasil penelitian menunjukkan bahwa ekstrak daging lidah buaya dan kombinasi kulit buah manggis dapat di formulasikan dalam sediaan krim pelembab yang homogen, tidak berubah warna, dan tetap stabil dalam penyimpanan 12 minggu dengan pH 6,0-6,6. Formula krim terbaik yang memberikan efek peningkatan kadar air tertinggi adalah krim dengan kombinasi ekstrak daging lidah buaya 15% dengan ekstrak kulit manggis 5% dengan peningkatan hingga 92,5%.

Abstract

Sistem penyimpanan obat berperan penting dalam menjaga mutu dan kualitas suatu obat, karena sistem penyimpanan obat merupakan kegiatan yang melaksanakan pengamanan terhadap semua obat dan perbekalan kesehatan lainnya yang dapat diterima agar aman, terhindar dari kerusakan fisik ataupun kimia, serta mutu suatu obat tetap terjamin. Tujuan penelitian ini adalah untuk mengetahui sistem manajemen dari obat rusak dan obat kedaluwarsa serta mengetahui faktor-faktor penyebab terjadinya obat rusak dan kedaluwarsa dilihat dari proses penyimpanannya di Instalasi Rumah Sakit Mokopido Tolitoli. Metode penelitian ini menggunakan metode deskriptif kualitatif secara retrospektif digunakan pedoman wawancara, pedoman observasi, pedoman dokumentasi, dan lembar-lembar pengumpulan data berupa buku pencatatan obat rusak dan kedaluwarsa. Hasil penelitian menunjukkan bahwa persentase nilai obat rusak sebesar 3,77% dan obat kedaluwarsa sebesar 3,41%, yang artinya jumlah obat tersebut belum memenuhi standar indikator yang telah ditetapkan. Kesimpulan dari penelitian ini adalah sistem penyimpanan obat di Rumah Sakit Mokopido Tolitoli sudah ada, tetapi masih belum berjalan optimal karena ada bagian tertentu yang belum dilaksanakan. Salah satu faktor penyebabnya, yaitu karena tempat penyimpanan obat yang masih kurang memadai. Kata kunci: Sistem penyimpanan obat, obat rusak, obat kedaluwarsa
