Abstract

Hypertension is a global health problem due to its increasing prevalence. Hypertension is a condition where there is a persistent increase in blood pressure exceeding 140/90 mmHg. Among the many choices of antihypertensive drug classes, ARB is one of the first-line drug classes for the treatment of hypertension. Inappropriate ARB administration can reduce the effectiveness of treatment and harm the patient. This study aims to determine the pattern of ARB use in hypertensive patients. This drug pattern study has been done on October – November 2021 at the Outpatient Department of Dr. Mohammad Hoesin Central Public Hospital Palembang. The sample was using the medical record data of all hypertensive patients’ period of July – December 2020 who met the inclusion criteria, with a total of 171 data. The data was processed by using the SPSS v25 and presented in the form of a table with a descriptive explanation referring to the research variables. This study found that the prevalence of ARB use was 59.0% with the highest was in male (30.4%) and age group 41-60 years (29.2%). The most widely used ARB is candesartan (94.0%) at a dose of 16 mg (52.6%), frequency once a day (97.9%) and duration more than one month (93.7%). The drug prescribed to the patient mostly didn’t have interaction with ARB (33.0%). The prevalence of ARB administration is high in hypertensive patients (59.0%) with the most commonly used ARB is candesartan (94.0%).

Abstract

Soil-Transmitted Helminths (STH) is defined as a parasitic intestinal worm that can infect humans through contaminated soil. STH is reported to have infected more than 1.5 billion people worldwide. STH infection can manifest in various gastrointestinal disorders such as diarrhea and malnutrition. Flies that are diptera insects can act as mechanical vectors of various parasites, including STH. Therefore, this study aimed to identify the types of flies and contamination of STH eggs on the outside of the fly’s body in traditional market. This type of research was a descriptive observational study with a cross-sectional design. The samples in this study were all flies caught using Aerial nets and WHO fly traps in Kamboja Market, Ilir Timur I district, Palembang. The place for processing and observing samples was carried out at the Medical Chemistry and Biooptics Laboratory of FK UNSRI. The results showed the number of flies obtained was 258, consisting of 4 species, 156 flies of Musca domestica 156 (60.5%), 97 flies of Chrysomya megacephala (37.6%), 3 flies of Fannia sp. (1.2%), and 2 flies of Lucilia sp. (0.7%). 258 flies were grouped into 36 samples was observed under the microscope to find STH egg contamination. Among the 36 samples examined, 9 samples (25%) were found consisting of 12 A. lumbricoides eggs, 1 T. trichiura egg, and 1 hookworm egg. It can be concluded that there was contamintaion of STH eggs on the outside of the fly’s body at traditional market.

Abstract

Ovarian cancer passed down in the family through inheritance of BRCA1 and BRCA2 genes that have been mutated. The objective of this research was to explore the relationship between dermatoglyphic patterns and ovarian cancer. Research method used an observational analytic using case-control design taking place at Obstetrics Inpatient Ward in RSUP Dr. Mohammad Hoesin Palembang. Samples of case and control groups were selected by using consecutive sampling method (Non probability). This method included all patients that meet the inclusion criteria as part of the sample in order to fullfill number of samples required to perform the analysis, and sample selection was done during certain period. Chi-Square test was used to analyze correlation between fingerprint patterns (Arch, Loop, and Whorl) and ovarian cancer. Independent sample t-test used to find average discrepancy of TRC (Total Ridge Count) and ATD angle. These statistical analyses were done by using SPSS for Windows. Research result indicated that most ovarian cancer samples had dermatoglyphic pattern; followed by Loop (52.4%) Whorl (44.2%), and Arch (3.3%). Dankmeijer and Furuhata indices of ovarian cancer were 3.91 and 68.45, respectively. There was a significant relationship between ovarian cancer patients’ and non-ovarian cancer patients’ fingerprints pattern, with Whorl pattern p value = 0.004 and Loop pattern p value = 0.005. There was no significant difference of Total Ridge Count (TRC) between ovarian cancer patients and non-ovarian cancer patients, with p value = 0.238. There was no significant difference of average angle of right ATD, with p value = 0.893, and average angle of left ATD, with p value = 0.323, between ovarian cancer patients and non-ovarian cancer patients.

Abstract

 The implementation of sex education to children is an application what children aged 8-10 years get about things related to sex education in the form of children tau and maintaining the cleanliness of their reproductive organs, children know what forms of sexual abuse are, children are able to protect themselves from sexual harassment.  The purpose of this study was to analyze the implementation of sex education in children at elementary school 108 Palembang. The population in this study were all 6th grade students aged 10-13 years, sample taken in this study 96 respondents using a total sampling technique. The chi square test results show that there is an association between the implementation of sex education in children with knowledge (P = 0.001, OR = 4.8, 95% CI = 1.9-12), role of parents (P = 0.000, OR = 6.95 CI = 2.6-13.6), role of homeroom teacher (P = 0,000, OR = 8.03, 95% CI = 3.05-21.1), role of friends (P = 0.010, OR = 3.8, 95% CI = 1.4-10), social media (P = 0.003, OR = 4.2, 95% CI = 1.6-10.4), facilities (P = 0.004, OR = 4.2, 95% CI = 1.6-11), religious norms (P = 0.036, OR = 2.7, 95% CI = 1.1-6.6). With the most dominant variable with the implementation of sex education in children is the role of the homeroom teacher (OR=8.519, 95% CI= 2.58-28.09). It is recommended that through this research, especially the parents to maintain the provision of good sex education to their students and start giving them to grade 5 children according to curriculum 13, and it is even better to provide sex education as early as possible to all students using appropriate language according to age.

Abstract

Abduction is the most important functional movement of the glenohumeral joint, and at the same time one of the most complex movements of the entire body. Brachial plexus injury can make shoulder fail to abduction. Saha’s procedure (trapezius transfer) one procedure that safe and significantly improve the shoulder movement especially abduction. We report a teenager male presented with inability to move his right shoulder and elbow after a motor vehicle accident 5 weeks before. He was unable to move the elbow and shoulder, but the hand and wrist still had a good function. From the right shoulder X-Ray there is proximal humerus fracture. Steindler flexorplasty was performed after 3 months strengthening program. 6 months after Steindler flexorplasty, Saha’s procedure was performed to improve the shoulder movement. The shoulder can abduction 200-550 and the elbow can flex 200-800 after the surgery. In this case, the superior trunk of the brachial plexus was injured due to the shoulder and the neck forcibly widens after direct shoulder trauma. Saha’s procedure after Steindler flexorplasty is best for superior trunk brachial plexus injury, a simple procedure with minimal blood loss, which provided functional improvement. Steindler flexorplasty and Saha’s Procedure are two procedures very beneficial to the patients who experienced superior trunk brachial plexus injury.

Abstract

Placenta Accreta Spectrum Disorder (PASD) is abnormal trophoblast invasion of part or all of the placenta into the myometrium of the uterine wall. Magnetic resonance imaging (MRI) examination is one of the tools that can help diagnosing PASD earlier, so that maternal morbidity and mortality can be reduced. This study aims to determine the prevalence, risk factors of PASD and the accuracy of Placenta Accreta Index Score (PAIS) and MRI, with histopathological examination in diagnosing PASD at dr. Mohammad Hoesin General Hospital (RSMH) Palembang during the 2018–2021. A descriptive study with a survey design on pregnant and intrapartum women with suspected PASD was performed at Department of Obstetrics and Gynecology at RSMH Palembang from 2018 until 2021. There were 72 study subjects who met the inclusion criteria. The relationship between the independent and dependent variables was analyzed using Chi Square and Fisher Exact. The cut-off point of the PAIS scores was analyzed using the Receiver Operating Curve (ROC). The comparison of the diagnostic value of PAIS and MRI scores used the Youden Index. Data was analyzed with SPPS version 22.0 From 72 subjects, 60 subjects (83.3%) were PASD and 12 subjects (16.7%) were not PASD. The risk factors of PASD in this study was surgical history more than once (PR = 4.600 (95% CI 1.261–16.781); p = 0.037). Youden Index values and PAIS accuracy were 0.782 and 0.953 while Youden Index values and MRI accuracy were 0.333 and 0.886. PAIS and MRI could be considered as diagnostic tools for PASD. However, overall, PAIS had a better diagnostic value than MRI.

Abstract

Monitoring the growth and development of children in the first 1,000 days of life is very important given the rapid growth and development at this age. This very rapid brain development at the age of under 2 years (a critical period of development) is the right time to make recovery, if there are growth disorders and nutritional deficiencies. Increasing the community's ability to assess children's growth and development and fulfillment of family nutrition is very important as an effort to optimize the process of child growth and development as the nation's next generation. The purpose of this programme is to improve the ability of the community (independently) in assessing children's growth and development and fulfilling family nutrition. The methods used in this programme are focus group discussions as the basis for making modules, health promotion with interactive lectures and discussion techniques, and simulations of how to assess toddler growth and development using maternal and child health books (KIA) and family nutrition fulfillment. This programme was carried out for 1 day and took place in Kaliasin Village, Tanjung Bintang District, South Lampung Regency. The participants were 30 people who were members of the community in Kaliasin Village, Tanjung Bintang District, South Lampung Regency. The results of the evaluation found that there was an increase in participants' understanding as much as 90% became a good understanding, 10% of participants had a fairly good understanding, and none of the participants had a poor understanding of how to assess the growth and development of toddlers and the fulfillment of family nutrition. The average pretest result was 50.5 and an increase in the posttest result was 83.5. Based on the analysis using the Wilcoxon test, it was found that there was a statistically significant mean difference between the pretest and posttest scores participants on how to assess toddler growth and family nutrition (p=0.000). Increasing the community's ability to assess children's growth and development and fulfillment of family nutrition with this technique of increasing knowledge and skills has proven to be effective.

Abstract

Pulmonary tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis which is currently a major global health problem. While our world still in COVID-19 pandemic situation. Anxiety is a natural human behaviour that manifests itself as a bodily response to dangers.Mycobacterium tuberculosis infection as a more common comorbidity for COVID-19 by 36% and from the situation of the spread of COVID-19 which has almost reached all provinces in Indonesia with the number of cases and the number of deaths increasing, researchers are interested in conducting research on the determinants of the level of Anxiety of pulmonary TB patients in the COVID-19 pandemic situation in Palembang City. Using the Hamilton Rating Scale for Anxiety (HARS) as instrument we collect data regarding anxiety level among pulmonary tuberculosis patients in COVID-19 pandemic settings. Researchers discovered that responders were 131 pulmonary tuberculosis patients who were recruited from public health centers in Palembang City, based on researcher observations and questionnaires. Overall, there are significant relation regarding anxiety levels and its determinant factor on tuberculosis patient, especially in COVID-19 pandemic settings.

Abstract

Health workers are at the forefront of responding to the COVID-19 emergency, which is at higher risk from various groups of workers. During the COVID-19 pandemic, workers may experience increased stress stemming from uncertain work situations, as well as increased stress for emergency response workers due to the application of strict K3 measures in preventing transmission, for example by using heavy PPE, applying physical isolation and etc. The purpose of this study was to determine the relationship between age, gender and length of work of health workers on stress with the application of an OHS management system during the COVID-19 pandemic. This research is analytic observational with cross sectional design. The study was conducted at the Kayu Agung Hospital. The population of this study were health workers at the Kayu Agung Hospital. The sample of this research is health workers, namely doctors and nurses with a total sample of 97 respondents. The data collection instrument used a questionnaire. Analysis of research data is univariate and bivariate using the chi square test. The results showed that there was no significant relationship (p>0.05) between age, gender, length of work on the incidence of stress during the COVID-19 pandemic and there was a significant relationship (p<0.05) between the implementation of the OHS management system and the incidence of stress, Odd value The ratio (OR) of 3.265 means that respondents who implement the OHS management system well are at risk of 3,265 times experiencing stress events compared to respondents who implement the OHS management system satisfactorily.The conclusion of this study is that only the application of the OHS management system has a significant relationship with the incidence of stress. It is hoped that it will increase the socialization of K3 and the application of the OHS management system, as well as manage stress with positive thinking patterns and apply a healthy lifestyle.

Abstract

Ageing is associated with the geriatric syndrome, characterized by several health issues frequently associated with immunosenescence. In the elderly, immunosenescence is characterized by low-grade chronic systemic inflammation manifested as elevated levels of inflammatory molecular and cellular markers. One of the inflammatory markers is interleukin 6 (IL-6). In addition, omega-3 polyunsaturated fatty acids are a class of biologically active and anti-inflammatory fatty acids. This study aims to determine the relationship between omega-3 and interleukin-6 levels in the elderly in RSMH Palembang who are in good health. This investigation is a cross-sectional, analytic observational study. The sampling method employed was consecutive sampling. The participants in this study were members of the RSMH Palembang healthy elderly community who were 60 years of age or older. People who were malnourished, had severe illnesses like cancer or autoimmune diseases, or were taking Omega-3 supplements were not allowed in this study. As a result, 22 (55%) of 40 research subjects had low serum omega-3 levels. On the other hand, there were 35 (87.5%) samples with elevated serum IL-6 levels and 5 (12.5%) samples with normal IL-6 levels among the samples obtained. The correlation analysis showed that there was a strong negative correlation between the levels of serum omega-3 and interleukin 6 (p = 0.005). Omega 3 levels and interleukin-6 levels are correlated in the healthy elderly population of RSMH Palembang.
