Abstract

BSTRACT One of the most common cardiovascular diseases with high morbidity andmortality rates is Congestive Heart Failure (CHF). Patients with CHF frequentlyexperience lower limb edema, which can cause significant discomfort. This study aimed to evaluate the implementation of contrast bath therapy in reducing pedal edema among CHF patients at TKII Putri Hijau Hospital, Medan. This research employed a descriptive case study approach using nursing care stages, including assessment, diagnosis, intervention, implementation, and evaluation, supported by observation sheets and physical examinations. The study involved two respondents at TK I/ Putri Hijau Hospital, Medan. The findings showed that after three days of intervention, edema was reduced: in Case I, pitting edema decreased from grade 2+ (4 mm) to grade I+ (1 mm), andin Case Il, from grade +2 (5 mm) to grade I+ (2 mm) following the application of contrast bath therapy. In conclusion, complementary therapy using contrast bath is effective in reducing pedal edema in CHF patients. It is recommended that contrast bath therapy be considered as an adjunct intervention in the managementof edema among CHF patients. ABSTRAK Salah satu penyakit kardiovaskular yang paling umum dengan tingkat morbiditas dan mortalitas yang tinggi adalah Congestive Heart Failure (CHF). Pasien dengan CHF sering mengalami oedema tungkai, yang dapat menyebabkan ketidaknyamanan. Tujuan untuk mengetahui implementasi contrast bath dalam menurunkan oedema kaki pada pasien CHF di Rumah Sakit TK II Putri Hijau Medan. Metode penelitian ini menggunakan deskriptif dengan pendekatan metode studi kasus dengan tahapan asuhan keperawatan yang mencakup pengkajian, diagnosa, intervensi, implementasi, dan evaluasi serta lembar observasi dan pemeriksaan fisik. Penelitian ini dilakukan dengan dua responden di Rumah Sakit TK II Putri Hijau Medan. Hasil penelitian diperoleh setelah dilakukan 3 hari penelitian didapatkan oedema berkurang, pitting oedema pada kasus I dari derajat 2+ 4mm turun menjadi 1+ 1mm, dan pada kasus 2 yaitu derajat 2+ 5mm turun menjadi derajat 1+ 2mm setelah diberikan contrast bath. Kesimpulan terapi komplementer contrast bath efektif menurunkan oedema kaki pada pasien CHF. Saran diharapkan terapi komplementer contrast bath dapat dijadikan sebagai intervensi pendamping dalam manajemen oedema pada pasien CHF.

Abstract

Congestive Heart Failure (CHF) is a complex syndrome characterized by impaired ventricular function in pumping blood, leading to symptoms such as shortness of breath. This study aims to describe the implementation of the semi-Fowler's position in CHF patients with ineffective breathing patterns. This research used a descriptive case study design, carried out through nursing care stages including assessment, diagnosis, intervention, implementation, and evaluation, along with observation sheets andphysical examinations. The study was conducted on two respondents at TK II Putri Hijau Hospital, Medan. The results showed an improvement in oxygen saturation (Sp02) in both patients after the semi-Fowler 's position was applied. In the first patient, Sp02 increased from 95% to 97%, while in the second patient, it increased from 94% to 98%. The findings indicate that the semi-Fowler's position helps improve respiratory quality and increase oxygen saturation in CHF patients. This intervention is recommended as an easily implemented non-pharmacological nursing approach for inpatient care. Nurses are encouraged to apply this intervention for CHF patients.

Abstract

Ischemic stroke is a serious non-communicable disease worldwide, characterized by functional impairment affecting the brain, such as nerve paralysis caused by a lack of oxygen supply to the brain. This study aims to describe the implementation of discharge planning regarding personal hygiene for ischemic stroke patients. This research used a case study method with a nursing care approach, including assessment, diagnosis. intervention, implementation, and evaluation. Data were collected using observation sheets and questionnaires. The results of this study showed an increase in knowledge regarding discharge planning on personal hygiene in both patients: in the first patient, knowledge level increased from 46% (poor) to 86% (good), and in the second patient, knowledge level increased from 46% (poor) to 86% (good). It can be conclude that discharge planning related to personal hygiene is effective in improving the understanding and ability of families to care for patients at home, thereby helping reduce the risk of complications. It is recommended that families regularly apply personal hygiene practices to prevent complications such as skin infections and body odor

Abstract

Diabetes mellitus is a chronic non-communicable disease whose incidence continues to increase globally. This disease occurs due to insufficient insulin production or insulin does not work effectively, making blood glucose levels difficult to control. The purpose of this study was to determine whether physical walking can stabilize blood glucose in type II diabetes mellitus patients at Putri Hijau Class II Hospital, Medan. This study used a quasi-experimental study with a control group. The sampling technique used was a purposive sampling technique so that the number of samples obtained according to the inclusion criteria was 40 participants who were divided into two groups: 20 participants in the intervention group and 20 participants in the control group. Based on the Wilcoxon test, there was a significant effect p <0.000. The results of the study showed that physical activity intervention walking can stabilize blood sugar levels in type II diabetes mellitus patients. It can be concluded that doing walking regularly can stabilize blood glucose. Suggestions are expected for type II diabetes mellitus patients to apply walking as a non-pharmacological therapy. It is hoped that it can be a complementary therapy for type II diabetes mellitus sufferers who are treated in hospital or at home
