Abstract

ABSTRACTIntracranial atherosclerosis is the most common cause of stroke in Asia including Indonesia. Transcranial Doppler (TCD) is a noninvasive tool which able to detect intracranial atherosclerosis. The purpose of this study was to determine the pattern of TCD waves in Javanese ischemic stroke patients and Carotid Intima Media Thickness (CIMT), and its correlation to demographic data. This is a cross-sectional study was conducted at Hospital X Surakarta. All Javanese ischemic stroke treated in stroke unit were examine for flow velocity using TCD and CIMT using ultrasound. Chi square was used to analyzes the relationship between age, gender, stroke type and the thickness of CIMT.  A total of 43 ischemic stroke patients were examined for TCD, consisting of 30 (69.77%) men and 13 (30,23%) women. The results showed that 31 (72.09%) had intracranial abnormalities in the form of stenosis or occlusion on MCA (23.26%), ACA (4.65%), PCA (4.65%), vertebral arteries (6.98 %) and other locations (32.56%). Thickening of CIMT was obtained as much as 22 (51,16%) of the patients. Age was a factor associated with CIMT thickening with (P: 0.041), while gender and type of stroke did not (P>0,05)). Transcranial Doppler (TCD) adalah alat noninvasif yang mampu mendeteksi aterosklerosis intrakranial. Tujuan dari penelitian ini untuk mengetahui pola gelombang TCD pada pasien stroke iskemik suku Jawa dan Carotid Intima Media Thickness (CIMT), dan korelasinya dengan data demografi. Penelitian cross-sectional ini dilakukan di Rumah Sakit X Surakarta. Semua stroke iskemik suku Jawa yang dirawat di unit stroke diperiksa untuk kecepatan aliran menggunakan TCD dan CIMT menggunakan ultrasound. Chi square digunakan untuk menganalisis hubungan antara usia, jenis kelamin, jenis stroke dan ketebalan CIMT. Sebanyak 43 pasien stroke iskemik diperiksa untuk TCD, terdiri dari 30 (69,77%) pria dan 13 (30,23%) wanita. Hasil penelitian menunjukkan bahwa 31 (72,09%) memiliki kelainan intrakranial dalam bentuk stenosis atau oklusi pada MCA (23,26%), ACA (4,65%), PCA (4,65%), arteri vertebral (6,98%) dan lokasi lain (32,56%). Penebalan CIMT diperoleh sebanyak 22 (51,16%) dari pasien. Usia adalah faktor yang terkait dengan penebalan CIMT dengan (p= 0,041), sedangkan jenis kelamin dan jenis stroke tidak (p> 0,05), sehingga ditarik kesimpulan bahwa kelainan kecepatan aliran intrakranial sering terjadi pada pasien stroke iskemik Jawa, sedangkan penebalan CIMT tidak berhubungan dengan patologi stroke iskemik.Kata Kunci: TCD, Stroke Iskemik, Flow Velocity, CIMT, Suku Jawa

Abstract

Background: Tuberous Sclerosis Complex (Bour­neville’s disease) is genetic disorder with autosomal pattern of heritence that affecting differentiation sellular in many organ such as brain, lungs, cardiac with varible severity. Birth incidence is estimated to be 1:6000. The purpose of this study was to reported a case of patients with Bourneville’s disease (tuberous sclerosis complex) in Moewardi Hospital. Also reported surveillance and complication in this patient.Case Presentation: A male, 18 yearsold, with seizures, onset from infancy, tonic secondary generalized seizure pattern simultaneously throug­h­out the body. Neurologic status exami­nation found right hemiparese and mental re­tarda­tion, Encountered in patients with skin disorders such as butterfly appearance with facial angiofibroma, forehead fibrous plaque dan shar­green patch. Abnormalities also appeared in the oral mucosa as multiple papules. When psych­ological tests patient had difficulty following instructions. Psychiatric examination showed mode­rate lower intelegence. EEG results slow waves at left parietal, there is severe electro­fisio­logic abnormality considered structural lesi­on in left parietal. CT showed multiple calci­fied nodul­es in subependymal right and left lateral ventri­cle. MRI showed  atrophy and left sclerotic hip­po­campus with glosis in sub cortex occipito left parietal. To overcome the seizure patients given Carbamazepine. There was in­creas­ing of trans­aminase enzim, liver suspect liver angio­myo­lipoma.Conclusion: Based on clinical criteria for definite tuberous sclerosis complex patients met the criteria. The patients responded positively to carbamazepine that reduced seizure frequency. But somehow, patient's prognosis is worst. Abstract

Case: A 28-years-old woman presented with sudden right extremities weakness, right sided facial droop, and not being able to speak. On examination, the patient had upper motor neuron (UMN) hemiplegia and motor aphasia. On blood laboratory, we found D-dimer level of 1068 ng/dl. A non-contrast head CT-scan and CT-angiography showed left cerebral hemisphere infarction, and bilateral carotid artery stenosis. Further investigation with transcranial Doppler demonstrated partial stenosis of internal carotid artery (ICA). On cerebral digital subtraction angiography (DSA), we observed pseudoaneurysm on the right ICA and flame-shaped appearance on the left ICA, both are common radiological finding of ICA dissection. Then we performed stenting angioplasty on bilateral ICA to provide recanalization, and aspirin 80mg q.d. and clopidogrel 75mg q.d. was initiated. Conclusion: Spontaneous dissection of the carotid artery can occur on young adults. DSA is considered as the gold standard for vascular examination, and stenting with angioplasty was performed for recanalization to prevent the progression to severe stroke.

Abstract

Background: Stroke, an acute neurological dysfunction, poses a health challenge with a potential fatality, emerging as a leading cause of long-term disability. This study explores the efficacy of repetitive transcranial magnetic stimulation (r-TMS) with medical rehabilitation in enhancing stroke patients' motor strength, comparing it with standard therapy and focusing on the Medical Research Council score. Objective: Stroke is an acute manifestation resulting from neurological dysfunction, persisting for ≥24 hours or leading to death due to blood vessel disorders. The study aims to determine the effectiveness of r-TMS therapy in improving stroke patients' motor strength, comparing it with standard therapy based on the Medical Research Council score. Methods: A quasi-experimental study with a pre-post control group design involved 30 respondents selected through consecutive sampling in stroke units and outpatient clinics. Upper extremity muscle strength, measured with the Medical Research Council score, was analyzed using the Mann-Whitney U test, with a significance level set at p < 0.05. Results: The post-test ranking value of the r-TMS group (20.56) exceeded that of the Non r-TMS group (10.46). The Mann-Whitney U test indicated a significant difference in the average ranking of the Medical Research Council scores between the two groups (p < 0.05, 0.001), highlighting the efficacy of r-TMS therapy in enhancing motor strength. Conclusion: This study shows that stroke patients receiving standard therapy and transcranial magnetic stimulation exhibit improvements in extremity motor strength compared to those undergoing medical rehabilitation alone. The r-TMS group notably showed a significant increase in extremity motor strength.

Abstract

Background: A 72-year-old male, experiencing dysphagia and left-sided weakness for six months post-stroke, encountered challenges such as coughing and choking during the consumption of liquids and soft foods, along with a prolonged meal-swallowing process. Magnetic Resonance Imaging (MRI) revealed bilateral thrombotic infarction and left lateralization. Case: The Gugging Swallowing Screening Scale (GUSS) assessment demonstrated severe impairment with a total score of 7, persisting despite conventional physiotherapy attempts to improve swallowing function. Subsequently, repetitive transcranial magnetic stimulation (rTMS) was implemented, involving high-intensity stimulation in the ipsilesional hemisphere and low-intensity stimulation in the contralesional hemisphere. Remarkably, one-month post-rTMS, the patient displayed significant progress, evidenced by an improved GUSS score of 15, indicating enhanced swallowing function. Discussion: This case emphasizes the positive impact of bilateral rTMS hemispheric stimulation on post-stroke dysphagia. The strategic application of high-intensity ipsilesional and low-intensity contralesional stimulation emerged as an effective intervention for alleviating swallowing difficulties. Conclusion: These findings highlight the potential of rTMS as an innovative therapeutic approach for persistent dysphagia following a stroke. Repetitive transcranial magnetic stimulation (rTMS) has shown significant potential as an innovative and effective therapeutic approach for managing persistent post-stroke dysphagia. This case highlights the role of tailored rTMS protocols in improving swallowing function, with recovery influenced by factors such as stroke severity, dysphagia severity, age, nutritional status, timing of intervention, and lesion location.

Abstract

BACKGROUND Stroke is the third leading cause of morbidity and second in mortality worldwide. Diabetes mellitus (DM) is a risk factor for stroke. Vaspin and single nucleotide polymorphism (SNP) rs2236242 involved DM pathogenesis. This study aimed to explore the correlation between SNP rs2236242, serum vaspin levels, and atherosclerosis in patients with ischemic stroke and DM. METHODS This study was conducted in Dr. Moewardi Hospital from 2022 to 2023. The case group included patients with ischemic stroke and DM, while the controls were those with ischemic stroke only. Tetra-primer amplification refractory mutation system-polymerase chain reaction was used to determine the genotypes. RESULTS There were 31 cases and 33 control. SNP rs2236242 showed that odds ratio (OR) (95% confidence interval [CI]) for AA-TT was 0.273 (0.241–0.305) and for TA-TT was 0.315 (0.298–0.341). The OR (95% CI) for allele A to T was 0.789 (0.373−1.669). The mean (standard deviation) serum vaspin level in the case group compared to the control was 1,570 (2,108) ng/ml versus 1,630 (1,428) ng/ml (p = 0.064). Higher vaspin levels were found in T allele of the TT (1,523 [2,269] ng/ml, p = 0.021) and TA (1,760 [1,349] ng/ml, p = 0.004) genotype groups than the A allele of the AA genotype group (0.914 [0.329] ng/ml). CONCLUSIONS Vaspin gene polymorphism AA genotype or A allele significantly reduces vaspin levels in patients with ischemic stroke with DM.

Abstract

Background: Stroke is a leading cause of disability worldwide, significantly impacting motor function, sensory, visual, and cognitive abilities. Conventional physical therapy has traditionally been used for stroke rehabilitation. However, technological advances have introduced virtual reality (VR) as a rehabilitation tool. Research has highlighted the therapeutic value of VR in improving motor functions, with some studies suggesting superior outcomes compared to traditional therapy. Despite this, the literature presents varied results, necessitating a systematic review to evaluate the breadth of evidence. Objective: This study aims to compare the effectiveness of VR therapy with conventional physical therapy in extremity function among stroke patients. Methods: A primary search was conducted across PubMed, Scopus, and ProQuest databases using keywords related to stroke, VR, and conventional therapy. Studies will be included if they compare the efficacy of VR with conventional therapy in stroke rehabilitation. Non-comparative studies, non-English publications, and irrelevant topics will be excluded. Results: A literature search yielded 15 eligible journals using a randomized clinical trial. Analysis showed that 4 out of 15 articles found VR therapy not superior to conventional therapy, while the other 11 articles demonstrated that VR therapy statistically improved patient conditions compared to conventional therapy. Conclusion: VR-based exercises stimulate cortical reorganization and strengthen neural systems involved in motor planning, learning, and execution. Most studies indicate that VR therapy is more effective in enhancing motor recovery, balance, and limb function in stroke patients and increasing patient motivation and engagement than conventional training.

Abstract

Stroke is a major contributor to long-term disability and its incidence continues to rise annually. This case report aimed to explore the promising benefits of combining stem cell therapy with repetitive transcranial magnetic stimulation (rTMS) in managing acute ischaemic stroke patients. A 62-year-old male presented with left-sided hemiparesis and hemineglect, as well as a cognitive disturbance in the attention domain. His medical history included uncontrolled hypertension over decades and diabetes mellitus for five years. A non-contrast head computed tomography (CT) scan revealed infarction in the right middle cerebral artery (MCA), with an initial National Institutes of Health Stroke Scale (NIHSS) of 13 upon admission to the Emergency Room. The infection and tumour markers were conducted to confirm no contraindication in this patient receiving stem cell therapy. Following the acute phase, the patient underwent a comprehensive treatment regimen involving both stem cell therapy and serial rTMS. Clinical assessments included NIHSS, Barthel Index, and Fugl-Meyer Assesement to evaluate neurological deficits. Additionally, the Montreal Cognitive Assessment-Indonesian version (MoCA-INA) assessment, electroencephalography examination and motor threshold were conducted. The results of this case report revealed noteworthy improvements in NIHSS, motoric strength, and cognitive function post-treatment. In this case report, improvement in clinical outcomes was obtained in the form of motor strength and higher cortical function. Stem cell therapy combined with rTMS has good potential in treating various neuroregenerative and rehabilitative aspects in ischaemic stroke patients.
