Abstract

Objective: This study aims to assess the impact of the COVID-19 pandemic on postpartum contraceptive methods. Methods: This retrospective descriptive study was conducted at a single secondary center, utilizing secondary data retrieved from medical records at the Inpatient Installation of Sebelas Maret University Hospital, Surakarta, covering the period from January 2020 to January 2022. Results: Among users of intra-uterine devices (IUDs), 85% were below 35 years old, 65% were primiparous, 67% had a history of previous injectable contraceptive use, 79% received routine antenatal care, and 51% had education below a college level. These individuals were educated about the importance of contraceptive programs during the COVID-19 pandemic. Sixty-six percent of IUD insertions were conducted via vaginal delivery, and 27% had health facilities within less than 1 km. In contrast, tubectomy contraceptive users comprised 106 patients, with the majority (54%) being aged 35 years or older, all being multiparous, and 25% tested positive for COVID-19. Among tubectomy users, 43% had a history of previous injectable contraceptive use, 85% received routine antenatal care, and 54% had education below a college level. Similar to the IUD group, they were educated about the significance of contraceptive programs during the COVID-19 pandemic. Eighty-four percent of tubectomies were performed via cesarean section, and 27% of patients lived within less than 1 km from health facilities. Conclusion: The usage rates of intra-uterine devices and tubectomy for contraceptives remained stable during the COVID-19 pandemic. However, there was a decrease in postpartum in-person visits and mobility, coupled with an increase in hospitalizations.Keywords: contraceptive; family planning; intrauterine device; tubectomy

Abstract

Introduction: Community welfare is confronted with a substantial challenge by Indonesia's burgeoning population, and the government's principal aim is the promotion of Long-acting reversible contraceptives (LARC). Post-placental IUD is an efficacious method that incorporates health promotion by means of midwife-delivered contraceptive counseling. Counseling promotes the development of positive relationships, knowledge, attitudes, and behaviors, thereby fostering responsible behavior and individual agency in decision-making. Nevertheless, the low utilization rate of postpartum IUD contraception can be attributed to the scarcity of information regarding the mother. The objective of this study is to examine the level of health promotion regarding post-placental IUD family planning in Malang Regency. Material and Methods: Descriptive methods will be employed, including verbal and written observations of behavior and descriptive data on individuals. The interview-based data collection process spanned the months of June to August 2023. Results: The research results suggest that inadequate efforts to promote postpartum IUD contraception are due to a dearth of understanding, motivation, resources, geographical proximity, and community customs. The objective is to improve the existing insufficiency of post-placental IUD family planning promotion in Malang Regency. Conclusion: From the beginning of pregnancy onwards, expectant women must be consistently motivated and educated about health promotion and family planning in order to develop an interest in and understanding of the choice of post-placental IUD family planning.

Abstract

The highest cause of maternal death in Banyumas Regency, Indonesia in 2022 is preeclampsia during the postpartum period (66.7 percent). However, preeclampsia detection is only carried out if there are symptoms or complaints. Complementary therapies provide a new modality for preventing preeclampsia without having to wait for symptoms to appear. A preliminary study showed that 95.31 percent of midwife who applied complementary therapies had not received training. This research aims to develop a Postnatal Complementary Care Preeclampsia training model for midwives. This research is RnD. Analysis stage with in-depth interviews (6 midwives) and FGD (6 stakeholders). At the Design and Development stage, the training model was prepared and validated by four experts. In the implementation phase, training interventions were carried out on 66 midwives and evaluated using different tests. Results: (1) Analysis: it is necessary to strengthen midwife resources with training to improve the care of preeclampsia postpartum; (2) Design, and create a postnatal complementary care preeclampsia training model; (3) Development, V-Aiken's expert assessment results are in the very high category (mean 0.95) shows the model is valid; (4) Implementation, Independent T-test results show the difference in performance (p-value 0.030 < 0.05), Mann Whitney test results also show the difference in competence (p-value 0.000 < 0.05) which is higher in intervention group, (5) Evaluation, Wilcoxon test results showed (P value 0.006<0.05) the training model was proven to reduce the incidence of preeclampsia in postpartum. The Postnatal Complementary Care Preeclampsia training model is feasible and can be applied as a training guide to improve the competency and performance of midwives to reduce the incidence of postpartum preeclampsia

Abstract

Molecular docking is an important computational method for drug design. It can be used to predict receptor binding interactions with ligans. In addition, phenol-derived compounds and Saponins are also reported to have various activities such as anti-hypertensive, anti-inflammatory and anti-apoptosis agents. The purpose of this study is to predict whether the Phenol and Saponin compounds are active as anti-inflammatory agents and also to ensure that the binding interactions are stable before and after the docking calculations. The protocol is carried out in accordance with the standards set by the LPPT-UGM Testing Laboratory. The tools, programs, and applications used in this study were Lenovo IdeaPad Flex 5 Processor, AMD Ryzen 5000 Series 5, AutoDock Tools (v1.5.6), Biovia Discovery Studio, AutodockVina, Swiss ADME, VegaZZ, Pubchem, and the and the pkCSM web server (http://biosig.unimelb.edu.au/pkcsm/). Trans-ADFH three-dimensional structure, as a test compound, and Prednisolone, as a standard compound, were downloaded from https://pubchem.ncbi.nlm.nih.gov/. The phytochemical analysis of ADFH includes phenol and saponin. Structure of PlGF 3D (IRV6) and VEGF (4KZN). Identification of ligands and proteins prepared using Pubchem results of molecular docking between preeclampsia through examination of PlGF and VEGF with phenols and saponins Identification of protein ligands using Pubchem, i.e., PlGF (IRV6) with phenol produced 2 interactions and 2 amino acid residues, while saponins produced 2 interactions and 6 residues. pharmacokinetics and toxicity using Swiss ADME, i.e., phenol and saponins are non-mutagenic to bacteria; the maximum safe dose for humans is 0.54 log mg/kg/day; it does not cause toxicity to the heart; the estimated dose for animals is 2.471 mol/kg; it does not cause allergies; it does not cause liver damage; a dose of 0.288 logμ/L can inhibit the growth of 50% of the protozoa T. Pyformis. The content of phenols and saponins in Apis Dorsata forest honey has a high docking score from the original ligand. Molecular docking on phenols and saponins identified PlGF with scores of -2.85, -2.85, and -2.85 kcal/mol, while saponins have +2.84, +2.29, and +2.29 kcal/mol values, which are stated to be better results than the original ligands. This means that phenol has a role as a standard drug that can have an effect on lowering PlGF levels for people with preeclampsia. Molecular docking on VEGF identified phenol with scores of -2.88, -2.88, and -2.88 kcal/mol, and saponins have +1.36, +3.46, and +1.36 kcal/mol values, which are stated to have better results than the original ligands. This means that saponins have a role as a standard drug that can have an effect on increasing VEGF levels for people with preeclampsia.

Abstract

BACKGROUND Stroke is the third leading cause of morbidity and second in mortality worldwide. Diabetes mellitus (DM) is a risk factor for stroke. Vaspin and single nucleotide polymorphism (SNP) rs2236242 involved DM pathogenesis. This study aimed to explore the correlation between SNP rs2236242, serum vaspin levels, and atherosclerosis in patients with ischemic stroke and DM. METHODS This study was conducted in Dr. Moewardi Hospital from 2022 to 2023. The case group included patients with ischemic stroke and DM, while the controls were those with ischemic stroke only. Tetra-primer amplification refractory mutation system-polymerase chain reaction was used to determine the genotypes. RESULTS There were 31 cases and 33 control. SNP rs2236242 showed that odds ratio (OR) (95% confidence interval [CI]) for AA-TT was 0.273 (0.241–0.305) and for TA-TT was 0.315 (0.298–0.341). The OR (95% CI) for allele A to T was 0.789 (0.373−1.669). The mean (standard deviation) serum vaspin level in the case group compared to the control was 1,570 (2,108) ng/ml versus 1,630 (1,428) ng/ml (p = 0.064). Higher vaspin levels were found in T allele of the TT (1,523 [2,269] ng/ml, p = 0.021) and TA (1,760 [1,349] ng/ml, p = 0.004) genotype groups than the A allele of the AA genotype group (0.914 [0.329] ng/ml). CONCLUSIONS Vaspin gene polymorphism AA genotype or A allele significantly reduces vaspin levels in patients with ischemic stroke with DM.

Abstract

Stunting merupakan masalah kesehatan yang menjadi isu prioritas di Indonesia, khususnya di Kabupaten Sukoharjo, dengan prevalensi sebesar 19,8%, melebihi target nasional 14%. Edukasi gizi bagi ibu hamil dan ibu dengan balita menjadi strategi utama dalam pencegahan stunting. Penelitian ini bertujuan untuk mengevaluasi efektivitas program edukasi gizi dalam meningkatkan pemahaman ibu terkait gizi seimbang dan pencegahan stunting dalam 1000 Hari Pertama Kehidupan (HPK). Penelitian ini menggunakan metode pre-eksperimental dengan desain one group pre-test dan post-test yang diolah menggunakan Orange Data Mining dan SPSS 22. Data dikumpulkan dari 37 peserta seminar penyuluhan gizi di Kecamatan Kartasura. Hasil penelitian menunjukkan peningkatan signifikan dalam pemahaman peserta, dengan rata-rata nilai pre-test 58,37 meningkat menjadi 74,05 pada post-test (p<0,05). Hasil ini membuktikan bahwa edukasi gizi yang terstruktur dan interaktif, efektif dalam meningkatkan literasi gizi ibu. Oleh karena itu, diperlukan program edukasi gizi yang berkelanjutan untuk menekan angka stunting dan meningkatkan kualitas kesehatan ibu dan balita

Abstract

Perindustrian di Indonesia belakangan ini semakin berkembang, hal ini mengantarkannya pada persaingan global, sehingga mendorong banyak perusahaan untuk semakin memperbaiki dan meningkatkan usahanya agar lebih efektif dan efisien. Salah satu yang berdampak signifikan pada keefektifan dan keefesianan suatu perusahaan adalah perencanaan fasilitas. Cellular Manufacturing (CM) adalah salah satu metode yang telah terbukti mampu menambah efisiensi serta fleksibilitas dalam lingkungan produksi manufaktur. Diantara faktor-faktor yang diperlukan dalam selular manufaktur adalah Cell Formation (CF), Group Layout (GL), dan Group Scheduling (GS). Biasanya, tiga faktor ini diselesaikan dalam keadaan terpisah atau diselesaikan secara berurutan. Tugas ahir ini membahas tentang metode untuk penyelesaian CF, GL, GS secara bersamaan. A hierarchical genetic algorithm (HGA) digunakan untuk menyelesaikan permasalahan tersebut.

Abstract

Permasalahan penentuan waktu terbang minimum dari sebuah pesawat terbang layang akibat pengaruh gerakan angin vertikal adalah merupakan permasalahan kontrol optimal non linear atau permasalahan optimasi non linier. Sedangkan penentuan waktu tercepat dengan lintasan bebas, adalah merupakan permasalahan penentuan fungsi pengontrol pesawat terbang layang yang dipengaruhi kecepatan angin sehingga dicapai sasaran yang tepat dengan waktu tercepat. Dalam makalah ini dikaji penentuan fungsi pengontrol yang sesuai untuk mendapatkan waktu minimum dengan keadaan awal dan akhir lintasan pesawat adalah tetap. Masalah ini diselesaikan secara numerik dengan menggunakan metode Runge-Kutta Order Empat. Hasil simulasi dilakukan untuk beberapa kecepatan angin yang berbeda, sehingga diperoleh fungsi pengontrol yaitu sudut angkat untuk masing-masing kecepatan angin dengan waktu terbang minimum.
