Abstract

Hip fracture in the elderly is a major health problem. Multi-professional rehabilitation approaches were recommended; however, there were various multi-professional rehabilitation models; therefore, identifying the multi-professional rehabilitation model, especially for older persons after hip fracture surgery, is needed. The aim of this review study is to identify a multi-professional rehabilitation model among the elderly with hip fracture surgery. A literature search for relevant articles in PubMed, Scopus, Embase, and CINAHL was used. Studies that met the following criteria were included: Patients with hip fracture surgery, multi-professional rehabilitation, and randomized controlled trial study, measured physical outcome, published during 2018 to 2024, English full-text article. Finally, six articles met inclusion criteria. The multi-professional rehabilitation model among older persons after hip fracture surgery includes geriatric interdisciplinary home rehabilitation, multicomponent home-based rehabilitation, comprehensive geriatric care, fragility integrated rehabilitation, and comprehensive orthogeriatric care. In addition, hip fracture rehabilitation problems include fear of falling and comorbid and postoperative complications. The multi-professional rehabilitation model improved the quality of life of the older persons after hip fracture surgery. Multi-professionals’ rehabilitation model effectively improved physical performance among older persons after hip fracture surgery. Identifying multi-professional rehabilitation models with specific needs for older persons after hip fracture surgery are recommended in clinical practice.

Abstract

Diabetes distress can significantly impact a person's mental health and motivation to manage the disease. Family support can reduce diabetes distress by providing comfort and understanding. This study aims to determine the relationship between family support and diabetes distress in type-2 diabetes mellitus patients at PKU Muhammadiyah Gamping Hospital, Indonesia.This study adopted a descriptive correlational design with a cross-sectional design. Samples were taken by accidental sampling technique from the population of outpatients at the PKU Muhammadiyah Gamping Hospital Polyclinic, with 107 respondents. The research instrument used was a family support questionnaire and the Diabetes Distress Scale-17. Data analysis was performed using Kendall's Tau correlation test. The results showed that most respondents had good family support (88.8%) and no distress (58.9%). Kendall's Tau correlation test showed a significant relationship between family support and diabetes distress with a P value = 0.014 and a correlation coefficient = 0.229. There is a significant correlation between family support and diabetes distress in patients with type-2 diabetes mellitus at PKU Muhammadiyah Gamping Hospital. Good family support can reduce diabetes distress in patients.

Abstract

Diabetes Mellitus (DM) is a chronic disease caused by the body's inability to use insulin optimally. Insulin therapy is an effective treatment to replace the role of the insulin hormone. Adherence to insulin therapy is crucial for the health of DM patients. This study investigates the correlation between family support and adherence to insulin therapy among type II DM patients at Gamping II Health Center, Yogyakarta. This study employed a quantitative cross-sectional design. Fifty-three type II DM patients who underwent control at Gamping II Health Center participated in this study. The family support instrument was measured using the Hensarling Diabetes Family Support Scale (HDFSS) Indonesian version, while adherence to insulin therapy was measured using the Morisky Insulin Adherence Scale-8 (MIAS). Kendall tau test was used for data analysis. The results showed a significant correlation between family support and adherence to insulin therapy among type II DM patients at Gamping II Health Center (p=0.011). Effective family support, in the form of assistance, attention, or information, is crucial in improving adherence to insulin therapy among type II DM patients. Therefore, family involvement is highly recommended to control blood sugar levels among type II DM patients.

Abstract

The Family Program for Healthy Physically-Mentally and Empowered Elderly (Klasikal-ya) in the management of household organic waste at the Aster Elderly Posyandu, Bantul, Yogyakarta is a Community Service activity aimed at maintaining and improving the physical and mental health of the elderly with physical and social activities in the form of managing household organic/kitchen waste using a composter for leftover kitchen waste (LOSIDA) which produces compost which is then used to grow productive home garden plants, of course adjusted to the functional capacity of the elderly participants. This program is integrated with TiM at the University (Universitas 'Aisyiyah Yogyakarta) which targets the elderly as the main focus by involving the active role of competent students and educators (lecturers). The implementation of activities begins with socialization to introduce the program for healthy physically and mentally and productive elderly; Training, carried out with counseling on healthy activities for the elderly in managing household organic waste using the LOSIDA method. Application of Technology with screening and activities to improve physical and mental health, making and implementing the LOSIDA composter; Mentoring and Evaluation; Monitoring the physical and mental health of the elderly, and utilizing compost from activities for home planting activities as a healthy physical and mental activity for the elderly. Sustainability of the Program; carried out with periodic communication/consultation channels. The benefits of this program are specifically to increase the physical activity of the elderly according to their functional capacity which will have a positive effect on the mental health of the elderly. Increasing community participation, especially the elderly, in dealing with organic waste which can then be used productively through useful home yard plants

Abstract

The increasing number of elderly people in Indonesia brings new challenges to the health sector, especially related to degenerative diseases such as chronic kidney disease (CKD). Elderly people with CKD undergoing hemodialysis face various changes in quality of life in physical, psychological, functional, and social aspects. Although hemodialysis can extend life expectancy, its impact on the quality of life of elderly patients often receives less attention. The purpose of this study was to use quantitative methodology to evaluate the effect of changes in quality of life in elderly people after hemodialysis in Indonesia. The four main dimensions of this analysis are physical, psychological, functional, and social. The method used in this study was a cross-sectional design with quantitative methods. The study sample consisted of 135 elderly patients undergoing hemodialysis at PKU Muhammadiyah Hospital, Yogyakarta. Data was collected using a structured questionnaire that measured quality of life based on four aspects. Data analysis was carried out using the regression method with the SmartPLS approach to assess each aspect's effect on patients' quality of life. The results showed that all aspects studied significantly affected the quality of life of elderly people after hemodialysis. Social aspects had the most significant influence (p = 0.004), followed by psychological (p = 0.048), functional (p = 0.038), and physical (p = 0.030) aspects. Elderly people undergoing hemodialysis experience limitations in physical activity, emotional stress, decreased body function, and social isolation that affect their overall well-being.

Abstract

Latar belakang: Pasien lanjut usia (lansia) paska operasi patah tulang pinggul atau Total Hip Arthroplasty (THA) memiliki permasalahan yang menyebabkan proses penyembuhannya menjadi terganggu, seperti dislokasi pinggul yang pada umumnya terjadi dalam kurun tiga bulan pertama setelah operasi THA. Tujuan: untuk mengetahui kualitas hidup lansia yang mengalami dislokasi paska operasi patah tulang pinggul. Metode: scoping review menggunakan Preferred Reportung Items for Systematic review and Meta-Analyses (PRISMA Flowchart), dengan kriteria inclusi (1) Artikel dari tahun 2017-2024, (2) metode Randomize control trial (RCT), (3) Artikel tentang dislokasi dan kualitas hidup lansia operasi patah tulang pinggul. Hasil: Pencarian literatur telah mengidentifikasi 1534 artikel, 198 duplikat artikel yang dikeluarkan dan didapatkan 10 artikel RCT yang memenuhi kriteria untuk direview. Ditemukan dua tema utama yaitu; (1) Risiko dislokasi setelah operasi patah tulang pinggul Lansia; a) penyebab terjadinya dislokasi patah tulang pinggul setelah operasi; b) dislokasi berulang; 3) kualitas hidup pasien pasca operasi patah tulang pinggul. (2) Faktor yang terkait dengan pemulihan fungsional dislokasi patah tulang pinggul lansia; a) meningkatkan mobilitas; b) Terapi hemiarthoplasty; c) Pendekatan anterior mengurangi dislokasi dibandingkan pendekatan posterior. Kesimpulan: Dislokasi paska operasi patah tulang pinggul menyebabkan perburukan qualitas hidup lansia. Program pencegahan dislokasi paska operasi patah tulang pinggul menjadi hal yang penting dalam proses rehabilitasi pasien.

Abstract

Gagal ginjal kronis (GGK) merupakan kondisi kerusakan ginjal struktural dan fungsional yang berlangsung lebih dari 3 bulan, menyebabkan penurunan fungsi ginjal secara progresif. Hemodialisis merupakan prosedur penting bagi pasien gagal ginjal kronis. Salah satu komplikasi GGK yang signifikan adalah anemia, yang dapat mempengaruhi kualitas hidup pasien secara langsung. Penelitian ini bertujuan untuk mengetahui hubungan antara anemia dan kualitas hidup pada pasien penyakit ginjal kronis yang menjalani hemodialisis di RS PKU Muhammadiyah Bantul. Penelitian ini menggunakan desain observasional dengan pendekatan cross-sectional. Sampel penelitian terdiri dari 68 pasien yang dipilih menggunakan teknik purposive sampling. Kualitas hidup diukur menggunakan kuesioner WHO-QoL Bref, sedangkan anemia ditentukan berdasarkan kadar hemoglobin yang diperoleh dari rekam medis pasien. Analisis data dilakukan menggunakan uji Spearman Rho untuk mengetahui hubungan antara variabel yang diteliti. Penelitian menunjukkan bahwa mayoritas responden memiliki kualitas hidup baik (100%, n=68). Tingkat anemia pasien gagal ginjal kronis sebagian besar berada pada kategori anemia sedang (47,1%, n=32), kategori anemia ringan (44,1%, n=30) dan 8,8% (n=6) memiliki anemia berat. Hasil analisis menunjukkan terdapat hubungan signifikan antara anemia dan kualitas hidup pasien GGK yang menjalani hemodialisis (p=0,001). Status anemia dapat menyebabkan penurunan kualitas hidup pasien gagal ginjal kronis. Oleh karena itu, kolaborasi diperlukan untuk memastikan asupan makanan yang tepat dan berpotensi meningkatkan kadar hemoglobin. Perencanaan aktivitas yang disesuaikan dengan kapasitas fungsional pasien terkait dengan status anemia juga sangat direkomendasikan dalam asuhan keperawatan untuk mempertahankan kualitas hidup pasien.

Abstract

Chronic kidney disease is a significant public health problem worldwide. Hemodialysis is the most common form of kidney replacement therapy in the world. Hemodialysis therapy is associated with deterioration in health-related quality of life. However, limited studies focused on health-related quality of life subscales among older persons with hemodialysis in the Indonesian population, especially with long-term follow-up. This study aims to explore the six-month longitudinal changes in health-related quality of life subscales among older persons with hemodialysis. This prospective cohort study evaluated 119 older adults undergoing hemodialysis using purposive sampling between March and September 2024 at PKU Yogyakarta Hospital, Indonesia. Outcomes status was evaluated with health-related quality of life at 1-, 3-, and 6-months. The Indonesian version of SF-36 assessed health-related quality of life. Data were analyzed using general estimating equations. In general, the health-related quality of life among older persons with hemodialysis was decreased. The longitudinal changes of eight subscales significantly deteriorated from the first month to the third month and remained consistently worse during the third to the sixth months (p=.001, respectively). Most deterioration was found in role limitation due to physical problem (β=-94.4; p=.001), general health (β=-70.5; p=.001), and vitality (β=-66.9; p=.001). Social function showed the better domain during the 6 months (β=-36.6; p=.001) than other health-related quality of life subscales. Different aspects of health-related quality of life deteriorate differently among older persons with hemodialysis. Specific attention needs to be paid to high-risk Indonesians among older persons with hemodialysis health-related quality of life, especially those who are older.

Abstract

Sectio caesarea merupakan tindakan bedah yang berperan penting dalam menyelamatkan nyawa ibu dan bayi jika dilakukan berdasarkan alasan medis yang tepat. Kebijakan terkait penerapan Enhanced Recovery After Cesarean Surgery (ERACS) untuk meningkatkan kualitas pelayanan kesehatan ibu dan anak. Bertujuan untuk mengeksplorasi lebih lanjut pengaruh metode ERACS dan Konvensional terhadap keberhasilan IMD dan mobilisasi dini. Panduan dalam Scoping Review menggunakan pernyataan PRISMA 2020. Menggunakan framework model PICO. Kriteria inklusi yaitu artikel/ sumber informasi terbit tahun 2020- 2024,kriteria eksklusi adalah laporan penelitian dalam bentuk monograf, artikel review/case report. Menggunakan 5 database yaitu Pubmed, Science Direct, Wiley Online Library, Google Scholar dan Research Rabbit. Bahwa   ada   pengaruh   SC   metode   ERACS terhadap   keberhasilan IMD dengan nilai p-value (sig) 0,000 < 0,05 dengan hasil observasi  memiliki tingkat nyeri yang rendah serta mobilisasi dini lebih cepat. Metode ERACS efektif dalam mempercepat pemulihan pascaoperasi, termasuk pengurangan lama rawat inap dan percepatan mobilisasi dinii dan meningkatkan keberhasilan IMD serta menyusui. Lakukan penelitian lebih lanjut untuk mengevaluasi implementasi dan dampak jangka panjang ERACS diberbagai konteks.
