Abstract

Patients with chronic kidney disease undergoing hemodialysis frequently experience various psychological problems, including anxiety, depression, and decreased quality of life. In recent years, spiritual well-being has received increasing attention as a potential factor that may help patients adapt to the burden of chronic illness. This study aimed to review and synthesize scientific evidence regarding the relationship between spiritual well-being, anxiety, depression, and quality of life among hemodialysis patients. This study employed a Literature Review approach following the PRISMA guidelines. Articles published between 2020 and 2025 were retrieved from PubMed, Scopus, ScienceDirect, and Google Scholar databases. The selected studies were screened based on predetermined inclusion and exclusion criteria. A total of ten quantitative studies met the eligibility criteria and were included in the review. The findings indicated that spiritual well-being was consistently associated with better psychological outcomes among hemodialysis patients. Higher levels of spirituality were linked to lower levels of anxiety and depression, improved quality of life, and more adaptive coping abilities. In addition, several studies reported that spirituality contributed to better treatment adherence and greater life satisfaction. However, variations in findings were observed due to differences in cultural contexts, participant characteristics, and measurement instruments. In conclusion, spiritual well-being is an important factor associated with the mental health and quality of life of hemodialysis patients. The integration of spiritual aspects into nursing care may support a more holistic approach to patient management. Further studies with more rigorous research designs are needed to clarify the underlying mechanisms of this relationship.

Abstract

Background: Cultural aspects play a crucial role in providing nursing care, particularly for patients and families with terminal or chronic illnesses undergoing palliative care. Understanding patients' and families' cultural aspects is essential to provide empathetic, effective, and culturally respectful palliative care. Method: This qualitative phenomenological study involved in-depth interviews with 10 participants (5 patients and 5 family members) selected through purposive sampling. Data were analyzed using Colaizzi's method. Result: Five main themes emerged reflecting the cultural perceptions of palliative care among Javanese patients and families: (1) Hope for recovery and a better life-serving as a spiritual and emotional anchor held by patients and families; (2) Family roles and responsibilities-encompassing caregiving duties, emotional support, and efforts to maintain family harmony and dignity; (3) Anxiety about the future-including concerns about dependency, financial burden, and the impact of loss, felt by both patients and families; (4) Life as a test and trial from God-a strong spiritual belief that illness is part of destiny, collectively accepted; (5) Surrender and acceptance (nrimo)-a characteristic Javanese attitude practiced by patients and families as a coping mechanism to find peace amidst uncertainty. Conclusion: Nrimo, sincere acceptance of illness as divine will, functions as an active, value-based coping mechanism that enhances resilience and spiritual well-being, differing from Western concepts of passive resignation. Healthcare professionals must validate nrimo as adaptive coping, not fatalism. Family-centered, indirect communication with early engagement of decision-makers is recommended. Spiritual care integration through religious leaders is essential for meaning-making.

Abstract

Cancer transcends physical pathology, manifesting as a chronic condition that imposes profound psychological, social, and spiritual burdens, often compromising the patient's quality of life (QoL). Recognizing that disease progression is a potential predictor of this decline, this study aims to analyze the correlation between cancer staging and quality of life. Conducted at PKU Muhammadiyah Gamping Hospital using an observational cross-sectional design, the study involved 75 cancer patients recruited via purposive sampling based on Slovin’s formula. Cancer staging data were validated through medical records and interviews, while QoL was assessed using the EORTC QLQ-C30 instrument. Data analysis using the Spearman Rank correlation test revealed that the majority of respondents presented with advanced-stage cancer and reported moderate-to-low QoL. A significant correlation was found (p-value 0,001), indicating an inverse relationship where advanced cancer stages are associated with lower quality of life. These findings underscore the urgency of early detection and comprehensive symptom management, including the optimization of palliative care. Consequently, this study recommends the integration of routine QoL assessments into clinical practice and the reinforcement of the nurse's role in the holistic management of cancer patients.

Abstract

Background: Patients with chronic kidney disease receiving hemodialysis often experience anxiety, irrespective of their treatment duration, due to the chronic nature of the condition. While anxiety is known to impact blood pressure, the correlation between anxiety levels and blood pressure in patients awaiting hemodialysis remains unclear and requires further study. Objective: The objective of this study is to determine the association between anxiety levels and blood pressure in patients awaiting hemodialysis. Methods: A quantitative cross-sectional study was conducted among 152 patients with chronic kidney disease undergoing hemodialysis, recruited through total sampling at the Hemodialysis Unit between April and May 2025. The Hamilton Anxiety Rating Scale (HARS) was used to evaluate anxiety levels, and blood pressure and Mean Arterial Pressure (MAP) were recorded before hemodialysis. The relationship between variables was analyzed using the Spearman rank test. Results: The characteristics of respondents were dominated by males (57.9%, n=88), aged late elderly (32.9%, n=50), with secondary education (73.7%, n=112), not working (61.8%, n=94), and had undergone hemodialysis therapy for more than 1-5 years (57.9%, n=88). The majority of respondents did not experience anxiety (78.3%, n=119). In addition, the majority of respondents had high blood pressure (57.2%, n=87). No significant relationship between anxiety levels and blood pressure in patients before hemodialysis was found (p=0.202). Conclusion: The findings indicate that patients undergoing hemodialysis for 1 to 5 years typically exhibit low anxiety levels. However, blood pressure remains elevated in this population, likely due to the underlying chronic kidney disease pathophysiology. Regular hemodynamic monitoring by nursing staff is crucial, even in patients with low anxiety. Additional research is needed to identify other factors related to blood pressure management in chronic kidney disease patients.

Abstract

Background: Death anxiety is a common psychological and existential concern among patients with chronic, terminal, and palliative illnesses, adversely affecting emotional well-being, quality of life, and coping ability. Spiritual care has been increasingly recognized as an essential component of holistic palliative care; however, evidence regarding its effectiveness in reducing death anxiety remains fragmented. Objective: This integrative literature review aimed to synthesize current evidence on the effectiveness of spiritual care in reducing death anxiety among patients receiving palliative care. Methods: An integrative literature review was conducted following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA 2020) guidelines. Six electronic databases (PubMed, ScienceDirect, SAGE Journals, ProQuest, SpringerLink, and Wiley Online Library) were searched for English-language studies published between 2021 and 2026. Eligible studies included randomized controlled trials, quasi-experimental studies, and analytical cross-sectional studies involving adults with chronic, terminal, or palliative illnesses. Methodological quality was assessed using the Joanna Briggs Institute (JBI) Critical Appraisal Tools, and findings were synthesized narratively using thematic analysis. Results: Eight studies met the eligibility criteria, comprising two randomized controlled trials, one quasi-experimental study, and five analytical cross-sectional studies. Three overarching themes emerged: (1) spirituality as a protective factor against death anxiety, (2) the effectiveness of spirituality-based interventions in reducing death anxiety, and (3) mechanisms through which spiritual care enhances psychological adaptation. Higher levels of spiritual well-being, meaning in life, and fulfillment of spiritual care needs were consistently associated with lower death anxiety. Structured interventions, including spiritual counseling, nurse-led spiritual care, and spirituality-oriented palliative care, significantly reduced death anxiety while improving resilience, psychological well-being, and quality of life. Conclusion: Spiritual care is an effective non-pharmacological intervention for reducing death anxiety and supporting psychological adaptation among patients receiving palliative care. Integrating evidence-based spiritual care into routine nursing practice may enhance holistic, person-centered care. Further multicenter randomized controlled trials using standardized interventions are needed to strengthen the evidence base

Abstract

Patients with end-stage chronic kidney disease undergoing hemodialysis experience complex lifestyle changes, including fluid and dietary restrictions, dependence on therapy, fatigue, changes in social roles, and psychological distress that affect psychosocial adaptation and quality of life. This literature review aimed to examine the factors influencing psychosocial adaptation among hemodialysis patients and their implications for nursing practice. The review was conducted using PubMed, Scopus, ScienceDirect, ProQuest, MDPI, and BMC databases with keywords related to hemodialysis and psychosocial adaptation. Inclusion criteria were full-text articles published between 2020–2025 in English or Indonesian that discussed psychosocial aspects of hemodialysis patients. Of 326 identified articles, 11 studies were analyzed using a narrative-thematic approach. The findings identified five major themes: illness acceptance, depression and anxiety, social support, burden of physical symptoms, and coping strategies and cultural values. Illness acceptance and social support positively contributed to psychosocial adaptation, whereas depression, anxiety, and physical symptom burden negatively affected patients’ quality of life. Overall, psychosocial adaptation in hemodialysis patients is influenced by psychological, social, physical, and cultural factors. Therefore, nurses should perform routine psychological screening, provide self-management education, strengthen family support, and facilitate adaptive coping strategies to improve patient outcomes.Top of Form
