Abstract

National Health Insurance program changed the pattern of health financing in Indonesia from fee to service system to capitation system. The capitation system is expected to reduce the risk of the community's own health costs and overcome inequalities of health services. The purpose of this literature review is to analyze the implementation of capitation payment system in overcoming health services problems. Although the implementation of NHI in Indonesia helps to equalize health services, there are quite number of problems in implementing NHI with the capitation system, among others, not all of the population is covered as participants, inequality of health facilities, varied quality of health services, health human resources, and management of capitation funds, monitoring, non-optimal referral systems, and varied geographic conditions had potential for widening health inequalities between community groups. Public health services should not only focus on treating individuals who are sick, but can do more important things such as making efforts to prevent and improve health. The capitation payment system is the most effective system for solving health services problems even though its implementation still not optimal and there are several shortcomings that can be fixed.

Abstract

Background: Visually impaired children have a low vision to visualize the process of plaque removal on teeth resulting in dental caries progression. Dental caries has an impact on children’s quality of life. The impact of dental caries on oral health related quality of life among visually impaired children remains mostly unknown. This study aimed to determine the correlation between dental caries and oral health related quality of life (OHRQoL) among visually impaired children.Method: This study was an observational study with a cross sectional design and conducted across four regencies in Yogyakarta and Central Java Province. A total of 70 visually impaired children aged 7-18 participated in this study. The participants underwent a dental examination to determine their caries experience using DMFT/dmft index. Oral health related quality of life was measured using the COHIP SF 19 questionnaire with interviewing method.Result: Data analysis showed that caries frequency was 94,3%. The mean number of DMFT/dmft index was 4,8 ± 2,743. Spearman correlation test showed the p value was 0,611 (p>0,05) for the correlation between DMFT/dmft index and OHRQoL.Conclusion: This study showed no correlation between dental caries and oral health related quality of life among visually impaired children.

Abstract

Background: Dental caries is an infectious microbial disease that begins as demineralization of an inorganic portion of tooth, followed by the destruction of organic parts, leading to cavity formation. Fluoride helps to prevent caries which can be administered systematically or topically. Fluoride mouth rinse has been used in school program to prevent caries.Method: This article aim is to describe the effectiveness of school-based fluoride mouth rinse (S-FMR) as one of the effective methods to prevent caries on children.Discussion: The School-based Fluor Mouth Rinse (S-FMR) program or gargling using fluoride in schools is the use of fluoride as a mouth rinse for children aged 6-18 years conducted in the school environment. Several studies have shown that S-FMR can reduce the prevalence of caries.Conclusion: S-FMR is considered effective in preventing caries due to the simple method, cost-effectiveness, time efficiency, and easily undertaken by the teacher’s supervision

Abstract

The results of Riskesdas (Baseline Health Research) by the Indonesian Ministry of Health in 2018 revealed that the caries rate in rural areas was higher than in urban areas. However, the number of villagers receiving dental and oral health care was lower than in urban communities. Karangtengah Village was located in the Baturraden District, Banyumas Regency, which is socio-demographically rural. This study aims to determine the correlation between knowledge and affective of oral health on the caries rate in Karangtengah Village. The study was observational analytical research. Subjects were selected using a purposive sampling technique. The total subjects were 388 residents. The knowledge and affective data were collected using a questionnaire, and the caries rate was scored using DMF-T. Data were analyzed using the Pearson correlation test (SPSS 22). The results revealed that the oral health knowledge and affective of the majority of Karangtengah Village residents were in the moderate category (48.7% and 66.8%, respectively), while the average DMF-T score was 13.12 (very high). The statistical analysis demonstrated no significant correlation between knowledge and DMF-T score and between the affective aspect of oral health and DMF-T score (p0.5).  The moderate category of oral health knowledge and affective aspect in rural areas may not have been applied as oral health behaviors contribute to high dental caries rates.

Abstract

Village Community Dental Health Efforts (UKGMD) is one of the breakthrough efforts or strategies to assist the community in reaching dental health services that have not been implemented yet and encourage the community to provide dental health services at the nearest health service. Dental and oral health survey data for the people of Karangtengah Village, Baturraden District in 2019 show an average caries with a DMF-T index of 13.12 which is classified as a very high category according to WHO. The cadre training activities were attended by 28 representative cadres from 7 posyandu in Karangtengah Village. Training activities include providing material through audio-visual forms and practice accompanied by the team. The assessment of this activity is carried out through the results of the pretest at the beginning of the activity session and the postest assessment at the end. The results of the pretest and postest showed an increase in the average knowledge score of cadres from 7.92 ± 1.56 to 8.85 ± 0.57. The paired t-test statistical test showed that there was a significant difference between the level of knowledge of cadres before and after receiving the training materials (p<0.05). The results showed that cadre training effectively increased UKGMD cadres' knowledge about oral health and simple dental examination methods.

Abstract

Patients with thalassemia generally have high caries rates. Dental caries in children can have a negative impact on daily activities and quality of life. Researchs on the comparison of caries and oral health related quality of life between thalassemia children and healthy children provides inconsistent results. The aim of this study was to compare dental caries and oral health-related quality of life between children with thalassemia major and healthy children. This study was a cross sectional study involving 17 children with thalassemia mayor and 22 healthy children who were selected by purposive sampling in Banyumas, Central Java. Caries was assessed using the DMFT/deft index through digital panoramic radiographs and clinical photographs of the oral cavity. Oral health-related quality of life was assessed using the Indonesian version of the Child Oral Health Impact Profile-SF 19 questionnaire. DMFT /deft scores in thalassemia major children and healthy children were 2.64 ± 2.06 and 2.23 ± 1.38, respectively. OHRQoL scores in thalassemia major and healthy children were 56.82 ± 10.35 and 56.73 ± 7.70, respectively. There was no significant difference between DMFT/deft scores and oral health-related quality of life scores between thalassemia major children and healthy children (p=0.663 and p=0.974, respectively). Nevertheless, oral health in children with thalassemia should be improved.

Abstract

Background: Oral health literacy (OHL) has an important role as a medium for health promotion and efforts to prevent oral diseases through oral health behavior changes. Purpose: This study aims to determine OHL and its correlation with oral health knowledge, attitude, and oral health practice among college students. Methods: This cross-sectional study was conducted on 583 students of Jenderal Soedirman University in 2020, as the students come from different provinces in Indonesia. The Indonesian Oral Health Literacy Questionnaire was used to assess OHL. A self-designed questionnaire was used to assess oral health knowledge, attitude, and oral health practice. Results: Most students had good OHL, knowledge, and attitude toward oral health (90.7%, 84.6%, and 90.5% respectively). A Pearson correlation test showed a correlation between OHL and oral health knowledge (p = 0.044) and attitude (p = < 0.001). The Fisher exact tests showed a correlation between OHL and snacking frequency, tooth-brushing frequency, tooth-brushing time, and toothbrush changing time. The better the OHL, the better the knowledge and attitude. There was no correlation between OHL and sweet food eating frequency, the use of cleansing aids, dentist visit frequency, and the first action taken when experiencing toothache. Conclusion: OHL has a correlation with oral health knowledge and attitude. However, OHL is only related to some oral health practice indicators. The results of this study are expected to be taken into consideration when formulating strategies to improve oral health within the university.

Abstract

Background: dental fluorosis is a condition of tooth growth and development that causes spots and brittle teeth. The cause of dental fluorosis is chronic fluoride intoxication due to consumption of fluoridated water during the tooth development period. One type of water with a high fluoride content is groundwater in karst areas. Groundwater in Darmakradenan village containing 1,9 mg/L fluoride. The vast majority of the locals consume groundwater as drinking water. Aim: to determine the association between groundwater consumption and the incidence of dental fluorosis. Methods: an analytic-observational with a cross-sectional approach. The research population consisted of residents aged 8–15 years in Darmakradenan Village, Ajibarang District, and Banyumas Regency. Purposive sampling was implemented for selecting research participants, with a total of 90 participants who divided into two groups: consumed groundwater and refilled drinking water. The Dean's Index is used to examine the status of dental fluorosis. Analysis data used Chi-square test. Result: there was an association between groundwater consumption and dental fluorosis (p < 0,05, CC = 0,277). Conclusion: there is an association between groundwater consumption in karst areas and the incidence of dental fluorosis.

Abstract

Siswa tunarungu memiliki keterbatasan fisik dalam mendengar dan umumnya diikuti oleh gangguan bicara.. Keterbatasan ini menjadi hambatan bagi penyandang tunarungu untuk memperoleh pengetahuan tentang kesehatan gigi dan mulut yang nantinya akan menentukan sikap dan tindakan anak dalam menjaga kebersihan rongga mulutnya . SLB B Yakut Purwokerto merupakan sekolah untuk siswa tunarungu wicara yang terletak di daerah Kranji, Banyumas, Jawa Tengah. Kegiatan ini bertujuan dalam untuk meningkatkan kemampuan pemeliharaan kesehatan gigi dan mulut pada siswa tunarungu di SLB B Yakut Purwokerto. Khalayak sasaran dalam kegiatan ini adalah siswa tunarungu SLB B Yakut Purwokerto berjumlah 74 siswa dari usia TK hingga SMA. Metode kegiatan ini berupa pemberian edukasi melalui demonstrasi sikat gigi dengan phantom gigi didampingi guru dan pemeriksaan gigi dan mulut. Pemeriksaan rongga mulut menunjukkan angka karies yang cukup tinggi. Hasil pemeriksaan rongga mulut akan diserahkan kepada pihak sekolah dan orang tua agar dapat ditindaklanjuti.

Abstract

Odontektomi merupakan prosedur untuk mengangkat gigi terpendam dengan pembedahan dan membutuhkan beberapa instrumen bedah tambahan. Prosedur odontektomi yang kompleks ini dapat menimbulkan masalah psikologis pada pasien yaitu munculnya kecemasan dental. Kecemasan dental dapat disebabkan oleh sejumlah faktor yaitu psikososial, perilaku, genetik dan sosiodemografi. Faktor sosiodemografi dapat terdiri dari usia, jenis kelamin, dan pendidikan. Tujuan penelitian ini untuk mengetahui hubungan faktor sosiodemografi dengan tingkat kecemasan dental pasien sebelum tindakan odontektomi di RSUD Prof. Dr. Margono Soekarjo Purwokerto. Metode penelitian ini adalah observasional analitik dengan pendekatan cross-sectional. Populasi dalam penelitian ini adalah pasien yang akan dilakukan tindakan odontektomi di RSUD Prof. Dr. Margono Soekarjo Purwokerto. Sampel diambil menggunakan teknik accidental sampling dengan jumlah sebanyak 75 responden pasien yang akan dilakukan tindakan odontektomi di RSUD Prof. Dr. Margono Soekarjo Purwokerto. Analisis data dalam penelitian ini menggunakan Uji Chi-square dan Uji Regresi Logistik Multinomial. Hasil penelitian didapatkan variabel usia mempunyai p (0,000) dan contingency coefficient cukup kuat (0,473), jenis kelamin mempunyai p (0,000) dan contingency coefficient kuat (0,673), dan pendidikan mempunyai p (0,041) dan contingency coefficient lemah (0,342). Simpulan penelitian ini adalah terdapat hubungan antara faktor sosiodemografi meliputi usia, jenis kelamin, dan pendidikan dengan tingkat kecemasan dental pasien sebelum tindakan odontektomi di RSUD Prof. Dr. Margono Soekarjo Purwokerto serta variabel yang paling berpengaruh dengan tingkat kecemasan dental pasien sebelum tindakan odontektomi di RSUD Prof. Dr. Margono Soekarjo Purwokerto yaitu jenis kelamin.
