Abstract

Sistem rujukan yang terdapat pada puskesmas menggunakan sebuah sistem informasi yang biasa dinamakan SIMPUS. Sistem Informasi Manajemen Puskesmas merupakan suatu tatanan atau peralatan yang menyediakan informasi untuk membantu proses manajemen puskesmas dalam mencapai sasaran kegiatannya (Depkes RI, 1997). Sistem informasi di Puskesmas banyak dikembangkan untuk mengatasi permasalahan sepertinya pengimputan data Rekam Medis sebagai sumber data primer yang digunakan untuk mengolah data asuhan medis menjadi statistik kesehatan. Oleh karena itu seorang perekam medis dituntut untuk bisa mengelola data yang ada sehingga menghasilkan sebuah informasi. Sistem informasi merupakan sarana untuk menyediakan informasi yang berguna dalam pengambilan keputusan organisasi dan menambah pengetahuan sehingga dapat mengurangi ketidak pastian bagi para pemakai informasi (Deni Maisa Putra & Dila Vadriasmi, 2020). Tahap PKM diawali dengan penjajakan dan studi pendahuluan dalam rangka pengumpulan data, permohonan izin lahan, dan kesepakatan terkait MOU yang akan dibahas. Persiapan kegiatan PKM dalam bentuk pembekalan ilmu pengetahuan, praktek terhadap penerapan SIMPUS yang ada di Puskesmas setelah mendapatkan izin dari Dinas Kesehatan Kota Padang, pelaksanaan PKM dilaksanakan selama 3 hari (15, 16, 17 Agustus 2019). Kegiatan PKM ini diharapkan dapat menghasilkan peningkatan pengetahuan dan bisa memberikan kontribusi terhadap petugas rekam medis untuk melih meningkatkan skill dalam penerapan SIMPUS.Kata kunci : SIMPUS, Rekam Medis, Puskesmas

Abstract

ABSTRAKPendahuluan: Kebutuhan masyarakat sekolah tentang media edukasi kesehatan  dalam meningkatkan personal hygiene anak masih belum banyak dikembangkan. Pendidikan kesehatan dengan menggunakan media edukasi dapat meningkatkan kemampuan kritis anak. Personal hygiene atau kebersihan diri perorangan perlu diaplikasikan pada diri pribadi serta keluarga agar terhindar dari penyakit. Tujuan penelitian ini adalah untuk menggali ide-ide atau persepsi masyarakat sekolah tentang kebutuhan untuk meningkatkan Personal hygiene pada anak di SD Sukagalih. Metode: Penelitian ini menggunakan desain kualitatif eksploratif untuk menggali ide-ide secara mendalam dari partisipan. Pemilihan partisipan menggunakan teknik purposive sample, berdasarkan kriteria inklusi. Sebanyak sembilan belas orang partisipan yang bersedia terlibat dalam penelitian ini. Pengumpulan data menggunakan teknik wawancara mendalam pada orangtua, guru, masyarakat, dan tenaga kesehatan, sedangkan pada anak-anak dilakukan focus group discussion (FGD). Waktu penelitian pada bulan September sampai dengan bulan November 2017, analisis data menggunakan teori Braun and Clarke (2006). Hasil: Penelitian didapatkan empat tema yaitu; kebutuhan akan media, kriteria media yang baik, jenis media yang dibutuhkan, dan penyitaan waktu dalam kebiasaan bermain. Diskusi: Bahwa masyarakat sekolah membutuhkan media dan metode untuk meningkatkan personal hygiene pada anak. Diperlunya kerjasama masyarakat sekolah untuk meningkatkan personal hygiene anak dan akan kebutuhan media edukasi yang dapat membantu pengetahuan, motivasi anak, dan  mendidik anak kearah yang lebih baik dalam kesehatan personal hygiene. ABSTRACTIntroduction: The needs of the school community about health education media in improving child hygiene are still not widely developed. Health education by using educational media can improve children's critical ability. Personal hygiene or personal hygiene needs to be applied to personal and family to avoid disease. The purpose of this study was to explore the ideas or perceptions of the school community about the need to improve personal hygiene in children in SD Sukagalih. Method: This study was conducted using a qualitative explorative design to explore the ideas in depth from the participants. The selection of participants was used purposive sample technique, based on the inclusion criteria. Nineteen participants were agreed to join in the study. Data collection used in-depth interview techniques for parents, teachers, communities, and health workers, while in children focus group discussions (FGDs) were conducted. During the study period from September to November 2017, data analysis used Braun and Clarke's theory (2006). Result: The research found four themes; the need for media, good media criteria, the type of media required, and the seizure of time in play habits. Discussion: That the school community needs media and methods to improve personal hygiene in children. There is a need for school community co-operation to improve the child's personal hygiene and the need for educational media that can help knowledge, motivate children, and educate children towards better health of personal hygiene.

Abstract

The medical record is also a documentation of the patient's condition and the contents of the medical record are medical secrets, especially in the storage of status in the filling room, which must be kept confidential, secure, and the medical record layout, whether it is appropriate or not by every health worker. The purpose of this study was to determine the Knowledge Review and Attitudes of Officers and Patients on the Confidentiality of Medical Record Files Primary Health Center at Kuranji Padang. This type of research is descriptive with a quantitative approach, the research sample is 90 patients and 4 health center officers using accidental sampling technique with data collection using questionnaires for patients and table cklis for officers. The results showed that there are still officers whose knowledge and attitude in maintaining the confidentiality of medical records (50%) or 2 people are still lacking and more than half of the knowledge (51.1%) and attitudes (52.2%) of patients towards maintaining the confidentiality of record files medical who do not understand and know. It can be concluded that Kuranji Primary Health Center, both patients and officers, is still lacking in maintaining the confidentiality of medical records. It is hoped that the head of the Primary Health Center will prepare Standard Operating Procedures (SOP) on medical records so that officers and patients know about medical records and about the confidentiality of medical records. In implementing the confidentiality of medical records, competent and professional medical record officers should know and understand more about medical records, participate in training and provide information to patients so that the confidentiality of medical records is maintained and can create good service.

Background: Accurate coding using appropriate diagnosis writing with appropriate medical terminology. And help the coding officer to code the disease according to ICD-10. This study aims to determine the relationship between medical terminology and the accuracy of coding based on ICD-10 in hospitals. Methods: The method used is a literature study carried out by describing the facts that exist and then being analyzed. Searching for articles through Google Scholar, with inclusion and exclusion criteria, the final results obtained 5 analyzed journals. Furthermore, the analysis is carried out by looking at the compare, contrast, critize, synthesize, summarize. Result: The results of this literature review show that from the 5 journals that have been analyzed there are 3 journals that have medical terminology accuracy with a percentage of more than 50%, 4 journals that have coding accuracy with a percentage of more than 50%, and 4 journals that have a relationship between terminology accuracy. Medical with coding accuracy. Conclusion: Based on the results of the literature review, it can be concluded that there is a need for uniformity and consistency in the use of medical terminology according to ICD-10 to further improve the accuracy of the code. So there should be communication between the coder and the doctor so that the perception between the two is the same and produces an accurate code.

Abstract

During the Covid-19 pandemic, Lecturers were able to improve their ability to make learning media because the lecture process was carried out boldly. To provide debriefing to lecturers who need training for making learning media with software. Understanding of information, in this case, making learning videos with bandicam and filmora applications. Mastery and understanding of practical programming languages, which can be applied directly, the results can be seen, are soft skills that must be taught by lecturers to teach courses during the Covid-19 pandemic. At the moment. This activity was attended by STIKES Dharma Landbouw Padang Lecturers, on Saturday, January 9, 2021, by explained the material and practice of making learning media.

Abstract

Keterlambatan pengembalian rekam medis dapat menghambat kegiatan pengelolaan dan beresiko hilangnya rekam medis. Salah satu faktor penyebab keterlambatan pengembalian berkas rekam medis diantaranya terlambat karena berkas yang belum lengkap. Sehingga diperlukan motivasi perawat dengan ketepatan waktu pengembalian rekam medis rawat inap di rumah sakit. Penelitian ini menggunakan pendekatan deskriptif analitik korelasi dengan desain penelitian cross sectional. Populasi pada penelitian ini berjumlah 46 perawat dengan sampel 32 perawat menggunakan proportional sampling dengan analisis univariat dan analisis bivariate. Berdasarkan hasil bahwa perawat yang kurang baik tanggung jawabnya terbukti 86% tidak tepat waktu, perawat dengan pengembangan dirinya terbukti 71% tidak tepat waktu, sedangkan perawat yang kurang mandiri dalam bertindak terbukti 79% tidak tepat waktu. Tanggung jawab dan pengembangan diri perawat memiliki hubungan dengan ketepatan waktu pengembalian rekam medis rawat inap di Rumah Sakit Islam Ibnu Sina Padang, sebaiknya perawat terus melakukan pengembangan diri guna meningkatkan keterampilan dan memperbaiki efektifiktas kerja untuk mencapai hasil kerja yang diharapkan seperti selalu mengikuti sosialisasi, mengikuti pelatihan dan bebas mengeluarkan pendapat saat berdiskusi.

Abstract

The development of information technology today affects various aspects of life and profession, one of which is in the field of health services, especially in patient medical records. Health data and information contained in medical records are very useful for the management of health care institutions to assess whether their services are of high quality, efficient and effective. The speed in providing medical record files to the poly is one of the indicators of service quality in medical records, both in and out of medical records from Filing. The purpose of this community service activity is to socialize the use of Trecer using Microsoft Access in managing medical records Air Dingin Health Center, Padang City in 2019. Good medical record management is very helpful in carrying out activities so that they are not misplaced (missfile) and are not lost when the file is removed from the filing. The method used in this activity is training and socialization in the use of simple theoretical and practical applications. Abstract

ABSTRACT. The completeness of the medical resume and the accuracy of the diagnosis code will affect the information generated due to the inaccuracy of the data presented so that it has an impact on the quality of information and the accuracy of the code in accordance with ICD-10. In RSI Ibnu Sina Padang, incomplete and inaccurate medical resumes are still found. This study aims to determine the review of the completeness of the medical resume and the accuracy of the diagnostic code of the surgical care unit for BPJS patients on filing BPJS claims for inpatient surgical care units. This type of research is descriptive, to see a description of the completeness of the medical resume and the accuracy of the diagnosis code of the surgical care unit for BPJS patients for submitting claims for inpatient services. The study was carried out in the case mix surgical treatment unit at Ibnu Sina Hospital. Data collection method using univariate analysis. The results showed that the completeness of the incomplete medical resume was (30.9%), the accuracy of the inaccurate diagnosis code was 14.8%, and there were no claims pending for BPJS claims. Based on the results of the study, it can be concluded that the incompleteness of the medical resume and inaccuracies are still not maximized, the researchers suggest paying attention to the medical record officer/coder starting from compliance with applicable regulations and policies, so that the completeness of the medical resume and the accuracy of the diagnosis code can be achieved as expected.

Abstract

ABSTRACT The accuracy of the code is needed so that the information resulting from the diagnosis and action is accurate. However, the authors found that at RSUD dr.Rasidin Padang there were still incorrect codes. This is because the diagnosis of Dengue Haemorrhagic Fever is not correct in giving the code according to the ICD-10. The purpose of this study was to determine the accuracy of the diagnostic code for dengue haemorrhagic fever in medical records at dr.racidin. This research method used is quantitative with a descriptive approach. This research was conducted from February to May 2022. The number of samples was 59 medical records using the total sampling method, and the tool used was a checklist table with univariate analysis. The results of this study were obtained from 59 medical records, there were 37 (62.7%) medical records with inappropriate codes, and a small proportion of 10 (16.9%) medical records that were not clearly written, and there were 39 incomplete diagnosis writings. (66.1%) this was due to errors in determining the diagnosis code, doctors' writings that were difficult to read, incomplete completeness, non-specific written diagnoses based on ICD-10, and the use of abbreviations that were not standardized in hospitals. Therefore, researchers suggest that coders should pay attention to the rules and procedures for coding a diagnosis based on ICD-10. Keywords: Coding Accuracy, Clarity, Completeness
