Abstract

A birth certificate issued by the hospital is the letter needed to obtain a baby's birth certificate at the population and civil registry office. At Hospital X, which is currently in the stage of implementing electronic medical records, it was found that there were problems related to the preparation of birth certificates, where delays often occurred and there was a lack of data that had not been included in the birth certificate. The purpose of this study was to design an information system for making birth certificates based on Microsoft Visual Studio 2012 in order to overcome delays and streamline the work of medical staff in making birth certificates. This study uses a descriptive examination technique. The process of collecting data in research through observation and interviews. The technique used in this design is through the waterfall development method and using software design with Data Flow Diagrams (DFD). The results of this system design make medical record management more accurate, effective and efficient in the activities of making birth certificates and processing birth data reports more quickly and precisely.

Abstract

By seeing that there are still problems related to the governance of emergency room patient services at Al-Ihsan Hospital, the problems that occur are due to the registration system often experiencing errors so that when patients register through the emergency room they experience obstacles. This research focuses on the design of information technology-based electronic medical record governance in emergency room patient services. This research was conducted with the aim of improving the quality of emergency room patient services so that patient information is available quickly and easily accessible by medical personnel in need. Agile method with Extreme Programming type is used to design the system to be made by going through the stages of planning, design, coding and testing. Needs analysis is done by conducting interviews and making direct observations, then designing the system using the Unified Modeling Language (UML). After the design stage is complete, the system testing stage is carried out to ensure that all functions can run as expected. The results of the application of this method show that the information technology-based RME governance system in emergency room patient services with the Agile XP method can design information technology governance well able to speed up the data entry process, improve data accuracy and facilitate data access by personnel. The conclusion obtained with this information system can improve IT governance-based information function services that are useful for increasing flexibility and responsiveness in emergency room services and making it easier for medical personnel to process data.

Abstract

Electronic medical records are recommended to be implemented in every hospital to facilitate fast data collection and the accuracy of the processed data is correct, but manual data collection still occurs in computerized systems such as entering data into Microsoft excel. The weaknesses in manual medical record data collection are quite a lot even though security is maintained by using a computer such as the length of time for data collection and tends to be incomplete data. The purpose of this research is a practical system in software that can facilitate medical record data collection because manual data entry occurs in the blood availability system at Limijati Mother and Child Hospital. The Agile method is used in this research as a method that involves users into the process of designing information systems that can be tailored to user needs without requiring a long time. Not only the system development method, the black box testing method is used as an application function test. The software used is Microsoft Visual Studio 2012 and Microsoft Access 2016 database. It is hoped that the resulting information system on blood availability can help easy data collection without any erroneous data so that data can be accepted.

Abstract

The current problem in the process of reporting Tuberculosis (TB) cases is the lack of effectiveness and efficiency in data collection and processing, which can hamper TB control efforts. In addition, poorly organized reporting also causes difficulties in monitoring the spread of TB disease and taking appropriate preventive action. The proposed method to address these issues is to apply the Rapid Application Development (RAD) method. This method allows customization and refinement of software according to the needs of users, including medical personnel and public health centers. Rapid and iterative software development, involving key stakeholders. Features that can be implemented in the software include electronic forms for patient data collection, real-time notification of TB disease spread, and structured report generation and data analysis. The results of applying the RAD method to the TB case reporting problem showed significant improvements in the efficiency and effectiveness of data collection, processing, and reporting. The system assists medical personnel in the management and monitoring of TB cases, and provides necessary information to public health centers and the Tuberculosis Disease Control Agency. With the use of RAD-based software, TB case reporting can be done more quickly, accurately, and structured, which in turn will help reduce the spread of TB disease and improve preventive measures.

Abstract

Problems faced in the governance of electronic-based medical records in outpatient daily visits include the lack of governance standards, inconsistencies in data collection, and low interoperability between existing systems. These problems can lead to errors in diagnosis, less efficient treatment, and increased patient safety risks. In order to solve these problems, the RAD (Rapid Application Development) method was proposed as a solution. This method involves a needs analysis phase conducted by listening to stakeholders, identifying existing problems, and designing a suitable solution. The use of the RAD method can provide significant results. After the implementation of the RAD method, the results showed significant improvement. Clear and measurable governance standards have been implemented to ensure consistency and accuracy in data collection. In addition, faster system development is also possible through the implementation of the RAD method, thus overcoming the technical challenges associated with electronic medical records. Overall, the results of applying the RAD method in the governance of electronic-based medical records on daily outpatient visits have successfully addressed the existing problems. This method can be widely applied in clinical practice to improve efficiency, quality of care and patient safety.

Abstract

This study aims to describe several problems related to data processing of the top 10 outpatient diseases. The research method used is a qualitative research method with a descriptive approach and a waterfall software development method, while the data collection techniques used are observation and interviews with medical record staff. In the research, problems were found such as diagnoses that were difficult to read by poly officers, the existence of duplicate tasks for medical record officers and recording still using manuals filled in by poly officers which were then inputted into the Microsoft Office Excel program and only made worksheets to then be processed back into the top 10 outpatient disease reports. The suggestions given for problem solving are the coordination between officers and doctors regarding the writing of diagnoses, the addition of medical record officers to reduce the workload and the design of a software that can facilitate the process of processing data on the top 10 outpatient diseases using Visual Studio 2012. So that the final report on the top 10 diseases can be accurate and can improve the quality of service in the hospital.

Abstract

The rapid development of information technology has had a huge impact on many things around us. This study aims to create effective patient registration by designing and developing systems to support better service in the hospital environment. In line with the objectives that the researchers have outlined, the problem behind this study is the inefficiency of patient registration when carrying out health care when researchers carry out field practice. As for the method of this study, it is a waterfall, which will produce an updated application output and can support the effectiveness of patient care. Based on this research, the researchers suggest that in order to improve the efficiency of hospital services, medical records should be able to develop existing systems and work with the field of system development to get more optimal results.

Abstract

The rapid development of technology is certainly very helpful for employee performance and services in hospitals, one of which is in the medical record installation, namely in the reporting of inpatient operations that have not been fully computerized. Problems were found during the Field Work Practice, one of which was the delay in the process of collecting patient surgery data, which made reporting hampered and did not run smoothly and efficiently. Therefore, an information system is needed to streamline employee performance in reporting complete, fast, and accurate operations. The research data collection methods used are literature study and interviews, with descriptive analysis. The method of making the system is using the waterfall method. It is hoped that the results of the design of this information system can streamline the performance of doctors, as well as officers in making reports on the top 10 index of surgical actions so as to produce complete and accurate data. And this information system can also maximize the quality of service in the hospital.

Abstract

Advances in technology that are increasingly developing can be a benefit for hospitals to support the effectiveness of officer performance. Methods for collecting research data used include literature studies, interviews, and observations with descriptive analysis. System developmenlt using the agile method. The reporting is made into an application that uses visual programming and a Microsoft Access database. The resultsl of thel information systeml design carried out using the selectedl method can increase the efficiency of reporting carried out by medicall record officers to produce accurate data to be sent to the Health Office.In addition, this information is able to maximize services for patients and the quality of service in the hospital can increase. Studies conducted at Hospital X Bandung City, one of which is in the medical recordsinstallation for Dengue Hemorrhagic Fever (DHF) reports that have not been computerized, show that technology improves the performance of hospital staff. There are several problems in the medical record installation, namely the delay in the process of making reports that still use a paper-based manual system (paperbase). By evaluating the information system electronically, it can help medical record officers makeDengue Hemorrhagic Fever (DHF) reports quickly, accurately, and completely.

Abstract

Technological developments have provided significant support for employee performance in hospitals, onel of which is in the medical record installation, namely the making of a visum et repertum in the medicolegal aspect. In the research that has been conducted, researchers found several problems in the medical records installation, one of which is the delay in making VeR because it is not fully computerized because it still uses Microsoft Word, resulting in less effective services. Therefore, a system is needed. The research methods used include literature study, observation, and interviews with descriptive analysis. The system development method uses the agile software development method. The results of this system design are expected to reduce the workload of officers, produce accurate information, maximize services and improve the quality of services in the hospital.
