Abstract

Background: Vulvovaginalis candidiasis (VVC) is an infection of the vaginal mucosa and or vulva caused by Candida species. As many as 70-75% of women are infected by CVV at least once during their lifetime, which most often occurs in women of childbearing age. Purpose: To evaluate the general overview of new VVC patients at the Sexual Transmitted Infection Division Dermato-Venereology Outpatient Clinic Dr. Soetomo General Hospital Surabaya from the period of 2010-2012. Methods: This is a retrospective study, and evaluating medical records of new CVV patients including basic information, history taking, physical and laboratory examination, management, follow up, and counseling. Result: There were 325 new CVV patients (11.5%) from the total 2.813 patients from the total of new patients at the STI Division Dermato-Venereology Outpatient Dr. Soetomo General Hospital Surabaya, range of age was mostly 25-44 years old  (53.2%) and 257 (79.1%) patients were married. The most main complaint was vaginal discharge accompanied with itchy sensation in 213 (65.5%) patients.  The most vaginal discharge appearance was white like cottage cheese in 245 (75.3%) patients. From the wet mount 220 (67.7%) patients showed either blastospore or pseudohyphae, while from the gram stain 220 (67.7%) patients showed 217 (66.8%) patients showed either blastospore or pseudohyphae. Therapy with ketoconasol is in 239 (73.2%) patients. Conclusion: Appropriate diagnosis, adequate therapy, also risk factors exploration and counseling are very important in the management of VVC.Key words:  Vulvovaginalis candidiasis, fluor albus, itch, pseudohyphae, ketoconazole.

Abstract

Latar belakang: Kusta adalah penyakit menular, menahun, yang disebabkan oleh Mycobacterium leprae. Penyakit kusta menyerang saraf tepi, kulit dan jaringan tubuh lainnya kecuali susunan saraf pusat. Endemis di beberapa negara di kawasan Asia Tenggara, Amerika dan Afrika. Tujuan: Mengevaluasi gambaran pasien baru kusta Unit Rawat Jalan (URJ) Kesehatan Kulit dan Kelamin RSUD Dr. Soetomo Surabaya periode tahun 2011-2015. Metode: Penelitian retrospektif dengan meneliti catatan medik pasien kusta di Divisi Kusta URJ Ilmu Kesehatan Kulit dan Kelamin RSUD Dr. Soetomo Surabaya selama Januari 2011 sampai Desember 2015. Hasil: Jumlah pasien kusta adalah 713 pasien. Kelompok umur terbanyak adalah 15-34 tahun (45,1%), jenis kelamin terbanyak adalah laki-laki (67,7%). Pasien sebagian besar berasal dari wilayah Surabaya (60,3%). Pasien dengan reaksi kusta tipe 1 (15,6%) dan reaksi kusta tipe 2 (23,8%). Penemuan pasien baru kusta terbanyak dari rujukan (56,6%) dan alasan rujukan untuk pemeriksaan Basil Tahan Asam (BTA) (59,2%). Sebanyak 671 pasien (94,1 %) tidak ada riwayat kontak dengan pasien kusta Pasien dengan reaksi kusta tipe 1 terbanyak pada tipe Mid Borderline (BB) (71,2%). Pasien dengan reaksi kusta tipe 2 terbanyak pada tipe Lepromatous Leprosy (LL) (68,2%). Kesimpulan: Gambaran pasien baru kusta dapat membantu dalam meningkatkan penatalaksanaan pasien kusta di masa yang akan datang.

Abstract

Latar Belakang: Erythema Nodosum Leprosum (ENL) pada pasien kusta perlu mendapat perhatian khusus oleh karena seringkali membuat pasien meragukan keberhasilan terapi kusta. Tujuan: Mengevaluasi gambaran umum kelainan sistemik dan laboratorik pada pasien kusta dengan reaksi tipe 2/ENL. Metode: Penelitian retrospektif terhadap pasien kusta dengan reakti tipe2/ENL di Divisi Kusta Unit Rawat Jalan (URJ) dan Instalasi Rawat Inap (IRNA) Kulit dan Kelamin RSUD Dr. Soetomo Surabaya tahun 2011-2013. Hasil: Dalam kurun waktu 2011-2013 didapatkan jumlah kunjungan pasien baru sebanyak 434 pasien, 106 diantaranya mengalami ENL. Sebagian besar pasien kusta dengan ENL berjenis kelamin laki-laki (69,8%), usia terbanyak antara 30-44 tahun (51,9%), 65% berasal dari kota Surabaya. LL (Lepromatous Leprosy) adalah tipe kusta terbanyak (62,3%). Keluhan sistemik tersering adalah demam (84,9%), mialgia dialami 57,5% pasien, mual muntah terjadi pada 47,2% pasien. Sebagian besar mengalami anemia (49,0% pasien laki-laki dan 16,9% pasien perempuan), 50,9% mengalami leukositosis, 42,5% dengan trombositosis, 53,8% dengan hipoalbuminemia. Simpulan: Sebagian besar pasien kusta dengan reaksi tipe2/ENL mengalami kelainan sistemik dan laboratorik.

Abstract

Background: Leprosy reaction is an acute episode in leprosy characterized by acute inflammation sometimes accompanied with systemic symptoms. Leprosy type 1 reaction often occurs in borderline leprosy patients. Leprosy type 1 reaction can cause peripheral  nerve damage through neuritis that can lead to disability. Interferon-γ induced protein 10 (IP10) is a chemokine with chemotaxis function to various immune cells such as macrophage, dendritic cells, Natural Killer cells, and activated T lymphocites. Several studies have found an increase of IP10 serum level when type 1 reaction occurred. Purpose: To evaluate the profile of IP10 serum levels in borderline patients with and without type 1 leprosy reaction. Methods: This is a descriptive cross-sectional study, with 34 borderline leprosy patients of which 17 had leprosy type 1 reaction and 17 without reaction that qualify inclusion and exclusion criteria. Results: Mean serum IP10 levels in leprosy type 1 reaction groups are 594,123 ± 327,628 pg/mL and 331,648 ± 101,819 pg/mL in groups without reaction. Conclusions: Serum IP10 levels in borderline leprosy patient with type 1 reaction  is higher than patients without reaction, although confounding factors in borderline leprosy patients with type 1 reaction that can influence serum IP10 levels cannot be eliminated. Further studies are needed to reveal the role of IP`10 in type 1 leprosy reaction.

Abstract

Latar belakang: Kusta adalah penyakit infeksi kronis, disebabkan oleh M. leprae, penyakit ini menyerang sistem saraf perifer, kulit, dan jaringan lain. Gangguan kusta pada saraf tepi menyerang juga saraf autonomik yang akan mengganggu kelenjar keringat yang dapat menyebabkan kondisi kulit kering. Transepidermal Water Loss (TEWL) adalah penilaian terhadap jumlah air yang menguap dari kulit. Semakin tinggi TEWL penguapan semakin besar, kemungkinan terdapat kerusakan pada barier kulit atau produksi keringat. Tujuan: Mengukur nilai TEWL pada lesi makula anestetika dan nonanestetika pada pasien kusta. Metode: Penelitian analitik observasional dengan populasi pasien kusta di Poli Kulit dan Kelamin RSUD. Dr. Soetomo Surabaya. Sesuai dengan kriteria inklusi, kemudian dilakukan pemeriksaan TEWL pada pasien tersebut. Hasil: Dari 22 pasien kusta didapatkan perbedaan rerata yang signifikan antara TEWL makula anestetika dan nonanestetika (p= 0,0001). Distribusi nilai TEWL pada makula anestetika 0-<10 g/m2/h (59,1%), kisaran 10-<15 g/m2/h (27,3%), 15-<25 g/m2/h (13,6%). Simpulan: Terdapat perbedaan rerata yang signifikan antara TEWL makula anestetika dan nonanestetika.

Abstract

Penyakit kusta masih menjadi masalah kesehatan utama. Penyakit kusta ditandai dengan berbagai spektrum manifestasi klinis danragam perbedaan antar spektrum yang ditentukan oleh respons imun dari host. Respons imun humoral di pasien kusta telah dilakukandengan penelitian mengukur kadar imunoglobulin (Ig). Tujuan penelitian ini adalah untuk mengetahui kenasaban jumlah keseluruhanIgG dan IgG anti PGL-I M.leprae dengan lama pengobatan dan reaksi kusta di pasien kusta tipe MB dengan mempelajarinya. Penelitiandilakukan dari bulan Juni sampai dengan Desember 2013 dengan sampel dari pasien kusta tipe MB di Kabupaten Sampang-Mandura.Serum pasien yang diperiksa adalah jumlah keseluruhan IgG dengan metode Radial Immunodiffusion (RID) dan IgG anti PGL-I M. lepraedengan Enzyme Linked Immunosorbent Assay (ELISA). Data dikumpulkan dan diuji kenasabannya. Median jumlah keseluruhan IgGyaitu 172IU/mL dan median IgG anti PGL-I yaitu 574,33U/mL. Median jumlah keseluruhan IgG di pasien yang menerima pengobatankurang dari tiga bulan lebih rendah dibandingkan dengan yang lebih dari atau sama dengan tiga bulan. Median jumlah keseluruhanIgG di pasien yang mengalami reaksi lebih tinggi dibandingkan dengan yang tidak mengalami reaksi. Median IgG anti PGL-I di pasienyang sudah diobati kurang dari 3 bulan lebih tinggi dibandingkan dengan yang lebih dari atau sama dengan tiga bulan. Median IgGanti PGL-I di pasien yang mengalami reaksi lebih tinggi dibandingkan dengan yang tidak. Tidak terdapat kenasaban yang bermaknajumlah keseluruhan IgG dan IgG anti PGL-I dengan lama pengobatan dan reaksi kusta pada penelitian ini. Tidak terdapat kenasabanbermakna jumlah keseluruhan IgG dan IgG anti PGL-I dengan lama pengobatan dan reaksi kusta.

Abstract

The existence of leprosy or kusta or Morbus Hansen or Hansens disease has been known for years, including in Indonesia. Starting from the discovery of Mycobacterium leprae isolates from ancient bone (about 1.000 years B.C), the archaeological excavations results in East Nusa Tenggara, interesting questions arise about how the development of leprosy in eastern Indonesia is. Biology molecular study would become a powerful tool to investigate the presence of leprosy bacillary whether there are similarities between the genomes of M. leprae isolates in the primeval and the present. PCR examinations were performed on mandibular bone fragments from ancient human who lived 1000 years B.C. discovered in archaeological surveys on the island of Lembata and three leprosy patients from East Nusa Tenggara. The DNA extraction was performed using a kit from Qiagen products and its TTC repeating pattern was seen with the method of direct sequencing. It turned out that the TTC profile obtained from samples of archaeological was as many as 13 copies, while the repetition of TTC in three samples of leprosy patients were 15, 17 and 26 copies. The different number of TTC repetition shows the different isolates of M. leprae between in the ancient times and the present. Further studies are needed to verify the differences in the genome that occur, for example from the study of SNPs (single nucleotide polymorphisms).

Abstract

Penderita kusta tahun 2015 di Kota Surabaya tertinggi pertama adalah Kecamatan Kenjeran, disusul Kecamatan Semampir, dan Kecamatan Tandes. Ada beberapa faktor risiko yang memengaruhi kejadian kusta, salah satunya adalah tingkat ekonomi. Penelitian ini bertujuan untuk mengetahui hubungan antara tingkat penghasilan dengan kejadian kusta. Jenis penelitian observasional analitik dengan rancangan penelitian case control matching untuk mengetahui hubungan tingkat penghasilan dengan kejadian penyakit kusta MB pada warga yang tinggal di wilayah Kecamatan Kenjeran, Semampir dan Tandes. Penelitian ini dilakukan antara bulan Januari-April 2017. Jumlah responden kasus sebanyak 28 orang dan kontrol sebanyak 28 orang, Pengumpulan data dilakukan dengan cara wawancara dan penelusuran rekam medis. Analisis bivariat dilakukan dengan menggunakan SPSS 16. Hasilnya didapatkan P value 0.003 < 0.05 menunjukkan bahwa ada hubungan antara tingkat penghasilan dengan kejadian kusta dengan nilai OR 5.200 (1.427-18.948). Simpulan : Terdapat hubungan antara tingkat penghasilan dengan kejadian kusta di Kecamatan Kenjeran, Tandes dan Semampir. Responden yang memiliki tingkat penghasilan rendah berisiko 5.2 kali terkena kusta dibandingkan responden berpenghasilan tinggi.

Abstract

Leprosy stills a public health problem in West Sulawesi which has a Case Detection Rate (CDR) around 43.69/100.000 population. Household contacts of leprosy are a high risk group to be infected, due to droplet infection mode of transmission of the disease. A nose swab examination and serological study was conducted to detect exposure of M. leprae of people who live in leprosy endemic area. Detection of M. leprae in the nasal cavity will represent the exposure rate from outside and the measurement of specific antibody is represented the result of exposure to the immune system. Two group of inhabitants (30 household contacts of leprosy and 30 nonhousehold contacts) were involved in the study. They live in Banggae district, a leprosy endemic area of Majene Regency, West Sulawesi. Sixty nose swab samples and sixty capillary blood samples from the same invidividuals of the two groups were collected and sent to Leprosy laboratory of the Institute of Tropical Disease, Airlangga University Surabaya. A Polymerase Chain Reaction (PCR) was performed to the nose swab samples for detection of M. leprae. The blood samples were examined serologically to measure the level of anti PGL-1 antibody. PCR examination of nose swab samples showed 1/30 positive result in the household contact group and also 1/30 positive result in non-household contact of leprosy (statistically no significant difference, p > 0.05). Serological study showed higher sero-positive result in the household contact group (15/30 or 50%) compared to non-household contact (11/30 or 36%), but statistical calculation revealed no significant difference between the two groups (p > 0.05) on sero-positive results of leprosy. It is concluded that household and non-household contact in leprosy have the same risk to be affected by the disease. The term of household and non-household contact need to be redefined. The possible role of exposure from the environment was also discussed, especially from non-human resource of M. leprae.

Abstract

Drug resistance in leprosy is important for Leprosy Control Program, since the WHO-Multidrug regiment (MDT) has been used for global treatment of leprosy for more than two decades already. A Dapsone resistance case in a Multibacillary (MB) leprosy case is reported. The patient was diagnosed and treated in Tajuddin Chalid Hospital Makassar, South Sulawesi. Previously he was treated in a health center at South Sulawesi and was given a treatment for one year, before referred to the hospital. The leprosy skin lesions are still active with erythematous skin lesions and thickened ear lobe. Bacteriological examination was positive for Acid Fast Bacilli, the Bacterial Index was 3+ and the Morphological Index was 1%. The specimens of M.leprae isolation was sent to the Institute of Tropical Disease Surabaya for drug resistance study. Using the Lp1-2 and Lp3-4 nested primers, PCR test was positive for M.leprae.Sequencing result for folP gene showed a double mutation at codon 53 (ACC / Threonin ) which become (AGG / Arginine). Simultaneous mutation at two nucleotides at one codon has never been reported in Indonesia before and this phenomenon is important for leprosy control policy.
