Abstract

Introduction: Acute care is a type of time-sensitive care, this is due to an acute condition requiring rapid intervention. Acute conditions that exist in the inpatient unit would require special care in handling. This acute condition can affect many things, one of which is the patient satisfaction. With proper treatment for acute cases, the course will improve patient satisfaction itself. However, to date, acute care has been poorly defined and inadequately supported in most developing health systems. Aims of Study: This research aims to determine nurses’ perception on the implementation of inpatient acute nursing care in the Hasanuddin University Hospital. Method: This research was a qualitative method, data collection using individual interviews against ten participants (inpatient nurses) and validity checks of the data that utilizes something beyond the data. Data analyze used thematic analysis. Result: The results of the research yield four themes. Those four themes are perception of acute conditions, supporting factors in the management of patients with acute conditions, inhibiting factors in the management of patients with acute conditions, and nurses’ hope for acute care. Conclusion: From this research it is known that the incidence of acute conditions in the inpatient unit of Hasanuddin University Hospital less than chronic conditions. To handle acute cases, nurse in the inpatient unit take action early by monitoring vital signs, then immediately gave emergency action if necessary, while collaborating with the doctor. Supporting factors for nurses is the availability of emergency trolly, as well as cooperation among nurses who are good. Whereas the inhibiting factor is the number of nurses who are still lacking and the absence of code blue team. Advice from researchers, to health institutions is expected to be able to form a rapid response team (code blue), so that patients with serious/acute condition can be handled more quickly and efficiently, so that the implementation of nursing, especially nursing acute to run well and the nurse also will not experience in serving patients overwhelmed.

Abstract

Stroke is the second leading cause of disability and death worldwide with 5.9 million deaths annually. Stroke can be followed by various clinical problems. Impaired motoric function of the extremities is the most severe post stroke disorder experienced and impacts on limitations of daily activities. The main treatment is by doing regular physical activity exercises. This review was conducted to identify physical activities that can be performed by stroke patients. This scoping review was compiled using Arksey and O'Malley's five-stage framework. A total of three databases (i.e., Pubmed, Proquest, and DOAJ) was searched with relevant keyword "physical activity OR exercise AND stroke". The articles selected are published in the last 5 years, in English, and in full text. The literature search retrieved 164 studies to be screened based on the exclusion-inclusion criteria which finally resulted in the 11 studies included in the review. The remaining 11 studies were fully read and data were extracted regarding the type of physical activity, goals, settings and outcomes. Qualitative (i.e., theme analysis) methods have been used to synthesis the data. Based on the results of the review, there are several physical activities that stroke patients can do, such as progressive resistance and balance (PRB) exercises, locomotors exercises: (walking on a treadmill with body weight support and walking on the ground), selective movements of the upper and lower trunk body in the Supine and sit using a stable support / pad or unstable support (Swiss ball), core stability training, aerobic (AT) and resistance training (RT), aerobic training, walking exercises, and task-oriented training. Several physical exercises have been identified where each of these exercises has a purpose and benefit in the recovery of physical functional stroke patients. Physical activity should be done regularly and take into account the duration, intensity, and frequency of the exercise based on patient’s ability and stamina.

Abstract

Introductions: Less critical nurses in Indonesia may happen due to the lack of contribution of the nursing institutions in enhancing the critical thinking skill. The PSGW is one of clinical teaching methods that facilitates students to criticize the current literatures. This study aimed at identifying the effectiveness of the PSGW on students’ critical appraisal skill (CAS) improvement during the medical surgical nursing clinical placement. Methods: This quasy-experimental study used pre-posttest control group design. The subjects were students who are currently in the professional practice stage of medical surgical nursing area. They were randomly assigned into two groups: 30 subjects in intervention group who conducted PSGW and 30 subjects in control group who conducted standard method of journal presentation. CAS questionnaires developed by Oxford University had been used to measure students’ CAS in criticizing the journal. The data was analyzed using Wilcoxon, Mann-Whitney, paired  t-test, and independent t-test with level of significance at 0.05.Results: The intervention group experienced a significant increase in CAS score (p=0.0001) after performing the PSGW, while the control group experienced an insignificant decrease (p=0.596) after performing the standard method of journal reading. When comparing the posttest CAS of both groups, the result showed that there were significant differences in the posttest CAS scores (p=0.0001) with large effect size (r=0.63).Conclusions: The PSGW is an effective learning method for clinical teaching. Further study needs to be performed to investigate the effectiveness of PSGW on the academic learning process.
