Abstract

Background: This literature review aims to present the latest scientific findings that explain the relationship between chronic prostatitis and male reproductive disorders. Method: This study was a narrative literature review that identified articles from PubMed, Scopus, and Google Scholar using the keywords "chronic prostatitis", "CP/CPPS", "male infertility", "erectile dysfunction", and "ejaculatory dysfunction". Inclusion criteria include publications in the last 10 years that discuss the association between chronic prostatitis and male reproductive disorders. The data were analyzed descriptively to identify patterns of interdependence. Results: Data synthesis showed that chronic prostatitis was associated with a significant decrease in sperm concentration, motility, morphology, and viability. Patients also had a threefold risk of erectile dysfunction, as well as a high prevalence of painful ejaculation (30–65%) and premature ejaculation (±40%). Inflammatory mechanisms and oxidative stress have been shown to play an important role in pathogenesis, while multimodal therapies, including antibiotics, antioxidants, PDE5 inhibitors, phytotherapy, and pelvic floor physiotherapy, show better effectiveness than monotherapy. Implications: These results confirm the need to evaluate the reproductive aspect in patients with chronic prostatitis, as well as a multidisciplinary approach to improve quality of life and fertility. Novelty: This study presents an up-to-date comprehensive review that integrates clinical evidence, pathophysiology, and therapeutic strategies, thereby enriching understanding and supporting the development of more effective management.

Abstract

Objective: This study aims to determine the effect of giving pelvic floor muscle biofeedback on improving erectile function in patients with LUTS. Improvement in erectile function was assessed by IIEF (International Index of Erectile Function), EHS (Erectile Hardness Score), and pelvic floor muscle strength. Improvement in LUTS symptoms was assessed by an IPSS (International Index of Prostate Symptoms). Material & Methods: This research was a randomized controlled trial study on outpatients at Cipto Mangunkusumo General Hospital. The research subjects were divided into 2 groups, the treatment group received 10 sessions of pelvic floor muscle biofeedback and home exercise. The control group only received home exercise. Results: There were 21 LUTS patients with ED. A total of 10 subjects (50%) had mild ED, 4 subjects (20%) had moderate ED, and 6 subjects (30%) had severe ED. From the results of the study, it was found that the scores for IIEF, IPSS, EHS, slow twitch, and fast twitch were improved in the treatment group (p-value <0.05), whereas in the control group there were only improvements in IIEF and IPSS scores (p-value <0.05). Conclusion: There was an improvement in erectile function (increased IIEF score, EHS score, decreased IPSS score, and increased pelvic floor muscle strength) after pelvic floor muscle biofeedback in ED patients with LUTS for 10 sessions. Abstract

Introduction: This study was aimed to described current anesthesia methods to facilitate no-scalpel vasectomy procedure. Methods: A systematic review based on PRISMA guideline was conducted which included all the studies discussing no-scalpel vasectomy and focusing on its anesthesia method. Outcomes searched were pain during anesthesia and procedure. Literature search was done through search engines (PubMed, EBSCO Host and Cochrane library) using (“no-scalpel vasectomy” OR “scalpel-free vasectomy” OR “vasectomy”) AND (“anesthesia”) as keywords and through manual search. Literature selection process was done by two reviewer and qualified studies were subjected to quality assessment. Results: A total of 446 literatures were found and five studies satisfying the eligibility criteria and discussed following anesthesia methods: local infiltration anesthesia (LIA), spinal cord block (SCB), no-needle anesthesia (NNA), mini-needle anesthesia (MNA) and eutectic mixture of local anesthetic (EMLA). This study showed that the average score of pain (VAS 1-10) during no-scalpel vasectomy procedure for LIA, combination of LIA and SCB, NNA, MNA and combination of LIA and EMLA were 1.86 – 2.7, 0.64, 0.66 – 2.13, 0.66 and 2.15, respectively. Conclusion: Combination of LIA and SCB, NNA and MNA showed promising results as anesthesia methods. However, these promising results should be further proven by more studies.

Abstract

The double-J stent is one of the ureteral catheters with curved ends for fixation. Since its introduction in 1978, catheterisation using a double-J stent has become one of the most frequently performed procedures in urology. However, double-J stents may lead to various complications, causing patient discomfort, pain, and bladder symptoms. Technological advancements in stent design aim to minimise these complications and enhance comfort. This review aimed to determine the association between the material and design used in ureteral stents and complications in ureteral catheterisation. We conducted a systematic review following the Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) checklist. Screening based on specific inclusion criteria was employed to select potential studies. A database search yielded a total of 124 studies. Nine full texts were evaluated for eligibility, resulting in the exclusion of one paper. Our findings indicate that ureteral stent insertion significantly increases the frequency of pain and urinary symptoms while decreasing overall health. The choice of materials and design plays a crucial role in minimising pain and improving the quality of life for patients. Physical properties such as firmness and tensile strength also affect the quality of life, with higher firmness and tensile strength causing lower quality of life. Stent material and design choice were context-dependent. Using soft stents and specialised soft distal pigtails has been shown to reduce patient stent-related symptoms. Silicone stents showed the least bladder symptoms than polymeric stents. Polymeric stents also showed no difference in utility from metallic stents for long-term usage.

Abstract

BACKGROUND Nephrolithiasis or kidney stone disease (KSD) is common worldwide. Despite various effective treatment strategies, KSD recurrence remains a problem. This study aimed to investigate the risk factors of KSD recurrence. METHODS This retrospective cohort study used medical records of all patients who came to the Department of Urology, Cipto Mangunkusumo Hospital, Jakarta, from January 2014 to December 2019, with asymptomatic and symptomatic KSD. Demographic information, clinical data, exposure to risk factors, and recurrent KSD diagnosis were collected. Univariate and multivariate analyses using logistic regression were performed to determine the significant risk factors. RESULTS We reported 325 patients with a median age of 52 years. More than half of the patients were males and from Java. Staghorn stone dominated the KSD types found in 181 patients (55.7%). After undergoing percutaneous nephrolithotomy, 214 patients (65.8%) became stone-free. However, about 40.6% of them later developed recurrent KSD. The adjusted odds ratio in recurrent KSD were 1.46 (95% confidence interval [CI] 1.33–1.59) for younger age, 1.86 (95% CI 1.61–2.07) for overweight–obese, 2.13 (95% CI 1.89–2.31) for less fluid intake, 1.81 (95% CI 0.97–2.12) for routine tea consumption, 1.24 (95% CI 1.06–1.84) for routine vegetables consumption, 2.27 (95% CI 1.83–2.84) for a family history of KSD, and 2.08 (95% CI 1.77–2.39) for diabetes mellitus (DM). CONCLUSIONS Most patients with recurrent KSD were younger, overweight/obese, had less fluid intake, a family history of KSD, and DM. Modifying a healthy lifestyle and a balanced diet is important to prevent KSD recurrence.
