Abstract

Preeklamsi merupakan penyebab utama morbiditas dan mortalitas baik maternal maupun perinatal. Tujuan: Menganalisis pengaruh pemberian likopen terhadap kadar sFlt-1 secara in vitro pada sel trofoblas model preeklamsi. Metode: Penelitian ini dilakukan secara eksperimental di Laboratorium Genetika Molekuler dan Kultur Sel Fakultas Kedokteran Universitas Padjadjaran selama periode 2017-2018. Sampel serum diperoleh dari RS. Dr. Hasan Sadikin Bandung, sesuai kriteria inklusi dan eksklusi. Kriteria inklusi adalah pasien yang telah didiagnosis preeklamsi berdasarkan pemeriksaan tekanan darah dan protein urin, dan kriteria eksklusi adalag pasien yang tidak datang kontrol atau tidak bisa dihubungi. Penelitian ini terdiri dari dua tahap, pertama ditentukan nilai LD50 likopen terhadap sel trofoblas kemudian dijadikan dasar nilai konsentrasi yang akan digunakan, selanjutnya dilakukan pemeriksaan sFlt-1 menggunakan metode Elisa pada kultur sel trofoblas kondisi normal dan preeklamsi dengan analisa data menggunakan analisis varians (Anova) dan uji Duncan untuk mengetahui perbedaan antar variabel. Hasil: Nilai LD50 adalah 250 ug/ml dan serial konsentrasi yang digunakan mulai dari 1,953 sampai dengan 125 ug/ml. Didapatkan kadar sFlt-1 menurun secara signifikan (p0,05) dari 39,5445 menjadi 31,1342 pg/ml pada sel trofoblas yang diinduksi dengan serum preeklamsi setelah pemberian konsentrasi likopen sebesar 15,625-31,25 ug/ml yang nilainya mendekati keadaan pada sel trofoblas yang diinduksi serum kehamilan normal. Pemberian likopen dapat menurunkan kadar sFlt-1 pada sel trofoblas preeklamsi. Simpulan: Likopen dapat merubah kadar sFlt-1 dan perubahannya dipengaruhi oleh konsentrasi dan jenis serum.Kata kunci: likopen, preeklampsi, sFlt-1

Abstract

Perineal rupture is a common complication during vaginal delivery and can lead to long-term health consequences for the mother. This study aims to identify the risk factors associated with perineal rupture in normal vaginal delivery. A scoping review was conducted by analyzing relevant literature from 2020 to 2024. The results indicate that major risk factors for perineal rupture include birth weight, parity, duration of the second stage of labor, and the use of obstetric instruments. Additionally, pregnancy exercise and perineal massage were found to be effective in reducing the incidence of perineal rupture, particularly in primiparous women. Proper management of labor, including monitoring fetal weight and employing appropriate medical techniques, plays a significant role in minimizing the risk of perineal rupture. This study recommends the implementation of evidence-based preventive measures to reduce the prevalence of perineal rupture and its long-term impacts on maternal health.

Abstract

Menopause is a natural phase in a woman's life characterized by physical and emotional symptoms and an increased risk of other serious health conditions, so exercise is needed to address these issues. This study aims to explore the types of exercise that are effective for the physical and mental health of menopausal women through a scoping review method. 180 articles were found in total, but 5 of them were duplicated. After screening, 175 articles were successfully screened and finally 8 articles were found that met the inclusion and exclusion criteria. The 8 articles that met the inclusion and exclusion criteria were selected. The results showed that several types of exercise can improve the physical and mental health of menopausal women, such as resistance training, aerobics, SaBang-DolGi walking program, ergonomic exercises, bone joint exercises and terra exercises. These types of exercise can increase muscle strength, reduce depression, improve sleep and cognitive function, improve physical and mental health (including vitality and bone mineral density), and improve overall quality of life. In conclusion, regular exercise has a significant positive impact on improving physical and mental health in menopausal women.

Abstract

Preterm labor is a multifactorial phenomenon involving inflammatory processes that will affect the balance of Th1 to Th2 by increasing dominance of pro-inflammatory Th1 cytokines and also lead to functional progesterone withdrawal.Progesterone maintains pregnancy mainly by promoting myometrial quiescence and labor is initiated by progesterone withdrawal. Progesterone also has antiinflammatory properties and as an immunomodulator. While it has been postulated that the effect of progesterone on preterm birth is related to its anti-inflammatory properties, the spesific mechanism of action remains unclear.The biological effects of progesterone are mediated by a 34-kDa protein named the progesterone-induced blocking factor (PIBF). PIBF is synthesized by lymphocytes of healthy pregnant women in the presence of progesterone. PIBF inhibits arachidonic acid production, reduced NK cell activity, and modifies the cytokine balance.The objective of this study was to compare serum concentrations of PIBF of women with preterm labor with those women with normal pregnancy. A comparative analytical study with cross-sectional design was conducted. This study consisted of 16 women in each group. All the subjects met the inclusion criteria and were admitted to the Dr . Hasan Sadikin hospital and it’s district hospital. This study conducted from July until November 2013. Serum PIBF concentrations were measured by enzyme-linked immunosorbent assay ( ELISA) .Mean PIBF concentrations in serum of patients with preterm labor were significantly lower (391.4051 ± 1021.162 ng/mL ) than in those of normal pregnancy (174.2165 ± 1297.675 ng/mL) with p = 0.029 . It was concluded that the levels of serum PIBF preterm labor is lower than in normal pregnancy.

Abstract

The Rebozo technique is a non-pharmacological method that is effective in supporting the delivery process. This research aims to explore the benefits, implementation challenges, and role of education in the application of this technique. The study was conducted through a scoping review of 19 selected articles and analyzed thematically based on certain inclusion criteria from various international databases. The results showed that the Rebozo technique improved maternal comfort, reduced the need for medical intervention, and supported a positive childbirth experience. The main obstacles include a lack of training and limited facilities. Structured education and training for health workers, as well as couple education to support mothers during childbirth, are important elements in the optimization of this technique. Research recommendations include practice-based training for health workers and strengthening the role of couples through educational programs to support the implementation of the Rebozo technique in physiological childbirth services.

Abstract

Calsium supplementation during pregnancy is a routine standard practice for antenatal care recommended by WHO, FIGO, POGI as well as Indonesia Ministry of Health. Calcium is one of essential micronutrients needed in human life, especially during pregnancy known for bone and teeth development, prevent osteoporosis, involved in blood coagulation, prevent haemorrhages during pregnancy and post partum periods, muscle contraction, prevent hypertension, preeclampsia, prematurity, IUGR, LBW, stunting, increase immunity, etc.Prevalence of hypocalcemia varies from 60 % to 70.5 % in low-medium income countries (LMIC) and 30 % to 60 % in high income countries (HIC).A systematic review study conducted in 2019 reported a prevalence of low calcium consumption less than 800 mg/day (normal consumption is around 1000-1200 mg/day) was 29 % in HIC and 82 % in LMIC, including Malaysia 377 mg/day and Indonesia < 400 mg/day.Calcium metabolism in human body is very much depend on the normal level of vitamin D, magnesium and parathyroid hormone in blood. Vitamin D is mainly needed for the absorption of calcium in intestine. The combination of vitamin D and calcium supplementation is very much recommended.

Abstract

The purpose of this study is to find the correlation between leptin levels and the onset of preeclampsi. This study used a cross sectional comparative study design that conducted in May 2018 - April 2019 in the SMF / Obstetrics and Gynecology department of RSUP dr. M. Djamil Padang, RSUD Achmad Mochtar, RSUD Solok, RST Reksodiwiryo. We used consecutive sampling method which consists of 69 pregnant women who fulfill the inclusion and exclusion criteria. Leptin level tests were done using ELISA method. The average level of leptin in early-onset preeclampsia is found to be the highest when compared to the late-onset preeclampsia and normal pregnancy, 64.07 ± 78.27 vs. 30.46 ± 31.99 vs. 16.61 ± 24.49. This differentiation is highly significant with the ANOVA statistical test (p <0.05). There is a significant correlation between leptin levels with the onset of preeclampsia.Keywords: preeclampsia early onset, preeclampsia late onset, leptin levels
