Abstract

Nowadays, the prevalence of hypertension and its concomitant risk of cardiovascular and kidney disease development is increasing as the disability evidence in the society also rises. One of the potential risk factors of prehypertension is anxiety and it has already well-known that cortisol is a marker of anxiety. There are some nonpharmacologic methods to relieve anxiety: exercise and relaxation. Taichi Chuan is a low intensity aerobic exercise that also gives a relaxation effect.This study is organised to find out the effect of Taichi Chuan (TCC) and Jacobson’s Progressive Muscular Relaxation (JPMR) on cortisol level in pre hypertension patients. This is a pre and post-test design study with a total of 26 pre hypertension patients included. They were randomly divided into 2 groups. Group I performed Taichi Chuan exercise, while group II performed JPMR for 18 times. The intervention frequency was 3x/week for 6 weeks with 30 minutes duration for each session. In the study, which was held in April-June 2015, there was a decrease but no significant difference of cortisol concentration in both group.The comparison between groups also did not show statistical difference. However there were significant difference noted on the blood pressure before and after intervention in both groups.

Abstract

Chronic Kidney Disease is a disorder of renal function are progressive and irreversible. Male patients with Chronic Kidney Disease undergoing hemodialysis may have an experience with sexual dysfunction problems is erectile dysfunction. The problem of erectile dysfunction can be a complex psychosocial problems such as depression. Perceptions and expectations of patients with chronic kidney disease who undergoing hemodialysis with erectile dysfunction to sexual fulfillment nursing care needs to be known and have not been studied until now. The research had toexplore the experiences of patients with chronic kidney disease who undergoing hemodialysis with erectile dysfunction. Methods of the research is a qualitative research design with a phenomenological approach. Subjects were patients with chronic kidney disease who undergoing hemodialysis with erectile dysfunction in Hemodialysis Unit RS Tk II.04.05.01 Dr.Soedjono Magelang many as seven people The collection of data by means of indepth interviews. The study was conducted in February-April 2016. In this research have the three themes, the perception initial of the chronic kidney disease and erectile dysfunction, the factors that cause erectile dysfunction, attitudes and feelings the patients related to the condition of erectile dysfunction.Patients with chronic kidney disease who undergoing hemodialysis with erectile dysfunction have a perception that the condition of the disease and hemodialysis therapy effect on erectile dysfunction problems.

Abstract

Patients with Terminal Chronic Renal Disease (ESRD) generally have impaired metabolism of minerals such as calcium. Probiotics can increase the balance of gastrointestinal microflora. Suplementation probiotics may increase levels of calcium. The aim of this study was to analyze the effect of probiotics on the increase of calcium levels. This study used a true experiment research using a randomized pre-post test control group design involving 24 subjects divided into 2 groups at random. The treatment group was given lactobacillus probiotics  (4.0x109CFU) each day in capsul, while the control group was given standard treatment. Statistical analyzes applied paired t-test and independent t-test. Data regarding characteristics of subject were collected using a structured questionnaire. Calcium levels measured by CPO methods and It conducted before intervension and after the intervention. Calcium levels in the treatment group increased at 0.9 g / dl, or higher than in the control group 0.7 g / dl. The treatment group showed a significant increase of calcium level (p = 0.02). But, this increases did not make a significant difference between the 2 groups. Giving of probiotics increases blood calcium levels even in a small level, therefore it is beneficial to be given to patients with ESRD.ABSTRAK Penderita Penyakit Ginjal Kronik (PGK) terminal pada umumnya mengalami gangguan metabolisme mineral seperti kalsium. Probiotik mampu meningkatkan keseimbangan mikroflora saluran pencernaan. Suplementasi probiotik mampu meningkatkan kadar kalsium. Tujuan penelitian ini adalah menganalisis pengaruh pemberian probiotik terhadap kadar kalsium darah pada pasien PGK terminal. Penelitian ini merupakan penelitian true experiment dengan rancangan randomized pre-post test control group design yang melibatkan 24 subjek yang dibagi menjadi 2 kelompok secara acak. Kelompok perlakuan  diberikan probiotik lactobacillus (4.0x109CFU) per hari dalam bentuk kapsul, sedangkan kelompok kontrol diberikan pengobatan standar. Pengumpulan data penelitian meliputi karakteristik subjek yang dilakukan dengan wawancara menggunakan kuesioner. Data kalsium darah diperoleh dari hasil pemeriksaan dengan metode OCP. Analisis statistik yang dilakukan adalah uji paired t-tes dan independent t-test. Kadar kalsium pada kelompok perlakuan meningkat 0,9 g/dl, atau lebih tinggi dibandingkan pada kelompok kontrol 0,7 g/dl. Kelompok perlakuan menunjukan peningkatan kadar kalsium secara bermakna (p=0,02).  Namun kenaikan ini tidak memberikan perbedaan yang bermakna antara dua kelompok (p= 0,21) Pemberian probiotik meningkatkan kadar kalsium darah walapun tidak banyak, sehingga bermanfaat untuk diberikan kepada pasien PGK terminal.  Kata kunci: probiotik lactobacillus, kadar kalsium, PGK terminal

Abstract

Hemodialisis merupakan salah satu terapi yang dilaksanakan oleh klien dengan penyakit ginjal kronis stadium akhir. Berbagai perubahan fisik dan psikologis klien yang menjalani hemodialisi berpengaruh pada kemampuan klien dalam melakukan aktivitas fisik. Pengalaman klien yang menjalani hemodialisis dalam melakukan aktivitas fisik merupakan hal penting untuk diketahui agar dapat menjadi dasar pengembangan program intervensi untuk mengurangi resiko komplikasi penyakit jantung yang menjadi penyebab utama kematian penyakit ini.Tujuan penelitian ini yaitu mengeksplorasi secara mendalam pengalaman pemenuhan kebutuhan aktivitas fisik klien yang menjalani hemodialisis.Metode yang digunakan dalam penelitian ini yaitu kualitatif dengan pendekatan fenomenologi. Data dikumpulkan melalui in-depth interview kepada 8 (delapan) orang klien yang menjalani hemodialisis.  Keabsahan data meliputi credibility, dependability, confirmability, transferability. Teknik analisis data menggunakan metode Collaizi.Hasil penelitian didapatkan 4 (empat) tema yaitu:  (1) Perubahan pemenuhan kebutuhan aktivitas selama menjalani hemodialisis; (2) Mempertahankan kemampuan perawatan diri selama menjalani hemodialisis (3) Perencanaan dalam upaya mempertahankan kemampuan aktivitas fisik; (4)  Harapan terhadap pelayanan terkait dengan intervensi keperawatanHasil penelitian pengalaman aktivitas fisik klien yang menjalani hemodialis dapat dijadikan dasar pengembangan program pelayanan keperawatan yang lebih bermutu untuk dapat meningkatkan produktifitas klien sehingga kualitas hidup klien hemodialisis menjadi lebih baik.

Abstract

Elevated monocyte count is correlated to the decrease of renal function and disease progressivity on end-stage renal disease (ESRD) patients. Probiotic that utilize lactobacillus species is known to play a role in maintaining imunity system balance by inducing the monocyte apoptosis. The combination between probiotic and calcium carbonate could increase probiotic colonization in the gatrointestinal tract. The aim of this study was to analyze the effect of probiotic and calcium carbonate combination toward reduction of monocyte count on ESRD patient at Rumah Sehat Terpadu Dompet Dhuafa Bogor Hospital. This study was true experimental research with randomized pre-post test control group design. Twenty four ESRD patient were randomly enrolled into treatment group (n=12) and control (n=12). The treatment group received probiotic and calcium carbonate, whereas control group received standardized calcium carbonate for 21 days. There was a significant decrease of monocyte (p=0.03) after administration of probiotic and calcium carbonate.

Abstract

Patients with chronic kidney disease (CKD) undergoing haemodialysis require fulfilment of physical, psychological and spiritual needs. In the effort to meet the spiritual needs, it is necessary to do an appropriate spiritual needs assessment of the patients. Assessing a Patient’s Spiritual Needs (APSN) and Spiritual Needs Questionnaire Scale (SpNQ) are such available instruments for use in spiritual assessment. This study aimed to determine the spiritual needs of patients with chronic kidney disease undergoing haemodialysis using the APSN and SpNQ instruments. This study used a quantitative observation with a cross-sectional design. The samples were 104 patients undergoing haemodialysis for more than three months at Kardinah Hospital and RSUI Harapan Anda Kota Tegal. The results showed that the need of prayers was the highest spiritual needs in APSN in the dimension ‘divine’ as reported by 80 respondents (76.92%). Meanwhile, in SpNQ, the dimension of religiosity was rated the highest with 98 respondents (94.23%). Patients with CKD had spiritual needs in all dimensions both in APSN and SpNQ. The result of chi-square test showed a p-value of 0.170, indicating that there was no difference in the assessment of spiritual needs of patients using the APSN and SpNQ instruments. The APSN and SpNQ instruments could be further considered as alternative instruments to assess the spiritual needs of patients with CKD undergoing haemodialysis.

Abstract

Hipertensi adalah peningkatan tekanan darah sistol ?140 mmHg atau diastol ?90 mmHg. Hipertensi sering disebut the silent killer karena tidak menimbulkan gejala sehingga pengobatannya seringkali terlambat. Hipertensi merupakan salah satu penyakit yang berhubungan dengan pekerjaan. Pegawai menghabiskan waktu kurang lebih delapan jam sehingga tidak memiliki kebiasaan olahraga secara teratur. Perilaku sedentari merupakan perilaku yang berisiko terhadap salah satu penyakit pembuluh darah. Proporsi hipertensi berdasarkan survei deteksi dini penyakit tidak menular pada pegawai kantor di daerah perimeter adalah 33,68%. Penelitian ini merupakan penelitian observasional analitik menggunakan desain case control dengan jumlah 76 sampel terdiri dari 38 kasus dan 38 kontrol yang diambil secara consecutive sampling pada populasi pegawai perimeter pelabuhan yang tercatat dalam survei deteksi dini tahun 2017. Data dianalisis secara univariat, bivariat dan multivariat. Hasil statistik yang diperoleh adalah jenis kelamin (p=0,010; OR adjusted 6,179; 95%CI 1,553-24,587) dan perilaku sedentari (p=0,034; OR adjusted 0,338; 95%CI 0,124-0,921). Umur, riwayat keluarga, kebiasaan olahraga, riwayat stres kerja dan jadwal kerja tidak terbukti sebagai faktor risiko hipertensi. Hormon merupakan salah satu penyebab hipertensi pada laki-laki cenderung lebih tinggi. Otot seseorang yang kurang melakukan aktivitas fisik cenderung akan mengendor sehingga peredaran darah akan terhambat dan kerja jantung akan lebih berat.   Kata kunci : Hipertensi, pegawai, pelabuhan, sedentari   SELF-CONCEPT OF PATIENTS WITH CHRONIC RENAL FAILURE WHO UNDERWENT HEMODIALYSIS   ABSTRACT Hypertension is an increase in systolic blood pressure ?140 mmHg or diastolic ?90 mmHg. Officers spend approximately eight hours so they do not have regular exercise habits. Sedentary is a risky behavior for one of the vascular diseases. The proportion of hypertension based on early detection of non-communicable diseases in the perimeter area is 33,68%. This research is an analytic observational study using a case control design with 76 samples consisting of 38 cases and 38 controls taken by consecutive sampling in the population of port perimeter officers recorded in the early detection survey in 2017. Data were analyzed by univariate, bivariate and multivariate. Results obtained were gender (p=0,010; adjusted OR 6,179; 95%CI 1,553-24,587) and sedentary behavior (p=0,034; adjusted OR 0,338; 95%CI 0,124-0,921). Age, family history, exercise habits, history of work stress and work schedules are not proven to be risk factors of hypertension. Hormone is one of the causes of hypertension in men tend to be higher. The muscle of someone who is less physically active tends to relax so that blood circulation will be hampered and the heart will work harder.   Keywords: Hypertension, officers, port, sedentary.
