Abstract

Background: The qualified health services are based on satisfaction level on common population and excellent service based on standards ethics professional codes, fulfilled exceed the patient's expectations and satisfaction. Hospitals must be working effective and efficient.. The center view of health care provider is no longer the caregiver, but the corner of services has been patient-oriented. In order to reach patient satisfaction, it is necessary to pay attention to things that play an important role in determining the perception of patient quality, including facilities, the role of physicians and nurses. Subjects and methods: The respondents was randomly selected by the Non Probability Sampling method. This study using a questionnaire containing the community satisfaction index on 14 measuring values ​​of hospital performance, the descriptive statistics used to quantitative analysing. Results: From 150 questionnaires, the average value of patient satisfaction with outpatient services at Harapan Insani Hospital was 87,614. and for admitted patients was 85,208, and for the price get an average value of 52,25. Conclusion: The respondents expressed their satisfaction at Harapan Insani Hospital, Pangkalan Bun. From the respondents, there is feedback for the problem of the rate of admitted patients due to the using of branded drugs, it makes problem for unit cost calculation. This analysis met solution to improve the performance of Harapan Insani Hospital, to fulfilled expectation of health care user in the Kotawaringin Barat region.

Abstract

Tingginya kasus COVID-19 di Indonesia dipengaruhi oleh kegagalan isolasi mandiri pada masyarakat tanpa gejala, ringan, dan sedang pasca rawat inap. Masalahnya adalah bagaimana mengontrol dan memastikan orang-orang diisolasi sampai waktu yang ditentukan. Untuk mengatasi masalah ini, Pemerintah dan Dinas Kesehatan Provinsi Riau menyediakan 18 tempat isolasi. Penelitian ini bertujuan untuk mengetahui hubungan program isolasi pemerintah di Provinsi Riau pada bulan Juni 2021 (sebelum peraturan jemput dan antar) dan Juli 2021 (setelah peraturan antar jemput) dengan jumlah kasus baru terkonfirmasi. Penelitian ini merupakan penelitian kuantitatif dengan desain cross sectional dan telaah data sekunder. Program isolasi mandiri di fasilitas pemerintah belum menunjukkan hasil yang signifikan dalam mengurangi kasus baru yang dikonfirmasi. Ketaatan dan kesadaran publik adalah masalah kurangnya isolasi di fasilitas pemerintah. Rekomendasi adalah optimalisasi program pendidikan di area publik, meningkatkan kontrol dari petugas kesehatan lokal di layanan kesehatan primer, paket obat dan suplemen pendukung, dan sistem rujukan ke rumah sakit untuk gejala sedang dan berat. Hasil yang diharapkan adalah pengendalian transmisi sistemik COVID-19 oleh Hospital Without Wall.

Abstract

Human resource and organizational capacity of public health programs in two sub-districts of East JakartaIntroduction: The current health policy priorities and focus are increasingly showing that public health programs are the same or even more important than medical treatment in improving the health status of the population. Although the government has asked the community health centers and local administrators to focus on the development and implementation of public health programs, very few studies in Indonesia have focused on the ability of government organizations to actually implement public health programs. This study evaluates the relationship between public health outcomes and the relationship with availability of human resources and program implementation capacity of government organizations. Methods: The data comes from reported minimum service standards (SPM) in public health and clean and healthy living behavior (PHBS), the survey and in-depth interviews of 46 respondents from 17 community health centers and 14 urban villages in two sub-districts - Matraman and Jatinegara - in East Jakarta. Results: The study found that health outcome indicators in the two study areas were lower than national average outcomes despite adequate availability of tbs. In addition, the administrative and management capacity of the implementation process in the field is not as expected. Conclusions: This study showed the paradox of resource availability and the weakness in intersectoral collaboration and in program implementation management. Based on this, we discuss three implications. First, the cross-sectoral authority of the mayor should be the advocacy focus among public health community interest groups. Secondly, the hamlet administrators should improve their implementation management capacity to have more effective programs. Third, community health centers should have human resources equipped with program management and intersectoral advocacy competencies.AbstrakLatar belakang: Prioritas dan fokus kebijakan kesehatan yang berkembang saat ini makin menunjukkan bahwa program public health adalah sama atau bahkan jauh lebih penting daripada tekanan pengobatan dalam meningkatkan status kesehatan penduduk. Meski pemerintah telah meminta puskesmas dan kelurahan fokus pada pengembangan dan pelaksanaan program-program public health, sedikit studi melaporkan kemampuan organisasi pemerintah yang benar-benar mengerjakan fungsi ini. Penelitian ini mengevaluasi hubungan capaian program public health dan apakah capaian itu didukung oleh ketersediaan sumber daya manusia dan kapasitas implementasi program dari organisasi pemerintah. Metode: Data berasal dari “standar pelayanan minimal” kesehatan masyarakat dan “perilaku hidup bersih dan sehat” (PHB), survei dan wawancara mendalam terhadap 46 responden dari 17 puskesmas dan 14 kelurahan di dua kecamatan - Matraman dan Jatinegara - di Jakarta Timur. Hasil: Penelitian ini menemukan bahwa indikator capaian kesehatan di daerah penelitian di kota besar seperti Jakarta adalah lebih rendah dari capaian rata-rata nasional meskipun memiliki ketersediaan sdm yang memadai. Selain itu, kapasitas administrasi dan manajemen proses implementasi di lapangan tidak seperti yang diharapkan. Simpulan: Studi ini menunjukkan paradoks antara ketersediaan sumber daya dan kapasitas yang lemah dalam kolaborasi lintas sektoral dan dalam manajemen implementasi program. Kami mendiskusikan 3 faktor penting yang harus menjadi perhatian dalam pengembangan dan implementasi upaya public health di Jakarta. Pertama, peran lintas sektoral yang jadi kewenangan dari walikota harus mendapat advokasi yang besar dari masyarakat public health. Kedua, administrator kelurahan memiliki kapasitas manajemen implementasi agar program-program dirasakan oleh penduduk setempat. Ketiga, puskesmas memiliki sdm dengan kemampuan manajerial dan bekerja sama dengan sektor lain yang bekerja fokus untuk upaya kesehatan masyarakat.

Abstract

Mother's readiness in healthy childbirth in Brebes Health Center of Central JavaPurpose: Although health workers have made classes of pregnant women to have a healthy childbirth, very few evaluations of these programs existed for Indonesian context. This study examines health workers and socioeconomic factors effects on mother's readiness to deliver healthy children. Methods: A survey of 100 pregnant women in the working area of the Brebes Health Center in Brebes Regency, Central Java Province, in 2018. Data collection using questionnaires and pregnancy records in the MCH Handbook. Results: A quarter of pregnant women in this study were not ready for a "healthy childbirth process" (24%). Mothers with higher education have three and a half times more than those with low education. Mothers who received health support in the ANC were three times more prepared than those who did not need ANC support. Conclusions: Most pregnant women have adequate readiness to deliver a healthy child. Mothers with low education were less likely to be ready to have a healthy childbirth. Health worker support to mothers during antenatal care is critical for a healthy childbirth. Puskesmas workers should reach mothers who have no ANC visit and educate them, in particular, those from low-education mothers.

Abstract

Patient safety is a global issue where the achievement is low, so that it needs to implement a patient safety culture. The patient safety culture is measured based on 12 elements of the patient's safety culture according to AHRQ and the application of 6 patient safety goals. Perceived causes of the problem is the work environment, team work, leadership, job satisfaction and job stress. At RSU Dharma Yadnya Denpasar, the staff's perception about patient safety culture is not known yet, but the incident rate is still high. The purpose of this research is to know the relation of determinant of factor which is related to patient safety culture. This research method is quantitative research with cross sectional design which analyzed by PLS, with sample of nurses and midwife implementer which is 72 respondent. The results of this study indicate that there is a significant correlation between work team, leadership, and work stress with the patient safety culture, respectively 3.707, 12.647, and 3.135> T Statistics 1.96. While there is no significant relation between work environment and job satisfaction with patient safety culture equal to 1,336 and 0,328 <T Statistic 1,96. This study concludes that teamwork, decreased levels of work stress and the application of transformational leadership models need to be applied in an effort to improve the patient safety culture in the hospital.

Abstract

The current and future of the healthcare system will face various problems, ranging from digitizing the health system, digital transformation and disruption of the health world or the problem of digital application during the Covid-19 pandemic in Health 4.0 and Community 5.0 and its leadership. The objective of the study is to explore digital information and the role of leadership in healthcare services in the future. This research is a narrative literature review and searched relevant articles from ProQuest and PubMed. Digital health is transforming healthcare delivery around the world to meet the evolving challenges of an aging population with a variety of chronic conditions. Digital transformation and disruptive innovation illustrate a comprehensive reorientation of the industry, including its business model due to the advent of digital technology in the form of digitizing products, services, and processes. Digital health products can be in the form of electronic health (eHealth), cellular health (mHealth), health information technology, teleconsultation (telehealth/telemedicine). All these digital products, if they cannot be applied now and in the future, will cause digital disruption in traditional healthcare services in hospitals. The current world of health also has an impact because of the COVID-19 pandemic, where this situation is a race for Health 4.0 and Society 5.0 (super smart people). Where Health 4.0 will relax and reflect digital health and implement it in Society 5.0. The leadership strategies that can be selected are systems thinking, contextual intelligence, and metacognitive strategies.

Abstract

Background: Laboratory information is increasingly recognized as a crucial factor to reduce diagnostic uncertainty and enhance quality care. Medical diagnoses and effective treatments depend on the accurate and timely reporting of laboratory test results, and the trend toward disease prevention and personalized care calls for more complex and effective tests and biomarkers. Today's clinical laboratory provides essential information for the diagnosis, monitoring, screening, prevention, early diagnosis, tailored treatment and more effective monitoring of human diseases. Response Time is an action that is carried out quickly and precisely to handle patients. While an ability for fast (responsive) service is the response time taken to the patient when the patient arrives to get a response. Response time is one indicator of service quality that affects patient satisfaction. The laboratory turnaround time is the starting point for the time of for receiving the sample, the time for registration, or the analytical sampling time and end point for analytical completion, the time for verification of results, the transfer of results to the time for medical record, and the time for report printing. Methods: In this systematic review, we searched Pubmed, Science Direct, and Scopus database published from January 2010 to March 2019. The included studies were broadly classified as reporting from healthcare/hospital and international survey. There were 21 studies that met keywords searching criteria. Then we analysed based on the content of studies, matching to our objectives Results: Out of 21 identified studies, we recognized 6 scholarly journals of turnaround time diagnostic laboratory results as quality improvement of medical care and medical diagnoses and that affects patient satisfaction. These studies concluded that turnaround time becomes a key element to patient safety that contributes to patient satisfaction. Conclusion: Turnaround time as one of quality indicator in a laboratory setting. Improvement in this parameter could give benefit to the patient and deliver service excellence. Standardized reporting method is mandatory as it will not only allow the accreditation of clinical laboratories according to the International Standard but also to assure guidance for promoting improvement processes and guaranteeing quality care to patients and patient satisfaction.

Abstract

Online learning and working from home for educators are changes that must be made by lecturers to keep teaching students. Distance education has the aim of increasing the quality of education and the relevance of education and increasing equitable access and expansion of education. Systematic review using database: Google Scholar. The search results that meet the criteria are then analyzed for articles. A systematic search yielded eight articles that best fit the study objectives. Studies vary greatly in study design, perspective, and also on the results of the evaluation of the COVID-19 health worker incentive policy. The covid 19 incentive program for health workers can be a solution to improve the performance of health workers in health services. The problem of delays, regulatory problems and also the availability of the budget are things that need to be evaluated at a later date. So that the success of the program can be a reflection of the government's success in handling COVID-19.

Abstract

Basically, this study want to know what is the impact of COVID-19 among Health Care Workers (HCWs), including effect (stress, burnout, and anxiety). This systematic review was performed according to the Preferred Reporting Items for Systematic review. We conducted a systematic review by searching the following databases Proquest, Science Direct, Scopus, Sage, and MDPI from January 2020 to December 2020. The search was limited to the studies that reported mental health impact of COVID-19 in healthcare workers, such as psychological (emotional) trauma, acute stress disorder (ASD) and post-traumatic stress disorder (PTSD), mass traumatic event, secondary traumatic stress (STS), moral injury, sleep disorder. Case reports, duplicate publications, journal reviews, were excluded. The methodological quality of studies was assessed by the Appraisal tools From JBI tools. The initial search returned 34 articles. twelve studies were included in final analysis, with total of 14.205 subjects. Healthcare worker directly giving healthcare to suspected or confirmed COVID-19 patients may develop various mental issues such as anxiety, poor sleep quality, insomnia, parasomnia, changing personal life and enhanced negative effect. Physician and Nurse are healthcare workers professions most at risk of infection of COVID-19. Healthcare Workers have significant mental pressure during the COVID-19 pandemic. The different healthcare workers professions and gender have different levels of risk of mental disorders during COVID-19 pandemic.

Abstract

Karyawan adalah salah satu faktor untuk membantu organisasi mencapai bisnis yang optimal. Oleh karena itu, sangat penting untuk merekrut, mengembangkan, dan mempertahankan staf berbakat yang memiliki keterlibatan dan komitmen terhadap kesuksesan perusahaan. Sayangnya, dalam industri Kesehatan di mana dokter adalah salah satu pemangku kepentingan yang paling penting, banyak dokter sama sekali tidak terlibat. Dokter memiliki keterlibatan terendah di seluruh tenaga kesehatan meskipun salah satu alasan mengapa seorang dokter tetap bekerja adalah jika dokter memiliki keterlibatan dengan rumah sakit. Untuk itu penting bagi pengelola rumah sakit untuk mengetahui faktor apa saja yang mempengaruhi keterikatan dokter dengan rumah sakit. Tujuan dari penelitian ini adalah untuk mengetahui faktor-faktor yang mempengaruhi keterlibatan dokter rumah sakit. Penelitian ini menggunakan metodologi scoping review. Pencarian artikel dilakukan menggunakan Pubmed, NCBI, Scopus, EMBASE menghasilkan 650 artikel yang berhubungan dengan doctor engagement. Penghapusan duplikasi dikecualikan dan sisa 35 artikel dimasukkan. Untuk metode yang kami gunakan: kerangka metodologis yang diusulkan oleh Arksey dan 'Malley, dan setelah itu, ulasan pelingkupan dilaporkan menggunakan metode Pedoman PRISMA (Prefered Reporting Items for Systematic Review and Meta-Analysis). Dari hasil penelitian yang dilakukan diketahui bahwa keterikatan tidak semata-mata karena materi atau penghasilan yang diperoleh, tetapi juga adanya faktor penghargaan dan pengakuan dalam bentuk immateriil. Lokasi kerja secara signifikan terkait dengan keterikatan. Berdasarkan spesialisasi, spesialis yang sangat dekat dengan mereka sebagian besar adalah spesialis non-bedah. Faktor lain yang berhubungan dengan keterikatan adalah motivasi bergabung dengan rumah sakit, seperti kelengkapan fasilitas sehingga dapat mengembangkan ilmunya, serta kesamaan visi dan misi rumah sakit. Keterikatan pada rumah sakit tidak dipengaruhi oleh sistem remunerasi dan pola kemitraan, tetapi dipengaruhi oleh persepsi dokter termasuk dokter spesialis mengenai dukungan rumah sakit berupa kelengkapan fasilitas dan tenaga pendukung, visi misi rumah sakit, dan peluang karir.
