Abstract

The Non-Communicable Disease (NCD’s) Prevention and Control Program is one of the essential Community Health Efforts implemented by the Puskesmas. Puskesmas accreditation is a form of program to maintain the quality and form of standardization to Puskesmas services in order to provide quality services. This research uses qualitative method, conducted in April-May 2018, aim to see improvement of performance at puskesmas especially at NCD’s Prevention and Control Program. Result of research, in general Output of program of implementation of NCD’s at Puskesmas accredited better than Puskesmas not yet accredited. Community-based partnership and funding activities have not been implemented, Iva test and CBE screening and DM screening are still around 5%, indicating the community and the lack of effective community empowerment. Input components of human resources, facilities, funds, screening instruction implementation are not adequated. Components of Planning Process (P1), Organizing and Moving (P2) at Accredited Puskesmas better than Puskesmas not yet accredited, P3 has been run even though not optimal in some Puskesmas. Need to improve community empowerment to support NCD’s Prevention and Control Program .. It is necessary to encourage the Puskesmas to improve the implementation of Puskesmas Management and conduct Continuous Improvement of Quality to achieve quality improvement as the main basis of Puskesmas Accreditation.

Abstract

Pandemi covid-19 membuat rumah sakit harus cermat dalam melakukan pengelolaan pendapatan khususnya untuk pembiayaan kegiatan belanja operasional. Salah satu kebutuhan belanja vital di rumah sakit adalah logistik kefarmasian oleh Instalasi Farmasi Rumah Sakit (IFRS) untuk melakukan pengelolaan ketersediaan sediaan farmasi, alat kesehatan, dan bahan medis habis pakai yang aman, bermutu, bermanfaat, dan terjangkau. Ada 4 tahap dalam analisis pengelolaan obat meliputi seleksi, perencanaan dan pengadaan, distribusi, dan penggunaan yang memerlukan dukungan dari organisasi, ketersediaan pendanaan, pengelolaan informasi dan pengembangan sumber daya manusia di dalamnya. Jenis penelitian yang dilakukan adalah penelitian non-eksperimental dengan metode deskriptif menggunakan pendekatan kualitatif. Dilakukan pada bulan Oktober-Desember tahun 2020. Lokasi penelitian yaitu unit farmasi rawat jalan, depo rawat inap, gudang farmasi, bagian keuangan, dan bagian pengadaan. Data primer didapat dari hasil observasi dan wawancara. Data sekunder didapat dari hasil telusur dokumen. Analisis data dilakukan dengan mengukur pencapaian indikator pengelolaan obat RSUA terhadap indikator standar menurut Kemenkes (2008), Pudjaningsih (1996), dan WHO (1993). Hasil : SDM di IFRS berjumlah 25 orang, terdiri dari 10 orang Apoteker, 11 orang D3 Teknis Kefarmasian, dan 4 orang lulusan SMF. Dari hasil analisis, didapatkan dari 21 indikator yang diukur, terdapat 12 indikator sudah sesuai dengan standar serta 9 indikator belum sesuai dengan standar.

Abstract

Background: A well-functioning multidisciplinary team optimizes resource utilization and reduces care redundancy, fragmentation, and wastage. Collaborative efforts yield a clearer understanding of older people’s needs and desires, significantly reducing hospitalization days. Despite limited studies, particularly in Indonesia, investigating professionals’ experiences in implementing multidisciplinary collaboration in government nursing homes, nursing care quality remains a concern. Objective: This study aimed to explore the implementation of multidisciplinary collaboration in nursing homes from the perspectives of various disciplines. Methods: A descriptive phenomenological study was used with semi-structured interviews and focus group discussions with multidisciplinary care providers, including nurses, doctors, social workers, physiotherapists, psychologists, occupational and recreational activity instructors, and clergy in nursing homes owned by the Jakarta provincial government. A total of 64 participants were involved, and data were collected from September 2022 to July 2023. Data were analyzed using content-based analysis. Results: Three main themes emerged: 1) context of multiple collaborations, 2) barriers to implementing multiple collaborations, and 3) impacts of non-optimal multidisciplinary collaborations. Nursing home management’s multidisciplinary teams predominantly implement professional-centered care with limited support systems. While providers generally perceive collaboration positively, shared responsibility and joint work among professionals are lacking. Conclusion: This study highlights the need to improve multidisciplinary collaboration in nursing homes to enhance care quality for older individuals. While providers view collaboration positively, barriers like a lack of shared responsibility and joint work persist. Enhancing teamwork cohesion through improved communication and integrated case reporting systems is crucial. Addressing human resource and systemic barriers is also vital. By overcoming these challenges, nursing homes can optimize resource use, reduce care redundancy, and better meet the diverse needs of older residents.

Abstract

Pengelolaan persediaan farmasi dilakukan dalam sebuah siklus berkesinambungan, yaitu siklus logistik yang sejalan dengan fungsi-fungsi dari manajemen logistik. Proses tersebut dimulai dari perencanaan kebutuhan, penganggaran, pengadaan, penyimpanan, pengdistribusian, penarikan, pemusnahan, serta pengendalian. Tujuan penelitian ini untuk memahami proses manajemen di unit  logistik Instalasi Farmasi Rumah Sakit (IFRS) dan dapat membuat sebuah strategi dalam pengelolaan logistik farmasi di RS X Kota Bengkulu ini. Penelitian ini adalah deskriptif kualitatif dengan pendekatan studi kasus. Dengan melakukan analisis data hasil observasi serta wawancara berjenjang pada kepala gudang logistik farmasi, staff asisten apoteker farmasi, kepala IFRS, wakil direktur penunjang pelayanan medis, serta penilaian pada sistem operasional prosedur yang ada pada IFRS  pada RS X yang dilakukan pada tahun 2020. Metode triangulasi dalam analisis ini digunakan sebagai konfirmasi serta validasi terhadap data yang sudah dikelola. Hasil penelitian menunjukkan bahwa parameter Input dinilai dengan 5M (man, money, material, machine and methode), didapatkan bahwa  SDM yang masih terbatas dinilai dengan uraian tugas dan Analisa beban kerja, dari segi material dan machine sudah sangat baik dengan adanya sistem informasi farmasi pada SIMRS maka memudahkan pendataan bahan-bahan medis yang selesai digunakan dan penggunaan metode sudah disesuaikan dengan SOP pada rumah sakit. Proses yang dinilai dari supply chain management.Kata Kunci: Siklus logistik, farmasi, rumah sakit

Abstract

This study analyzes the correlation between patient age with hospitalization cases and cost in Indonesia. The design of this study was cross-sectional quantitative using open sources data from Indonesia National Health Insurance Agency (BPJS Kesehatan). The sample units in this study were hospitalization cases from 2015-2021 of National Health Insurance program member. Sample size of this study are 43,921 that weighted to get 3,901,373 hospitalization cases. This study show an uptrend in cases and the total cost of cardiovascular disease hospitalization in 2015-2019, while it show downtrend during the Covid-19 pandemic. The average cost of cardiovascular disease hospitalization per case is 7.5 million rupiah (495.15 USD) per year. The highest average cost per case is 185.47 million rupiah (12,244.62 USD) per year; the lowest is 1.5million rupiah (99.03 USD) per year; and the average total cost is 3.99 trillion rupiah (263,417.41 USD) per year. The accumulated costs for 2015-2021 is 27.94 trillion rupiah (1,844,582.04 USD). Spearman's test shows there is a correlation between age and cases, average and total costs of cardiovascular disease hospitalization. Only correlation between age and average cost that show a negative pattern, which means that the higher the age, the lower the hospitalization average cost.

Abstract

Chronic kidney disease is a global health issue affecting more than 10% of the world's population and poses a significant disease burden, especially in developing countries. In Indonesia, the National Health Insurance program (JKN) has enabled access to dialysis care for patients with chronic kidney failure, but it also presents challenges in sustaining financing. This study aims to analyze the trends in dialysis service claims in Pematangsiantar City during the period from 2017 to 2022 and project the service needs until 2025. The research design is descriptive-analytic, using claim data from BPJS Health related to dialysis procedures. Analysis is conducted using polynomial trends to identify historical patterns and future projections. The results show that the average annual cost of dialysis service claims in Pematangsiantar City is 85 billion rupiah. There was an annual increase in visits and claims costs by 25.56% and 23.52%, respectively. In 2021, dialysis visits surged by 91.6%, with claim costs reaching Rp128.2 billion. Projections up to 2025 indicate a significant upward trend, with an estimated number of visits approaching 350,000 in that year. The study's conclusions emphasize the importance of maintaining sustainable health financing through the JKN program and adjusting dialysis service capacity according to demand to prevent over- or undersupply. Additionally, primary prevention efforts through health education and early detection of kidney disease should be prioritized to reduce the number of new patients requiring dialysis.

Abstract

Penelitian ini bertujuan untuk mengeksplorasi perbedaan gaya kerja, stres kerja, dan loyalitas kerja antar Generasi X, Y, dan Z di Rumah Sakit Pusat Pertamina. Data dikumpulkan melalui kuesioner terstruktur yang telah diuji validitas dan reliabilitasnya. Hasil penelitian menunjukkan bahwa generasi secara signifikan memengaruhi aspek Work Centrality (p<0,001; aOR=0,253), Work-Life Balance (p<0,001; aOR=3,455), Leadership (p<0,001; aOR=2,365), dan Recognition (p<0,001; aOR=4,261). Generasi Z mencatat skor tertinggi pada Work Centrality dan Recognition, sementara Generasi Y unggul dalam Work-Life Balance. Loyalitas kerja tertinggi ditemukan pada Generasi X (p=0,017; aOR=2,909). Generasi Z menunjukkan kemungkinan tertinggi untuk mengalami stres kerja sedang (p<0,001; 6,9 kali lebih tinggi dibandingkan Generasi Y). Hasil analisis multivariat menunjukkan bahwa berbagai faktor demografis, termasuk jenis kelamin, pendidikan, status kepegawaian, jabatan, dan lama kerja, juga memiliki pengaruh signifikan terhadap dimensi-dimensi ini. Penelitian ini menyoroti pentingnya organisasi untuk mengadopsi pendekatan manajemen yang komprehensif dan berbasis kebutuhan untuk memaksimalkan potensi karyawan dari semua generasi.

Abstract

Pictorial health warnings already implemented for more than two years since June 24, 2014, however, there are no changes made following the Minister of Health Regulation No. 28 of 2013 until now. According to the results of the National Survey of Assessment of the Implementation of Pictorial Health Warning (PHW) in Indonesia in 2015 towards the compliance of the tobacco industry, it is known that the cigarette industry has not yet adhered to pictorial health warnings. The Government hopesthat the existence of the Minister of Health Regulation No. 28 can reduce the prevalence of smokers, but data from 2016 SIRKERNAS shows that the prevalence of smoking is increasing. The purpose is to dig deeper towards the implementation of the inclusion of health warnings and health information policy on cigarette packaging, henceforth it can be input into the formulation of tobacco control strategies. This study uses descriptive studies with qualitative analysis through in-depth interviews and document review. Aspects of the process (communication, bureaucratic structure and disposition), aspects of the actor (commitments and relationships) aspects of the content (level, benefits and policy objectives) as well as aspects of the context (political culture and socio-economic) are interrelated and influence each other in the implementation of the inclusion of warnings and health information on the cigarette packs. The implementation of the inclusion of warnings and health information on cigarette packaging has been carried out, and the level of industry compliance in including the PHW at the retail level has reached 99.91%. However, the cigarette industry is not compliant in fulfilling the provisions because the size is not appropriate, PHW images covered by excise tapes, and some terms indicate quality, superiority, security, and imaging.

Abstract

Insan Permata Woman and Child Hospital was facing Patient Safety Incidence especially for periode of mid to late 2016 there were 12 incidents and 11 phlebitis cases. This indicated a gap between the conditions in the field and the ideal situation that should be achieved. This study aimed to see how the behavior of employees in implementing patient safety program and the factors that influence it. The sample of this study were 151 respondents as sample, consist of health and non health workers. The patient safety program focused on the accuracy of patient identification, reduction of risk of infection and reduction of risk of falling patients. This research was a quantitative research followed by qualitative method. The results showed that most employees already had good patient safety behavior, age and attitude were the most affecting factors in patient safety behavior. The lack of socialization of accreditation and patient safety, poor monitoring and some incomplete facilities should became the concern for the hospital as it was considered to have great influences in the daily implementation of patient safety program.

Abstract

Many cases of medical and dental malpractice in Indonesia are still related to patient safety and healthcare quality. This narrative review aims to identify solutions that can be implemented. The main focus is to compare the incidence of malpractice in clinical medical and dental practice, emphasizing legal implications, coverage, and internal organizational issues. A systematic literature search was conducted through databases such as Pubmed, ScienceDirect, Google Scholar, Springer, Garuda, and other online databases. Articles published no later than 2024. Eleven studies were selected that discussed the problem of malpractice in Indonesia. Independent screening and critical appraisal were carried out to minimize bias, and data were retrieved and assessed through narrative synthesis. The results showed limited or lack of informed consent, non-compliance, administrative negligence, poor communication, and laws on medical malpractice. Causal factors, such as negligence in the operating room, misdiagnosis, and lack of consent forms, were common and most often resulted in patient injury or death. This review also explains a lack of studies related to this due to the poorly organized data collection system on complaints of medical malpractice in the country and the lack of adequate legal governance regarding dental malpractice. Regulations related to legal practices in Indonesia are still poorly organized, and even in similar cases, consistent enforcement is not guaranteed. These factors indicate the need for an evaluation of the healthcare system in Indonesia. Suggestions that can be given are to involve the community in the informed consent process, improve medical training, and improve the legal system so that these incidents can be reduced in number and patients feel protected.
