Abstract

Salah satu permasalahan dalam program penanggulanan tuberculosis (TB) adalah rendahnya cakupan penemuan kasus TB paru di puskesmas, serta bervariasinya angka pencapaian Case Detection Rate (CDR). Penelitian ini bertujuan membandingkan pengelolaan Program P2TB di dua puskesmas kecamatan dengan pencapaian CDR TB tinggi dan rendah di wilayah Jakarta Timur. Metode penelitian adalah kualitatif yang diperkuat data kuantitatif faktor individu dan sosial dengan pendekatan cross sectional. Hasil penelitian menunjukkan adanya perbedaan dalam manajemen yang mana puskesmas dengan CDR TB tinggi mempunyai Kepala Puskesmas dengan kemampuan manajerial Program P2TB yang baik, mekanisme transfer of knowledge yang baik (komponen input); perencanaan target berbasis masalah untuk meningkatkan CDR TB, program penjaringan kasus secara aktif dengan melibatkan kader dan lintas sektor, adanya antisipasi hasil mutu laboratorium serta monitoring dan evaluasi yang baik (komponen proses); dan memenuhi seluruh target indikator penemuan penderita TB (komponen output). Hasil tersebut diperkuat faktor individu (pengetahuan, sikap suspek terhadap bahaya dan cara pencegahan, serta persepsi suspek terhadap pelayanan kesehatan) dan faktor sosial (dukungan kader, KIE oleh petugas) lebih baik pada puskesmas dengan CDR TB tinggi. Dapat disimpulkan bahwa untuk meningkatan CDR, diperlukan pelatihan TB kepada top manajemen, penjaringan kasus aktif dengan melibatkan kader dan sektor non kesehatan, serta kerjasama lintas sektor.

Abstract

Hospitals that already have sports medicine services need to constantly make improvements and innovations to ensure the implementation of complete services. This study aims to explain the description of the availability of personnel and equipment at a hospital in Jakarta and the extent of their suitability based on literature studies in order to support sports medicine services. This research method is descriptive qualitative with triangulation methods, namely observation, indepth interviews with 4 key informants, review of hospital documents, and is equipped with a literature study to match the personnel and equipment available with those found in the literature study. This research is located at a hospital in Jakarta, which runs from November 3, 2020 to December 1, 2020. The results show that the hospital has sufficiently complete personnel and only needs to complete the availability of sports psychologists. Meanwhile, nutritionists and dentists can ask for the willingness of the staff in the hospital to join as a sports medicine team. For podiatrists and health education providers, their role can be carried out by other personnel in the field of sports medicine who have similar competencies. The equipment owned by that hospital is also quite complete, but it should be equipped with MRI, as well as tools that have become gold standards, including CPET and DXA Scan. The fulfillment of these personnel and equipment should be a development plan for sports medicine services in stages in the future.

Abstract

The utilisation of Electronic Medical Record (EMR) is an implementation of information and communication technological development in healthcare. EMR improve quality and documentation of medical record. Integrated Electronic Surgical Report (IESR) prototype is an EMR development that concerned on surgical report fulfillment, allows the surgeon fullfill an android based medical record efficiently. This research is mixed method study, using qualitative study to get feedback from the IESR user, and a cross sectional analytic study to know the comparison of medical records filling completeness between Integrated Electronic Surgical Report (IESR) and conventional medical records at Sultan Imanuddin hospital. Secondary data of 31 medical record sample from medical record department in october until december 2020 was obtained. Bivariate analysis result using Mann_Whitney test showed p=0.061. There are few positive aspect including: simplicity , short duration and efficiency in medical record fulfilment using IESR. But there is no significant difference between IESR prototype utilisation and conventional medical record in term completeness fulfilment of surgical ward patients medical record.

Abstract

ABSTRACKBackground: The rapid change from presidential regulation no. 19 year 2016 on health insurance into presidential regulation no. 28 year 2016 on health insurance get a big attention.Objectives: This research was purposed to analyze about health insurance policy which changed very quickly. It changed from presidential regulation no.19 year 2016 into presidential regulation no.28 year 2016 on health insurance.Research Methods: The researcher used qualitative methods.Results: The analysis from the input processing and output showed that the change of presidential regulation is a responsive form from president when he looked public rejection response for the increase of fee.Conclusions: The president extended it through the department of health affairs.This change has not been described a process of democracy because there’s still a lack of cross-sectoral coordination role in the discussion. This change of presidential regulation not yet affected to appropriate the fee adequacy on BPJS Implementation. The department of health affairs as a leader of health sector was recommended to increase the cross-sectoral coordination which can manifest the better product of health policy and to complete the policy instrument that yet to be determined. It also used to be concern from the department of health affairs, DJSN and BPJS which explained the increase of fee must be offset by a quality improvement rather than the implementation of national health insurance. Keywords; change, policy, presidential regulationABSTRAKLatar belakang: Perubahan Peraturan Presiden No.19/2016 tentang Jaminan Kesehatan menjadi Peraturan Presiden No. 28/2016 tentang Jaminan Kesehatan yang sangat cepat menjadi sorotan yang mencolok.Tujuan: penelitian ini untuk menganalisis perubahan yang begitu cepat tentang kebijakan jaminan kesehatan Peraturan Presiden No.19/2016 tentang Jaminan Kesehatan menjadi Peraturan Presiden No.28/2016 tentang Jaminan KesehatanMetode penelitian: pendekatan kualitatif.Hasil: Berdasarkan analisis bahwa dalam proses input, proses dan output, perubahan Peraturan Presiden ini merupakan bentuk responsif Presiden melalui lembaga pemerintah Kementerian Kesehatan dengan melihat respon penolakan masyarakat akan kenaikan iuran. Proses perubahan ini belum menggambarkan sebuah proses yang demokrasi dikarenakan masih kurangnya koordinasi peran lintas sektoral dalam pembahasannya.Kesimpulan: Dengan adanya perubahan Peraturan Presiden ini berdampak belum memadainya kecukupan iuran dalam penyelenggaraan BPJS. Peran Kementerian Kesehatan sebagai leader dalam regulasi bidang kesehatan disarankan dapat meningkatkan koordinasi lintas sektoral untuk dapat mewujudkan produk kebijakan kesehatan yang lebih baik serta melengkapi instrument kebijakan yang belum ditetapkan, serta untuk menjadi perhatian sektor terkait Kementerian Kesehatan, DJSN dan BPJS Kesehatan bahwa kenaikan iuran harus dapat diimbangi dengan peningkatan kualitas daripada penyelenggaraan jaminan kesehatan nasional. Kata kunci: Perubahan, Kebijakan, Peraturan Presiden

Abstract

Analysis of Policy Implementation of  Inclusion of Health  Warnings and Health Information on Cigarette PacksBackground: The picture health warning has been applied for more than two years since June 24, 2014, but until now there has been no change in accordance with Minister of Health Decree No. 28 Year 2013. The National Survey of Implementation of Health Illustration Implementation Implementation in Indonesia in 2015 on cigarette industry compliance is known that the cigarette industry has not complied with the illustrated health warning. The Government's expectation with the Ministry of Health 28 could reduce the prevalence of smokers, but the 2016 Sirkernas data shows the prevalence of smoking is increasing. Objective: To dig deeper into the implementation of the policy of inclusion of health warnings and health information on cigarette packs for further input into the formulation of tobacco control strategies. Method: It is a descriptive study with qualitative analysis through in-depth interview and document review. Result: Outcome: Process aspects (communication, bureaucratic structure and disposition), actor aspect (commitment and relationship), content aspect (level, benefit and policy objectives) and context aspect (political culture and social economy) have interrelationship and influence each other in the implementation of inclusion health and health information on cigarette packaging. Conclusion: The implementation of health warning and health information on cigarette packing has been done and industry compliance level in listing PHW at retail level has reached 99.91%.Keywords:  PHW,  Cigarette, Tobacco, Policy implementationLatar Belakang: Peringatan kesehatan bergambar sudah diterapkan lebih dari dua tahun sejak 24 Juni 2014, akan tetapi sampai saat ini belum dilakukan pergantian sesuai dengan Permenkes No 28 Tahun 2013. Hasil Survei Nasional Penilaian Implementasi Peringatan Kesehatan Bergambar di Indonesia tahun 2015 terhadap kepatuhan industry rokok, diketahui bahwa industry rokok belum patuh terhadap peringatan kesehatan bergambar. Harapan Pemerintah dengan adanya Permenkes 28 dapat menurunkan prevalensi perokok akan tetapi data Sirkernas 2016 menunjukan prevalensi merokok semakin menningkat. Tujuan: Untuk menggali lebih dalam terhadap implementasi kebijakan pencantuman peringatan kesehatan dan informasi kesehatan pada kemasan rokok untuk selanjutnya dapat menjadi masukan dalam perumusan strategi pengendalian tembakau. Metode: Merupakan studi deskriptif dengan analisis kualitatif melalui wawancara mendalam dan telaah dokumen. Hasil: Aspek proses (komunikasi, struktur birokrasi dan disposisi), aspek aktor (komitmen dan hubungan), aspek konten (level, manfaat dan tujuan kebijakan) serta aspek konteks (budaya politik dan social ekonomi) memiliki keterkaitan dan saling mempengaruhi dalam pelaksanaan pencantuman peringatan kesehatan dan informasi kesehatan pada kemasan rokok. Kesimpulan: Bahwa pelaksanaan pencantuman peringatan kesehatan dan informasi kesehatan pada kemasan rokok sudah terlaksana dan tingkat kepatuhan industri dalam  mencantumkan PHW ditingkat  ritel telah mencapai 99.91%.Kata kunci:  PHW, Rokok, Tembakau, Implementasi kebijakan

Abstract

Background: Healthcare workers (HCWs) serve as front liners in the battle against COVID-19. They are often expected to be in constant contact with infected patients, posing higher risk of transmission. To date, literature on risk factors of COVID-19 transmission in HCWs are still limited. Purpose: We conducted this systematic review to identify the risk factors for SARS-CoV-2 virus transmission among HCWs. Methods: We conducted a systematic review using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses protocol. Four internet databases were searched using predefined search terms. The keywords used were {“COVID-19” OR “SARS-CoV-2”}, {“transmission” OR “transmission risk”}, and (“healthcare professionals” OR “healthcare workers” OR “healthcare staff”). All English articles which were published from January 2020 to June 2021 involving HCWs in hospitals treating COVID-19 were included into the study. Results: Out of initial 636 studies found using the predefined search terms, 18 studies fulfilled the inclusion criteria and were subsequently analyzed. We found that risk factors for COVID-19 could be divided into 5 categories: use of personal protective equipment (PPE), infection control procedures, unsafe workplace practices, provision of institutional support, and presence of individual risk factors in healthcare workers. Good hospital governance is necessary to establish policies regarding control of COVID-19 infection which will protect the patients and HCWs. Conclusion: Proper use of PPE, compliance to proper infection control procedures, establishment of safe workplace practices, provision of institutional support for prevention measures, and lack of individual risk factors may reduce the transmission risk of SARS-CoV-2 virus among healthcare workers. Further studies are warranted to identify methods for reducing incidence and mortality rate due to COVID-19 at HCWs.

Abstract

In giving injection drugs to patients, the dispensing of sterile preparations is carried out by nurses in rooms with limited facilities and infrastructure. Dispensing of sterile preparations must be carried out by pharmacists in health care facilities. The purpose of this study was to examine the authority of nurses in dispensing sterile preparations in hospitals. This study uses a qualitative descriptive approach with a case study method, the research location is at the Batam Business Administration Hospital. The results of the study there are no rules that state nurses in hospitals are given the authority to dispensing sterile preparations. The dispensing service for sterile preparations at the Batam Management Agency Hospital has not been carried out by pharmacists but is carried out by nurses. This condition will affect the quality of service in the hospital. Recommendation: Implementing the rules for dispensing sterile preparations by pharmacists, hospitals need to make clear the division of tasks between nurses and pharmacists. Keywords: Authority, Nurse, Hospital, Dispensing Sterile Preparations

Abstract

The Corona virus is known to have originated from the Wuhan city China, starting December 2019 years. Data from March 30, 2020, more than 170 countries reported a number of positive cases of Covid-19 as many as 720,117, including Indonesia. The hospital experienced a decrease in revenue but the hospital still guaranteed patient recovery in accordance with hospital service standards. This will affect the financial strategy that can be used in dealing with the pandemic by analyzing financial performance through SWOT analysis in hospitals with profitability ratios (Strength, Weakness, Opportunity, and Threat). We include research for the 2020 period, types of national and international articles published in the field of public and hospital health connected to the COVID-19 pandemic relating to infirmary financial strategies with a SWOT analysis. Conducted a search, using a multi-method strategy between January 2020 and December 2020. In carrying out the search, if any, complete download references, quotes and abstracts to Vancouver, the database reference program, for further evaluation. Systems thinking for decision making, several hospital strategies are needed to survive this pandemic, namely a change in leadership style in terms of financial management by not only focusing on COVID-19 but essential services for other diseases can be improved with rapid and comprehensive preparation and response using an integrated health system understanding in every hospital stakeholder.

Abstract

Latar Belakang: Surat Edaran Kementerian Pendidikan dan Kebudayaan (Kemendikbud) Nomor 3 Tahun 2020 tentang Pencegahan COVID -19 di Satuan Pendidikan dan Surat Edaran Kementerian Pendidikan dan Kebudayaan Nomor 4 Tahun 2020 tentang Pelaksanaan Kebijakan Pendidikan Pada Masa Darurat Penyebaran Corona Virus Disease (COVID -19) menetapkan kebijakan belajar dari rumah. Tujuan: Menganalisis coping stress pada mahasiswa pascasarjana FKM UI tahun 2020 selama masa pandemi COVID -19 yang melakukan perkuliahan secara daring (Study From Home / SFH) ditambah WFH. Metode: Penelitian ini dilakukan dengan menggunakan metode penelitian kualitatif dengan pendekatan studi kasus. Pengumpulan data dengan Focus Group Discussion (FGD) dilakukan kepada mahasiswa pascasarjana FKM UI angkatan 2020, sedangkan triangulasi sumber dilakukan dengan cara mengumpulkan informasi dari salah satu Dosen FKM UI dan Psikolog UI. Hasil: Pembelajaran jarak jauh menyembabkan banyak mahasiswa mengaku mengalami stres sehingga sebagian harus mengakses layanan dari psikiater untuk mengurangi pengaruh buruk yang dirasakan. Tak jarang, manifestasi stres muncul dalam bentuk gangguan fisiologis dan psikologis. Kesimpulan: Koping terhadap stres perlu dilakukan untuk mengurangi gangguan yang dirasakan. Terdapat dua strategi coping yang cukup efektif dalam mengatasi stres akibat pembelajaran jarak jauh, antara lain emotional focus coping dan problem focus.

Abstract

Background: The qualified health services are based on satisfaction level on common population and excellent service based on standards ethics professional codes, fulfilled exceed the patient's expectations and satisfaction. Hospitals must be working effective and efficient.. The center view of health care provider is no longer the caregiver, but the corner of services has been patient-oriented. In order to reach patient satisfaction, it is necessary to pay attention to things that play an important role in determining the perception of patient quality, including facilities, the role of physicians and nurses. Subjects and methods: The respondents was randomly selected by the Non Probability Sampling method. This study using a questionnaire containing the community satisfaction index on 14 measuring values ​​of hospital performance, the descriptive statistics used to quantitative analysing. Results: From 150 questionnaires, the average value of patient satisfaction with outpatient services at Harapan Insani Hospital was 87,614. and for admitted patients was 85,208, and for the price get an average value of 52,25. Conclusion: The respondents expressed their satisfaction at Harapan Insani Hospital, Pangkalan Bun. From the respondents, there is feedback for the problem of the rate of admitted patients due to the using of branded drugs, it makes problem for unit cost calculation. This analysis met solution to improve the performance of Harapan Insani Hospital, to fulfilled expectation of health care user in the Kotawaringin Barat region.
