Abstract

Dalam penelitian ini, peneliti akan membahas apa sebenarnya penatagunaan antimikroba itu dan mengapa hal ini sangat penting untuk organisasi pelayanan kesehatan. Dan juga, tulisan ini berfokus pada strategi utama kebijakan untuk mengimplementasi program penatagunaan antimikroba. Metode penelitian menggunakan pendekatan kualitatif dengan systematic literature. Sehingga dipilih 22 artikel yang memenuhi kriteria. Diperoleh hasil penelitian bahwa Kebijakan Penatagunaan Antimikroba bertujuan untuk merasionalisasi penggunaan antibiotik dan mengurangi resistensi antimikroba. Program ini menunjukkan bahwa staf medis dan farmasis harus bekerja sama untuk melaksanakan program Penatagunaan Antimikroba. Untuk mengurangi resistensi antimikroba, dokter dan pasien memerlukan edukasi antibiotik. Untuk mengevaluasi program Penatagunaan Antimikroba, penggunaan antibiotik harus terus dipantau dan dievaluasi. Kebijakan dan pedoman yang jelas dan terukur dapat meningkatkan Penatagunaan Antimikroba. Dengan demikian, manajemen rumah sakit dan pemangku kepentingan lainnya harus mendukung Penatagunaan Antimikroba untuk mempertahankannya. Program Penatagunaan Antimikroba harus mengurangi resistensi antimikroba dan meningkatkan penggunaan antibiotik.

Abstract

Latar belakang: Jumlah dokter spesialis di Indonesia masih kurang untuk memenuhi kebutuhan pelayanan spesialistik. Adaptasi dokter spesialis Warga Negara Indonesia lulusan luar negeri (WNI LLN) di fasilitas pelayanan kesehatan (fasyankes) merupakan salah satu upaya yang dilakukan Pemerintah dalam memenuhi kebutuhan dan peningkatan mutu layanan dokter spesialis. Tujuan: Penelitian ini bertujuan untuk mengetahui gambaran kebijakan yang terkait dengan adaptasi dokter spesialis WNI LLN di fasyankes. Metode: Penelitian ini melakukan pemeriksaan literatur (metode literatur review, research terhadap semua peraturan perundang-undangan yang terkait, yaitu tujuh peraturan dan tiga artikel yang didapatkan melalui pencarian pada Google Scholar. Selain itu penelitian ini juga menggunakan analisis pustaka/ library research dari berbagai sumber diantaranya laporan Badan Pusat Statistik (BPS), data registrasi Konsil Kedokteran Indonesia (KKI), profil tenaga kesehatan, dan sumber data lainnya dari berbagai artikel internasional yang terkait. Hasil: Hasil penelitian ini menunjukan bahwa peraturan adaptasi dokter spesialis WNI LLN di fasyankes telah dipersiapkan dengan cukup baik oleh Pemerintah dimana leading sektor nya adalah Kementerian Kesehatan dan KKI. Sampai bulan Agustus 2023 atau dalam kurun waktu 1(satu) tahun kebijakan ini dilaksanakan sudah memberikan kontribusi yang cukup baik yaitu ada 16 dokter spesialis WNI LLN yang telah ditempatkan di RSUD khususnya wilayah terpencil yang membutuhkan layanan spesialistik. Kesimpulan: Diharapkan ke depan, implementasi kebijakan adaptasi dokter spesialis ini di dukung oleh semua stakeholder terkait khususnya Pemerintah daerah sehingga dapat memenuhi kebutuhan layanan spesialistik di daerah secara optimal.

Abstract

Pelayanan kesehatan primer atau Primary Health Care (PHC) merupakan satu-satu nya cara untuk mencapai Universal Health Coverage (UHC) yang efektif, efisien dan equitable. Namun demikian terdapat tantangan dalam pelaksanaan PHC tesebut yaitu masih sulitnya masyarakat untuk mengakses layanan kesehatan. Untuk mengatasi pemenuhan akses tersebut, pemerintah memiliki beberapa alternatif solusi yang salah satunya adalah menggandeng peran sektor swasta sebagai bagian dari Fasilitas Kesehatan Tingkat Pertama (FKTP) dalam bentuk kerjasama pemerintah dan swasta (KPS). DKI dalam pemenuhan pelayanan kesehatan primer untuk masyarakatnya telah bekerjasama dengan Fasilitas Kesehatan Tingkat Pratama Lainnya. Penelitian ini bertujuan untuk memberikan gambaran kebijakan dan pengembangan strategi kebijakan kerjasama sektor swasta dengan pemerintah yang di DKI diimplementasikan melalui kerjasama FKTP lainnya dengan Puskesmas. Metode penelitian melakukan literature review terhadap kebijakan dan peraturan yang mendukung kebijakan dan pelaksanaan kerjasama pemerintah swasta dalam layanan primer di beberapa negara. Hasil dari penelitian ini didapatkan bahwa pemerintah telah mengeluarkan kebijakan atau peraturan yang dapat dijadikan sebagai landasan dalam pelaksanaan kerjasama dengan sektor swasta. Pelaksanaan kebijakan kerjasama dibeberapa negara memberikan hasil yang positif yang terlihat dari meningkatnya indikator kesehatan. Kesimpulan: Pelaksanaan kebijakan kerjasama dengan FKTP lainnya di DKI Jakarta telah didukung oleh kebijakan atau peraturan sebagai dasar pelaksanaan. Pemerintah Jakarta selaku pelaksana desentralisasi memiliki kewenangan dengan tetap mengacu kepada kebijakan atau peraturan pemerintah pusat. Perlunya pemda Jakarta untuk membangun kapasitas agar dapat mengelola sektor swasta dalam sistem kesehatannya lebih baik lagi guna memastikan bahwa semua penyedia layanan, baik pemerintah maupun swasta, berkontribusi secara efektif terhadap tujuan layanan kesehatan primer dalam mencapai cakupan kesehatan universal.

Abstract

Latar Belakang: Pelayanan kesehatan di rumah sakit harus mengutamakan mutu pelayanan dan keselamatan pasien. Leadership Walkrounds merupakan metode yang efektif untuk meningkatkan budaya keselamatan pasien dan kualitas pelayanan, melalui komunikasi langsung antara manajer dan karyawan. Hal ini berkontribusi pada pengembangan keterampilan karyawan, menjaga budaya keselamatan pasien, meningkatkan keterlibatan karyawan, dan mengurangi burnout pegawai. Tujuan tinjauan literatur ini adalah untuk mengetahui efektivitas Leadership Walkrounds dalam meningkatkan budaya keselamatan pasien di rumah sakit. Metode: Studi ini dilakukan dengan metode scoping review dengan pedoman Arksey dan O’Malley. Studi dilakukan dengan mengikutsertakan publikasi dari tahun 2013 hingga 2023. Pencarian literatur dilakukan pada 3 database elektronik menemukan sebanyak 137 artikel yang kemudian menghasilkan 6 artikel yang akan masuk ke dalam tinjauan. Analisis dilakukan dengan mencari dampak dan faktor yang memengaruhi efektivitas Walkrounds. Hasil: Hasil tinjauan menunjukkan bahwa Walkrounds efektif dalam meningkatkan keselamatan pasien, budaya keselamatan, keterlibatan tenaga kerja, dan mengurangi kelelahan. Implementasi yang tepat dan partisipasi aktif tenaga kerja penting untuk keberhasilan Walkrounds. Walkrounds bukanlah solusi tunggal, pendekatan terintegrasi dan komprehensif juga diperlukan. Komitmen kuat dari pimpinan dan lingkungan yang mendukung komunikasi terbuka, umpan balik, dan kolaborasi sangat penting. Kesimpulan: Walkrounds membantu mengidentifikasi masalah, mencegah kesalahan, meningkatkan komunikasi tim, dan melibatkan pasien dalam perawatan. Implementasi Walkrounds harus menjadi bagian dari strategi yang lebih luas, dengan pedoman yang jelas, kolaborasi antar organisasi, dan keterlibatan pasien untuk hasil yang optimal.Kata Kunci: Budaya keselamatan pasien, Leadership walkrounds, Rumah sakit Background: Healthcare services in hospitals should prioritize the quality of care and patient safety. Leadership walkrounds are an effective method to enhance patient safety culture and service quality through direct communication between managers and employees. It contributes to the development of employee skills, maintaining patient safety culture, improving employee engagement, and reducing burnout. The aim of this review is to assess the effectiveness of Leadership walkrounds in enhancing patient safety culture in hospitals.Methods: This study was conducted using the scoping review method following the Arksey and O'Malley framework. Publications from 2013 to 2023 were included in the search. Literature search was conducted in 3 electronic databases resulting in a total of 137 studies, which then resulting 6 articles will be included on this study. Analysis was performed on all identified studies, focusing on the impact and factors influencing the effectiveness of walkrounds.Results: The review findings indicate that Walkrounds are effective in improving patient safety, safety culture, employee engagement, and reducing fatigue. Proper implementation and active employee participation were crucial for the success of Walkrounds. Walkrounds were not a standalone solution; an integrated and comprehensive approach was also required. Strong commitment from leadership and an environment that supports open communication, feedback, and collaboration were highly important. Conclusion: Walkrounds help identify issues, prevent errors, improve team communication, and involve patients in care. Implementation should be part of a broader strategy, with clear guidelines, interorganizational collaboration, and patient involvement for optimal outcomes.Keywords: Hospital, Leadership walkrounds, Patient safety culture

Abstract

WHO reported that 30-50% of teenagers pregnance have until 19 year old in the world (2011) .in Indonesia, Teenagers having sex in Bandung, 54%, 51% Jakarta, and Surabaya 47% (JPNN, 2014). Campaign for teenagers condom use not accordance with Indonesian culture. Indonesia is not justified HIV prevention and teen pregnancy with condom use campaign. Phenomenological study method by in-depth interview nurses in the community about the experience of involving parents, teens, and peers to prevent free sex in adolescents. The result is that in order to prevent free sex in adolescents should involve parents, teens, and peers.

Abstract

Effective and efficient pharmaceutical logistics management is a crucial component in ensuring the quality of healthcare services in hospitals. The hospital pharmacy plays a strategic role in ensuring the timely availability of medicines, medical devices, and consumables in the right quantity, quality, and at an efficient cost. However, many hospitals still face challenges such as stock-outs, overstocking, expirations, and weak procurement and distribution systems. This study aims to identify and analyze various logistics management control strategies implemented in hospital pharmacies using a scoping review approach. A systematic literature review was conducted based on the PRISMA-ScR framework by searching articles in databases such as Scopus, ScienceDirect, and ProQuest for publications from 2016 to 2025. Out of 72 identified documents, 10 articles met the inclusion criteria. The results show that logistics control strategies can be categorized into five main approaches: conventional methods (EOQ, ROP), classification-based methods (ABC-XYZ), digital technologies (JIT, RFID), artificial intelligence (AI, Faster R-CNN), and predictive models (MPC). Each strategy presents its own advantages and challenges in terms of cost efficiency, data accuracy, infrastructure readiness, and human resource capacity. This review recommends strengthening logistics information systems, investing in technology, and enhancing human resource capabilities as key pillars to improve the effectiveness and sustainability of pharmaceutical logistics management in hospitals.

Abstract

The Hospital Infection Prevention and Control Program (HIPCP) is an effort to reduce the risk of Healthcare-Associated Infections (HAIs), including hand hygiene compliance. WHO issued the Multimodal Hand Hygiene Improvement Strategy as one of the strategies to tackle low hand hygiene compliance. Dr. M. Goenawan Partowidigdo Pulmonary Hospital (RSPG) has implemented hand hygiene regulations referring to the current policies, but it has not met the target over the last three years. This study aims to analyze the implementation of the regulation of hand hygiene compliance at RSPG according to the WHO Multimodal Hand Hygiene Improvement Strategy, using an analytical descriptive qualitative approach with a case study method. The analysis uses the combination of the theory of George Edward III, Weaver, and the WHO Multimodal Hand Hygiene Improvement Strategy. WHO Multimodal Hand Hygiene Improvement Strategy assessment was conducted by scoring the Hand Hygiene Self-Assessment Framework (HHSAF). The results showed the lowest percentage of HHSAF scores on the bureaucratic structure variable, but the most important variable was the resource variable, namely human resources, related to activeness and behavior issues. These issues were obtained from in-depth interviews developed from structured questions on HHSAF. Hand Hygiene of RSPG is at an intermediate level. Researchers recommend behavioral enforcement with appreciative inquiry in addition to reward and punishment as an effort to improve compliance with the implementation of hand hygiene regulations at RSPG.

Abstract

Since the implementation of the National Health Insurance, hospitals must be able to manage JKN claims properly. One way to properly manage the situation is by employing a medical internal verification doctor. This study aims to determine the factors that influence the performance of medical internal verification doctor based on the percentage of claim eligibility that has been verified. The research was conducted using a case study method with a qualitative and quantitative approach. The study was conducted on 16 medical internal verification doctors using questionnaires and in-depth interviews. The results of the study found that the performance of medical internal verification doctors in January-June 2022 was 81.45%, lower than other hospital owned by the ministry of health (85-94%). This study also found that the factors related to the verification performance of medical internal verification doctors were demographic in the form of years of service as medical internal verification doctors and other workloads, knowledge, job satisfaction in general, satisfaction with salary, benefits and rewards for work, satisfaction with the nature of work, as well as satisfaction with the operational situation of work and colleagues. Other factors in medical internal verification doctors verification performance is organizational support, such as standard operating procedures, training and education opportunities and opportunities for communication with external parties. Other factors related to the verification performance of VIM doctors are the passion as a medical internal verification doctor, tasks assignment, feedback on performance and criteria or standards for becoming a medical internal verification doctors.

Abstract

Delivering high-quality healthcare services relies on the performance of hospital staff. Various determinants, both internal and external, significantly impact this performance. While optimal performance typically fosters employee satisfaction, suboptimal performance can result in dissatisfaction, subsequently diminishing the overall organizational effectiveness of the hospital. The objective of this scoping review is to identify the factors influencing hospital employee performance and their satisfaction. This study adopted the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews guidelines. Literature searches was performed across three major databases: Scopus, PubMed, and ProQuest databases. 1,332 literature items identified from the selected databases. The quality of the literature was assessed using the Joanna Briggs Institute Critical Appraisal Tools. Fifteen literatures included in this review was of medium quality (score 7-8) and high quality (score 9-10) This review identified factors affecting hospital employee performance. Internal factors affecting employee performance include emotional intelligence, work stress, digital stress, work-family conflict, benefits, housing, rewards, satisfaction, and spirituality. External factors affecting employee performance include environment, hospital type, organization, performance management system, work conflict, flexi-time, open communication, toxic leaders, planning, and team focus. It was concluded that a holistic understanding of these variables is essential for hospital management to enhance service quality and organizational performance.

Abstract

Indonesia faces a critical shortage and maldistribution of medical professionals, particularly in Disadvantaged, Border, and Outermost (DTPK) regions. This study analyzes the effectiveness of the Ministry of Health's Educational Funding Assistance Program (Bandikdok) within the new regulatory framework of Law No. 17 of 2023. A qualitative Document and Policy Analysis was conducted. Data were collected from statutory regulations, Ministry of Health workforce reports (2022–2025), and a comparative review of successful retention models from Japan (Jichi Medical University) and Thailand (CPIRD). The analysis reveals that the program is supported by a robust legal foundation and has successfully lowered entry barriers, attracting over 2,278 participants through a centralized digital platform. However, the study identifies a critical structural weakness: the reliance on temporary Non-Civil Servant (Non-ASN) contracts and administrative sanctions creates a transactional "financial bond." This stands in contrast to the "social bond" models utilized in Japan and Thailand, which achieve higher retention through local recruitment and community integration. While the current policy effectively accelerates workforce production, long-term retention remains fragile due to the lack of career security. To ensure sustainability, the policy paradigm must shift from a punitive financial approach to a supportive ecosystem. Recommendations include decentralizing recruitment to create a "rural pipeline" and offering conditional Civil Service (ASN) tracks for graduates committed to long-term service in underserved areas.
