Abstract

The basic cause of death is used as an evaluation of a health service, knowing the general health status, reporting mortality and prevention of deadly diseases and preparing for medical needs in the future. The purpose of this research is to know the description of the accuracy of the coding accuracy of the cause of death on the perinatal case death certificate at RSIA Tiara. The research method used in this research is a descriptive mix method where a mix of quantitative and qualitative is by describing and explaining the accuracy of the UCoD coding on medical certificates causing death in perinatal cases. The population in this study were all medical records of babies who died in 2020, 2021 and 2022, totaling 32 medical records. The sample used in this study was a saturated sample where all populations were sampled, namely 32 medical records. The results of the presentation of the accuracy of the coding on the medical certification cause of death were as much as 69% while those that were not correct were as many as 31%. It is preferable to code all diagnoses in the prenatal case death certificate using the guideline and rules P1,P2,P3,P4 to give the correct code. Factors causing inaccuracy in codification of the underlying cause of death on the death certificate, the doctor's writing on the infant's medical resume in the final diagnosis section using cursive letters creates obstacles for the coding officer in determining the correct code. There is no special SPO for coding prenatal cases. Lack of human resources with a background in medical records.

Abstract

The development of the health care industry, one of which is hospitals, and the enactment of Permenkes 24 of 2022 concerning RME, which requires all health facilities to implement RME. This study aims to determine the readiness of inpatient RME implementation at Port Medical Center Hospital which is in the process of transmitting conventional medical records to RME. This study aims to determine the readiness of health facilities in implementing RME. Involving 100 respondents involving; Decision Making Team, Doctors, Outpatient and Emergency Room Units, Inpatient and ICU Units, Central Opname, Radiology Unit, Unit, Laboratory, Nutrition Unit, Physiotherapy Unit, Pharmacy Unit, and Information Systems Technology Unit. The research time was conducted October 2022 - June 2023. The data collection technique used a questionnaire. The results of the study, Readiness for Implementation of Inpatient EMR at Port Medical Center Hospital is in category II, which is quite ready with aspects of organizational alignment at a score of 24.91 and aspects of organizational capacity at a score of 44.24 in the category of quite ready. Readiness in the implementation of RME needs to be improved in terms of work culture regarding discussions of the framework, decision-making teams, discussions regarding accountants, and infrastructure improvements.

Abstract

Shrinkage is the reduction of medical records on storage shelves by moving inactive medical records to a different room from the active shelf seen from the last patient visit. The purpose of this study was to determine the implementation of shrinkage at the Kembangan Regional General Hospital. This research method uses a qualitative descriptive method by means of interviews and observations in the medical records unit. So it can be concluded from the results of the study that the Kembangan Regional General Hospital already has SOP related to shrinkage which is divided into SOP sorting, media transfer and destruction. In the implementation of shrinkage, officers carry out the process of sorting, transferring and transferring media every day. But officers do not conduct an assessment based on use value. The destruction process is carried out when the number of medical records has reached 2 tons by using a third party to make medical records in the form of pulp. The main obstacles to the implementation of shrinkage are officers who do not have a decree on duties and teams so that officers often delay the implementation of shrinkage, paper quality that makes it difficult for officers to see medical record sheets in the past year and scan machines that heat up quickly due to continuous use. Therefore, the Kembangan Regional General Hospital should complete the SOP related to shrinkage, make a decree of duties and conduct an assessment stage so that the implementation of shrinkage is carried out properly, and add a scan machine so that the implementation of media transfer is not delayed.

Abstract

Pending BPJS Health claims at RSIJ Pondok Kopi are divided into 3 (three) aspects including administration, coding and medical. This research aims to determine the factors of causes pending claims, resolution, and pending trends using a descriptive quantitative approach, which provides an overview of the factors causing pending claims and pending trends in 2023. Results with 1,143 samples of pending claim reports due to administration 523 (46%) with the most pending categories due to inappropriate treatment class 120 (23%), causative factors due to coding 570 (50%) with the most pending categories due to inaccurate diagnosis 525 (92.1%), and factors medical causes 50 (4%) with the most pending category due to inappropriate medical indications 36 (72%). So the pending settlement is carried out after receiving the verification results report by the casemix officer by completing the claim requirements according to the pending claim category and confirming with the DPJP if necessary. The pending trend in 2023 for 1st semester period January to June  showed decreasing results with a significant decrease in March of 12%. To minimize pending figures, hospitals should recruit employees according to their field of competence, empower existing human resources by clinical coding training, implement RME so that RM documentation is more complete, and internal verification officers are more thorough in carrying out their work.

Abstract

Online/distance learning is learning that is carried out without face-to-face meetings, or using assistive platforms. Offline or face-to-face learning is a learning system that does not use an internet connection or use a learning assistant platform, in other words students and lecturers meet face to face. This study aims to find out the differences and effectiveness of learning online and offline methods in disease classification and coding courses conducted by Medical Record Students at Esa Unggul University. This study uses a quantitative approach to the research method, namely inferential analysis using the t dependent test instrument. The effectiveness of online learning methods is 33 or 44.0%, and the effectiveness of offline learning methods is 40 or 53.3%. based on the results of the dependent t-test, a significant score of 0.000 was obtained between the effectiveness of online learning and the effectiveness of offline learning. So it can be concluded that there are differences between online learning methods and offline learning methods. So it is advisable to combine learning methods or use hybrid learning methods.

Abstract

ABSTRAK Latar Belakang : Kepuasan pasien merupakan tingkat perasaan pasien yang timbul sebagai akibat dari kinerja layanan kesehatan yang diperoleh dibandingkan dengan apa yang diharapkannya. Tujuan penelitian : mengetahui gambaran kepuasan pasien terhadap mutu pelayanan di bagian pendaftaran Puskesmas Kecamatan Penjaringan. Metode penelitian deskriptif dengan pendekatan analisis kuantitatif. Sampel dalam penelitian ini adalah pasien yang melakukan pendaftaran di TPP rawat jalan sebanyak 106 responden. Hasil penelitian : Petugas pendaftaran sudah menjalankan tugasnya sesuai dengan SPO yang berlaku. Karakteristik responden terbanyak Wanita (60%), Usia terbanyak antara 20 – 30 tahun (72%), Pendidikan terbanyak D4/S1 (34%). Hasil kepuasan pada pada dimensi Tangible didapatkan persentase terendah sebesar 15%, sedangkan persentase tertinggi sebesar 49%. Dimensi responsive didapatkan persentase terendah sebesar 12%, sedangkan persentase tertinggi sebesar 54%. Dimensi reliability didapatkan persentase terendah sebesar 12%, sedangkan persentase tertinggi sebesar 47%. Dimensi empathy didapatkan persentase terendah sebesar 7%, sedangkan persentase tertinggi sebesar 44%. Dimensi Assurance didapatkan persentase terendah sebesar 10%, sedangkan persentase tertinggi sebesar 42%. Maka persentase kepuasan pasien didapatkan pada kategori Sangat Memuaskan 32%, Memuaskan 50%, Cukup Memuaskan 15%, Kurang Memuaskan 2%, Tidak Memuaskan 0%. Kesimpulan : Maka didapatkan persentase pada kategori tertinggi yaitu yang menyatakan memuaskan sebesar 50%, sedangkan persentase terendah yaitu yang menyatakan kurang memuaskan sebesar 2%. Saran : Petugas pendaftaran lebih meningkatkan dan memahami dalam melayani dan menyampaikan informasi yang lebih tepat dan jelas. Kata Kunci : Kepuasan Pasien, Mutu Pelayanan, TPP. ABSTRACK Background Patient satisfaction is the level of patient feelings that arise as a result of the performance of health services obtained compared to what they expected. Descriptive research method with a quantitative analysis approach. The sample : in this study was 106 patients who registered at the outpatient TPP. Reserach results: resgistration officers have carried out their duties in accodance with the applicable SPO. Characteristics of most respondents: female (60%). Most aged between 20 – 30 years (72%), most education is D4/S1 (34%). The satisfaction results for the Tangible dimension obtained the lowest percentage of 15%, while the highest percentage was 49%. The lowest percentage obtained for the responsive dimension was 12%, while the highest percentage was 54%. The lowest percentage obtained for the reliability dimension was 12%, while the highest percentage was 47%. The lowest percentage obtained for the empathy dimension was 7%, while the highest percentage was 44%. The Assurance dimension obtained the lowest percentage of 10%, while the highest percentage was 42%. So the percentage of patient satisfaction was obtained in the category Very Satisfactory 32%, Satisfactory 50%, Quite Satisfactory 15%, Unsatisfactory 2%, Unsatisfactory 0%. Conclusion: So the percentage obtained in the highest category is 50% which states it is satisfactory, while the lowest percentage is 2% which states it is unsatisfactory. Suggestion: Registration officers improve and understand more in serving and conveying more precise and clear information. Keyword: patient satisfaction, service quality, TPP.

Abstract

Codification is the activity of classifying diseases based on certain criteria. Coding of disease diagnoses according to ICD-10 rules. One form of classification coding is using External Cause, which is a code used to classify the external cause of a disease caused by accidents, injuries, poisoning, burns, or side effects of drugs. This study was conducted at the Emergency Department of the Jakarta Islamic Hospital Pondok Kopi with the aim of the study to determine the percentage of accuracy of diagnosis and external cause codes and identify SPO related to classification coding. This study uses a quantitative descriptive method, with the technique of taking total sampling or saturated samples on 66 medical records for the period July 2022. The results of the research obtained with 66 samples related to traffic accident patients. Obtained the results of the accuracy of 66 appropriate injury diagnosis codes (100%) and 0 inappropriate (0%). The results of the accuracy of the external cause code obtained 66 (0%) external cause codes cannot be assessed because external cause coding activities have not been carried out. Only have SPO specifically for coding diagnoses in general, SPO specifically for external causes is not yet available. Of the 5 inhibiting factors studied, 3 inhibiting factors were obtained that caused the inaccuracy of external cause coding, namely Man, Money, and Material.

Abstract

Self-Registration Patient is a health service facility related to registration independently by patients who are provided by the machine. The purpose of this study is to identify what the user/patient needs and also to analyze the design for the doctor's appointment in hospitals. The method used a literature review approach through a database of Google Scholar obtained by 6 national-based journals, while Google Patent obtained 4 international-based patents. The results are related to the process of basic needs that will be used by users/patients such as QR Barcodes; touch screen monitors; Numpad and mouse; speakers; receipt printing machine; SEP paper printing; and CPU. The design supported by distribution model system such as; Entity-Relationship Diagram (ERD); Data Flow Chart (DFD); Use Case Diagram (UCD); flow chart, and interface design. In conclusion, the research could help the hospital's needs and also the patients, including knowing the latest status of patients, making appointments according by intended polyclinic, doctors, and time visits based on doctor's time schedule. It is also could print SEP as well as the pharmacy queue number receipt after being examined. The system is also expected to reduce long queues at the Outpatient Registration Place (TPPRJ), especially for BPJS Insurance users.

Abstract

The completeness of filling in the medical record can be checked using a documentation audit, one of which is quantitative analysis by reviewing certain parts of the contents of the medical record with the intention of finding deficiencies, especially those related to medical record documentation. The purpose of this study was to get an overview of the completeness of filling out inpatient medical records at the Kembangan Regional General Hospital. This research method uses a descriptive method with a quantitative approach. The population in this study were inpatient medical records at the Kembangan Regional General Hospital with a sample of 89 medical records. Based on the results of calculations using quantitative analysis obtained an average completeness of 82%, the highest completeness is the patient identification component of 87% and the lowest completeness is the important reporting component of 76%. Based on the research that has been done, it can be concluded that filling in the medical record at the Kembangan Regional General Hospital is not complete, this is due to the lack of awareness of health workers to complete filling in the medical record so that it has an impact on incomplete filling in medical record files

Abstract

Personal Health Record (PHR) is a collection of health-related data that is archived and managed by an individual or parties associated with the PHR. Its purpose is to provide a comprehensive and accurate summary of a person's health history that can be accessed online. This research is a Literature Review study which aims to determine the features and implementation of Personal Health Records in Patients with Diabetes Mellitus. The method used is the Literature Review method from 7 journals which uses several methods including the prototype method, qualitative methods, heuristic evaluation methods, applied development methods, participatory action research methods, and randomized controlled trial methods with data collection techniques through google scholar, direct science and pubmed. The results of the research are seen in the features and implementation of the Personal Health Record. The Personal Health Record feature for diabetes mellitus with dialysis measures has 25 features on the main page, the PHR feature for diabetes mellitus has 7 main features. PHR implementation allows users to access their personal health records anytime and anywhere. Thus, the PHR system can increase accessibility and allow users to access PHR without time restrictions and also the Personal Health Record can assist in monitoring the health condition of Diabetes Mellitus patients. Research conclusion According to the journals analyzed, PHR is effective because it can encourage patient participation in individual health controls, and can access information about diabetes mellitus through monitoring physical activity, diet, weight, and glucose levels.
