Abstract

Kanker payudara merupakan keganasan paling sering dan penyebab kematian tertinggi. Di Indonesia, kanker payudara juga merupakan kanker paling sering dan penyebab kematian kedua setelah kanker paru. Hal ini tejadi karena kanker payudara sering terdiagnosis pada stadium lanjut sehingga menyebabkan kegagalan pengobatan dan kematian. Kejadian kanker payudara stadium lanjut ini dapat dicegah dengan deteksi dini, namun masyarakat masih mempunyai pengetahuan yang rendah tentang tanda awal kanker payudara. Tujuan dari kegiatan ini adalah untuk memberikan penyuluhan tentang tanda awal dan deteksi dini tumor ganas di payudara. Kegiatan dilaksanakan pada 11 November 2020, diikuti oleh 91 orang ibu PKK di wilayah kerja Pusat Kesehatan Masyarakat (Puskesmas) Sememi, Kecamatan Benowo Kota Surabaya. Kegiatan diawali pre-test dan pengisian kuisioner faktor risiko, kemudian dilanjutkan dengan pemberian penyuluhan melalui aplikasi zoom meeting dan diakhiri dengan post-test. Evaluasi dilakukan dengan membandingkan hasil pre-test dan post-test. Hasil kuisioner menunjukkan terdapat peningkatan pemahaman tentang tanda awal dan deteksi dini kanker payudara sebesar 6,96%. Faktor risiko peserta menderita kanker payudara menunjukkan bahwa sebagian besar peserta penyuluhan tidak berisiko menderita kanker payudara karena tidak merokok (100%), tidak mempunyai riwayat benjolan pada payudara (97,14%), menyusui lebih dari 6 bulan (88,57%), tidak terpapar radiasi sinar X (88,57%), mempunyai anak (85,71%), tidak mempunyai riwayat keluarga yang pernah menderita tumor atau kanker (71,43%), rutin berolah raga (60%), namun terdapat 97,14% yang berusia lebih dari 25 tahun sehingga masih mempunyai faktor risiko menderita kanker payudara. Kesimpulan. Terdapat peningkatan pengetahuan masyarakat sebesar 6.96% dan sebagain besar peserta penyuluhan tidak memiliki faktor risiko untuk terjadi kanker payudara.

Abstract

Introduction: Breast cancer is still a burden by being the most frequent malignancy and the highest cause of cancer death in women. The death rate from breast cancer can be reduced by early case finding and appropriate immediate treatment. Early case finding can be started with a simple periodic examination that can be carried out by each individual independently, namely by breast self-examination. This study aims to determine the effect of education on early detection and screening of breast cancer on the level of knowledge among women in Gandul village, Madiun district. Methods: This study involved 101 participants aged from 25 to 87 years old, including female residents of Gandul village and cadres of public health center.  We conduct health education about breast cancer and early detection of breast cancer, especially regarding breast self-examination. Pre-test and post-test were also conducted to assess the the knowledge of the participants before and after the seminar held. Results: The pre-test score of participants with low scores (<75) was 78.21% (79 people) and decreased in the post-test to 43.56% (44 people), while participants with high scores (>75) were only 21.78% (22 people) in the pre-test and increased to 56.44% (57 people) in the post-test. Conclusion: There is a knowledge improvement about the early detection of breast cancer among participants after the intervention. The knowledge improvement can be seen through the increased score from pre-test to post-test.

Abstract

This study aims to increase understanding about early detection of cervical cancer and pap smear evaluation among residents, as well as to increase the pap smear sampling skills by health workers in Pilangkenceng District, Madiun Regency. This study was a descriptive observational study using a cross-sectional approach which was conducted on 100 women, from the Pilangkenceng Primary Health Center in Madiun Regency, aged 20 to 70 years. Pretest and post test assessments were carried out before and after the education of cervical cancer. Specimen adequacy assessment was carried out on 100 Pap smear samples taken by the health workers based on the Papaniculaou classification and the Bethesda system. Pap smear results were also compared with the results of the visual inspection of acetic acid (IVA) which was done before. Education of cervical cancer to the community can increase people's understanding of cervical cancer by 8.05% to 82.26%, while training for health workers on the technique of pap smear sampling shows an increase in knowledge of 51.62% to 100%. Adequacy of pap smear samples by health workers in Pilangkenceng District, Madiun Regency showed that 99% were adequate and satisfactory. Ten percent of IVA examinations showed positive results and those cases showed signs of inflammation and infection on pap smear examination. Community service activities of early detection of cervical cancer through the pap smear method can increase the understanding of early detection of cervical cancer in the community and the knowledge and sampling skills of health workers, hence the health workers at the Pilangkenceng Primary Health Center are ready to provide pap smear services for the community.   Keywords: adequacy, early detection, pap smear, IVA, cervical cancer.   Abstrak Kegiatan ini bertujuan untuk mengetahui peningkatan pemahaman tentang deteksi dini kanker serviks dan serta evaluasi pap smear pada warga, serta peningkatan kemampuan dalam melakukan pap smear oleh tenaga kesehatan di Kecamatan Pilangkenceng Kabupaten Madiun. Penelitian observasional deskriptif dilakukan dengan metode pendekatan cross sectional yang dilakukan pada 100 wanita, dari wilayah Puskesmas Pilangkenceng Kabupaten Madiun, dengan usia 20-70 tahun. Penilaian pretest dan post test dilakukan sebelum dan sesudah penyuluhan. Penilaian adekuasi spesimen dilakukan pada 100  sediaan  pap smear  yang diambil oleh para tenaga berdasarkan klasifikasi Papaniculaou dan sistem Bethesda. Hasil pap smear juga dibandingakan dengan hasil pemeriksaan inspeksi visual asam asetat (IVA) yang dilakukan sebelumnya. Penyuluhan terhadap masyarakat dapat meningkatkan pemahaman masyarakat tentang kanker serviks sebesar 8,05% menjadi 82,26%, sedangkan pelatihan pada tenaga kesehatan tentang tehnik pengambilan pap smear menunjukkan peningkatan pengetahuan sebesar 51,62% menjadi 100%. Adekuasi sediaan pap smear oleh tenaga kesehatan Kecamatan Pilangkenceng Kabupaten Madiun menunjukkan bahwa 99% adekuat dan memuaskan. Sepuluh persen pemeriksaan IVA menunjukkan hasil positif  dan setelah dilakukan hapusan  pap smear, kasus IVA positif tersebut menunjukkan gambaran proses keradangan dan infeksi. Kegiatan pengabdian masyarakat dapat meningkatkan pemahaman tentang deteksi dini keganasan serviks pada masyarakat dan pengetahuan para tenaga kesehatan tentang deteksi dini kanker serviks melalui metode pap smear, sehingga diharapkan bahwa tenaga kesehatan di Puskesmas Pilangkenceng siap memberikan pelayanan pap smear bagi warga. Abstract

HIGHLIGHTS This study aimed to identify the characteristics of Gestational Trophoblastic Neoplasia (GTN). GTN is chemosensitive, but without appropriate therapy and follow-up, GTN will develop into complications and fatalities.   ABSTRACT Objective: Gestational Trophoblastic Neoplasia (GTN) is a pregnancy-related malignancy due to abnormal proliferation of trophoblastic tissue. This study aimed to identify the characteristics of patients with GTN to help diagnose cases of GTN earlier and provide better treatment. Materials and Methods: This was a descriptive retrospective study on medical records of patients with GTN in Dr. Soetomo General Academic Hospital Surabaya, Indonesia, during the period of January 2018 to December 2020 with a total sampling technique. There were 41 patients with GTN included as study subjects.  Results: Forty-one cases of GTN met the inclusion criteria out of the fifty medical records collected. The majority of patients aged 21 – 30 years old (34%) and had parity status without data (42%). Regarding the clinical profile based on prognostic factors, the predominant patients (71%) also had no data about the time interval between the end of the last pregnancy and the first time diagnosed by GTN, Those with more than 100,000 mIU/ml of beta-hCG levels were 32%, and those without metastases were 41.5%. Most patients belonged to the low-risk group (49%) and received chemotherapy (71%) with the MTX LD regimen (69%).  Conclusion: GTN occurred predominantly in reproductive women that belonged to the low-risk group. Furthermore, chemotherapy is one of the chosen therapy for those patients.

Abstract

HIGHLIGHTS Squamous cell carcinoma majorly covered in histopathologic of the records, while adenosquamous followed second. Most of the subjects were referral patients to Dr. Soetomo General Academic Hospital. They were mostly originated from Java outside Surabaya.   ABSTRACT Objective: To identify the distribution of age, histopathology type, clinical stage, treatment type, parity, first complaint, and referral origin of geriatric cervical cancer patients at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia. Materials and Methods: This study was a retrospective study observing data from medical records and presented the data descriptively. The medical records were taken from Oncology Outpatient Clinic in Dr. Soetomo Hospital, from 2020-2021, covering geriatric patients aged more than 55 years old. The data were screened and processed. Results: At Dr. Soetomo Hospital, in 2020-2021 there were 228 cervical cancer patients. From 176 patient data that met the inclusion criteria, the average age was 65.38 ± 4.86 years, with 4 types of histopathology dominated by squamous cell carcinoma (82.39%), adenocarcinoma (11.93%), adenoquamous (3.41%) and others (4%), divided into 8 clinical stages and dominated by stages IIIB (77.27%), IIB (15.9%), IVB (2.84%), IIIA and IB had same number (1.14%), 1A (0.57%) and no cases of IIA were found. The treatments were dominated by chemotherapy (86.36%), radiation therapy (7.38%), no treatment (3.41%), hysterectomy (1.7%), while for conization and palliative therapy each in 1 case (0.57%). Most experienced 3 parity (29.5%), followed by 4 parity (17.61%), >5 (13.07%), 5 (10.23%), 1 (6.82%) and no parity (2.27%). The three first complaints were. vaginal bleeding (82.38%), vaginal discharge (46.02%), and pain (82.38%), and the patients were dominated by referrals from Java Island other than Surabaya City (78.40%), outside Surabaya in Java Island as many as 36 referrals (20.45%) and outside Java Island 2 referrals (1.14%). Conclusion: There were 176 geriatric patients with cervical cancer at Dr. Soetomo General Academic Hospital, Surabaya, Indonesia, in the 2020-2021 period, dominated by age 56-65 years, the histopathology type of squamous cell carcinoma, stage IIIB patients, most received therapy was chemotherapy, most were multiparous with 3 parities, the majority experienced complaints of vaginal bleeding when diagnosed with cervical cancer, and were dominated by referrals from Java Island outside Surabaya.

Abstract

Excessive release of interleukin-6 (IL-6) during the progression of coronavirus disease 2019 (COVID-19) induces cytokine storms, resulting in multi-organ damages including liver injury, similar in nature with mechanism of viral hepatitis. Systemic IL-6 has been associated with the incidence of liver injury among COVID-19 patients; however, studies on IL-6 expression in the liver tissue are completely lacking. The aim of this study was to measure the IL-6 expression in the liver tissues and to determine its correlation with the degree of liver injury in fatal COVID-19 patients. Through this first cross-sectional study, IL-6 expression was measured through immunohistochemical staining and the degree of liver injury was identified based on level of serum alanine aminotransferase (ALT). The Spearman correlation test was used to identify the correlation between IL-6 expression and the degree of liver injury. A total of 47 deceased COVID-19 patients were included and IL-6 expression was observed in all post-mortem liver specimens, ranging from mild to strong expression. Liver injury at various degrees (mild to severe) was found in more than half (59.5%) of the cases. The Spearman correlation analysis suggested a statistically insignificant correlation between liver IL-6 expression and the degree of liver injury (r=0.152; p=0.309). In conclusion, even IL-6 expression was observed in all post-mortem liver specimens, there was an insignificant correlation between IL-6 expression in the liver tissue with the degree of liver injury among fatal COVID-19 patients, suggesting that IL-6 was not the only main factor contributing to liver damage in COVID-19 patients.

Abstract

Breast cancer is the most common cancer found in the world and this cancer is the second cause of death due to cancer after lung cancer in Indonesia. This high mortality rate can be prevented by carrying out early detection. However, this is still not done well, especially in communities living in remote areas with low to medium economic and educational levels. The aim is to convey knowledge about breast cancer and training in the self-examination of the breast. It was held on Tuesday, 12 September 2023 at the Ngluyu Community Health Center, Nganjuk, from 07.00 to 14.00 WIB and was attended by 111 people. Data on breast cancer risk factors was collected through questionnaires. There were 96 participants who filled out the questionnaire, aged between 22-63 years. Risk factors data showed that 3.13% did not have children, 15.63% breastfed children less than 6 months, 5.21% were exposed to X-rays more than once a year; 1.04% had breast tumors, and 4.17% had family members suffering from cancer. In addition, 89.58% were over 25 years old and 42.71% had doing self-examination of breast. There was an increase in public understanding at the Ngluyu Community Health Center from 77.70% to 84.12%, namely an increase of 6.42%. This can increase public understanding about breast cancer and procedures to self-examination of breast and contribute in early detection of breast cancer as well as reducing the incidence of breast cancer. The results of this counseling can be used as recommendations by health centers in the Nganjuk district area.

Abstract

Background: Papillary thyroid microcarcinoma (PTmC) is a subtype of papillary thyroid carcinoma characterized by a tumor that is no larger than 10 mm. This tumor is frequently discovered by accident during autopsies or in thyroidectomy specimens for other thyroid disorders. This study was conducted to ascertain the incidence of incidental PTmC in thyroidectomy specimens and analyze the clinicopathological characteristics.Methods: A descriptive cross-sectional study design was used. Pathology archives of thyroidectomy specimens between 2012 and 2019 in a single institution were collected. Cases with fine-needle aspiration biopsy (FNAB) before surgery were also studied. The clinical information was retrieved from medical records. Results: 1459 patients underwent thyroid surgery. The clinical diagnosis in 706 cases was neoplasm and in 753 cases was non-neoplasm. There were 697 cases of malignant thyroid tumors. Forty-two cases of papillary thyroid microcarcinoma were found, with 85.71% affected females, with the male-to-female ratio was 1:6. The majority of PTmC were clinically diagnosed as benign lesions with multinodular goiter as the most common diagnosis. Fine needle aspiration biopsies were performed on 18 patients. There were 2 cases found in laryngectomy specimens for laryngeal cancer. There were 2 cases presented initially as metastasis nodules in lymph nodes and the lung, respectively.Conclusions: PTmC comprises 6.03% of all malignant thyroid neoplasms and is mainly found incidentally in thyroidectomy specimens for benign thyroid lesions with clinical diagnosis as multinodular goiter. The male-to-female ratio is 1:6. Regional and distant metastasis are not common in PTmC.

Abstract

Penelitian ini bertujuan untuk meningkatkan pemahaman masyarakat tentang deteksi dini kanker serviks dan evaluasi Pap smear, serta meningkatkan keterampilan pengambilan sampel Pap smear pada penyedia layanan kesehatan di Kecamatan Mawaddah, Kabupaten Mojokerto. Studi deskriptif observasional dengan desain cross sectional dilakukan pada 100 wanita berusia 20-70 tahun dari Rumah Sakit Mawaddah. Penilaian dilakukan sebelum dan sesudah pemaparan materi. 100 sampel Pap smear yang dikumpulkan oleh bidan setempat dievaluasi untuk menilai kecukupan spesimen menggunakan sistem Papanicolaou dan Bethesda. Kegiatan ini secara signifikan meningkatkan pemahaman tentang kanker serviks (8,05% menjadi 82,26%), sementara kegiatan pelatihan tenaga medis meningkatkan pengetahuan mengenai pengambilan sampel Pap smear (51,62% menjadi 100%). 99% dari sampel Pap smear yang dikumpulkan oleh tenaga medis di Kecamatan Mawaddah dinilai memadai dan memuaskan. Program pengabdian masyarakat yang berfokus pada deteksi dini kanker serviks melalui Pap smear, secara efektif meningkatkan pemahaman masyarakat, meningkatkan pengetahuan dan keterampilan pengambilan sampel oleh tenaga medis, yang dapat meningkatkan pula efektifitas layanan Pap smear di Rumah Sakit Mawaddah pada masyarakat.

Abstract

Kegiatan pengabdia masyarakat ini bertujuan untuk meningkatkan pemahaman tenaga kesehatan dan masyarakat mengenai deteksi dini serta pencegahan kanker serviks. Pelatihan tenaga kesehatan dilakukan agar mereka dapat melakukan prosedur Pap smear dengan benar dan sesuai standar, sementara penyuluhan masyarakat bertujuan untuk meningkatkan kesadaran akan pentingnya deteksi dini dan pencegahan kanker serviks. Studi ini menggunakan metode deskriptif observasional dengan pendekatan cross-sectional, mencakup dua kegiatan utama: pelatihan tenaga medis secara daring dan penyuluhan masyarakat secara langsung. Pelatihan Pap smear diikuti oleh 51 peserta yang terdiri dari perawat, bidan, dan dokter. Evaluasi sebelum dan sesudah pelatihan menunjukkan peningkatan pemahaman yang signifikan, di mana peserta dengan skor 80-100 meningkat dari 58,82% menjadi 82,35%. Penyuluhan kanker serviks diikuti oleh 74 peserta. Sebelum penyuluhan, hanya 12,16% peserta yang memperoleh nilai 80-100, namun setelahnya meningkat menjadi 100%. Sebanyak 62,2% peserta telah menikah, 8,1% memiliki riwayat hubungan seksual sebelum usia 17 tahun, dan 9,5% memiliki lebih dari satu pasangan. Selain itu, 98,6% belum pernah menerima vaksin HPV, dan 78,4% jarang melakukan Pap smear, menunjukkan rendahnya kesadaran pencegahan kanker serviks. Setelah penyuluhan, 60 peserta menjalani skrining kanker serviks. Mayoritas (41,7%) berusia 31-40 tahun. Hasil skrining dengan sistem Bethesda menunjukkan 100% NILM. Berdasarkan klasifikasi Papanicolaou, 88,1% termasuk kelas II (96,2% inflamasi, 1,9% bacterial vaginosis, 1,9% atrofi), dan 11,1% kelas I. Program ini terbukti efektif dalam meningkatkan pemahaman tenaga kesehatan dan masyarakat, serta keterampilan tenaga kesehatan dalam prosedur Pap smear, yang berkontribusi pada peningkatan kualitas layanan deteksi dini kanker serviks di RSIA Hikmah Sawi Bangkalan.
