Abstract

AbstrakLeukemia merupakan penyakit keganasan sel darah yang berasal dari sumsum tulang ditandai oleh proliferasi sel-sel darah putih, dengan manifestasi adanya sel-sel abnormal dalam darah tepi. Pada tahun 2006, leukemia berada pada urutan ke-5 dari keseluruhan penderita kanker di Indonesia. Leukemia kronik merupakan leukemia yang paling sering terjadi pada dewasa dan lanjut usia. Secara umum leukemia kronik diklasifikasikan atas Leukemia Granulositik Kronik (LGK) dan Leukemia Limfositik Kronik (LLK). Leukemia kronik yang perjalanannya lambat dan diiringi oleh gejala yang tidak khas, maka penelitian ini bertujuan untuk mengetahui gambaran laboratorium leukemia kronik di bagian Peyakit Dalam RSUP DR. M. Djamil Padang. Jenis penelitian ini adalah deskriptif retrospektif. Instrumen yang digunakan pada penelitian ini adalah data sekunder yang diperoleh dari Instalasi Rekam Medik RSUP Dr. M. Djamil Padang berupa data pasien leukemia kronik yang dirawat di Bagian Penyakit Dalam RSUP Dr. M. Djamil Padang sejak 1 Januari 2010 – 31 Desember 2012. Hasil penelitian ini menunjukkan bahwa dari 16 kasus leukemia granulositik kronik terdapat 37,5% pasien mengalami anemia sedang, 100% leukositosis, jumlah trombosit dapat menurun, normal, dan meningkat dengan presentase masing-masing 25%, 25%, dan 50%. Gambaran eritrosit sebagian besar normositik anisositosis. Separuh pemeriksaan darah tepi menunjukkan peningkatan persentasi mielosit, 31,25% menunjukkan peningkatan persentasi metamielosit dan eosinofil, serta sebagian besar menunjukkan presentasi blast. Sedangkan gambaran sumsum tulang hiperseluler, penekanan eritropoetik, mielopoetik hiperaktif, dan trombopoetik dalam batas normal. Leukemia limfositik kronik yang terdiri dari 1 kasus menunjukkan gambaran laboratorium berupa anemia sedang, leukositosis, trombositopenia, gambaran eritrosit nomokrom anisositosis, peningkatan jumlah leukosit, peningkatan jumlah limfosit, presentasi smudge cell, dan ditemukan presentasi blast pada darah tepi, tetapi selularitas tidak dapat dinilai.Kata kunci: leukemia kronik, darah tepi, BMPAbstractLeukemia is a malignant disease of blood cells derived from the bone marrow characterized by the proliferation of white blood cells, with the manifestation of the abnormal cells in the peripheral blood. In 2006, leukemia was ranked 5th of all cancer patients in Indonesia. Chronic leukemia is the most common leukemia in adult and the elderly. In general, chronic leukemia classified on chronic myelocytic leukemia (CML) and chronic lymphocytic leukemia (CLL). The onset of chronic leukemia is insidious and accompanied by symptoms that are not typical, this research aims to describe the laboratory findings of chronic leukemia patients treated at Internal Medicine Department of Dr. M. Djamil Hospital Padang.This research is a retrospective descriptive research. The instruments used in this research are the secondary data derived from the Medical Record Departement Dr. M. Djamil Hospital Padang in the form of leukemia chronic patients’ data who were treated in Internal Medicine Department of Dr. M. Djamil Hospital Padang since January 1st 2010 – December 31st 2012. The results of this research showed that of 16 cases of chronic myelocytic leukemia contained 37.5% of the patients had moderate anemia, leukocytosis 100%, platelet count can be decreased, normal, and increased the percentage of each 25%, 25%, and 50%. The morphology of erythrocytes mostly normocytic anisocytosis. Half of peripheral blood examination showed an increase in the percentage of myelocyte, 31.25% showed an increase in the percentage metamyelocyte and eosinophils, as well as most of the shows presentation blast. The bone marrow are hypercellular, compressing erythropoietic, myelopoietic hyperactivity and thrombopoietic mostly normal in number. Chronic lymphocytic leukemia consisting of 1 case shows the laboratory findings are moderate anemia, leukocytosis, thrombocytopenia, the morphology of erythrocyte is normochromic anisocytosis, leukocytes increase in number, increase in the number of lymphocytes, presentations smudge cells, and blast presentation is found in the peripheral blood, but the cellularity not be assessed.Keywords: chronic leukemia, peripheral blood, BMP

Abstract

Background Adipose tissue is not merely a site for energy storage, but is also the largest endocrine organ, secreting various adipocytokines. Plasma visfatin, an adipocytokine predominantly secreted from visceral adipose tissue, has insulin-mimetic effects, and has been closely linked to insulin resistance.Objective To compare plasma visfatin levels between obese and non-obese adolescents, as well as between obese adolecents with and without insulin resistance.Methods This cross-sectional study was conducted in students who attended three senior high schools in Padang. Subjects comprised 28 obese and 28 non-obese adolescents. The age of the subjects ranged from 14-18 years. Obesity criteria were based on body mass index (BMI) measurements. Fasting serum glucose level was measured by glucose hexokinase photometry and serum insulin was measured by chemiluminesence immunoassay. Plasma visfatin was measured by enzyme-linked immunosorbent assay (ELISA). The insulin resistance index was estimated from fasting serum insulin and glucose levels using the homeostatic model assessment for insulin resistance (HOMA-IR). Differences in the variables were tested using independent T-test and Mann-Whitney test, depending on the distribution of the variables.Results The mean plasma visfatin level was significantly higher in the obese than in the control group [2.55 (SD 1.54) vs. 1.61 (SD 0.64) ng/mL, respectively; (P=0.005)]. The insulin resistant group had significantly higher mean plasma visfatin level than the non-resistant group [3.61 (SD 1.59) vs. 1.96 (SD 1.18) ng/mL, respectively; (P=0.004)].Conclusion Obese adolescents with insulin resistance have signifcantly higher plasma visfatin levels compared to those without insulin resistance.

Abstract

NS1 protein is a nonstructural protein of dengue virus which are secreted into the blood. NS1 protein could be detected in blood upto nine days after the onset of illness that almost simultaneously with the occurrence of viremia. The purpose of this study is to knowthe result of diagnostic test method of immunocytochemical NS1 by RT-PCR on dengue virus infected patients which is diagnosed at Dr.M. Djamil hospital. Padang. The method of the research uses cross sectional analytically study with consecutive sampling of patientswho diagnosed as infected with dengue virus and treated in the interne ward of Dr. M. Djamil General Hospital Padang from Januaryup to August 2011. Patient ‘s venous blood was taken about 5 mL and then centrifuged 3500 rpm and then the serum is separated andstored at-20° C until the sample is examined. The examination of NS1 serum is carried out by immunocytochemical method followedby the inspection of RNA viruses and their serotypes by RT-PCR and continued by agarose gel electrophoresis on 1.5 up to 2%. From thesixty samples obtained showed: 71.6% tested positive immunocytochemical NS1, 28.4% negative immunocytochemical NS1, 76.7%positive RT-PCR and 23.3% negative RT-PCR, 23.9% Den-1, 43.3% Den-2, 26.1% Den-3, and 6.5% Den-4. The immunocytochemicalNS1 diagnostic test obtained RT-PCR sensitivity of 85%, specificity 71%, positive predictive value 90%, and negative predictive value59%. The immunocytochemical NS1 has a quite high sensitivity, and low specificity, where as the positive predictive value is quite high,but the negative predictive value is lower than the RT-PCR for the diagnosis of dengue virus infection.

Abstract

Asidosis metabolik adalah kasus yang paling sering ditemukan pada pasien critically ill. Pendekatan Henderson-Hasselbalch gagalmenjelaskan gangguan metabolik yang rumit karena hanya tertuju pada kenasaban pH dengan tekanan parsial karbondioksida dan ionbikarbonat. Pendekatan keseimbangan asam-basa metode Stewart lebih akurat dan dapat menggambarkan gangguan metabolik yangrumit. Modifikasi metode Stewart yang digunakan saat ini adalah metode Figge-Stewart dan Fencl-Stewart yang disederhanakan, dapatdigunakan di tempat dengan sumber daya terbatas. Tujuan penelitian ini adalah mengetahui kesesuaian metode Fencl-Stewart yangdisederhanakan dengan Figge-Stewart dalam mendiagnosis asidosis metabolik di pasien critically ill. Penelitian analitik potong lintangterhadap 40 pasien critically ill yang dirawat di ICU, CVCU dan HCU RSUP Dr. M. Djamil Padang masa waktu Oktober–November 2015.Analisis gas darah diukur dengan potensiometri, elektrolit dengan ion selective electrode dan kadar albumin dengan immunoturbidimetri.Hasil dianalisis dengan program komputer. Kesesuaian metode Fencl-Stewart yang disederhanakan dengan Figge-Stewart dianalisisdengan uji Kappa, bermakna jika nilai p<0,05. Sebanyak 40 orang pasien (18 laki-laki, 22 perempuan) diikutkan dalam penelitian ini.Rerata umur pasien adalah 48,35(18,4) tahun dan diagnosis terbanyak adalah gagal jantung (30%). Hiponatremia, hipernatremia,hipokalemia, hiperkalemia, hipokloremia dan hipoalbuminemia ditemukan masing-masing sebanyak 37,5%, 12,5%, 2,5%, 12,5%,17,5%, 20% dan 87,5% pasien. Terdapat kesesuaian yang baik antara metode Fencl-Stewart yang disederhanakan dengan Figge-Stewartdengan nilai kappa=0,529 dan bermakna secara statistik (p<0,001). Kesesuaian metode Fencl-Stewart yang disederhanakan denganFigge-Stewart dalam mendiagnosis asidosis metabolik di pasien critically ill adalah baik.

Abstract

Acute Myocardial Infarction (AMI), one of the primary manifestation of coronary heart disease, is a significat cause of death worldwide. Hyperhomocysteinemia, a risk factor for cardiovascular disease, is caused by nutritional or genetic disturbances in homocysteine metabolism. The role of hyperhomocysteinemia in altered lipid metabolism presumed holds the key to an increased risk of cardiovascular disease. Hyperhomocysteinemia causes the reduction of serum High-Density Lipoprotein (HDL) cholesterol level by inhibiting hepatic synthesis of apo-A1 (significant apolipoprotein HDL). The aim of this study was to know the correlation between hyperhomocysteinemia and decreased HDL cholesterol levels for the management of cardiovascular disease risk factors. This research was an analytical study with cross-sectional design in 40 patients AMI who meet the inclusion and exclusion criteria and conduct blood test at the Central Laboratory of Hospital Dr. M. Djamil Padang and Biomedical Laboratory Faculty of Medicine Andalas University. The study was conducted in May 2016-Agustus 2017. Homocysteine level was measured by ELISA method. High-Density Lipoprotein level was performed by enzymatic colorimetric method. Data were analyzed by Spearman’s correlation test. Research subjects were 40 people with male gender 30 (75%) and female 10 (25%), mean age 61.08 (11.09) year. The mean level of HDL cholesterol in patients with AMI is 41.93 ± 13.12 mg/dL. The mean level of homocysteine in patients with AMI is 25.36 ± 22.2 µmol/L. Spearman’s correlation test showed a strong correlation between the levels of homocysteine and HDL cholesterol with r=-0.603 and p<0.01.

Abstract

Metabolic acidosis is prevalent among critically ill patients and the common cause of metabolic acidosis in ICU is lactic acidosis. However, not all ICUs can provide lactate measurement. The traditional method that uses Henderson-Hasselbach equation (completed with BE and AG) and alternative method consisting of Stewart and its modification (BDEgap and SIG), are acid-base balance parameters commonly used by clinicians to determine metabolic acidosis in critically ill patients. The objective of this study was to discover the association between acid-base parameters (BE, AGobserved, AGcalculated, SIG, BDEgap) with lactate level in critically ill patients with metabolic acidosis. This was an analytical study with a cross-sectional design. Eighty-four critically ill patients hospitalized in the ICU department Dr. M. Djamil Padang Hospital were recruited in this study from January to September 2016. Blood gas analysis and lactate measurement were performed by potentiometric and amperometric method while electrolytes and albumin measurement were done by ISE and colorimetric method (BCG). Linear regression analysis was used to evaluate the association between acid-base parameters with lactate level based on p-value less than 0.05. Fourty five (54%) were females and thirty-nine (46%) were males with participant’s ages ranged from 18 to 81 years old. Postoperative was the most reason for ICU admission (88%). Linear regression analysis showed that p-value for BE, AGobserved, AGcalculated, SIG and BDEgap were 119; 0.967; 0.001; 0.001; 0.689, respectively. Acid-base balance parameters which were mostly associated with lactate level in critically ill patients with metabolic acidosis were AGcalculated and SIG.

Abstract

Nefropati diabetik merupakan komplikasi mikrovaskular di pasien Diabetes Melitus (DM) yang menyebabkan End Stage RenalDisease (ESRD). Proses glikasi non-enzimatik asam amino bebas di hiperglikemia kronis menghasilkan advanced glycation end-products(AGEs). Advanced glycation end-products dimetabolisme di ginjal sehingga terjadi penumpukan yang memicu kerusakan glomerulusginjal. Advanced glycation end-products terbukti berperan dalam perjalanan penyakit nefropati diabetik. Pemeriksaan mikroalbuminuriadengan rasio air kemih albumin kreatinin merupakan pemeriksaan yang disarankan untuk mendeteksi nefropati lebih awal. Tujuanpenelitian ini untuk mengetahui kenasaban kadar AGEs dengan rasio air kemih albumin kreatinin di pasien DM tipe 2. Penelitian inimenggunakan metode analitik dengan desain potong lintang di 30 orang pasien DM tipe 2 yang memenuhi patokan kesertaan dannonkesertaan serta melakukan pemeriksaan darah di Instalasi Laboratorium Sentral RSUP Dr. M. Djamil Padang masa waktu Mei2015-Maret 2016. Pemeriksaan kadar AGEs dilakukan dengan metode sandwich ELISA. Pemeriksaan mikroalbumin air kemih dilakukandengan metode imunoturbidimetri. Data dianalisis dengan uji kenasaban Spearman, bermakna jika p<0,05. Rerata kadar AGEs di DMtipe 2 adalah 1052,18±750,25 ng/L. Rerata nilai rasio air kemih albumin kreatinin di pasien DM tipe 2 adalah 23,77±16,58 mg/g.Uji kenasaban Spearman menunjukkan kenasaban sangat kuat antara kadar AGEs dan rasio air kemih albumin kreatinin dengannilai r=0,85 dan nilai p<0,05. Terdapat kenasaban sangat kuat antara kadar AGEs dengan rasio air kemih albumin kreatinin di DMtipe 2.

Abstract

Atherogenic dyslipidemia is the main risk factor of Acute Coronary Syndrome (ACS), caused atherosclerosis plaque and stenosis of artery coronary. Lipid profile ratio used as a marker of cardiovascular disease severity. Atherogenic Index of Plasma (AIP) calculated as logarithm triglycerides/high-density lipoprotein cholesterol (TG/HDL-C) is a reflection of plasma atherogenicity degree and indicator of small dense low-density lipoprotein (LDL). Small dense LDL is the best predictor for cardiovascular disease, but expensive and difficult to examined. The aim of this study to determine the correlation of AIP with stenosis level of the coronary artery in ACS.  This research was an analytical study with cross-sectional design in 24 ACS patients meet the inclusion and exclusion criteria and conduct blood tests at the Central Laboratory Installation of Dr. M. Djamil Hospital Padang in January 2017- September 2017. Triglycerides and HDL-C performed by an enzymatic colorimetric method and stenosis level determined by coronary angiography. Spearman correlation was used to analyze correlation of atherogenic plasma index with stenosis level of the coronary artery, significant if p<0.05.  The subjects of this study were 20 males (83.3%) and 4 females (16.6%) with mean age 57.6(7.9) years. The mean level of HDL cholesterol and triglycerides in ACS were 34.8 (8.7) mg/dL and 155.8 (51.8)  mg/dL. The mean level of  AIP in ACS was 0.28 (0.18). Median of stenosis level of coronary artery was 80% with range 50% - 95%. Spearman correlation test showed a moderate positive correlation between AIP with stenosis level of the coronary artery in ACS (r= 0.426 ; p<0.05).  There is a moderate positive correlation between AIP with stenosis level of the coronary artery in ACS.

Abstract

AbstrakResistensi insulin merupakan penurunan respons biologis jaringan terhadap insulin dalam kadar normal. Pada DM tipe 2 terjadi resistensi dan gangguan sekresi insulin. Terdapat indeks baru 20/(C-peptide puasa x glukosa darah puasa) untuk menilai resistensi sekaligus gangguan sekresi insulin. Penelitian bertujuan membuktikan korelasi indeks 20/(C-peptide puasa x glukosa darah puasa) dengan HOMA-IR untuk menilai resistensi insulin pada DM tipe 2 di RSUP. Dr. M. Djamil Padang. Dengan menggunakan sampel darah dari pasien, kadar glukosa darah puasa, insulin puasa dan C-peptide puasa ditentukan. Data yang diperoleh kemudian dianalisis menggunakan korelasi Pearson. Hasil penelitian menunjukkan rerata kadar glukosa puasa 9,83 (3,53) mmol/L [177 (63,54) mg/dL], insulin puasa 10,58 (3,61) μU/L, dan C-peptide puasa 0,97 (0,29) nmol/L dan terdapat korelasi yang sangat kuat dan bermakna secara statistik (p<0,0001) antara indeks 20/(C-peptide puasa x glukosa darah puasa) dengan HOMA-IR (r= -0,838). Dapat disimpulkan bahwa indeks 20/(C-peptide puasa x glukosa darah puasa) dan HOMA–IR berkorelasi kuat untuk menilai resistensi insulin pada DM tipe 2 di RSUP. Dr. M. There is a new index available to assess resistance and impaired of insulin secretion all at once, with the formula 20/(fasting C-peptide x fasting blood glucose). This study aimed to prove the correlation of this new index to the HOMA-IR (Homeostasis model assesment of insulin resistance) in type 2 diabetes at Dr.M.Djamil Padang hospital. Level of fasting glucose, fasting insulin and fasting C-peptide of blood were measured, followed by statistical data analysis using Pearson correlation test.The result showed the mean of fasting blood glucose, fasting insulin and fasting C-peptide were 9.83 (3.53) mmol/L[177 (63.54) mg/dl], 10.58 (3.61) μU/L, and 0.97 (0.29) nmol/L respectively.There was a strong and statistically significant correlation (p<0.0001) found between the new index and the HOMA-IR ( r= -0.838). To be concluded, the index 20/(fasting C-peptide x fasting blood glucose) and HOMA-IR was strongly correlated to assess insulin resistance in type 2 diabetes at Dr. M. Djamil Padang hospital.

Abstract

AbstrakPenelitian ini bertujuan mengetahui korelasi indeks massa tubuh (IMT) dengan estimated Glomerular Filtration Rate (eGFR) berdasarkan formula MDRD pada populasi sehat di kota Padang. Penelitian analitik dengan rancangan potong lintang ini mengikutsertakan 121 orang peserta medical check up pada beberapa rumah sakit di Kota Padang pada bulan Agustus 2013-Oktober 2014. Nilai eGFR dihitung menurut persamaan MDRD, kadar kreatinin diperiksa dengan metode enzimatik yang telah distandarisasi. Korelasi IMT dengan eGFR berdasarkan formula MDRD dianalisis menggunakan korelasi Pearson. Kemaknaan secara statistik ditentukan jika nilai p < 0,05. Korelasi IMT dengan eGFR berdasarkan formula MDRD adalah korelasi negatif yang sangat lemah dan tidak bermakna (r = -0,19 ; p > 0,05). Simpulan: tidak didapatkan korelasi antara IMT dengan eGFR berdasarkan formula MDRD.AbstractThe purpose of this study was to evaluate the correlation between BMI and eGFR by MDRD formula among healthy population in Padang. This cross-sectional analytic study enrolled 121 participants taking medical check-up in several hospitals in Padang from August 2013 to October 2014. Value of eGFR was determined by MDRD formula, and a standardized enzimatic method was used for measuring creatinine levels. The correlation between BMI and eGFR by MDRD formula was analyzed using Pearson correlation. Statistical significancy was concluded if the value of p < 0.05. The correlation of BMI and eGFR by MDRD formula was found very weak, negative, and not significant (r = -0.19, p >0.05). Thus, there was no correlation between BMI and eGFR by MDRD formula.
