Abstract

Background: Acute myocardial infarction (AMI) remains an important health problem globally. A diagnosis of AMI can be made by finding at least two of the three criteria, namely typical angina pain, electrocardiography (ECG) abnormalities, and increased cardiac biomarkers. Variations in troponin levels based on examination time and infarct location were obtained from previous studies. High-sensitive troponin I (hsTnI) is a very specific biomarker in detecting myocardial damage. This study aims to determine the description of hsTnI levels based on the location of the infarction in AMI patients of Dr. M. Djamil General Hospital Padang. The location of the infarction in AMI determines the management and prognosis for the clinician. Methods: This research is a retrospective study with a cross-sectional design at the Laboratory Installation of Dr. M. Djamil General Hospital Padang. The sample was adult patients diagnosed with AMI with complete data that met the inclusion and exclusion criteria, taken from medical records. Results: This study showed that from a total of 140 AMI patients, 67 samples met the inclusion criteria. A total of 67 AMI patients consisted of 59 men (88.06%) and 8 women (11.94%) with an average age of 60.1 + 10.5 years. The highest hsTnI levels were found at the posterior infarction location, 40,000 ng/L (16,639-48,997). The location of the infarction was dominated by the inferior infarction location in 20 people (29.9%). Conclusion: The highest hsTnI levels were found at the posterior infarction location. The location of the infarction is dominated by the inferior infarction location in AMI patients with ST elevation at Dr. M. Djamil General Hospital Padang.

Abstract

Background: Diabetic foot ulcers (DFUs) are a serious complication of diabetes mellitus with a high risk of infection. Patterns of causative bacteria and antibiotic sensitivity can vary between hospitals and time periods. Methods: This descriptive research involved all swabs and pus samples from DFU patients at Dr. M. Djamil General Hospital Padang between January and June 2023. Samples were processed through culture, Gram staining, and identification using VITEK 2. Patient medical record data was analyzed for demographic characteristics. Results: Of the 23 samples, 65% were gram-negative bacteria, dominated by Escherichia coli (30%). Gram-positive bacteria were found in 35%, with Staphylococcus aureus (18%) as the most. E. coli showed resistance to ampicillin and ceftriaxone but was sensitive to meropenem and amikacin. S. aureus is resistant to vancomycin but sensitive to benzylpenicillin. Conclusion: Gram-negative bacteria, especially E. coli, were more dominant in DFU at Dr. M. Djamil General Hospital Padang during the research period. The pattern of antibiotic resistance found emphasizes the importance of selecting appropriate antibiotic therapy based on sensitivity test results.

Abstract

Background: Acute myocardial infarction (AMI) occurs due to rupture of atherosclerotic plaque which triggers platelet activation and aggregation to form a thrombus. Several studies have examined the relationship between PDW and AMI, but there is no specific correlation with hsTnI as the most sensitive and specific cardiac biomarker for AMI. This study aims to analyze the correlation between hsTnI levels and PDW values in AMI patients. Methods: Cross-sectional study of 60 AMI patients in the emergency room of Dr. M. Djamil General Hospital Padang from April to August 2022. Research subjects who met the inclusion and exclusion criteria underwent hsTnI and PDW examinations. The correlation of hsTnI with PDW was analyzed using the Spearman correlation test. Correlation is declared significant if the p-value is <0.05. Results: The research sample consisted of 48 men (80%) and 12 women (20%). The average age is 60.67 (10.73) years with an age range of 33-82 years. The mean hsTnI level was 2,606.10 (2,404.79) ng/Land the mean PDW value was 10.97 (2.26) fL. There was a strong and significant positive correlation between hsTnI and PDW in AMI patients (r = 0.618, p = 0.000). Conclusion: There is a correlation between hsTnI levels and PDW values at AMI.

Abstract

Background: Peripartum cardiomyopathy (PPCM) is a rare but serious condition characterized by left ventricular dysfunction in late pregnancy or early postpartum. Early diagnosis is essential to prevent complications such as thromboembolism. N-terminal pro-B-type natriuretic peptide (NT-proBNP) is a promising biomarker for the diagnosis of PPCM. Case presentation: An 18-year-old female presented with progressive shortness of breath and generalized edema. Physical examination and echocardiography revealed acute heart failure with multiple left ventricular thrombus. Very high levels of NT-proBNP (1,796 pg/mL) supported the diagnosis of PPCM. The patient was successfully treated with standard heart failure therapy and anticoagulants. Conclusion: This case report highlights the importance of NT-proBNP in the early diagnosis of PPCM and detecting of thromboembolic complications. Significant increases in NT-proBNP levels, even without obvious symptoms, should be promptly evaluated for PPCM, especially in high-risk populations.

Abstract

Background: ST-elevation myocardial infarction (STEMI) is a leading cause of mortality worldwide. Despite advancements in treatment, especially with primary percutaneous coronary intervention (pPCI), 30-day mortality rates remain significant. The monocyte-to-high-density lipoprotein cholesterol ratio (MHR) has emerged as a potential predictor of mortality in STEMI patients, reflecting the balance between inflammation and anti-atherosclerotic processes in atherosclerotic plaques. This study aimed to evaluate the association between MHR and 30-day mortality in STEMI patients undergoing pPCI. Methods: This prospective observational study included 55 STEMI patients treated with pPCI at Dr. M. Djamil General Hospital in Padang, Indonesia, between January and July 2024. Patients were included if they were ≥18 years old, undergoing their first pPCI, and had blood tests done within 24 hours of admission. Patients with prior revascularization, acute/chronic infections, malignancies, autoimmune diseases, or on lipid-lowering therapy were excluded. Blood samples were collected within 24 hours of admission. Monocyte counts were measured using flow cytometry, and HDL cholesterol levels were determined using a homogeneous enzymatic colorimetric method. The MHR was calculated by dividing the monocyte count by the HDL cholesterol level. The primary outcome was 30-day mortality, assessed through hospital records and telephone follow-up. Statistical analysis included chi square, t-tests, and Mann-Whitney U tests. Results: The mean age of the study participants was 59.5 (±11.4) years, with 81.8% being male. The mean monocyte count and MHR were 968 (±212)/mm3 and 28.3 (±6.06), respectively. The median HDL cholesterol level was 33.4 (27-49) mg/dL. Both monocyte count and MHR were significantly higher in patients who died within 30 days compared to those who survived (p<0.001). Conclusion: The MHR is an independent predictor of 30-day mortality in STEMI patients undergoing pPCI. This readily available and cost-effective biomarker may aid in risk stratification and guide treatment strategies for this high-risk population.

Abstract

Kadar adiponektin dalam plasma secara signifikan menurun pada penyandang obes. Adiponektin telah dilaporkan memiliki efek langsung anti-aterosklerosis. Indeks Aterogenik Plasma (IAP) dihitung sebagai log10(TG/ HDL-C), merupakan biomarker aterosklerosis. IAP dapat menjadi biomarker baru untuk risiko dan prognosis penyakit kardiovaskular. Tujuan: Menentukan korelasi kadar adiponektin dengan indeks aterogenik plasma pada penyandang obes. Metode: Penelitian ini merupakan studi analitik dengan rancangan potong lintang. Subjek penelitian adalah 30 orang penyandang obes dengan Indeks Massa Tubuh (IMT) ≥25 kg/m2 yang memenuhi kriteria inklusi dan eksklusi. Subjek penelitian melakukan pemeriksaan darah di Instalasi Laboratorium Sentral RSUP. Dr. M. Djamil Padang pada bulan Januari 2019 sampai September 2019. Pemeriksaan kadar adiponektin dengan metode enzyme linked immunoassay (ELISA). Pemeriksaan kolesterol high density lipoprotein (HDL) dan trigliserida menggunakan alat kimia klinik otomatis. Data univariat diolah dan disajikan dalam bentuk distribusi frekuensi dan rerata (standar deviasi). Data bivariat dianalisis dengan uji korelasi Pearson, bermakna secara statistik jika p˂0,05. Hasil: Rerata umur adalah 34,5 (6,2) tahun. Rerata kadar adiponektin adalah 2,5 (1,1) μg/mL. Rerata indeks aterogenik plasma (IAP) adalah 0,29 (0,17), termasuk risiko tinggi untuk penyakit kardiovaskular. Uji korelasi Pearson menunjukkan korelasi negatif sangat lemah antara adiponektin dengan IAP dan tidak bermakna secara statistik (r=-0,114, p=0,550). Simpulan: Tidak terdapat korelasi antara kadar adiponektin dengan indeks aterogenik plasma pada penyandang obes.Kata kunci: adiponektin, indeks aterogenik plasma, obes

Abstract

Akumulasi lemak tubuh abnormal dan berlebih pada obesitas menyebabkan low grade inflammation sel adiposit yang berkontribusi terhadap penurunan kadar adiponektin. Adiponektin berperan dalam metabolisme glukosa, sehingga kondisi hipoadiponektinemia dapat menyebabkan gangguan metabolisme glukosa. Tujuan: menentukan korelasi kadar adiponektin dengan kadar glukosa puasa pada penyandang obes. Metode: Penelitian ini merupakan studi analitik dengan rancangan potong-lintang terhadap 25 orang penyandang obes yang bekerja di Instalasi Laboratorium Sentral RSUP Dr. M. Djamil Padang yang memenuhi kriteria inklusi dan eksklusi. Penelitian dilakukan mulai bulan September 2018 sampai September 2019. Kadar adiponektin diperiksa dengan metode Elisa two-step sandwich enzyme immunoassay dan kadar glukosa puasa diperiksa dengan metode heksokinase. Data dianalisis dengan uji korelasi Pearson, bermakna jika p<0,05. Hasil: Subjek penelitian terdiri dari laki-laki 8 orang (32,0%) dan perempuan 17 orang (68,0%). Rerata umur adalah 33,5 (6,0) tahun dengan rentang 23-52 tahun. Rerata indeks massa tubuh adalah 34,0 (3,6) kg/m2. Rerata kadar adiponektin adalah 2,8 (1,5) μg/mL dan rerata kadar glukosa puasa adalah 92,8 (11,4) mg/dL. Uji korelasi Pearson menunjukkan korelasi negatif lemah antara log-adiponektin dengan kadar glukosa puasa dan tidak bermakna secara statistik (r= -0,217, p= 0,298). Simpulan: Tidak terdapat korelasi kadar adiponektin dengan kadar glukosa puasa pada penyandang obes.Kata kunci: adiponektin, glukosa puasa, inflamasi adiposit, obesitas

Abstract

Atherogenic dyslipidemia is one of the risk factors for the acute coronary syndrome (ACS). Castelli's risk index-1 (CRI-1) is one of which risk markers for ACS. Castelli's risk index-1 is the ratio of total cholesterol and HDL cholesterol. The test is cheap and easy to do in the hospital setting. Objectives: To described CRI-1 in ACS patients in Dr. M. Djamil Padang Hospital. Methods: This descriptive study was carried out in the central laboratory and the CardioVascular Care Unit (CVCU) of Dr. M. Djamil Hospital Padang from September 2017 to September 2018. The population was all ACS patients who have been diagnosed by the clinician. The samples were part of the population that meet the inclusion and exclusion criteria. Colorimetric enzymatic method using automated clinical chemistry used to measure total cholesterol and HDL cholesterol. Results: This study used CRI-1> 4. Data were presented descriptively in a frequency distribution table. Seventy ACS patients were consisting of 50 (71.43%) males and 20 (28.57%) females, with a median age of 60.1 (8.93) years old. The median total cholesterol and HDL cholesterol levels were 178.66 (46.84) mg / dL respectively and 35.71 (10.86) mg / dL. CRI-1 mean is 5.43 (2.27). 81.43% CRI-1 subject results were more than four. Conclusion: The low levels of HDL cholesterol and within normal median total cholesterol level made CRI-1 value increased.Keywords: Acute coronary syndrome, Castelli’s risk index-1

Abstract

Rapid and accurate measurement of capillary glucose level using point-of-care testing (POCT) is needed to maintain the patient’s normoglycemic status in obtaining adequate management. The glucose POCT method should be evaluated to determine analytical performance by comparing with the hexokinase method as a reference method to provide accurate and reliable results. Objectives: To determined the correlation between capillary glucose using glucose dehydrogenase-nicotinamide adenine dinucleotide (GDH-NAD)  and serum glucose hexokinase methods. Methods:  This analytic cross-sectional study on 42 outpatients who underwent fasting blood glucose examination at Dr. M Djamil Padang General Hospital. This study was conducted from February until September 2020. Capillary fasting blood glucose was measured using glucose POCT GDH-NAD method and serum glucose with hexokinase method. The Spearman correlation test was used to analyze data, significant if p<0.05. Results: The subjects were 28 male (66.7%),14 female (33.3%) with mean age and hematocrit level was 56.12±12.97 years and 40.90±2.42%, respectively. The median capillary glucose GDH-NAD method and serum glucose hexokinase method were 100.00 mg/dL each, with a median difference was 3.00 mg/dL. Spearman correlation test showed very strong positive correlation and statistically significant (r=0,961;p=<0,001). Conclusion: Capillary glucose GDH-NAD method had a very strong positive correlation with serum glucose hexokinase method.Keywords:  hexokinase, fasting glucose level, GDH-NAD, glucose POCT
