Abstract

Abstract Malaria is a serious and potentially fatal public health problem. Malaria elimination efforts are carried out in Indonesia to create a society that is free from malaria transmission in 2030. This study aims to analyze the implementation of malaria control policies at the Sidodadi Community Health Center, Asahan District. This study used a qualitative approach involving the head of the Community Health Center and the malaria program manager as respondents. The results showed that the management of malaria sufferers was quite good, but there were still obstacles in the availability of malaria drugs at the Sidodadi Health Center. Prevention and management of risk factors for malaria are also not optimal due to the lack of facilities and infrastructure for health workers. Epidemiological surveillance and epidemic control still need to be improved according to the standards of the Ministry of Health. Communication, information, and education in malaria prevention is quite good. The improvement of human resources at the Sidodadi Health Center has also been carried out well, but the Asahan District Health Office needs to continue to prioritize the malaria elimination program and continue to improve education for field workers. Abstract

Abstract The hospital has a process for implementing continuity of care in the hospital and integration between care-giving professionals (PPA) assisted by a patient service manager (MPP)/case manager. The current challenge that must be faced by hospitals in implementing the JKN program is implementing good quality control and cost control. The existence of MPP in the hospital case management system is expected to be able to support quality control and cost control. The purpose of this study was to analyze the functions of the case manager in improving quality control and cost control for inpatients (case study). This research was conducted at the Grandmed Lubuk Pakam Hospital, Deli Serdang Regency from September 2022 to May 2023. The type of research in this study was qualitative. The sample in this study were case managers at Grandmed Lubuk Pakam Hospital, Deli Serdang Regency, totaling 3 people and 1 director of medical services and 3 heads of inpatient rooms. The results showed that the implementation of case managers in improving quality control and cost control for inpatients was quite good. The case manager utility assessment has not gone well because there is no clear format for the initial screening of patients. Patient care planning by case managers has not been maximized. Not all case managers do patient care planning. The case manager's facilitation and advocacy has not been maximized. There are 2 case managers who do not coordinate with other fields, only coordinate with DPJP and nurses. The case manager evaluation has not gone well because there is no instrument format for case manager tasks so evaluation cannot be carried out. Post-discharge planning follow-up by the case manager has not gone well because the case manager has never followed up post-discharge planning. Case managers should carry out case manager functions for patients who are being treated in case management in accordance with the standard criteria set by STARKES 2022 accompanied by proper and accurate documentation to improve service quality and patient satisfaction. Abstract

Health services are a benchmark in determining the expected quality of health services in hospitals. In providing services, patients must receive the best service from the hospital to improve public health. This type of research is quantitative research using an analytical survey method with a cross-sectional approach. The population and sample in this study were all 112 inpatient rooms at Granmed Lubuk Pakam Hospital. The influencing variables are building components, air ventilation, and bed equipment. This research uses primary data in the form of observation sheets and checklists. Meanwhile, secondary data was obtained directly from the Grandmed Lubuk Pakam Hospital office. Analysis of the data used univariate and bivariate analysis. The highest Inpatient Class Building Component is in the very good category, namely 58 rooms (51.8%). The highest Inpatient Class air ventilation factor was in the very Good category with 47 rooms (42.0%). The highest Inpatient Class Bed Completeness Factor was in the Very Good category with 60 rooms (53.6%). Each factor's p-value obtained is 0.001; 0.004; 0.002 means that there is an influence of building components, air ventilation, and bed equipment on the readiness to implement standard inpatient classes at Grandmed Lubuk Pakam Hospital in 2023.

Abstract

Catastrophic diseases, such as stroke, heart disease, and kidney failure, are the leading causes of morbidity and mortality in Indonesia. Early detection is crucial to prevent further complications. Health cadres play a strategic role in the early detection of these diseases; however, their understanding of emergency conditions remains limited. Therefore, emergency education is necessary to enhance the ability of health cadres in the early detection of catastrophic diseases.Objective: To analyze the impact of emergency education on improving the ability of health cadres in the early detection of catastrophic diseasesThis study employed a quasi-experimental design with a pre-test and post-test approach involving 50 health cadres in the working area of Puskesmas X. Data were collected through questionnaires before and after the emergency education intervention. Data analysis was conducted using univariate and bivariate statistical tests, with a paired t-test to examine differences before and after the intervention Univariate Analysis: Before the education was provided, 60% of health cadres had low knowledge levels, 30% were at a moderate level, and only 10% had a high level of knowledge. After the intervention, there was a significant increase, with 70% of cadres having a high level of knowledge and 30% in the moderate category.Bivariate Analysis: The paired t-test results showed a significant difference (p < 0.05) in the knowledge level of health cadres before and after receiving emergency education. Emergency education significantly improves the ability of health cadres in the early detection of catastrophic diseases. Therefore, this educational program should be continuously implemented to enhance the capacity of health cadres in efforts to prevent and detect catastrophic diseases in the community

Abstract

Latar belakang: Alat Pelindung Diri (APD) merupakan elemen penting dalam sistem perlindungan bagi tenaga medis, terutama perawat, yang berinteraksi langsung dengan pasien dan berada di lingkungan dengan risiko tinggi penularan penyakit. Penelitian ini bertujuan untuk mengetahui determinan kepatuhan penggunaan APD pada perawat. Metode: Penelitian ini menggunakan desain cross-sectional dan dilaksanakan di Rumah Sakit Grandmed Lubuk Pakam pada tahun 2024. Subjek yang terlibat dalam penelitian ini adalah seluruh perawat yang bekerja di Rumah Sakit Grandmed Lubuk Pakam, khususnya perawat yang bertugas di instalasi bedah sentral, dengan jumlah 57 orang. Teknik pengambilan sampel yang digunakan adalah total sampling. Data dikumpulkan melalui penyebaran kuesioner kepada perawat. Analisis data dilakukan dengan menghitung distribusi frekuensi dan melakukan pengujian hipotesis menggunakan uji chi-square dengan tingkat signifikansi α = 0,05. Hasil: Kepatuhan penggunaan APD pada perawat sebesar 19,3%. Pengetahuan perawat (p = <0,001), sikap perawat (p = 0,003), pengawasan (p = 0,004), ketersediaan APD (p = <0,001) berpengaruh signifikan terhadap kepatuhan penggunaan APD. Kesimpulan: Pengetahuan perawat yang kurang baik, sikap perawat yang negatif, pengawasan yang tidak baik, ketersediaan APD yang tidak memadai berpengaruh signifikan terhadap kepatuhan penggunaan APD.

Abstract

Heart disease is a major cause of morbidity and mortality in Indonesia, with increasing health care costs. Optimizing the role of case managers is seen as a potential solution to improve service quality while reducing care costs. This study aims to analyze the effect of optimizing case managers in improving patient satisfaction and reducing care costs in heart disease patients at Mitra Mulia Husada Hospital. This study used a quantitative design with a pre-experimental one-group pretest-posttest approach. Data were collected through a patient satisfaction survey using a SERVQUAL-based instrument and analysis of care cost data from medical record reports before and after the implementation of case managers. The implementation of case managers showed an increase in the average patient satisfaction score from 3.55 to 4.45 (Likert scale 1-5) and a decrease in the average daily cost of care by 20%. In addition, there was a reduction in the length of hospitalization from 7 to 5 days. Optimal implementation of case managers can significantly improve patient satisfaction and efficiency of care costs. These findings support the integration of case managers as an important part of health service management for heart disease patients in hospitals.

Abstract

Sistem pembiayaan yang digunakan oleh BPJS kesehatan yaitu sistem Indonesia Case Based Group’s (INA-CBGs) untuk biaya pengganti dari BPJS untuk klaim pembiayaan. Metode operasi caesar menjadi salah satu layanan BPJS yang mengalami peningkatan signifikan dari tahun ke tahun di mana operasi caesar di rumah sakit terus meningkat sehingga pembiayaan juga mengalami peningkatan. Tarif INA-CBGs adalah besaran pembayaran klaim oleh BPJS kesehatan kepada fasilitas kesehatan rujukan tingkat lanjut atas paket layanan yang didasarkan kepada pengelompokan diagnosis penyakit dan prosedur (PMK nomor 52, 2016). Tujuan dalam penelitian ini adalah untuk menganalisis perbandingan biaya rumah sakit dengan paket INA-CBGs pada pembayaran klaim sectio caesarea pasien BPJS di RSU Indah Kota Bagan Batu Riau Tahun 2025. Jenis penelitian ini dilakukan dengan menggunakan pendekatan kualitatif. Teknik pengambilan sampel dalam penelitian ini yaitu menggunakan informan. Teknik pengumpulan data yang utama dalam penelitian ini dilakukan melalui wawancara yang mendalam atau indept interview. Analisa data dalam penelitian ini menggunakan analisa naratif. Hasil analisa menunjukkan bahwa rata-rata biaya pelayanan tindakan Sectio Caesarea di RSU Indah Kota Bagan Batu Riau meliputi komponen obat-obatan, alat kesehatan, jasa medis, dan biaya administrasi. Total biaya aktual yang dikeluarkan rumah sakit lebih tinggi dibandingkan dengan tarif klaim yang diterima melalui sistem INA-CBGs, terutama pada kasus dengan komplikasi medis. Diharapkan Manajemen rumah sakit sebaiknya mengadakan evaluasi berkala terhadap perbedaan antara biaya aktual dan tarif klaim INA-CBGs.

Abstract

Latar belakang: Rendahnya kepatuhan dalam mengonsumsi obat antihipertensi menjadi hambatan bagi keberhasilan pengendalian tekanan darah, serta berkontribusi pada peningkatan risiko komplikasi kardiovaskular dan beban pelayanan kesehatan. Penelitian ini bertujuan untuk mengidentifikasi determinan kepatuhan konsumsi obat pada penderita hipertensi. Metode: Studi ini menggunakan desain potong lintang (cross-sectional). Populasi penelitian mencakup seluruh pasien hipertensi yang berobat di Puskesmas Klinik Pratama Polkes 01.10.02 Lubuk Pakam pada tahun 2025, dengan jumlah 95 orang. Seluruh populasi dijadikan sampel penelitian melalui teknik total sampling. Pengumpulan data dilakukan menggunakan kuesioner yang disebarkan kepada pasien hipertensi, sedangkan analisis data dilakukan dengan uji Chi-Square pada tingkat signifikansi α = 0,05. Hasil: Sebanyak 65,3% responden diketahui tidak patuh dalam mengonsumsi obat, sedangkan 34,7% responden patuh dalam konsumsi obat. Faktor-faktor seperti tingkat pendidikan yang rendah, pengetahuan yang kurang, jarak tempat tinggal ≥1 km, minimnya dukungan keluarga, lama konsumsi obat, serta tidak adanya paparan edukasi kesehatan terbukti berhubungan signifikan dengan kepatuhan konsumsi obat. Kesimpulan: Kepatuhan konsumsi obat pada penderita hipertensi dapat dipengaruhi oleh aspek pendidikan, pengetahuan, jarak tempat tinggal, dukungan keluarga, durasi konsumsi obat, dan edukasi kesehatan.

Abstract

Background: High-quality medical records play a crucial role in modern healthcare services as they serve as the primary foundation for medical decision-making and contribute significantly to patient safety and service satisfaction. The success of medical record management is highly dependent on the implementation of effective quality control, which not only includes the accuracy and completeness of data but also ensures that the data is securely managed and easily accessible to healthcare providers. Objective: This study aims to analyze the relationship between the implementation of quality control in medical record services and patient satisfaction at RS Grandmed Lubuk Pakam. Research Methods: This study employs a quantitative descriptive correlational approach. The sample consists of medical record staff and patients, selected through total sampling and purposive sampling techniques. Data was collected through questionnaires and analyzed using Pearson correlation and simple linear regression. Results: The analysis indicates a significant positive relationship between the implementation of quality control and patient satisfaction, with a correlation coefficient of r = 0.652 and a p-value of < 0.05. The regression analysis shows that quality control contributes 42.6% to the variation in patient satisfaction. Conclusion: The better the implementation of quality control in medical record services, the higher the patient satisfaction perceived. Therefore, hospitals need to continue enhancing quality control implementation through staff training, optimization of information technology, and strengthening data protection systems to improve service quality and patient satisfaction.

Abstract

Background: Medical records play a crucial role in supporting safe, high-quality, and continuous healthcare services. However, the quality of medical record services at many healthcare facilities, including Granmed Hospital Lubuk Pakam, still faces various challenges.Objective:This study aims to analyze the factors affecting the quality of medical record services, including individual factors (knowledge, perception, workload), organizational factors (policies, training, supervision), and technological factors (facilities and infrastructure). Research Methods: A quantitative analytic observational study with a cross-sectional design was conducted. A total of 75 healthcare and administrative workers involved in medical record management were selected using total sampling. Data were collected through structured questionnaires and medical record audits, then analyzed using chi-square tests and multiple logistic regression. Results: There were significant relationships between knowledge (p=0.002), workload (p=0.015), training and supervision (p=0.049), and availability of facilities and infrastructure (p=0.022) with the quality of medical record services. Most respondents reported a heavy workload and inadequate facilities.Conclusion:Knowledge, workload, training-supervision, and facility availability significantly influence the quality of medical record services. Efforts to strengthen human resources, manage workloads better, enhance supervision, and improve infrastructure are essential to enhance the quality of medical record documentation.
