Abstract

Chronic obstructive pulmonary disease (COPD) is a lung disease characterized by obstruction to airflow that is persistent and progressive. Shortness of breath and low PEFR is major problem in COPD. Shortness of breath during sleep can increase reticular activating system and release norepinephrine which cause waking state and sleep disorder. The purpose of this study was to analyze the correlation between PEFR and sleep quality in COPD patients.The study design was correlation analytic with cross-sectional approach involving 53 patients as a sample. Data were collected by using peak flow meter to measure PEFR and PSQI questionnaire to measure quality of sleep. Data were analyzed by using Spearman rank test with significance level of 0.05. The result showed that the average value of PEFR was 41.09% and more than half of respondents (71.7%) had PEFR value in red zone category. Mean value of sleep quality was 18.62 and all of respondents had sleep quality in poor category. The p value was 0.000 (p <α) with r value = -0,876. It indicates that there was significant correlation between PEFR and sleep quality in COPD patients. Nurses should provide appropriate intervention to improve PEFR so that COPD patients can have better sleep quality.   Keywords: PEFR, sleep quality, COPD

Abstract

Diabetes Mellitus (DM) is a chronic disease that requires a continous self-care. A low level ofknowledge about self-care can worsen health condition and cause diabetes distress. The continuingstress can cause hyperglycemia that lead to complications of DM. DSME/S is a structured healtheducation that facilitates DM patients in implementing and maintaining their behavior for sustainableself-care. This research aimed to analyze the effect of DSME/S on diabetes distress in patients withtype 2 DM. This research was quasi experimental with non randomized control group pretestposttest design. Thirty respondents in this study were divided into 15 respondents as theintervention group and 15 respondents as the control group by consequtive sampling technique.DSME/S was conducted in 6 sessions: 4 sessions in the hospital and 2 sessions at respondent'shouse. The data were analyzed by using dependent t test and independent t test with significancelevel of 0.05. The result of dependent t test revealed a significant difference of diabetes distressbetween pretest and posttest both in the intervention group (p=0.001) and in the control group(p=0.046). Furthermore, independent t test showed a significant difference between interventiongroup and control group (p=0.001). The decrease of diabetes distress was higher in the interventiongroup than control group. This result indicates that there was a significant effect of DSME/S ondiabetes distress in patients with type 2 DM. Nurses are expected to apply DSME/S as a dischargeplanning to reduce diabetes distress in patients with type 2 DM.Keywords: diabetes distress, DSME/S, Diabetes Mellitus

Abstract

Pathological change experienced by patients with type 2 diabetes mellitus is impaired peripheralblood circulation, especially in the lower limbs and feet. Peripheral blood circulation can be examinedby measuring Ankle Brachial Index (ABI). Therapeutic exercise walking is one of physical activitieswhich can improve blood circulation. This research goal was to analyze the effect of therapeuticexercise walking on peripheral blood circulation in patients with type 2 diabetes mellitus. Theresearch design was randomized control group pretest-posttest design. The sample size was 15respondents as control group and 15 respondents as treatment group. Data was analyzed by usingdependent t-test and independent t-test with significant level of 0.05. This result showed a significantincrease of ABI after therapeutic exercise walking in the treatment group (p= 0.001). However therewas a significant decrease of ABI between pretest and posttest in control group (p=0.010).Independent t-test indicated a significant difference of ABI between treatment group and controlgroup (p=0.000). So, it can be concluded that there is an influence of therapeutic exercise walking onthe peripheral blood circulation in patients with type 2 diabetes mellitus. Abstract

Airway obstruction is an important physiological disease in asthma clients that will inhibits the flow of air during inspiration and expiration, so that lung ventilation is not optimal. This situation makes clients lose base ability to reach normal airflow during breathing, especially when expiratory, which leads to reduction in peak expiratory flow. Non-pharmacological therapies can be used as a supplementary or complementary pharmacological therapy, one of which is progressive muscle relaxation. The objective of this research was to analyze the effect of progressive muscle relaxation on peak expiratory flow of clients with bronchial asthma. Respondents in this research were 11 people. Data analysis used dependent t-test with CI 95% (α=0,05). The results of data analysis showed the results of p value = 0,000 (p<0,05). The conclusion of this research is that there is a significant effect of progressive muscle relaxation on peak expiratory flow of clients with bronchial asthma. This research is expected to be applied and administered in health care, especially in the rehabilitation of clients with asthma to improve respiratory function optimally.   Keywords: progressive muscle relaxation, asthma, peak expiratory flow

Abstract

Diabetes Mellitus is metabolic disease characterized by hyperglycemia. Diabetic foot ulcer is serious complication in diabetic patients which can lead to lower extremity amputation. Therapeutic exercise walking is one of physical activities which can improve blood circulation in the body. This research aimed to analyze the effect of therapeutic exercise walking on the risk of diabetic foot ulcerin patients with type 2 diabetes mellitus. This research employed randomized control group pretest postest design. The sampling technique was simple random sampling involving 15 respondents as intervention group and 15 respondents as control group. Therapeutic exercise walking was done three times a week within a month with the duration of 40 minutes. The data were analyzed using dependent t test and independent t test with significant level of 0.05. The results revealed a significant difference between pretest and postest either in the intervention group (p=0.001) or control group (p=0.048). Furthermore independent t test showed a significant difference between intervention group and control group (p=0.015). This result indicates that there is a significant effect of therapeutic exercise walking on the risk of diabetic foot ulcer. Nurse is expected to apply therapeutic exercise walking to prevent diabetic foot ulcer in diabetes mellitus patients. Keywords: type 2 diabetes mellitus, therapeutic exercise walking, risk of diabetic foot ulcer

Abstract

Background: The incidence of pneumonia in sufferers being treated in the ICU is one form of nosocomial infections. VAP is one of the founders of factors of complications from Acute Respiratory Distress Syndrome (ARDS). VAP incidence figures can be reduced by having the care oral hygiene is good and correct. oral hygiene solution Clorhexidin 0.2% effective in preventing colonization of Staphylococcus Aureus. Research objectives: knowing the difference of Oral hygiene by using Chlorhexidine 0.2% 0.9% Nacl and to prevent the occurrence of Ventilator Associated Infection (VAP) in patients hospitalized in ICU Dr. Soeradji Tirtonegoro Klaten. Research methods: the research was included in a quasi experiment design or artificial treatment with non equivalent control group design. Engineering samples are non probability sampling with technical purposive sampling. Data analysis techniques using Paried test T test. Results of research: Characteristics of respondents ages 41-60-year-old as much as 9 (64,3%) respondents. Diagnosis of the disease the patient Post Craniotomy Operation by as much as 5 (35.7%) respondents. The male gender as much as 9 (64.3S%) of the respondents. The difference in Oral Hygiene use Clorhexidine obtained results VAP the value of p = 0.023 and because p < 0.05 0.05 then the Ha accepted and Ho is rejected. Conclusion: There is a significant difference between the oral hygiene of events VAP value mean different 1.428 oral hygiene means using Clorhexidine was able to lower the incidence of VAP figure 1.428 versus oral hygiene using Nacl 0.9%.

Abstract

Public Health Center of Ngemplak I is one of Public Health Center at Sleman regency which has hospitalization facilities. Based on Bed Occupancy Rate (BOR) at Public Health Center of Ngemplak I, hospitalization has been utilized optimally. BOR for 2001 was 89,11% and 2002 was 93,8%. Compared with another Public Health Center which has hospitalization facilities at Sleman regency, the utilization of hospitalization facilities at Public Health Center of Ngemplak I is the highest from the other. More over, some people of Prambanan and Kalasan district choose to utilize hospitalization facilities at Public Health Center of Ngemplak I although there is Public Health Center with hospitalization facilities in their district.  The objective of  this research was to know the description of factors that encourage family’s  decision in utilizing hospitalization facilities at Public Health Center of Ngemplak I Sleman. The method of this research was qualitative. The subject of research was family of patient who hospitalized at Public Health Center of Ngemplak I. Sample was defined by using accidental sampling and consist of 30 people. The instruments of this research were questionnaire and guide interview. The result showed Public Health Center of Ngemplak I was used for hospitalized by family with middle to lower socio economic characteristic with average income Rp.200.000,00-Rp.500.000,00, most of responden work as farmer, labour, lower employee; low to middle education; lived in Ngemplak district, distance from home to Community Health Center < 5 km; and paid the treatment cost by themselves. The factors from service provider that encourage family’s decision to utilize hospitalization at Public Health Center of Ngemplak I were cheap, near, quickly treatment, friendly  service, easy administration, good facilities, and complete diagnostic tools. The internal factors that encourage family decision were income, health insurance participation, disease severity, and experience of utilitation  before. Abstract

Self-care management is essential for maintaining blood glucose control so that the complications of diabetes mellitus can be prevented. Coping skills are needed to overcome obstacles in self-care. This study aimed to identify the effect of coping strategy training on self-care management in patients with type 2 diabetes mellitus. The design of study was a non-randomized control group pretest-posttest design. The population was patients with type 2 diabetes in the area of Public Health Center of Jember Kidul. The sampling technique was purposive sampling involving 20 respondents which were divided into 10 respondents as the intervention group and 10 respondents as the control group. The training was conducted once by providing education about diabetes self-care and stress management. Self-care management was measured by using the Summary of Diabetes Self-care Activities (SDSCA) questionnaire. The data were analyzed by Wilcoxon, t dependent, and Mann-Whitney test (α = 0.05). The result revealed a significant increase in the average value of self-care in the intervention group after coping strategy training (p: 0.008) and there was no difference between pretest and posttest in the control group (p: 0.197). Mann-Whitney test showed a significant difference of self-care betweenthe intervention group and control group after the intervention (p = 0.000). It can be concluded that there was a significant effect of coping strategy training on self-care management in patients with type 2 diabetes mellitus. Coping strategy training can be applied to improve self-care management in diabetic patients.Keywords: Diabetes mellitus, self-care management, coping strategy training

Abstract

Patients with type 2 Diabetes Mellitus (DM) experience changes related to self-care management. This condition can lead to a negative psychological reaction such as stress. People deal with stress depending on their coping strategy. One of the factors affecting coping strategy is spirituality. This study aimed to analyse the correlation between spirituality and coping strategy in type 2 DM patients. This research applied an observational analytic design with cross-sectional approach. A total of 84 respondents were enrolled in this study by using consecutive sampling technique. Data collection was conducted by administering questionnaires of Daily Spiritual Experience Scale (DSES) and Cope Inventory. Data were analysed by using Spearman correlation test with significance level of 0.05. The result showed that the mean value of spirituality was 70.25 and coping strategy was 75.88. There was a significant positive correlation between spirituality and coping strategy (p-value: 0.001; r: 0.616). The correlation is strong and positive which means the higher the level of spirituality the better the coping strategy. This study suggests the importance of assessing the aspect of spirituality to optimize coping strategy in patients with type 2 DM. Keywords: Type 2 Diabetes Mellitus; Spirituality; Coping Strategy

Abstract

Diabetes mellitus (DM) is a chronic disease that requires continuous self-care. Education by nurses can support self-care behavior of patients with type 2 DM. Good knowledge affects self-care of type 2 diabetes patients so that metabolic control can be achieved. This study aimed to identify the correlation between the role of nurse as educator and self-care behaviour in patients with type 2 diabetes mellitus. This research applied a descriptive analytic design with cross sectional approach. A total of 63 respondents were enrolled in this study by using consecutive sampling technique. Data collection was conducted by administering questionnaires of nurse role as educator and Summary of Diabetes Self Care Activities (SDSCA). Data were analyzed by using Spearman correlation test with significance level of 0.05. The result showed that the mean value of nurse role and self-care behaviour was 59.84 and 3.79 respectively. The p value was 0.000 (p<0.05) with the correlation coefficient (r) of 0.851. It indicates that there was a significant correlation between nurse role as educator and self-care behaviour in patients with type 2 DM. The correlation was strong and positive which means the better the nurse's role as educator the better the patient's self-care behaviour. The education gained by the patients can affect the motivation to perform self-care behaviour. This study suggests the importance of nurse role in providing education in type 2 diabetes patients to optimize self-care behaviour.
