Abstract

Diabetes Mellitus is a chronic disease that can harm the body, both physically and psychologically (stress). DM patients will carry out adaptive coping strategies to properly manage treatment or therapy. The process of self-empowerment in people with type 2 diabetes mellitus will appear when they have information or knowledge about the disease so that it can influence the patient's self-skills for further treatment. This study aimed to analyze the relationship between self-empowerment and coping strategies in patients with type 2 diabetes mellitus. The design of this study was an analytical observational study using a cross-sectional method. The sampling technique used is consecutive sampling. The sample of this study was type 2 DM patients with 84 respondents. The instrument used in this study is the Diabetes Empowerment Scale-Long (DES-LF) questionnaire and the Cope Inventory questionnaire. Data were analyzed using the Spearman test. Self-empowerment and coping strategies in type 2 diabetes mellitus patients showed close to maximum results. In the results of the study, it was found that there was no significant relationship between self-empowerment and coping strategies in type 2 DM patients with p = 0.086. It means that other factors can influence coping strategies. Health services can educate patients about the importance of coping strategies for DM clients so that they can reduce the impact of DM disease

Abstract

Patients with diabetes can affect cardiac output, such as blood pressure dysfunction. Changes in insulin resistance and hyperglycemia can lead to blood pressure dysfunction and increased cardiac output problems. Warm compresses affect macrovascular function and impact blood pressure in patients. This study analyzed the effectiveness of warm compresses in reducing blood pressure in type 2 DM patients with decreased cardiac output problems. The study used a case report method. Warm compresses were applied for three days, for 2 hours per day, using a warm water sack placed on the abdomen at 40-42°C. Blood pressure was measured before and after the intervention. Results showed that after 3 days, there was a reduction in blood pressure, with an average decrease of 16.667 mmHg in systolic and 6.667 mmHg in diastolic pressure. Warm compresses effectively reduce blood pressure in patients with cardiac output problems. Nurses can apply warm compress therapy as a simple intervention to improve blood pressure in patients with decreased cardiac output.

Abstract

Latar belakang: pasien kritis yang dirawat di Ruang Intensive Care Unit (ICU) dengan penurunan kesadaran dan bedrest total beresiko mengalami dekubitus. Penerapan hasil penelitian massage menggunakan olive oil ini bertujuan untuk mencegah terjadinya dekubitus pada pasien kritis yang dirawat di Ruang ICU. Metode: analisis masalah dilakukan untuk mendapatkan informasi tentang efektifitas massage menggunakan olive oil untuk mencegah terjadinya dekubitus. Intervensi massage menggunakan olive oil dilakukan pada 12 orang pasien kritis yang dirawat di ruang ICU dan dipilih berdasarkan kriteria yang telah ditentukan. Tindakan massage menggunakan olive oil dilakukan satu kali sehari selama 5 hari dan selanjutnya dilakukan evaluasi. Instrumen yang digunakan yaitu GCS (Glasgow Coma Scale) untuk mengukur tingkat kesadaran pasien dan skala Braden untuk penilaian luka baring/decubitus. Hasil: 12 pasien yang dilakukan massage menggunakan olive oil menunjukkan hasil p-value 0,04 (<0,05) yang berarti bahwa massage olive oil signifikan dalam mencegah dekubitus. Diskusi: olive oil yang diaplikasikan pada kulit dengan cara massage dapat memberikan dampak meningkatkan aliran darah, merilekskan jaringan kulit, menjaga kelembaban kulit dan sebagai anti inflamasi. Kesimpulan: penerapan aplikasi jurnal massage olive oil yang dilakukan pada pasien kritis dengan bedrest total di ruang ICU memberikan manfaat mencegah dekubitus, sehingga dapat menurunkan peluang terjadinya dekubitus dan berdampak menurunkan hari perawatan pasien di ICU.

Abstract

Latar Belakang: pasien yang dirawat di Ruang Intensive Care Unit (ICU) dengan kondisi kritis yang mengalami hemodinamik yang tidak stabil ditandai dengan peningkatan MAP (Mean Arterial Pressures), Heart Rate (HR), Respiratory Rate (RR), serta penurunan saturasi oksigen (SPO2). Penerapan hasil penelitian foot massage ini bertujuan untuk menstabilkan hemodinamik selama pasien dirawat di Ruang ICU. Metode: analisis masalah dilakukan untuk mendapatkan informasi tentang efektifitas dalam pemberian foot massage untuk menstabilkan hemodinamik. Intervensi foot massage dilakukan pada 10 orang pasien kritis yang dirawat di Ruang ICU dan dipilih berdasarkan kriteria yang telah ditentukan. Tindakan foot massage dilakukan selama 20-30 menit dan selanjutnya dilakukan evaluasi. Alat ukur yang digunakan yaitu lembar observasi untuk melihat perubahan hemodinamik pada bed side monitor pasien. Hasil: sepuluh pasien yang dilakukan foot massage menunjukkan hasil p-value 0,001 (<0,05) pada RR; 0,000 (<0,05) pada HR; 0,004 (<0,05) pada MAP; 0,591 (>0,05) pada SPO2 yang berarti bahwa signifikan pada hemodinamik non invasif pada pasien yaitu pada HR, RR, dan MAP, akan tetapi tidak adanya perubahan yang signifikan pada SPO2. Diskusi: foot massage yang diaplikasikan pada pasien dengan ketidakstabilan hemodinamik dapat memberikan dampak meningkatkan sirkulasi, vasodilatasi arteri, mengurangi ketegangan otot, dan memberikan efek relaksasi otot polos. Kesimpulan: penerapan aplikasi jurnal foot massage yang dilakukan pada pasien kritis dengan ketidakstabilan hemodinamik di Ruang ICU memberikan manfaat menstabilkan hemodinamik pada HR, RR, dan MAP, sehingga dapat menurunkan hari perawatan pasien di ICU.

Abstract

Background: The government has made vaccination the primary strategy to control the COVID-19 pandemic. However, the public still needs to implement COVID-19 prevention behavior even though they have been vaccinated because COVID-19 infection with or without symptoms can still occur. Health literacy is the basis for behavior change. Purpose: This study aimed to determine the relationship between health literacy and post-vaccination COVID-19 prevention behavior of the community in the work area of "‹"‹the Patrang Public Health Center, Jember Regency. Methods: A descriptive correlation study with a cross-sectional approach was performed in May 2022. The sample in this study was 435 selected by purposive sampling with the inclusion criteria of those aged >17 years old and receiving a full primary dose of vaccination. The data were collected using the Health Literacy Survey Coronavirus Disease Questionnaire 22 (HLS-COVID-Q22) and COVID-19 prevention behavior questionnaire. The data were analyzed by Spearman with a significance level of 0.05. Results: The results showed that the majority of health literacy was inadequate health literacy (43,2%), and the post-vaccination COVID-19 prevention behavior was in the moderate category (42.7%). There was a significant moderate positive correlation between health literacy and post-vaccination COVID-19 prevention behavior (p < 0.001; r = 0.513). Conclusion: The higher the health literacy, the better the post-vaccination COVID-19 prevention behavior. The nurse was importance to provide health education by paying attention to the health literacy ability. Focusing on improving health literacy is an important strategy to enhance COVID-19 prevention behavior and reduce transmission of the coronavirus.

Abstract

Angka pasien dengan kanker setiap tahun terus mengalami peningkatan. Ca lidah merupakan keganasan jenis karsinoma yang mengenai lidah dan hampir 95% berupa karsinoma sel skuamosa. Ca lidah yang tidak ditangani secara komprehensif dapat bermetastasis pada jaringan sekitar. Tumor primer atau metastasis di wilayah kepala leher, paru atau mediastinum dapat menyebabkan obstruksi jalan napas pada tingkat laring, trakea atau bronkus sehingga dapat menyebabkan gangguan pada saluran pernapasan. Penatalaksanaan pada pasien dengan bersihan jalan napas tidak efektif atau obstruksi saluran napas dapat dilakukan secara non-farmakologi seperti latihan pernapasan, salah satunya yaitu buteyko breathing technique agar pasien dapat mengontrol pernapasannya sehingga proses pernapasan dapat berjalan dengan cukup baik. Penelitian ini bertujuan untuk menganalisis penerapan buteyko breathing technique dengan masalah keperawatan bersihan jalan napas tidak efektif pada pasien ca lidah di Ruang Seruni RSD dr. Soebandi Jember. Metode yang digunakan dalam penelitian ini adalah studi kasus deskriptif, dimana penelitian ini dilakukan selama 3 hari dengan durasi pemberian intervensi selama 20 menit. Instrumen yang digunakan berupa lembar SOP buteyko breathing technique dan lembar observasi bersihan jalan napas pasien. Hasil penelitian didapatkan bahwa dengan menggunakan buteyko breathing technique, pasien dapat mulai mengontrol pernapasannya, terlihat dari indikator frekuensi napas (RR) membaik, batuk dan sputum menurun, ronkhi tidak ada serta penggunaan otot bantu pernapasan seperti retraksi dada minimal dan pernapasan cuping hidung tidak ada. Penerapan buteyko breathing technique cukup efektif dalam mengatasi masalah keperawatan bersihan jalan napas tidak efektif pada pasien ca lidah. Karya ilmiah ini dapat dijadikan sumber referensi bagi perawat untuk memberikan terapi dalam mengatasi pasien dengan masalah bersihan jalan napas tidak efektif.

Abstract

The COVID-19 pandemic has impacted lifestyle changes, especially physical activity, which tends to decline. Lack of physical activity can worsen glycemic control in patients with type 2 diabetes mellitus, causing complications affecting their quality of life. This study aimed to analyze the correlation between physical activity and the quality of life of type 2 diabetes mellitus patients during the COVID-19 pandemic. This study used a descriptive correlational research design with a cross-sectional approach and 84 respondents obtained by a simple random sampling technique. Data were collected using a Physical Activity Questionnaire (GPAQ) and a Diabetes Quality of Life (DQOL) questionnaire and analyzed by the Kendall Tau correlation test with a significance level of 0.05. The results showed that 51.2% of respondents had moderate physical activity levels, and 57.1% had a high quality of life. Kendall's tau b test revealed a significant positive correlation between physical activity and the quality of life of type 2 diabetes mellitus patients during the COVID-19 pandemic (p-value = 0.015 and r = 0.251). Providing health education about the importance of physical activity is essential, especially during the COVID-19 pandemic, to enhance quality of life of type 2 diabetes mellitus patients.

Abstract

Aims : Lumbar fractures increased from 46.8% to 56.0% between 2009 and 2019, causing severe pain and discomfort, a critical nursing issue requiring attention to enhance patient care quality. Methods : the study was conducted in the Seruni Surgical Room, RSUD dr. Soebandi Jember, focusing on a patient with multiple lumbar fractures and nerve root injury. The patient was selected based on inclusion criteria and pain complaints. The researchers provided a distraction technique, involving listening to classical music and imagining positive things to alleviate pain. Results : the research results showed that there was a difference between the pain scale from the first day to the third day. This distraction technique is given according to the patient's needs when the pain felt by the patient appears. Conclusions : Acute pain is a major nursing issue in patients with multiple fractures and nerve injuries, requiring pain management interventions like music distraction therapy to address physical, psychological, cognitive, and social needs. Researchers hope that nurses can apply therapy independently to patients who have pain.

Abstract

The emergence of complications in patients with type 2 DM significantly impacts the quality of life. Resilience as part of self-coping may be connected with the quality of life and the severity of the complications. This study analyzed the correlation between resilience and quality of life in type 2 DM patients with chronic complications. The method was an observational study with a cross-sectional approach. Thirty-five respondents were recruited in this study using a consecutive sampling technique. Data were collected using Connor Davidson Resilience Scale 25 (CD-RISC 25) and Diabetes Quality of Life (DQOL) questionnaires and analyzed by Pearson Product Moment (p <0.05). The result showed that the mean value of resilience was 63.06 (±12.47), while the average value of quality of life was 72.46 (±11.78). A strong positive correlation existed between resilience and quality of life (p-value = 0.000; r = 0.782). Increased resilience score equals increased quality of life of type 2 DM patients with chronic complications. Providing interventions to increase resilience is essential to optimize the quality of life in type 2 DM patients with chronic complications.

Abstract

Abstrak Ulkus diabetik adalah komplikasi DM yang paling sering terjadi. Angka mortalitas diabetisi dengan ulkus diabetik juga semakin meningkat, sehingga dibutuhkan strategi pencegahan untuk mengidentifikasi risiko. Tujuan penelitian adalah teridentifikasi diabetisi yang berisiko mengalami ulkus diabetikum berbasis Diabetic Foot Screening di wilayah Kabupaten Jember. Penelitian ini menggunakan pendekatan kuantitatif dengan jumlah sampel sebesar 100 responden. Teknik sampling yang digunakan adalah multistage random sampling. Instrumen yang digunakan dalam penelitian Michigan Diabetic Neuropathy Score (MDNS), Michigan Neuropathy Screening Instrument (MNSI), Ipswich Touch Test (IpTT) dan Monofilament Test. Alat pengumpulan data berupa ceklist yang telah disesuaikan dengan diabetic foot screening, tensimeter aneroid, dan stetoskop. Penelitian ini dianalisis secara univariat, ditampilkan dalam bentuk tabel distribusi frekuensi. Hasil penelitian memberikan gambaran identifikasi risiko ulkus diabetik berdasarkan nilai neuropathy perifer diabetik, nilai ankle brachial index (ABI), deformitas pada kaki, gangguan mobilisasi, kuku patologis, riwayat ulkus dan amputasi. Diabetisi memiliki risiko ulkus diabetik dengan kategori rendah sebanyak 41 diabetisi (41%), risiko sedang sebanyak 56 diabetisi (56%) dan kategori risiko tinggi sebanyak 2 diabetisi (2%). Diabetisi di Kabupaten Jember memiliki risiko ulkus diabetik. Identifikasi risiko terjadinya ulkus diabetik dibutuhkan oleh diabetisi sebagai rujukan untuk melakukan tindakan preventif agar tidak terjadi perburukan komplikasi dari DM. The mortality rate of people with diabetes who have diabetic ulcers is also increasing, prevention strategies to identify risks are required. The study sought to identify diabetics in Jember that were at risk of developing diabetic ulcers. With a sample size of 100 respondents, this study takes a quantitative approach. The sampling technique used is multistage random sampling. The instruments used were Michigan Diabetic Neuropathy Score, Michigan Neuropathy Screening Instrument, Ipswich Touch Test, and Monofilament Test. Data collection are a checklist form that has been adjusted to the diabetic foot screening, aneroid sphygmomanometer, and stethoscope. This was analyzed univariately and displayed as a frequency distribution table. The results provide an overview of ulcer risk identification based on diabetic peripheral neuropathy values, ankle-brachial index values, foot deformities, impaired mobilization, pathological nails, history of ulcers, and amputations. Diabetes patients have a low risk of diabetic ulcers 41 people (41%), a moderate risk 56 people (56%), and a high risk 2 people (2%). Diabetics in Jember are at risk of developing diabetic ulcers. Identifying the risk of diabetic ulcers is necessary for people with diabetes to take prevention. Keywords: diabetic foot screening, DM type 2, diabetic ulcer
