Abstract

ABSTRACTIntroduction: Dentures play a role in returning both physiological function and esthetics of a patient that experiences tooth loss, one of the steps in denture fabrication is vertical dimension measurements that consists of the Vertical Dimension of Occlusion (VDO) and the Vertical Dimension at Rest (VDR), this step is crucial in ensuring the rehabilitation of function and esthetics are achieved. The purpose of this study is to analyze the suitability of vertical dimension measurements before and after insertion of complete dentures. Methods: This is an observational study using the cross sectional method done on 30 patients indicated for removable dentures treated by coassistents and residency at prosthodontic clinic RSGM UNPAD. Sample size is determined using the purposive sampling method. Measurements are done using the 2 dot method, first measurements are done prior to insertion using the Niswonger method which is VDR subtracted by freeway space to get the first VDO measurement. The second measurement is done after denture insertion which then patients are put in the centric occlusion position, the distance between the 2 dots subnation and gnation are measured to get the second VDO after insertion Results: Probability value of p=0,000 using paired t-test analysis shows that there is significant difference between VDO and DVF before and after insertion off dentures (p-value 0,000 ≤0,05). Conclusion: There is a difference in the measurement of the VDO between before and after the insertion of complete removable dentures, and this difference remains within an acceptable tolerance range.KEY WORDS:  complete dentures, vertical Dimension of Occlusion, physiologic vertical dimension, before and after denture insertionEvaluasi pengukuran dimensi vertikal sebelum dan sesudah pemasangan  gigi tiruan lengkap lepasan: Studi observasionalABSTRAKPendahuluan: Langkah penting pada rehabilitasi gigi tiruan yang berperan untuk mengembalikan fungsi dan estetik dari pasien yang kehilangan gigi adalah pengukuran dimensi vertikal yang terbagi menjadi Dimensi Vertikal Oklusi (DVO) dan Dimensi Vertikal Fisiologis (DVF). Tujuan dari penelitian ini adalah untuk menganalisis  kesesuaian dimensi vertikal sebelum dan sesudah pemasangan gigi tiruan lengkap lepasan. Metode: Jenis penelitian ini merupakan metode observasional dengan desain cross sectional. Subjek pasien poliklinik Prostodonsia RSGM UNPAD. Data dimensi vertikal oklusi pasien sebelum pemasangan gigi tiruan (diukur dengan rumus Niswonger, DVO= DVF-2 mm freeway space pada posisi istirahat fisiologis) dan sesudah pemasangan gigi tiruan saat oklusi sentrik, diukur dengan mengukur jarak 2 titik subnation dan gnation. Hasil: Hasil nilai probabilitas p=0,000 menggunakan uji paired t-test menunjukan terdapat perbedaan yang signifikan pada pengukuran DVF dan DVO sebelum dan sesudah pemasangan gigi tiruan lengkap lepasan (p-value 0,000 ≤0,05). Simpulan: Terdapat perbedaan hasil pengukuran Dimensi Vertikal Oklusi (DVO) antara sebelum dan sesudah pemasangan gigi tiruan lengkap lepasan, tetapi perbedaan hasil ini masih dalam batas toleransi yang dapat diterima.KATA KUNCI: Gigi Tiruan Lengkap Lepasan, dimensi Vertikal oklusi, dimensi vertikal fisiologis, sebelum dan sesudah pemasangan gigi tiruan

Abstract

ABSTRAKPendahuluan: Hilangnya struktur anatomi mandibula dapat disebabkan oleh karena malformasi kongenital, trauma, serta reseksi dengan menghilangkan jaringan tumor. Tumor ameloblastoma seringkali melibatkan sejumlah tulang pada mandibula dan metastasis dapat terjadi sehingga harus dilakukan reseksi. Defek yang mempengaruhi fungsi bicara, pengunyahan serta penelanan merupakan konsekuensi dari dilakukannya reseksi, sehingga pembuatan obturator menjadi penting untuk dilaksanakan. Laporan kasus ini bertujuan untuk memaparkan tatalaksana rehabilitasi fungsi dan estetika pada pasien post hemimandibulektomi dengan obturator definitif. Laporan kasus: Seorang laki-laki berusia 19 tahun datang ke Departemen Prostodonsia Rumah Sakit Hasan Sadikin Bandung untuk dibuatkan protesa pasca reseksi. Pasien telah menjalani operasi hemimandibulektomi disertai rekonstruksi plat dengan riwayat medis terdiagnosa ameloblastoma tipe plexiform ar. mandibula sinistra. Pasien merasa fungsi bicara, penelanan serta penampilannya terganggu karena setiap pergerakan membuka mulut, rahang mengalami deviasi dan kesulitan untuk mengunyah makanan yang halus sekalipun. Secara klinis pada bagian defek yang dipasang plat tidak ada rasa sakit, inflamasi serta infeksi sehingga dilakukan penatalaksanaan obturator definitif dengan pertimbangan agar dapat memulihkan fungsi pengunyahan dengan segera dan untuk memperoleh stabilitas oklusi. Pasien dibuatkan obturator definitif mandibula kerangka logam dengan metode pencetakan alter cast. Simpulan: Penatalaksanaan rehabilitasi dengan obturator definitif mandibula dapat memulihkan fungsi bicara, pengunyahan dan penelanan serta meningkatkan kepercayaan diri pasien.Kata kunciobturator definitif mandibula, pencetakan alter cast, hemimandibulektomi, ameloblastoma Aesthetic and functional rehabilitation on post hemimandi-bulectomy patient with obturator definitiveABSTRACTIntroduction: Losing part of the mandible structure might be caused by congenital malformations, trauma, and surgical resection by removing a tumor. It is frequently found that ameloblastoma involves several bones of the mandible and metastasis might occur resulting in mandible resection. As a consequence, the defects would occur and influence some functions including speech and swallowing so the fabrication of an obturator was necessarily needed. Case report: A 19 years old male came to the Department of Prosthodontics Rumah Sakit Hasan Sadikin Bandung required a prosthesis after surgical resection. The patient has been diagnosed with ameloblastoma plexiform type ar. mandibula sinistra and underwent hemimandibulectomy including plate reconstruction inserted. There are some impairments in speaking, swallowing also reduced visual performance because of mandibular deviation and chewing difficulty, even soft food most of the time. Mandibula definitive frame obturator was then delivered to the patient with the alter cast impression method. Conclusion: The rehabilitation with the mandible definitive obturator gained some function including speech, chewing, and swallowing and there’s an improvement in the patient’s self-esteem.Keywordsmandible definitive obturator, alter cast impression, hemimandibulectomy, ameloblastoma

Abstract

Temporomandibular joint dysfunction is a term that covers a number of clinical problems that involves masticatory muscles, temporomandibular joints, and related structures, or both. Loss of tooth was an etiology of temporomandibular joint dysfunction. The purpose of this study was to obtain the description of temporomandibular joint dysfunction level that caused by tooth loss of elderly in three nursing home Bandung. This was a descriptive study using the survey method of the elderly in three nursing home Bandung. A total of 34 people consist 6 males and 28 females. The subjects were examined by symptoms of temporomandibular joint dysfunction and the dysfunction level was assessed by Helkimo Clinical Dysfunction Index. The result of this study shows that elderly in Nursing Home Bandung have mild dysfunction level was 7 (14.71%), moderate dysfunction level was 22 (64.71%), and severe dysfunction level is 5 (20.58%). It can be concluded that loss of the teeth is one of the etiologies of temporomandibular joint disorder. Based on the research conducted, it can be concluded that all elderly with teeth loss will have the temporomandibular joint disorder and the most severity happens based on teeth loss by using the Helkimo Clinical Disfunction Index score was the moderate disorder.

Abstract

The eyes are a vital component of facial esthetics and expression. Ocular defects can have a significant psychological impact on patients. An ocular prosthesis is an artificial maxillofacial prosthesis used to restore the appearance of a missing eye. It can be customized to replicate the patient’s natural eye in terms of color, shape, size, and movement, resulting in a realistic and natural-looking outcome. The objective of this case report is to explain the procedural steps involved in the fabrication of a custom ocular prosthesis, employing an impression technique using a custom tray and incorporationg the iris button technique. A 36 years old female patient came to RSGM Padjadjaran University to fabricate a new eye prosthesis. The patient had a prior medical history of a severe ocular infection, which necessitated enucleation surgery. The ocular prosthesis was fabricated using an impression technique with a custom tray, employing a physiological impression method to capture the deep regions of the superior palpebral socket. The sclera, iris, and pupil were fabricated using the iris button technique. Custom ocular prostheses incorporating physiological impression technique and iris button technique offer ocular prosthesis with good stability and more natural appearance. Abstract

Absolute glaucoma is a progressive condition that causes irreversible optic nerve damage from elevated intraocular pressure. When it no longer responds to treatment, patients may develop severe pain and complete vision loss, making enucleation the final option for relief. After enucleation, prosthetic rehabilitation is essential to restore facial appearance, improve function, and support psychological well-being. This case report outlines the clinical course and prosthetic management of an anophthalmic socket after enucleation for absolute glaucoma, highlighting key fabrication steps and the importance of structured follow-up care. A 23-year-old male with worsening, treatment-resistant glaucoma experienced persistent pain, scleral redness, and complete vision loss, leading to enucleation in 2018. His first prosthesis loosened after two years, and recurrent socket bleeding in 2023 necessitated an additional surgery. A new custom prosthesis was fabricated using a polyvinyl siloxane impression, followed by wax try-ins and precise iris–pupil positioning with callipers and a PD ruler. Acrylic painting was applied to create a natural ocular appearance. After insertion, the prosthesis was evaluated for comfort, stability, and esthetics, with follow-ups at one day, one week, and six months. This case highlights the value of personalized prosthetic design and consistent aftercare in achieving optimal outcomes for patients with anophthalmic sockets.

Abstract

Introduction. Intraradicular posts are usually used on teeth that have lost a lot of hard tissue. Many in vitro studies on the tensile bond strengths of various cements used to retain post and core have reported conflicting results.Purpose: The purpose of this study was to compare the tensile bond strength of customized cast post and core cemented with glass ionomer or self adhesive resin cement. Methods. Eight extracted human insisivus were endodotically treated. The root preparations were accomplished using reamers (9 mm in depth and 1.375 mm in diameter). Pattern for cast post and core were made using inlay resin pattern, and specimens were cast in chromium-cobalt alloy. Specimens divided into 2 groups. Cast post and core in group 1 (n=4) were cemented using glass ionomer cement and in group 2 (n=4) cemented using self adhesive resin cement. Tensile bond strength were measured in universal testing machine at crosshead speed of 0.5 mm/min. The results (N) were statistically analyzed by ANOVA (?=.05). Results. There is a difference in the strength of the adhesive between glass ionomer cement and self adhesive resin cement in numerical data, although the difference is not statistically significant.The ANOVA indicated that there were no significant differences among the groups tested. Tensile bond strength mean between glass ionomer and self adhesive resin cement were statistically similar. The mean value of group 1 (glass ionomer) adhesion strength is 209,292 N, group 2 (self adhesive resin) is 196,190 N. Conclusion. Customized cast posts and cores cemented with glass ionomer and self adhesive resin cement demonstrated similar mean tensile bond strength.
