Abstract

Patients with paralytic ileus generally have complaints of abdominal pain of uncertain duration. The pain that appears can get worse with anxiety about something being experienced, such as a surgical action plan. Feelings of anxiety can be a stressor and cause nursing problems in the form of anxiety. Guided imagery therapy is known to be a non-pharmacological therapy in dealing with pain and feelings of anxiety in preoperative patients. This study aims to analyze the administration of guided imagery relaxation therapy to the problem of acute preoperative pain in patients with paralytic ileus. The research method used in this study was a descriptive case study. The research was conducted for three days on one patient with preoperative paralytic ileus in the Mawar Room at RSD dr. Soebandi Jember. Guided imagery therapy is carried out twice daily for 20-30 minutes. The results obtained from this study were that after pain management was carried out with guided imagery therapy, the patient's pain level decreased from the NRS 6 scale to the NRS 5 scale, respiratory frequency 20x/minute, SpO2 96%, blood pressure 130/100 mm Hg, pulse rate 101x/minute. Reducing the pain scale results indicate that guided imagery therapy can be applied to treat pain and anxiety in preoperative patients to improve their condition. respiratory frequency 20x/minute, SpO2 96%, blood pressure 130/100 mmHg, pulse rate 101x/minute. Abstract

Pain is the main problem experienced by many postoperative patients. One of the therapies for patients with rectal cancer is surgery, so patients with rectal cancer also feel pain after getting surgery. > 80% of patients experience postoperative pain, but appropriate pain management only occurs in approximately 30-50%. Benson's relaxation therapy reduces pain intensity in postoperative patients without causing side effects. This study aims to analyze benson therapy to reduce pain in postoperative surgery for patients with rectum cancer. The method used in this research is a descriptive case study. The research was conducted for three days on one patient with postoperative surgery in the Seruni Room at RSD dr Soebandi Jember. Benson therapy is carried out twice a day for 10-15 minutes. This study found that Benson therapy can reduce the level of acute pain in postoperative patients from the NRS scale 6 to the NRS scale 3, respiration rate 20 x/minute, SpO2 98%, blood pressure 120/80mmHg, pulse 87x/ minute. The result of reducing the pain scale indicates that Benson therapy can be applied to treat acute pain in postoperative patients to make certain patients' conditions feel better.

Abstract

Hyperglycemia is the main symptom in patients with diabetes mellitus. One of the risk factors that can increase blood sugar levels in diabetes mellitus (DM) patients is anxiety and stress, as a result of diabetes mellitus sufferers who experience anxiety, their physical condition does not improve, and their blood sugar levels are always high. Progressive muscle relaxation therapy (PMR) is a therapy that aims to focus the patient's attention so that they can differentiate the feelings that occur when the muscles are relaxed or relaxed from when they are contracting so that the patient can manage their body condition against their anxiety. This study aims to analyze the provision of progressive muscle relaxation therapy to control anxiety levels and blood sugar levels in patients with type 2 diabetes mellitus in the Gardena medical ward of Dr. Soebandi Hospital Jember. This research uses descriptive methods in the form of case studies on a patient with Type 2 DM. Therapy was given once a day for 20-30 minutes. Anxiety levels were measured using the DASS questionnaire, and blood sugar measurements were made using a blood sugar stick. After being given PMR therapy for 3 x 24 hours, the patients experienced a decrease in anxiety levels and blood sugar levels. Providing PMR therapy is effective in treating unstable blood glucose levels and anxiety for patient with Type 2 DM.

Abstract

Background: Multiple Fractures or broken bones are a condition where the continuity of the bones is broken in a complex manner, this is caused by trauma caused by frequent traffic accidents. Objective: To analyze the effect of a combination of non-pharmacological therapies Quantum Spiritual Touch Therapy" (QST) and Finger Holding in multiple fraktur patients with acute pain at RSD dr. Soebandi Jember. Method: This research is an observational study with a case study design. Primary data obtained through anamnesis and physical assessment. Assessment of the effect of QST therapy and finger holding was carried out a total period of 35 minutes, 20 minutes for QST therapy and 15 minutes for finger holding therapy, based on initial, process, and final data. Results: The results showed a reduction in pain levels with a Numerical Rating Scale (NRS) was carried out for 6 meetings with the implementation of therapy once a day. Pain levels decreased in the pre-implantation measurements with an average of 5,4 to 4,7 at the time of post-implementation. Discussion: The combination of pharmacological therapy from QST therapy and finger holding aims to help the effectiveness of analgesic drugs by improving the psychological structure with implementations that contain spiritual elements.

Abstract

Patients with fracture conditions often experience complaints of excessive anxiety, where anxiety is felt due to the pain that is felt and the provision of invasive or surgical action plans. Preoperative patients will certainly feel excessive anxiety. Feelings of anxiety will cause stress and cause nursing problems, namely anxiety. Dhikr therapy is a non-pharmacological therapy and includes complementary therapy, where this therapy can reduce the level of anxiety felt by preoperative patients. The purpose of this study was to analyze the administration of dhikr therapy in patients with preoperative close fractures of the left distal radius. This research uses the case study method. This research was conducted in the Seruni room at dr. Soebandi Regional Public Hospital of Jember. Dhikr therapy is carried out for four consecutive days with a time of approximately 20 minutes. The results obtained from this study after administration of anxiety reduction combined with dhikr therapy obtained the patient's anxiety scale level from VAS-A4 to the VAS-A0 scale, visualization of anxiety complaints decreased, restless behavior decreased, and eye contact increased, respiratory rate 16 x/minute, SpO2 98%, pulse 90 x/minute, and blood pressure 130/80 mmHg. Reducing anxiety with dhikr therapy can be given to pre-surgery patients, this can improve health quality, sleep quality, and physical condition.

Abstract

Introduction:the pain of post-operative laparotomy is caused by tissue discontinuity or surgical wounds due to surgical incisions, so the skin's nerve cells become damaged. Laparotomy pain is often found in severe and moderate levels of pain intensity due to damage to the integument, muscle tissue, and vessels, and it causes in longer pain during the recovery period. Purpose: describe the actions of nurses in dealing with the pain of post-operative laparotomy patients in the hospital. Methods: this literature review uses the Google Scholar, PubMed, and Science Direct databases with cohort study designs, cross-sectional studies, case studies, randomized control trial studies, quasi-experimental studies published between 2019-2023. Result: the 10 articles reviewed in this study showed that the pain felt by post-operative laparotomy patients in the hospital experienced a decrease in the pain scale, from moderate pain to mild pain. Nursing actions related to pain can be done with pharmacological and non-pharmacological approaches. Analysis:the pharmacological approach can be carried out by administering analgesic therapy to reduce the intensity of pain felt by the patient while the non-pharmacological approach can be carried out by providing therapy such as; murottal Al-Qur'an therapy, lemon aromatherapy, deep breathing relaxation, finger grip relaxation, and hand massage therapy. Abstract

Post-laparotomy pain is one of the major problems in nursing practice frequently experienced by patients after undergoing surgical procedures. Poorly managed pain can lead to discomfort, sleep disturbances, increased physiological stress, limited mobility, and delayed wound healing. Effective pain management is an essential part of enhancing patient comfort and accelerating recovery. In addition to pharmacological approaches, several studies showed that non-pharmacological interventions such as hand massage can serve as effective, safe, and easily applicable pain management. This study aims to identify patient’s outcome after receiving hand massage therapy using baby oil in reducing pain in post-laparotomy. The research design employed a descriptive quantitative case study approach. The subject was a single post-laparotomy patient at dr. Soebandi Hospital, Jember, experiencing moderate pain, as measured by the Numeric Rating Scale (NRS). The patient was given nursing care, and one of the interventions was hand massage, as well as pain relief medication. The hand massage intervention was administered twice daily for four consecutive days, with each session lasting 10 minutes. The patient's pain scale was observed daily. The results of the case study showed a decrease in pain from a scale of 6 on the first day to a scale of 3 on the fourth day Patient said that everytime he got pain medication, he still feel the pain, but the massage made it better. Therefore, this therapy can be considered as an effective therapy and applicable complementary nursing intervention in managing post-laparotomy pain

Abstract

Abstrak Ulkus diabetik adalah komplikasi DM yang paling sering terjadi. Angka mortalitas diabetisi dengan ulkus diabetik juga semakin meningkat, sehingga dibutuhkan strategi pencegahan untuk mengidentifikasi risiko. Tujuan penelitian adalah teridentifikasi diabetisi yang berisiko mengalami ulkus diabetikum berbasis Diabetic Foot Screening di wilayah Kabupaten Jember. Penelitian ini menggunakan pendekatan kuantitatif dengan jumlah sampel sebesar 100 responden. Teknik sampling yang digunakan adalah multistage random sampling. Instrumen yang digunakan dalam penelitian Michigan Diabetic Neuropathy Score (MDNS), Michigan Neuropathy Screening Instrument (MNSI), Ipswich Touch Test (IpTT) dan Monofilament Test. Alat pengumpulan data berupa ceklist yang telah disesuaikan dengan diabetic foot screening, tensimeter aneroid, dan stetoskop. Penelitian ini dianalisis secara univariat, ditampilkan dalam bentuk tabel distribusi frekuensi. Hasil penelitian memberikan gambaran identifikasi risiko ulkus diabetik berdasarkan nilai neuropathy perifer diabetik, nilai ankle brachial index (ABI), deformitas pada kaki, gangguan mobilisasi, kuku patologis, riwayat ulkus dan amputasi. Diabetisi memiliki risiko ulkus diabetik dengan kategori rendah sebanyak 41 diabetisi (41%), risiko sedang sebanyak 56 diabetisi (56%) dan kategori risiko tinggi sebanyak 2 diabetisi (2%). Diabetisi di Kabupaten Jember memiliki risiko ulkus diabetik. Identifikasi risiko terjadinya ulkus diabetik dibutuhkan oleh diabetisi sebagai rujukan untuk melakukan tindakan preventif agar tidak terjadi perburukan komplikasi dari DM. Kata kunci: diabetic foot screening, DM tipe 2, ulkus diabetik   Abstract The most common complication of diabetes is diabetic ulcers. The mortality rate of people with diabetes who have diabetic ulcers is also increasing, prevention strategies to identify risks are required. The study sought to identify diabetics in Jember that were at risk of developing diabetic ulcers. With a sample size of 100 respondents, this study takes a quantitative approach. The sampling technique used is multistage random sampling. The instruments used were Michigan Diabetic Neuropathy Score, Michigan Neuropathy Screening Instrument, Ipswich Touch Test, and Monofilament Test. Data collection are a checklist form that has been adjusted to the diabetic foot screening, aneroid sphygmomanometer, and stethoscope. This was analyzed univariately and displayed as a frequency distribution table. The results provide an overview of ulcer risk identification based on diabetic peripheral neuropathy values, ankle-brachial index values, foot deformities, impaired mobilization, pathological nails, history of ulcers, and amputations. Diabetes patients have a low risk of diabetic ulcers 41 people (41%), a moderate risk 56 people (56%), and a high risk 2 people (2%). Diabetics in Jember are at risk of developing diabetic ulcers. Abstract

Background: Spiritual distress is a problem with the patient's response to health. Spiritual distress is a disturbance in the meaning of life about oneself, others, and God. Spiritual distress occurs due to spiritual needs not being met. Purpose: This study aims to determine spiritual distress in post-operative orthopedic patients in the surgical room at dr. Soebandi Regional Hospital Jember. Methods: This study employed a descriptive, consecutive sampling method involving 150 respondents from both major and minor post-operative orthopedic patients in 2023. Results: The majority of respondents in this study were men with varying ages and types of surgery, 80.7% experienced major surgery, and 19.3% underwent minor surgery. Data collection was conducted using the Spiritual Distress Scale questionnaire, and the results showed a median value of 74.00, with a minimum value of 33 and a maximum value of 103. This indicates that the resulting value is close to the maximum value of the questionnaire, suggesting a high level of spiritual distress. This can be caused by the high value placed on self-relationship, which encompasses the strengths that exist within a person, including knowledge and attitudes about oneself. This high value can occur due to a lack of knowledge about oneself and a lack of confidence in the future, leading to a loss of peace of mind. Conclusion: This research provides additional data and knowledge regarding spiritual distress in post-operative orthopedic patients. Patients with spiritual distress require maintenance of spiritual well-being, which helps them find meaning in life and experience a sense of peace. The results of this research make it possible for nurses to develop and understand the importance of spiritual care for patients

Abstract

Dengue fever or hemorrhagic fever is an acute infectious disease that can be transmitted by the Aedes aegypti mosquito. The virus carried by the Aedes aegypti mosquito will attack cells in the liver, disrupting liver function and causing swelling and bleeding. Symptoms that can occur are high fever, muscle and joint pain, and rash. One of the nonpharmacological therapies is deep breath relaxation therapy to reduce pain. This study aims to analyze deep breath relaxation techniques to reduce pain. This research design used a case report approach.   The pain can decrease after being given deep breath relaxation therapy for 3 days. On the first day, on a scale of 7, it decreased to a scale of 2 on the third day of hospitalization. Applying deep breath relaxation techniques can help reduce pain intensity by relaxing skeletal muscles that experience spasms due to increased prostaglandins, so that vasodilation of blood vessels will increase blood flow that is spasmed and ischemic. This technique can help a person or patient control themselves when feeling emotional discomfort, physical stress, or pain. Deep breath relaxation techniques can be given to patients who experience pain as a nonpharmacological therapy. The application of this relaxation technique is proven to reduce pain intensity.
