Abstract

Latar Belakang: Kecemasan pada pasien sindrom koroner akut (SKA) yang menjalani rawat jalan masih sedikit diteliti, padahal kecemasan pada pasien rawat jalan merupakan hal yang penting untuk dikaji karena pasien berada di rumah dan jauh dari fasilitas kesehatan. Beberapa konsep menyatakan bahwa kecemasan pada pasien SKA dapat memperburuk keadaan jantung, namun demikian bagaimana kaitan antara cemas dengan frekuensi angina masih belum banyak diteliti.Tujuan: Penelitian ini bertujuan mengidentifikasi hubungan kecemasan dengan frekuensi angina pada pasien SKA.Metode: Desain penelitian ini adalah correlational study. Penelitian dilakukan di Poliklinik Jantung di satu Rumah Sakit di Jawa Barat. Jumlah sampel sebanyak 100 responden dengan SKA yang di ambil menggunakan teknik consecutive sampling. Data dikumpulkan dengan menggunaan kuesioner ZSAS dan SAQ selama satu bulan, kemudian data dianalisis menggunakan analisis deskriptif dan korelasi Rank Spearman.Hasil: Mayoritas responden memiliki tingkat kecemasan normal dan frekuensi angina minimal. Sedangkan responden yang mengalami cemas berat sebesar 3% dan mengalami frekuensi angina berat sebesar 8%.Hasil perhitungan p-value sebesar  0,00 yang lebih kecil dari α (0,05) dan r -0,508 maka terdapat hubungan yang bermakna antara kecemasan dengan frekuensi angina.Kesimpulan: Terdapat hubungan antara kecemasan dengan frekuensi angina pada pasien dengan SKA. Semakin tinggi tingkat kecemasan seseorang maka frekuensi angina semakin berat.

Abstract

Fraktur merupakan salah satu masalah yang sering muncul di Instalasi Gawat Darurat (IGD). Fraktur mengakibatkan penderitanya mengalami gangguan fisiologis maupun psikologis yang dapat menimbulkan respon berupa nyeri. Tujuan penelitian ini adalah untuk mengetahui manajemen farmakologis nyeri yang dapat dilakukan pada pasien dengan fraktur di ruang instalasi gawat darurat (UGD). Literature Review dengan pencarian artikel menggunakan database Scopus, Pubmed, Cambridge Core, Google Scholar, dan Cinahl. Artikel yang digunakan adalah artikel yang diterbitkan dalam kurun waktu tahun 2014 sampai dengan tahun 2023, dengan menggunakan metode PCC setelah dilakukan seleksi dan diidentifikasi sesuai kriteria inklusi, terdapat 5 artikel yang akan direview. Berdasarkan hasil analisis 5 artikel penelitian yang direview, maka didapatkan hasil bahwa penggunaan ultrasound-guided block saraf femoral dan parenteral opioid, penggunaan ultrasound-guided fascia iliaca block, penggunaan intranasal ketamin dan intranasal fentanyl, penggunaan ultrasound-guided serratus anterior plane block, dan penggunaan ultrasound-guided block hematoma dan prosedural sedasi analgesi dapat menjadi manajemen farmakologis nyeri pada pasien dengan fraktur di IGD. Peran perawat sebagai edukator diharapkan dapat memberikan edukasi terhadap klien tentang apa saja manajemen nyeri pada pasien fraktur beserta kekurangan dan kelebihannya. Selain itu sebagai kolaborator, perawat dapat berkolaborasi dengan dokter mengenai pemberian obat dan dosis yang sesuai untuk klien untuk menangani masalah nyeri pada pasien fraktur di IGD.

Abstract

Artificial intelligence atau kecerdasan buatan telah digunakan dalam analisis data yang rumit dan besar untuk memberikan keluaran tanpa input manusia dalam berbagai konteks perawatan kesehatan. Area keperawatan kritis atau perawatan intensif memiliki banyak data yang tersedia serta memiliki kebutuhan akan peningkatan efisiensi dalam perawatan pasien. Hal ini menjadi salah satu alasan, perawatan intensif dianggap cocok untuk penerapan artificial intelligence. Penelitian ini bertujuan untuk mengkaji penggunaan artificial intelligence oleh tenaga kesehatan pada area keperawatan kritis serta mengidentifikasi kekuatan dan kelemahan penggunaannya, serta untuk mengidentifikasi peluang perbaikan di masa depan. Protokol scoping review ini disusun menggunakan framework Arksey & O’Malley. Peneliti akan mencari literatur menggunakan teknik advance search pada database dan search engine CINAHL, PubMed, Scopus, Science Direct, SAGE journals dan Google Scholar. Kata kunci yang akan digunakan dalam review ini yang memenuhi unsur population yaitu, healthcare professional dengan alternatif terminologi seperti healthcare professional or healthcare providers or physician or nurse(s) or doctor(s) dan konsep artificial intelligence, dengan alternatif ai or a.i. or machine learning or deep learning, serta konteks critical care dengan alternatif intensive care or icu. Setelah itu, dua reviewer akan melakukan skrining pada abstrak secara independen sesuai dengan kriteria inklusi. Peneliti akan melaporkan hasil yang didapatkan dalam diagram alur PRISMA. Peneliti akan melakukan analisa pada hasil yang didapatkan berdasarkan data bibliografi serta hasil studi terkait penggunaan artificial intelligence pada area keperawatan kritis. Hasil dari review ini diharapkan dapat memberikan informasi terkait pelaksanaan penggunaan artificial intelligence pada area keperawatan kritis, juga kekuatan dan kelemahan penggunaannya, serta peluang perbaikan di masa depan.

Abstract

The family plays an important role in determining the method or treatment needed by the patient in the intensive care unit. However, vulnerable families experience anxiety which will prolong the decision-making process and patient care, so it is very important for nurses to overcome family anxiety so as not to interfere with the functioning of the family's role properly. The purpose of this study was to identify interventions to reduce anxiety in families accompanying patients in the intensive care unit. This research method is a literature review that analyzes articles from five reputable databases, namely Pubmed, CINAHL Plus, Wiley, Science Direct, and Google Scholar with the year of publication of the article between 2016-2021. The keywords used in the article search consisted of 'Patient's family', 'caregivers', 'family', 'therapy', 'anxiety in critical care', and 'fear'. The search results found 11 articles that met the criteria from 85,759 identified articles. The results of the article review identified that interventions that could reduce family anxiety consisted of Aurosoma Therapy, Nurse Social Support, Cognitive Behavioral Therapy (CBT), Spiritual-Religious Intervention, Tele-Mental Health Intervention, Family Integrated Care (FICare), and Nursing Intervention. As well as three interventions that can reduce other psychological symptoms such as depression and stress; Communication Facilitator, Nursing Consultation, and Early Integrated Palliative Care. It is necessary to develop research with a combination of interventions that reduce the level of anxiety in families in Indonesia and be integrated with a critical care setting system to develop comprehensive and holistic nursing care with the patient's family as one of the essential aspects that need to be considered by nursing.

Abstract

Nurses are frontliners during the COVID-19 pandemic and are vulnerable to ethical challenges when caring for patients with COVID-19. The ethical challenge of nurses is basically due to the gap between nursing practice and nursing goals. This study aims to identify empirical evidence related to the ethical challenges nurses face when caring for COVID-19 patients. This research method uses a literature study that analyzes articles from five international reputable databases such as CINAHL, Pubmed, Proquest, Science Direct, and search engines Google Scholar. The keywords used to facilitate the search for articles were 'COVID-19', 'critical care nursing', 'intensive care nursing', 'intensive care nurses', 'critical nurses', 'intensive care nurses', 'caring', 'ethical challenges', 'ethical issues', 'ethic dilemmas', ethical challenges', 'ethical issues', and 'ethical dilemmas'. with the help of boolean operators and then the articles are selected gradually using PRISMA. From the search results obtained 7 articles that deserve to be analyzed. There are three themes related to ethical challenges that occur, namely ethical challenges in the nursing care process, ethical challenges in nursing professionals, and ethical challenges in health interprofessional relationships. Ethical challenges cause nurse discomfort at work, and can even cause job stress. Regarding these results, nurses need to get support to overcome ethical dilemmas that occur through the provision of resources according to needs, preparation of SOPs regarding the roles and duties of nurses, as well as increasing knowledge regarding the care of COVID-19 patients.

Abstract

Many emergency situations require immediate first aid. The community plays a crucial role in saving victims of cardiac arrest or traffic accidents, which are among the leading causes of premature death in Indonesia. Unfortunately, the level of public knowledge and preparedness in providing first aid remains low, highlighting the need for improvement through education and training. Community health cadres were chosen as the target group because they are closely connected to the community, easily accessible, and can act as first responders before professional medical personnel arrive. This study aimed to enhance the knowledge and readiness of community cadres in recognizing and providing effective first aid through CPR and first aid training in Pangandaran Village. A pre-experimental one-group pre-test and post-test design was used, involving 33 participants selected through accidental sampling. Data analysis was conducted using a paired sample t-test. The results showed a significant improvement (p<0.05) in the cadres’ knowledge and preparedness after the training. Based on these findings, it is recommended that similar training programs be conducted regularly by health institutions and community organizations to strengthen the capacity of cadres in providing first aid, thereby increasing the survival rate of victims of accidents and cardiac arrest within the community.Keywords: Accident, cardiac aArrest, knowledge, preparedness.

Abstract

Many countries have high earthquake activities. As an integral part of the emergency response team, nurses play a critical role in disaster management. This review aimed to identify nurses' skills in dealing with earthquakes and their implications for the importance of nurse education in this area. This study used a narrative review. The literature was searched using three databases: CINAHL, PubMed, and Scopus, using the keywords "Nurses OR Nurse OR Personnel Nurses OR Registered Nurses AND Clinical Competence OR Competency, Clinical OR Clinical Skill AND Earthquakes." The analysis was carried out descriptively and qualitatively using a thematic analysis approach. The resultsshows that eight studies were analyzed in this review. Nursing competency in earthquake disasters includes skills and knowledge important for preparing, responding to, and recovering from pre- and post-disaster conditions. The results of this research classify nurses' skills into five categories: knowledge of disaster management, clinical skills and patient care, protective measures and safety leadership, communication and coordination, and psychological and social support. Conclusion: The review results highlight the importance of increasing nurses' skills through training and knowledge in disaster preparedness for nurses, emphasizing a deep understanding of emergency medical management and post-disaster recovery. The importance of adapting nursing education curricula and using effective teaching strategies to ensure that nurses have sufficient skills and knowledge to face complex disaster challenges. Keywords: Clinical competence, disaster management, nursing skills.

Abstract

Background: The families of patients play a crucial role in supporting the care and recovery process of patients in the Intensive Care Unit (ICU). One critical aspect of this role is the need for comprehensive and relevant information about the patient's condition and care in the ICU. Purpose: To explore the information needs of families of patients in the intensive care unit (ICU). Method: Literature search was conducted in Scopus, PubMed, and CINAHL databases and google scholar search engine. The initial articles found were 12,056, then 9 articles were included in the study. Data were extracted and synthesized to provide a comprehensive picture of the information needs of patient families in the ICU. Results: Families need in-depth information about the patient's condition, the care provided, and the medical team involved in the care. A variety of communication and education methods have been used to meet this information need, including the use of easy-to-understand language and the provision of written or visual materials. Education that is tailored to the family's preferences and needs and pays attention to the language used is key to providing effective support to families of patients in the ICU. Conclusion: Families play a vital role in understanding and addressing the informational needs of ICU patient families. Enhancing communication and education tailored to family needs can improve family involvement in patient care and overall care outcomes.   Keywords: Information Needs; Intensive Care Unit; Patient's Family.   Pendahuluan: Keluarga pasien memiliki peran penting dalam mendukung proses perawatan dan pemulihan pasien di Intensive Care Unit (ICU). Salah satu aspek krusial dalam peran keluarga ini adalah kebutuhan akan informasi yang komprehensif dan relevan tentang kondisi dan perawatan pasien di ICU. Tujuan: Untuk mengeksplorasi kebutuhan informasi keluarga pasien di intensive care unit (ICU). Metode: Pencarian literatur dilakukan di database Scopus, PubMed, dan CINAHL serta search engine Google Scholar. Artikel awal yang ditemukan adalah 12,056, kemudian 9 artikel disertakan dalam penelitian. Data diekstraksi dan disintesis untuk memberikan gambaran komprehensif tentang kebutuhan informasi keluarga pasien di ICU. Hasil: Keluarga membutuhkan informasi yang mendalam tentang kondisi pasien, perawatan yang diberikan, dan tim medis yang terlibat dalam perawatan. Berbagai metode komunikasi dan edukasi telah digunakan untuk memenuhi kebutuhan informasi ini, termasuk penggunaan bahasa yang mudah dipahami dan penyediaan materi tertulis atau visual. Edukasi yang disesuaikan dengan preferensi dan kebutuhan keluarga serta memperhatikan bahasa yang digunakan menjadi kunci dalam memberikan dukungan yang efektif kepada keluarga pasien di ICU. Simpulan: Keluarga memiliki peran penting dalam memahami dan memenuhi kebutuhan informasi keluarga pasien di ICU. Memperbaiki komunikasi dan edukasi yang disesuaikan dengan kebutuhan keluarga dapat meningkatkan keterlibatan keluarga dalam perawatan pasien serta meningkatkan hasil perawatan secara keseluruhan.   Kata Kunci: Intensive Care Unit; Kebutuhan Informasi; Keluarga Pasien.

Abstract

Communication between health workers is crucial in providing nursing care and improving patient safety. Communication problems can cause incidents that lead to disability or death. This study aimed to identify the implementation of handoff communication using sbar in the emergency department and intensive care unit. This review used a scoping review design that uses the Arksey and O'Malley framework. Article searches through two databases, namely Sciencedirect and Pubmed, and the search engine, Google Scholar. The keywords used were "Health workers or Healthcare professionals AND SBAR implementation or SBAR or Handoff Communication AND Emergency rooms or Emergency departments AND Intensive Care Unit". There were nine articles that fulfilled the inclusion criteria and were analyzed in this study. The results of the review showed that the description of the implementation of handoff or SBAR communication, studies showed variations in the effectiveness of its implementation in various countries and health settings. There are several factors that influence the implementation of SBAR and the implementation of handoff, namely work experience, attitudes, knowledge, and formal training. Implementation of handoff communication and SBAR still shows substansial challenges, both in terms of effectiveness and quality. Although SBAR has been widely recognized as an effective tool to improve communication, its implementation is uneven across countries and health settings. SBAR implementation is often disrupted by factors such as external interference, lack of training, or incomplete information conveyed during the handover process, which ultimately affects patient safety.

Abstract

Out-of-hospital cardiac arrest (OHCA) remains a major global health challenge with low survival rates. Artificial intelligence (AI) has emerged as a promising tool to enhance early detection, response, and management of OHCA cases. This study explores the current use of AI in OHCA, identifying challenges and opportunities related to its implementation. This scoping review followed the PRISMA-ScR guidelines, utilizing a systematic search of international databases to identify relevant literature. A total of 10 studies were included, comprising cohort studies, observational studies, randomized controlled trials (RCTs), and pilot projects from 10 different countries. AI implementation in OHCA management demonstrated several opportunities, including improved early detection (increasing sensitivity by 5.5–15% and reducing EMS response time by up to 26 seconds), enhanced decision support for termination of resuscitation (with specificity up to 99.0%), and increased bystander engagement through real-time CPR guidance. However, challenges remain, such as data privacy, ethical concerns (especially with visual surveillance and GDPR compliance), infrastructure limitations, and variability in local protocol. The paradox between faster detection and improved CPR quality was also noted. AI has significant potential to improve OHCA outcomes by optimizing detection, response, and clinical decision-making. Successful implementation requires multidisciplinary collaboration, robust external validation, and ethical considerations to address privacy and local adaptation. Integrating AI into emergency systems and public training can enhance survival rates, but further large-scale studies are needed to ensure effectiveness and equity.
