Abstract

Penelitian ini dilatarbelakangi oleh tingginya angka kematian dan kecacatan di dunia akibat sindrom koroner akut (SKA). Perawatan klien dengan SKA masih terfokus pada aspek fisik, sedangkan aspek spiritualitas banyak dilupakan.Tujuan penelitian ini adalah untuk menggali secara mendalam makna spiritualitas yang dipersepsikan oleh klien dengan SKA yang menjalani perawatan di ruang intensif jantung RSUP dr. Hasan Sadikin Bandung. Untuk mencapai tujuan tersebut penelitian ini menggunakan desain deskriptif kualitatif, dengan jumlah partisipan sebanyak sepuluh orang. Pengumpulan data dilakukan dengan wawancara dan observasi. Analisis data dilakukan dengan menggunakan content analysis.Berdasarkan hasil penelitian teridentifikasi empat kategori makna spiritualitas dengan sepuluh buah tema, yaitu: (1) Spiritual adalah hubungan dengan diri sendiri dengan dua buah tema antara lain, menerima penyakit sebagai suatu teguran atau cobaan, dan hidup menjadi lebih baik; (2) Spiritual adalah hubungan dengan Tuhan dengan lima buah tema yaitu: kepasrahan pada Tuhan, ibadah atau komunikasi dengan Tuhan, harapan, permohonan ampunan (pertobatan), dan rasa syukur; (3) Spiritual adalah hubungan dengan orang lain dengan dua buah tema yaitu: perhatian, cinta, dan kasih sayang dari orang lain, serta keberhasilan keluarga, memberikan manfaat bagi sesama; dan (4) Spiritual adalah hubungan dengan alam dengan satu buah tema yaitu menyalurkan hobi atau aktivitas di alam. Berdasarkan hasil penelitian tersebut diharapkan perawat lebih sensitif terhadap makna spiritualitas klien dan mendorong penggunaan sumber spiritualitasnya untuk proses penyembuhan klien. Institusi pendidikan diharapkan mampu mempersiapkan perawat yang peka terhadap kebutuhan spiritualitas klien serta diperlukan penelitian lebih lanjut dan lebih luas dalam lingkup perawatan spiritualitas.Kata kunci:Persepsi, spiritualitas,sindrom koroner akut AbstractThis research was conducted due to the high of mortality and disability rate in the world caused by Acute Coronary Syndrome (ACS), however the treatment of ACS’ clients are still focusing on physical aspects rather than spirituality aspects. Actually, these aspects are equally important. Through awareness of the meaning of spirituality, the clients can achieve spirituality comfort. Spiritual comfort can give peacefulness and positive impact to clients’ health. The purpose of the study was to explore the meaning of spirituality in clients with ACS who undergone treatment in the cardiac intensive care RSHS Bandung. The study used a descriptive exploratory design with 10 participants. The data was collected through interviews and observations. The data were analysed using content analysis the data analysis performed using content analysis. The result identified four categories of the meaning of spirituality followed by 10 themes, namely : (1) Spirituality was self-relationship that consists of two themes, include a. Accepted of disease as a reproach or temptation b. Better in life; (2) Spirituality was relationship between human and God, it consists of five themes, namely : a. Self-reliance, b. Worship or communication with God, c. Hope, d. Asking forgiveness or repentance, e. Grateful; (3) spirituality was a relationship with others, it consists of two themes, namely : a. caring, love, affection from others, and the success of the family b. Giving to others; (4) Spirituality was relationship between human with nature, consist of one theme, namely : doing personal interest or activity in the nature environment. Based on the results, nurses are expected to be more understood to the meaning of clients’ spirituality and encouraging them to use their spirituality sources for their healing process. Educational institutions are expected to prepare the student with high sensitivy of clients’ spirituality needs, and then further research of spirituality care is needed with broader scopesKey words: Acute Coronary Syndrome, perception, spirituality

Abstract

Tingginya angka kematian yang terjadi di unit perawatan intensif, menuntut peningkatan pelayanan perawatan paliatif termasuk perawatan pasien menjelang ajal, yang melibatkan perawat perawatan kritis. Tujuan penelitian ini untuk memperoleh gambaran persepsi perawat terhadap perawatan pasien menjelang ajal di ruang Neurosurgical Critical Care Unit(NCCU). Delapan perawat pelaksana di ruang NCCU RSHS Bandung dilibatkan dalam penelitian deskriptif kualitatif ini dengan rentang usia antara 27– 43 tahun, dan bekerja selama 3–20 tahun. Teknik sampling yang digunakan adalah purposive samplingdan jumlah informan dibatasi setelah data jenuh. Pengumpulan data dengan melakukan wawancara semi terstruktur, dan analisis yang digunakan adalah content analysis. Hasil penelitian didapatkan 4 tema dan 15 subtema yaitu: (1) Pemahaman perawat tentang perawatan pasien menjelang ajal yaitu: membantu pasien meninggal dengan tenang, menghadirkan keluarga untuk memberikan dukungan, dan lebih berfokus pada bimbingan spiritual; (2) Cara menghadapi kematian yang sering terjadi yaitu: adaptasi perawat terhadap kondisi pasien menjelang ajal, kesulitan menentukan fase menjelang ajal pasien kritis, dilema dalam pengambilan keputusan, dan empati; (3) Peran perawat dalam mempersiapkan pasien menjelang ajal yaitu: pembimbing spiritual pasien, komunikator, fasilitator, dan pemberi dukungan emosional keluarga; (4) Hal-hal yang perlu diperbaiki dalam perawatan menjelang ajal yaitu: diperlukan pelatihan perawatan paliatif pada pasien kritis, diperlukan ruangan khusus pasien menjelang ajal, diperlukan pembimbing rohani khusus, dan diperlukan standar operasional prosedur (SOP) perawatan pasien menjelang ajal. Perawat perlu memberikan perawatan yang membantu pasien meninggal dengan tenang, memberikan dukungan untuk keluarga, dan lebih difokuskan untuk memenuhi kebutuhan spiritual pasien.Kata kunci: Intensive Care Unit, perawatan akhir hidup, persepsi perawat AbstractThe high number of death that occurred in the Intensive Care Unit, strive to improve palliative care services including the provision of care of dying patients by critical care nurses. The purpose of this study was to obtain a perception of nurses toward the care of dying patients in the Neurosurgical Critical Care Unit ( NCCU ). Eight nurses who work at NCCU were involved in this study, age between 27- 43 and have been working from 3 to 20 years. The sampling technique used the purposive sampling method and a limited number of informants after data saturated. Data collection was done by conducting semi- structured interviews, content analysis was used to analyse the data. There are four themes with 15 sub-themes include: 1) Nurse understanding about caring for dying patients: help the patients to die peacefully, presenting the family to provide supports, and more focused on spiritual guidance. 2) Way of handling the frequent of death occurance: adaptation of nurses to dying condition, difficulty determining the critical phase of the dying patient, dilemmas in decision-making, and empathy. 3) The role of nurses in preparing for the dying patient: the patient spiritual guides, communicators, facilitators, and providers of family emotional support. 4) The Things that need to be improved in end of life care: the palliative care training is required in critically ill patients as well as separate unit for dying patients, exclusive spiritual guide, and standard operating procedures (SOP) of care for the dying patients. It can be concluded that nurses need to provide treatment that helping patients to die peacefully, and providing support for the family, which is focused on meeting the spiritual needs of patients.Key words:Intensive Care Unit, end of life care, nurses perception

Abstract

Luka tekan merupakan salah satu dampak hospitalisasi pada pasien kritis. Perawat memiliki peran yang penting dalam pencegahan luka tekan tetapi banyak perawat yang belum melakukan upaya pencegahan luka tekan secara maksimal sehingga perlu kajian untuk mengetahui faktor –-faktor yang memengaruhinya. Intensi merupakan antiseden terdekat dengan perilaku seseorang. Berdasarkan Theory of Planned Behavior (TPB), intensi seseorang untuk melakukan sesuatu dipengaruhi oleh sikap, norma subyektif, dan pengendalian perilaku. Penelitian ini bertujuan untuk mengetahui faktor yang memengaruhi intensi perawat melakukan pencegahan luka tekan. Penelitian kuantitatif ini menggunakan deskriptif analinitik observasional dengan pendekatan korelasional dilakukan secara cross sectional. Populasi pada penelitian ini adalah perawat yang melakukan asuhan keperawatan langsung pada pasien di ruang perawatan intensif. Teknik pengambilan sampel dengan menggunakan total sampling, yaitu 70 perawat di ruang perawatan intensif. Hasil penelitian ini menunjukkan adanya hubungan antara sikap dengan intensi (r = 0,632, p < 0,05), norma subyektif dengan intensi (r = 0,625, p < 0,05), dan pengendalian perilaku dengan intensi (r = 0,633, p < 0,05). Semakin favorable sikap perawat maka akan semakin tinggi intensi perawat melakukan pencegahan luka tekan. Semakin tinggi tekanan normatif dari sekitar perawat maka akan semakin tinggi pula intensi perawat, dan semakin tinggi pengendalian perilaku yang dirasakan perawat maka akan semakin tinggi pula intens perawat.Kata kunci: Intensi, pencegahan luka tekan, perawat, ruang intensif.Nurses’ Intention to Prevent Pressure Ulcers in ICU based on Theory of Planned BehaviourAbstractPressure ulcer is one of the effects of hospitalization in critically-ill patients. Nurses have an important role in preventing pressure ulcer but many nurses do not implement preventive measures well. Factors influencing it need to be studied. Intention is the closest antecedent to behaviour. Based on the Theory of Planned Behavior (TPB), intention to do something is influenced by attitude, subjective norms, and perceived behavior control (PBC). The purpose of this study was to examine the relationship among antecedents: attitude, subjective norm, perceived behavior control and the intention of nurses to prevent pressure ulcer. This research was a correlational study using observational analytic description design with cross sectional approach. Population of this study was nurses who worked in the intensive care unit. Total sampling technique was used to select 70 nurses. Results of this study showed  there was relationship between attitude and intention (r = 0.632, P <0.05), subjective norms and intention (r = 0.625, P <0.05), and PBC and intention (r = 0.633, p <0.05). The more favorable the attitude, the higher the intention to prevent pressure ulcer. The higher the normative pressure from surrounding nurses, the higher the intention. The higher the perceived behavioral control, the higher the intention to prevent pressure ulcer.Key words: Intense, intensive care unit, nurse, pressure ulcer prevention.

Abstract

The goals of decision making process about Do Not Resuscitate orders (DNR) are supporting the autonomy of patient or family and preventing futile medical interventions. Families can experience challenges and difficulties when making decision about DNR. The exploration of the decision-making process involving family is therefore crucial. This study aimed to explore in-depth the decision making of DNR among End of Life (EoL) patients within inpatient area from the perspective of family caregiver. A qualitative descriptive phenomenology study was conducted that recruited 6 family caregivers, within purposive sampling. Data collection used in-depth interviews with face to face and video calls, and data analysis followed the Colaizzi’s method. Twelve major themes emerged from interview finding of family caregiver: decision making made through family counselling, inadequate information communication, consideration of patient health status, family’s beliefs and values, family accompaniment, time allocation, knowledge, trust in health workers, financial consideration, information provision, and needs of second opinions. The findings of this study highlighted providing communication and sharing information were ineffective and inadequate. Several barriers factors were also found related to culture and religious beliefs and values, lack of knowledge and formal training, time constrains, caring experience, and financial consideration. Conclusion thus suggest need to inprove their ability to provide adequate information, to sharing and to perform an effective communication. Assessing appropriate and providing standards of operational procedure is essential to be done in order to improve communication styles and facilitate a shared decision making, partculary in DNR decision making for EoL patients.

Abstract

Engagement is a characteristic of an employee who is committed to the organization. An engaged nurse shows strong work dedication and works productively in advancing the hospital organization. Nurses with good work engagement can improve the quality of nursing in the hospital. This study aims to determine the factors that influence nurses' work engagement to serve as a knowledge base to guide the best practice of nursing services. This study used a narrative review design. This review starts by identifying the main topics in work engagement. This study used narrative review, a method that uses four databases namely PubMed, CINAHL, Google Scholar and ProQuest. Inclusion criteria included full text articles and publications in English and Indonesian between 2015-2021. The study found 13 articles. Several factors were found to influence nurses' work engagement both from within and from outside (motivation, moral competition, supervision and others).

Abstract

Abstrak  Stunting merupakan kondisi dimana tinggi atau panjang badan anak lebih rendah dibandingkan usia berdasarkan kurva pertumbuhan World Health Organization, yang diakibatkan kekurangan gizi kronis dan infeksi berulang. Dampak stunting bagi anak dapat bertahan sampai dengan anak berusia dewasa dan dikaitkan dengan kemunduran di berbagai aspek kehidupan. Stunting perlu diatasi melalui pendekatan multidisipliner, termasuk salah satunya melalui program pengabdian kepada masyarakat. Artikel ini bertujuan untuk mengetahui pengaruh penyuluhan kesehatan terhadap pengetahuan ibu mengenai pemberian gizi seimbang sebagai upaya pencegahan stunting pada balita. Metode yang digunakan pada kegiatan ini, yaitu penyuluhan kesehatan dengan media powerpoint. Populasi pada kegiatan ini adalah ibu yang memiliki balita dari 17 rukun warga Desa Sukamanah, dengan sampel sebesar 57 orang yang ditentukan dengan teknik total sampling. Perubahan tingkat pengetahuan dinilai dengan kuesioner yang diberikan sebelum dan sesudah penyuluhan kesehatan. Data yang didapatkan kemudian diuji secara statistik dengan aplikasi pengolah data SPSS  Berdasarkan hasil analisis diketahui hasil pre-test (mean 65.6, median 80) dibandingkan post-test (mean 71.2, median 80) menunjukkan pengaruh terhadap tingkat pengetahuan peserta kegiatan sebelum dan sesudah dilaksanakan penyuluhan kesehatan yang ditunjukkan dengan p-value 0.012 (<0.05). Penyuluhan kesehatan terbukti bermanfaat dalam meningkatkan pengetahuan ibu yang memiliki balita mengenai gizi seimbang yang mendukung upaya pencegahan stunting. Hasil ini diharapkan dapat menjadi dasar bagi ibu untuk memberikan asupan gizi yang tepat pada balita. Kata kunci : Balita, Gizi, Penyuluhan Kesehatan, Stunting   Abstract   Stunting is a condition where a child's height or body length is lower than age based on the World Health Organization growth curve, causing chronic malnutrition and recurrent infections. The impact of stunting on children can last into adulthood. This is related to backwardness in various aspects of life. Stunting must be managed through a multidisciplinary approach that includes community service programs. This community service activity aims to increase the knowledge of mothers with toddlers regarding providing nutrition and balanced nutrition as an effort to prevent stunting in toddlers. The method used in this activity is health education using PowerPoint media. The target of this activity is mothers who have children aged 0-5 years (toddlers) and come from 17 community units in Sukamanah Village. Changes in knowledge level were assessed through questionnaires administered before and after health education. The data obtained was then tested statistically using the SPSS data processing application. Based on the results of the pre-test and post-test analysis, a p-value of 0.012 (<0.05) was obtained, indicating that there was an influence on the level of knowledge of activity participants before and after health education was carried out. This activity program has proven to be useful in increasing the knowledge of mothers with toddlers regarding nutrition and balanced nutrition which supports efforts to prevent stunting. It is hoped that these results can become a basis for mothers in providing proper nutrition and nutritional intake to toddlers. Keywords : Health Education, Nutrition, Stunting, Toddlers

Abstract

Introduction: The development of a structured curriculum, relevant teaching and learning methods, and support facilities and access is necessary to improve palliative care education by preparing nursing students as future professional nurses in the palliative care area. This review aims to synthesize available literature explaining the barriers and enablers in implementing palliative care education in undergraduate nursing students.Method: This review uses a systematic narrative review approach. Studies were identified from the PubMed, Scopus, CINAHL, and ProQuest Nursing databases from January 2012 to August 2022 and then selected based on the inclusion criteria that had been set.Results: The search results found that 23 of 890 studies were included. The findings were grouped into obstacles and enablers in four parts, namely curriculum and learning strategies, personal and professional competencies, relationships and enablers, and facilities and access.Conclusions: Identifying barriers and enablers can help educational institutions to provide palliative care education according to the needs and expected developments in clinical practice. Collaboration and cooperation between stakeholders must be increased effectively to help provide maximum education.Keywordsbarriers, enablers, nursing students, palliative care education, undergraduate student

Abstract

This study aims to identify the level of spiritual care competence in Ners Professional Program (PPN) students. The research method used was descriptive quantitative, with data collection techniques using a questionnaire. The results showed that the level of spiritual care competence was 76.6% in the high category and 23.4% in the medium category. In conclusion, almost all respondents have a level of spiritual care competence in the high category, but in the aspects of referring, personal support and client counseling, as well as professionalization and improving the quality of spiritual care, only some respondents have a level of competence in the high category. Abstract

Adolescents with fathers who have schizophrenia are more likely to experience emotional dysregulation, negative decisions, a failure to form an identity, inconsistency in decision-making, and failure to adapt. This study is aimed to investigate the lived experiences of adolescents who live with a father who has schizophrenia in an urban area of Bandung City, West Java. The qualitative phenomenological approach was used in this study. In-depth interviews were conducted with six adolescent participants, aged 13 to 22, who live in Bandung City with a father diagnosed with schizophrenia. The Colaizzi method was then used to analyze the interview transcripts. Data analysis produced eight major themes: i) dealing with psychosocial problems and moral dilemmas, ii) a lack of closeness between father and children, iii) exposures to violence inside and outside the home, iv) getting the wisdom behind the catastrophe, v) changing roles and responsibilities, vi) a lack of social support, vii) difficulties to share personal issues, and viii) seeking to build a better future. The adolescents still have opportunities to continue building a better future. Improving health promotion and education programs to manage negative emotions and developing Self-Help Groups for adolescents whose fathers have schizophrenia is thus necessary.

Abstract

The prevalence of cancer in Indonesia has increased and is one of the biggest causes of death. Symptom clusters, a collection of symptoms in cancer patients that appear together and are related to one another, can affect the quality of life of cancer patients. This study aims to identify the relationship between symptom clusters and the quality of life of advanced cancer patients. This research used cross-sectional quantitative survey data from a cancer patient care unit at a referral hospital in West Java Province with a total of 140 respondents. Consecutive sampling was conducted for three months in stage III or IV cancer patients who were undergoing therapy. This study used descriptive analysis, factor analysis with the Principal Components Analysis (PCA) approach, and multiple linear regression analysis. Five symptom clusters were identified: the psychological cluster, the gastrointestinal cluster, the numbness cluster, the pain cluster, and the respiratory distress cluster. The results showed that symptom clusters influence the quality-of-life dimension. The symptom clusters’ coefficient of determination (R2) for the physical dimension was 0.231 (weak), the role dimension was 0.191 (very weak), the emotional dimension was 0.484 (moderate), the cognitive dimension was 0.011 (very weak), the social dimension was 0.420 (moderate), and the general-health dimension was 0.202 (weak).
