Abstract

AbstractBackground: Hospital accreditation provides guidelines for implementing national program standards related to the reduction of tuberculosis morbidity. However, each accreditation body has a different interpretation of the established standards, resulting in variations in the assessment elements across these bodies.Objective: This research aims to identify differences in hospital accreditation institutions in assessing tuberculosis control programs, facilities and infrastructure provision, and services in the effort to control tuberculosis risk factors according to regulations.Method: The method used in this research is a narrative review method.Results: This study found that clear differences in approaches are evident among various accreditation institutions in evaluating factors related to improving tuberculosis services.Conclusion: Accreditation institutions differ in assessing the improvement of TB services. Their focus includes facilities, clinical practices, drug procurement, MDR-TB services, and case recording. There is variation in assessment from compliance with standards to field practice implementation. Evaluation methods also vary such as observation, document examination, and interviews. Abstract

Program utama dalam pembangunan berwawasan kesehatan adalah Program Indonesia Sehat. Salah satu aspek yang diukur untuk menilai kesuksesan Program Indonesia Sehat dengan Pendekatan Keluarga (PIS-PK) yaitu ketersediaan sarana air bersih dan fasilitas jamban keluarga, dan juga bagaimana keluarga mengoptimalkan penggunaan keduanya dalam melaksanakan hidup sehari-hari. Penelitian ini bertujuan untuk mengamati bagaimana masyarakat memanfaatkan sarana air bersih dan jamban keluarga dapat menjadi penilaian terhadap capaian PIS-PK. Penelitian ini memanfaatkan data sekunder dengan survei observasional dilakukan di wilayah Puskesmas Bojong Rawalumbu, Kota Bekasi, selama periode 2 tahun (dari Mei 2018 hingga April 2020). Sampel dalam penelitian ini sebanyak 16.982 orang yang ditentukan menggunakan total sampling. Data diolah dengan program SPSS, dan kemudian ditampilkan kedalam tabel dengan analisis univariat. Hasil penelitian menunjukkan bahwa 99,9% dari total responden memiliki akses terhadap sarana air bersih, yang berjumlah 16.970 orang, responden mempunyai sarana air bersih terlindung sebanyak 16.954 orang (99,8%), responden berperilaku menggunakan air bersih sebanyak 12.486 orang (73,5%). Sedangkan untuk responden yang mempunyai jamban keluarga sebesar 16.857 orang (99,3%), responden memiliki jamban saniter sebanyak 16.734 orang (98,5%), dan responden yang berperilaku BAB di jamban sebanyak 12.385 orang (72,9%). Dapat disimpulkan bahwa indikator sarana air bersih dan jamban keluarga di masyarakat untuk area kerja Puskesmas Bojong Rawalumbu masih berada pada tingkat yang baik. Abstract

In this study, we explore the success of implementing an e-prescription system in the case of electronic medical records, and the factors that influence it. The introduction explains the importance of e-prescribing in increasing the efficiency and accuracy of the treatment process in health facilities. The method used involved observations of 40 journals covering various approaches, from qualitative to quantitative, with a focus on system design, management support, staff training, technology infrastructure, user participation, system development models, evaluation and maintenance, and user perceptions. This research uses a descriptive analytical approach by collecting data from various literature sources and previous research related to the implementation of e-prescriptions. Data were analyzed to identify challenges, benefits and development strategies in the use of e-prescriptions in the context of transforming medical records from physical to electronic. The research results show that the application of e-prescriptions has the potential to improve operational efficiency, patient safety and service quality. However, there are still obstacles related to user readiness, evaluation effectiveness, and e-prescription system development that need to be overcome. The discussion highlights the need to increase user readiness, evaluate the effectiveness of using e-prescriptions, as well as system development strategies that are appropriate to technological developments and user needs. Collaboration between stakeholders is also considered important in achieving successful e-prescription implementation. Taking these findings into account, coordinated and sustainable efforts are needed to increase the use of e-prescriptions in the context of transforming medical records into electronic form. By overcoming existing challenges and implementing appropriate strategies, it is hoped that the implementation of e-prescribing can make a significant contribution in improving the quality of health services and patient safety.

Abstract

Prospective payment system with INA-CBG’s fare for cases using BPJS Insurance demands hospital to control their cost. This study aims to see the cost containment in XY Hospital through the implementation of clinical pathway, drug formulary, and incentive structure. The study looked into Sectio Caesarea cases from January to March 2016, using quantitative method, comparing BPJS claim with hospital billing and assesst the implementation of clinical pathway using qualitative method through in depth interview. Result shows there is deficit amount of Rp.1.014.125.684,00 and the average of deficit per case is Rp.4.899.157,89. Eighty four percent of cases have length of stay in accordance with clinical pathway. From those cases, 96% has concordant doctors visit, 21% has concordant drug usage, and 48% has concordant laboratory diagnostic test. The hospital formulary uses the national formulary. It is found that XY Hospital does not have an evaluation system for clinical pathway implementation and drug usage. The incentive structure that is used is fee-for-service system which is not suitable for prospective payment method.

Abstract

ABSTRACT The incidence of tuberculosis in children is increasing and becomes a serious health problem because it can cause long-term effects such as death. TB can cause nutrient deficiencies and metabolic changes that delay recovery by suppressing immune function. Nutritional support is known to promote recovery in subjects treated for TB. The purpose of this study was to determine the effect of nutritional interventions on the success of childhood tuberculosis treatment. Literature review with 3 databases as literature search that are Pubmed, Proquest, Ebsco with analysis conducted using PRISMA.  Five research articles were obtained from 3 databases relevant to the topic. Tuberculosis infection causes anorexia, malabsorption of nutrients and metabolic disorders, also causing malnutrition characterized by a decrease in fat and muscle mass. Malnutrition can be addressed with nutritional interventions that can be provided in the form of macronutient (high-protein diet) and micronutrients (vitamins and minerals). The combination of macronutrients and micronutrients can increase BMI, albumin levels, and improve fever and cough resolution. This can increase the ability of the child's immune response so that it can accelerate the recovery of children with TB. Improvement of nutrition in children is also supported by nutritional counseling that given to parents. Macronutrients and micronutrients are one of the factors supporting the success of TB treatment in children. Research on nutrition interventions is not enough, so it needs to be supported by further research on this topic so that it can provide better TB treatment results.   ABSTRAK Angka kejadian tuberkulosis pada anak semakin meningkat dan menjadi masalah kesehatan yang serius karena dapat menimbulkan dampak jangka panjang seperti kematian. Tuberkulosis dapat menyebabkan defisiensi nutrisi dan perubahan metabolisme yang menunda pemulihan dengan menekan fungsi kekebalan tubuh. Dukungan nutrisi diketahui mampu mendorong pemulihan pada subjek yang mendapatkan pengobatan TB. Tujuan penelitian ini adalah untuk mengetahui pengaruh intervensi gizi terhadap keberhasilan pengobatan tuberkulosis anak. Kajian literatur dengan 3 database sebagai pencarian literatur yaitu Pubmed, Proquest, Ebsco dengan analisis dilakukan menggunakan PRISMA. Lima artikel penelitian diperoleh dari 3 database yang relevan dengan topik. Infeksi tuberkulosis menyebabkan anoreksia, malabsorbsi nutrisi dan gangguan metabolisme, juga menyebabkan malnutrisi yang ditandai dengan penurunan massa lemak dan otot. Gizi buruk dapat diatasi dengan intervensi gizi yang dapat diberikan dalam bentuk makronutrien (diet tinggi protein) dan mikronutrien (vitamin dan mineral). Kombinasi makronutrien dan mikronutrien dapat meningkatkan BMI, kadar hemoglobin, dan memperbaiki resolusi demam dan batuk. Hal ini dapat meningkatkan kemampuan respon imun anak sehingga dapat mempercepat pemulihan anak penderita TBC. Perbaikan gizi pada anak juga didukung dengan penyuluhan gizi yang diberikan kepada orang tua. Makronutrien dan Mikronutrien merupakan salah satu faktor pendukung keberhasilan pengobatan tuberkulosis pada anak-anak. Penelitian terkait intervensi gizi belum cukup banyak, sehingga perlu didukung dengan penelitian lebih lanjut, sehingga dapat memberikan hasil pengobatan TB yang lebih baik.

Abstract

Penerbitan STR dan SIP Tenaga Kesehatan Masyarakat (Kesmas) mendapat kritik dari para sejawat. Kebijakan penerbitan STR hilang-timbul karena tata kelola regulasinya masih tumpang tindih dan belum sampai ke tatanan bawah. Satu sisi melihat pada quality control, berfikir bagaimana peran dan marwah tenaga kesmas dapat terjaga dan diakui selayaknya bagian dari tenaga kesehatan. Sisi lain melihat pada dampak/ fraud yang mungkin dapat terjadi akibat regulasi yang masih banyak celah. Pemerintah dituntut untuk mengatur melalui regulasi pelaksana sehingga tenaga kesmas dapat bekerja sesuai dengan kapasitas kompetensinya. Undang-Undang Nomor 17 Tahun 2023 Tentang Kesehatan hadir sebagai regulasi baru (hasil omnibus law) dalam penerapan registrasi dan perizinan tenaga kesmas. Tujuan penelitian ini ialah untuk mengetahui bagaimana aturan terkait syarat dan proses registrasi, serta perizinan praktik tenaga kesmas. Menggunakan metode penelitian deskriptif kualitatif dengan pendekatan Undang-Undang (statute approach), metode pengumpulan data melalui diskusi dengan pakar dan scoping review dengan elaborasi data sekunder, kemudian dilakukan analisis isi (content analysis). Penelitian ini menunjukkan bahwa registrasi dan perizinan tenaga kesmas merupakan proses unik yang belum memiliki pengaturan khusus, yang menunjukkan pengetahuan, kompetensi, dan keahlian dari praktik tenaga kesmas. Proses registrasi dimaksudkan untuk memverifikasi kompetensi melalui asesmen, sementara perizinan menentukan atribut profesional dan kemampuan untuk menerapkan kompetensi dalam konteks praktik yang berbeda. Kebijakan terkait registrasi dan perizinan tenaga kesmas penting untuk diaplikasikan, untuk menghindari risiko intervensi yang tidak tepat atau salah sasaran. Regulasi tersebut juga diperlukan untuk melindungi masyarakat dengan jaminan kelayakan praktik tenaga kesmas yang terstandar.

Abstract

Background: Antimicrobials such as antibiotics, antivirals, antifungals, and antiparasitics are vital for treating infections in humans, animals, and plants. Antimicrobial Resistance (AMR) occurs when microorganisms become resistant to these drugs, posing a serious global threat. In Indonesia, high rates of infectious diseases increase the risks associated with AMR, highlighting the need for effective control measures. This study reviews research on efforts to control antimicrobial resistance in Indonesia within the One Health context, analyze related policies, and identifies key implementation challenges to provide recommendations for improvement. Method: A literature review was conducted, identifying 1,029 articles from databases including EBSCOHost, ProQuest, PubMed, and Google Scholar. After screening and applying inclusion criteria, 14 relevant articles were selected for analysis to evaluate policies related to AMR in Indonesia and their alignment with the One Health approach. Result: The findings showed that the increase in antimicrobial resistance (AMR) in Indonesia is mainly due to the misuse of antimicrobial drugs in different areas. While there are policies in place, putting them into practice has proven difficult, especially in the health sector, which faces more challenges than the livestock and environmental sectors. These results highlight the need for better coordination and alignment of policies across all sectors. Conclusion: Although current AMR policies in Indonesia incorporate the One Health approach, significant challenges remain, including limited resources, insufficient guidelines, and weak coordination among sectors. Strengthening collaboration between health, agriculture, and environmental sectors and improving monitoring systems is essential for effective AMR control in Indonesia.

Abstract

Support management in the Pharmaceutical Installation has the aim of supporting the drug management system so that drug management in hospital agencies runs optimally. Pharmaceutical support management consists of human capital, organizational capital, information capital and finance capital, the four of which are interrelated so as to make the system that has been running effective and efficient. The purpose of this study was to determine the condition of support management at the Permata Serdang Hospital Pharmacy Installation. This research is a qualitative research with a case study approach and data collected from interviews and secondary data from the review of hospital documents. The results of interviews with informants from the Pharmacy Installation of Permata Serdang Hospital show that from the human capital aspect there are still problems related to the lack of human resources in related units. In organizational capital, it is known that an organizational structure has been formed that supports a work culture that focuses on patient safety and service efficiency. In information capital, the use of SIMRS has been utilized to support the management of medicines and services, although there are still some technical problems such as slow application and has not been fully utilized for decision making in the management of medicines. There is no information obtained from the finance department for the finance capital aspect, but the Hospital Pharmacy Installation carries out a control process to prevent losses from medicines that have been purchased. Therefore, the Hospital needs to improve the quality of management support such as increasing the number of human resources who also support the organization in the Permata Serdang Hospital Pharmacy Installation, as well as maximizing the function of SIMRS as a decision-making tool in the hope that drug management will run more effectively and efficiently.

Abstract

Health law in Indonesia has seen significant development through the new policy, one of which is the Minister of Health Regulation (Permenkes) No. 22 of 2023, aimed at improving accessibility, efficiency, and quality of healthcare services. Dharmais Cancer Hospital, as a national referral center, plays a crucial role in implementing this policy. This study aims to evaluate the implementation of this policy at this hospital, focusing on regulatory strengthening, healthcare service distribution, and patient rights protection. The findings indicate that while the policy has enhanced regulatory consistency and patient rights access, challenges remain in drug distribution and patient education. The study suggests that improved logistical coordination and staff training are necessary to strengthen policy implementation on the ground. Hukum kesehatan di Indonesia mengalami perkembangan yang signifikan melalui kebijakan baru, salah satunya adalah Peraturan Menteri Kesehatan (Permenkes) No. 22 Tahun 2023, yang bertujuan meningkatkan akses, efisiensi, dan kualitas layanan kesehatan. Rumah Sakit Kanker Dharmais sebagai pusat rujukan nasional memainkan peran penting dalam implementasi kebijakan ini. Penelitian ini bertujuan untuk mengevaluasi implementasi kebijakan Permenkes tersebut di rumah sakit ini, dengan fokus pada penguatan regulasi, distribusi layanan kesehatan, dan perlindungan hak pasien. Hasil penelitian menunjukkan bahwa meskipun kebijakan ini telah meningkatkan konsistensi regulasi dan akses hak pasien, tantangan masih ada dalam distribusi obat dan edukasi pasien. Penelitian ini menyarankan perlunya peningkatan koordinasi logistik dan pelatihan staf untuk memperkuat implementasi kebijakan di lapangan.

Abstract

In relation to the importance of health for every human being, the Ministry of Health provides a large enough budget for improving the quality of health workers through training. However, the evaluations commonly used to measure training effectiveness are generally limited to pre-test and post-test to assess participants' knowledge before and after training (Kirkpatrick level 2 evaluation model). The Kirkpatrick evaluation model is a training evaluation model that is often used to measure the effectiveness of training, namely by measuring the achievement of results based on four levels (level 1 – level 4). Therefore, the authors intend to investigate various literature related to the evaluation model from various levels, which can be developed to evaluate the training of health workers by the Ministry of Health. This literature review follows PRISMA guidelines. Database searches were carried out through PubMed, Scopus and ProQuest search channels in the last 5 years (2017-2022). The results show that level 2 evaluation is most often used to assess the effectiveness of health training outcomes, namely by assessing the improvement of participants' knowledge and skills after training. Fewer level 3 and level 4 evaluations are conducted due to limited time, funds, experience, and tools. However, since each level in Kirkpatrick's evaluation model is interrelated, it is important to be able to evaluate each level sequentially so that information related to training effectiveness can be clearly depicted. Therefore, the Ministry of Health needs to prepare all the necessary resources to be able to assess the effectiveness of training at each level.
