Abstract

Nutritional status is one of the parameters in the control of diabetes disease. Nutritional status can be measured with anthropometry, SGA, PG-SGA etc. But PG-SGA SGA and its still subjective, so it is necessary to use other parameters as reference in disease control type 2 diabetes. The purpose of this study was to identify the nutritional status using the method (SGA and PG-SGA) and their effects on blood sugar control in patients with type 2 diabetes mellitus with complications of nephropathy in Sanglah. This type of research is observational analytic cross-sectional design. Total sample of this study is 50 people, who were taken consecutively at Outpatient Polyclinic and Inpatient in Internist Department General Hospital Sanglah - Denpasar. Univariate and bivariate variables were analyzed using SPSS, with significance ? = 0:05. The results showed that of the chi-square statistical test, there was no significant association between SGA nutritional status (p = 0.923) and PG-SGA (p = 0.566) to control blood sugar in diabetic nephropathy patients in Sanglah. Then, PG-SGA has a sensitivity 66.6% and specificity of 43% .in detect nutritional status by SGA in patients with type II diabetes mellitus with complications of diabetic nephropathy in Sanglah. From the results of this study concluded there was no relationship nutritional status based on SGA and PG-SGA with blood sugar control in patients with diabetic nephropathy in Sanglah. Sensitivity and specificity of the PG-SGA SGA low based on nutritional status in patients with diabetic nephropathy. Abstract

Type 2 diabetes mellitus (T2DM) is group with high cardiovascular disease (CVD) risk. Inflammation which is reflected by increasing leukocyte number, CRP, and lipid profile. Despite that, infection as one of diabetic complication commonly occurs in patient with T2DM. The dyslipidemia status reflected by lipid profile composition. So here, we aim to evaluate the relationship between leukocyte count with lipid profile to know the association between traditional and non-traditional risk factor of CVD in T2DM patient. The type of this research is cross sectional analytic research with 94 samples was enrolled into this study. The data is obtained initially by collecting from the quistionare and from laboratorium examination. Mean of samples’ age is 55 years and large number of patients had leukocytosis as reflected by high value of its mean (15.02±8.78 x 103/?L). We found that 56.4% of the subjects fall into worst category of glycemic control (HbA1C). Only 24.5% of the subjects had good glycemic control. Further evaluation showed that there were significant strong negative correlations between leukocyte count with HDL (r= -0.555; p=0.000). Inflammation which mainly related to infectious disease is common among T2DM patients which increase CVD risk by lowering HDL and worsening glycemic control.Keywords: Type 2 Diabetes Mellitus, lipid profile, leukocyte count

Abstract

Background: Diabetes mellitus (DM) is an interminable issue that can modify glucose, protein, and fat digestion caused by the absence of insulin discharge due to either the dynamic or checked powerlessness of the β-Langerhans islet cells of the pancreas to create insulin. The prevalence of type 2 diabetes mellitus parallels the expanding prevalence of obesity and central obesity. The prevalence of obesity and central obesity in Indonesian among the adult populations are 23.1% and 28%, respectively. Obesity and central obesity are associated with a higher risk of DM. The accumulation of adipose tissue in certain parts of the body, such as in the abdominal cavity, causes an increased risk of insulin resistance until the onset of the metabolic syndrome. This study aims to determine the correlation between waist circumference and glycated haemoglobin (HbA1c) among type 2 DM patients in Diabetic Polyclinic Sanglah General Hospital. Methods: This study was an observational analytic study using a cross-sectional method. Samples selected using consecutive sampling, which determined based on inclusion and exclusion criteria from the population. The total of study subject was 70 respondents. Result: The results of the study showed a significant relationship between waist circumference and HbA1c among type 2 diabetes mellitus patients (p = 0.012) with a low correlation (r = 0.300). Conclusion: Conclusion of this study that there is a relationship between waist circumference and HbA1c among type 2 diabetes mellitus patients in Diabetic Polyclinic Sanglah General Hospital.

Abstract

Background : Obesity has been occured by deposit of fat in subcutaneous and intra abdominal tissue. The prevalance of obesity in Indonesian womens were 32,9% and mens is 19,7 % respectively. In general side, the prevalence of obesity in 18 years old adult peoples were 15,4%. The major risk factors for obesity including lifestyle dietary changes such as consuming fast food habit. Objective : to find out overview of consuming fast food habit and obesity in medical students. Method : This research was cross-sectional decriptive study. The respondens were 127 medical students on 5th semester in Udayana University on 2014. The qualitative data were be measured by questionnaires. The variables were including age, gender, general knowledge of parents, parental income monthy, body mass index (BMI), habit of consuming fast food, average outcome monlty of medical students, and daily frequency of consuming fast food.Result : the characteristic respondens including : a) mostly 20 years old, b).women more than men, c). level education of parents were university, d). monthly parental income more than 5 million rupiah, e). BMI in normal limit, f).degree of obesity were grade I, g).inconvience consuming fast food daily, h).monthy outcome of medical students were aproximately 500.000 rupiah, and i).regular consuming fast food mostly once weekly.Conclusion : mostly responden were women with normaly of BMI and inconvenience consuming fast food daily.

Abstract

Background: Diabetic Nephropathy (DN) is Diabetes Mellitus’s microvascular complication with albuminuria and decrease glomerular filtration rate. DN pathogenesis is related with inflammatory process induced by chronic hyperglycaemia. Mean platelet volume (MPV)  and platelet-lymphocyte ratio (PLR) are potential for reviewing systemic inflammation. The combination of MPV and PLR index into MPVLR shows significant results in prognostic patients with diabetes mellitus and acute myocardial infarction and predicted DN. This study aims to determine the MPVLR index of DN risk in type II diabetes mellitus patients at Sanglah Hospital, Denpasar.Method: Cross-sectional study of 50 DM-2 patients at Sanglah Hospital in 2016. The data is secondary data from the medical records of DM-2 patients according the inclusion and exclusion criteria with purposive sampling technique. Diabetic nephropathy is diagnose based on diagnostic criteria. Complete blood count (CBC) using the CELL-DYN Ruby System. MPVLR is an absolute comparison of the calculation of MPV and total lymphocytes. T-test and Mann-Whitney Test to assess differences mean of data. The MPVLR index for diabetic nephropathy was assessed by Receiver Operating Curve (ROC) and Area Under the Curve (AUC). p <0.05 is statistically significant.Results: 50 data were grouped into nephropathy and without nephropathy groups. Men is dominant in both groups (p>0.05). The MPVLR ratio was statistically significantly higher in the nephropathy group (5.11 ± 2.20; P = 0.004). The MPVLR index cut point value was 3.835 with a sensitivity value of 73.7% and specificity of 71%.Conclusion: MPVLR can use as independent predictor of diabetic nephropathy in diabetic patients. Latar Belakang: Diabetik Nefropati (DN) merupakan komplikasi mikrovaskular Diabetes Melitus dengan albuminuria dan penurunan estimasi laju filtrasi glomerulus. Patogenesis DN berkaitan dengan proses inflamasi terinduksi hiperglikemia kronis. Indeks rerata volume trombosit (MPV) dan rasio trombosit-limfosit (PLR) potensial untuk meninjau inflamasi sistemik. Gabungan indeks MPV dan PLR menjadi MPVLR menunjukan hasil yang signifikan dalam diagnostik prognostik pada pasien diabetes melitus dan infarksi miokardial akut serta menjadi prediktor DN. Penelitian ini bertujuan untuk mengetahui indeks MPVLR terhadap risiko DN pada pasien diabetes melitus tipe II di RSUP Sanglah, Denpasar.Metode: Studi potong lintang (cross-sectional) terhadap 50 pasien DM-2 di RSUP Sanglah tahun 2016. Data penelitian adalah data sekunder dari rekam medis pasien DM-2 yang memenuhi kriteria Inklusi dan Eksklusi dengan teknik purposive sampling. Diabetik nefropati ditegakkan berdasarkan kriteria diagnosis. Pemeriksaan darah lengkap (DL) menggunakan CELL-DYN Ruby System. Rasio MPV terhadap limfosit (MPVLR) adalah perbandingan absolut dari perhitungan MPV dan limfosit total. Uji-t dan Uji Mann-Whitney untuk menilai perbedaan antara rata-rata data dua kelompok. Indeks MPVLR terhadap diabetic nefropati dinilai dengan Receiver Operating Curve (ROC) dan Area Under the Curve (AUC). Nilai p<0,05 bermakna secara statistik.Hasil: 50 data dikelompokkan menjadi kelompok dengan nefropati dan tanpa nefropati. Jenis kelami laki-laki dominan di kedua keompok tersebut (p>0,05). Rasio MPVLR secara statistik bermakna lebih tinggi pada kelompok dengan nefropati (5,11±2,20; P=0,004). Nilai titik potong indeks MPVLR sebesar 3,835 dengan nilai sensitifitas sebesar 73,7% dan spesifisitas 71%.Simpulan: MPVLR dapat sebagai prediktor kuat dan independen diabetik nefropati pada pasien diabetes.

Abstract

Background: Coronavirus disease (COVID-19) infection has become a worldwide pandemic. Diabetes Mellitus is a comorbid disease that can increase in severity. The most common clinical finding in severe COVID-19 infection is a high Neutrophil Lymphocyte Ratio (NLR). In patients with Diabetes Mellitus (DM) type 2, it is also known that high NLR values ​​have a poor prognosis. This study analyzes the risk factors associated with high NLR values ​​in type 2 DM patients infected with COVID-19 at Sanglah General Hospital. Methods: A cross-sectional analytical study was conducted on the medical records of 70 patients with COVID-19 infection with type 2 DM who were treated at Sanglah General Hospital, Denpasar, for the period April 2020-October 2020. The samples were divided into two groups, namely high and normal NLR. The analysis was conducted to see which risk factors were significantly associated with high NLR values ​​in COVID-19 patients with type 2 diabetes. The analysis was carried out using an independent T-test for numerical variables and chi-square for nominal variables. Results: Most of the study samples had high levels of NLR (65.71%). Risk factors that had a significant relationship with high NLR values ​​were random blood sugar levels (p=0.00), fasting blood sugar levels (p=0.00), HbA1c (p=0.002), CRP levels (p=0.00), leukocyte level (p=0.00), BUN level (p=0.004), history of type 2 DM before COVID-19 infection (p=0.009) and control HbA1c (0.001). Conclusion: The risk factors associated with high NLR values ​​in COVID-19 patients with type 2 DM at Sanglah Hospital Denpasar are random blood sugar levels, fasting blood sugar levels, HbA1c, CRP levels, leukocyte levels, BUN levels, history of type 2 diabetes before COVID-19 infection and HbA1c control.   Latar Belakang: Infeksi Coronavirus disease (COVID-19) telah menjadi pandemik di seluruh dunia. Diabetes Mellitus merupakan penyakit komorbid yang dapat meningkatkan keparahan. Temuan klinis yang banyak ditemukan pada infeksi COVID-19 berat adalah nilai neutrophil lymphocyte ratio (NLR) yang tinggi. Pada pasien Diebetes Mellitus (DM) tipe 2 juga diketahui nilai NLR yang tinggi memiliki prognosis buruk. Penelitian ini bertujuan untuk menganalisis faktor risiko yang berhubungan dengan nilai NLR tinggi pada pasien DM tipe 2 yang terinfeksi COVID-19 di RSUP Sanglah. Metode: Studi analitik potong lintang dilakukan pada rekam medik 70 pasien dengan infeksi COVID-19 dengan DM tipe 2 yang dirawat di RSUP Sanglah Denpasar periode April 2020-Oktober 2020. Sampel dibagi menjadi dua kelompok yakni NLR tinggi dan normal. Analisis dilakukan untuk melihat faktor risiko yang signifikan berhubungan dengan nilai NLR tinggi pada pasien COVID-19 dengan DM tipe 2. Analisis dilakukan menggunakan uji independent T-test untuk variabel numerik dan chi-square untuk variabel nominal. Hasil: Mayoritas sampel penelitian memiliki kadar NLR tinggi (65,71%). Faktor risiko yang memiliki hubungan signifikan dengan nilai NLR tinggi adalah kadar gula darah acak (p=0,00), kadar gula darah puasa (p=0,00), HbA1c (p=0,002), kadar CRP (p=0,00), kadar leukosit (p=0,00), kadar BUN (p=0,004), riwayat DM tipe 2 sebelum infeksi COVID-19 (p=0,009) dan kontrol HbA1c (0,001). Kesimpulan: Faktor risiko yang berhubungan dengan nilai NLR tinggi pada pasien COVID-19 dengan DM tipe 2 di RSUP Sanglah Denpasar adalah kadar gula darah acak, kadar gula darah puasa, HbA1c, kadar CRP, kadar leukosit, kadar BUN, riwayat DM tipe 2 sebelum infeksi COVID-19 dan kontrol HbA1c.

Abstract

Background: The pathogenesis of COVID-19 infection is the occurrence of excessive inflammatory reactions in which one of the markers of inflammation that is easy to work with is the value of the Neutrophil Lymphocyte Ratio (NLR). The NLR value increases in uncontrolled type 2 DM patients and is a prognostic factor for the occurrence of severe symptomatic COVID-19 infection. This study aims to examine the correlation between NLR values ​​and various laboratory parameters of type 2 DM patients with COVID-19 infection at Sanglah Hospital. Methods: This study used a cross-sectional analytical design and involved 70 research samples from the medical records of type 2 DM patients with COVID-19 who were treated at Sanglah Hospital from April 2020 to October 2020. Analysis with Pearson and Spearman tests to see the correlation between NLR values ​​and 12 laboratory parameters such as GDS, GDP, HbA1c, Leukocytes, Hemoglobin, BUN, D-Dimer, CRP, serum creatinine, SGOT, SGPT and Ferritin using SPSS software version 21 for Windows. Results: From the results of the correlation test, laboratory parameters that were found to be significantly positively correlated with NLR values ​​were GDS levels (r=0.451; p=0.000), GDP (r=0.037; p=0.001), leukocytes (r=0.479; p=0.000), BUN (r=0.368; p=0002), D-dimer (r=0.348; p=0.011), CRP (r=0.557; p=0.000), serum creatinine (r=0.313; p=0.009) and ferritin (r=0.337; p=0.011). Conclusion: The NLR value in type 2 DM patients with COVID-19 is positively correlated with glycemic markers, namely GDS and GDP. In addition, it is also positively correlated with leukocytes, BUN, D-dimer, CRP, serum creatinine and ferritin.   Latar belakang: Patogenesis infeksi COVID-19 adalah terjadinya reaksi inflamasi berlebihan dimana salah satu penanda inflamasi yang mudah dikerjakan adalah nilai Neutrophil Lymphocyte Ratio (NLR). Nilai NLR meningkat pada pasien DM tipe 2 yang tidak terkontrol dan menjadi faktor prognostik untuk terjadinya infeksi COVID-19 gejala berat. Penelitian ini bertujuan untuk meneliti korelasi antara nilai NLR dengan berbagai parameter laboratorium pasien DM tipe 2 dengan infeksi COVID-19 di RSUP Sanglah. Metode: Studi ini menggunakan desain analitik potong lintang dan melibatkan 70 sampel penelitian dari rekam medik pasien DM tipe 2 dengan COVID-19 yang dirawat di RSUP Sanglah bulan April 2020 hingga Oktober 2020. Analisis dengan Uji Pearson dan Spearman untuk melihat korelasi antara nilai NLR dengan 12 parameter laboratorium berupa GDS, GDP, HbA1c, Leukosit, Hemoglobin, BUN, D-Dimer, CRP, serum kreatinin, SGOT, SGPT dan Ferritin menggunakan piranti lunak  SPSS versi 21 untuk Windows. Hasil: Dari hasil uji korelasi, parameter laboratorium yang ditemukan signifikan berkorelasi positif dengan nilai NLR adalah kadar GDS (r=0,451; p=0,000), GDP (r=0,037; p=0,001), leukosit (r=0,479; p=0,000), BUN (r=0,368; p=0002), D-dimer (r=0,348; p=0,011), CRP (r=0,557; p=0,000), serum kreatinin (r=0,313; p=0,009) dan ferritin (r=0,337; p=0,011). Kesimpulan: Nilai NLR pada pasien DM tipe 2 dengan COVID-19 berkorelasi positif dengan penanda glikemik yaitu GDS dan GDP. Selain itu berkorelasi positif juga dengan leukosit, BUN, D-dimer, CRP, serum kreatinin dan ferritin.

Abstract

Background: Single Nucleotide Polymorphisms (SNPs) of the Transcription Factor 7-Like 2 (TCF7L2) diabetes susceptibility gene were located in the noncoding region. Intronic SNP may play a role in mRNA splicing. This study aimed to elucidate whether SNPs of the TCF7L2 gene expressed different mRNA isoforms in the peripheral blood. Methods: Across sectional study was conducted in Legian Bali. A total of 56 subjects were grouped into carrier of TCF7L2SNPs (28 Balinese age 30-74 years,eitherheterozygote or mutant of the rs12255372 SNP [GT or TT], rs7903146 [CT or TT], and rs10885406 [AG or GG])and wild-type of TCF7L2 (28 matched in age and sex Balinese,rs12255372 [GG], rs7903146 [CC], and rs10885406 [AA])which has been identified in the previous study.In this study, the mRNA isolation was done using LS Trizol reagent (Nitrogen®) and variants of the mRNA isoform in the TCF7L2 gene was identified by reverse transcriptase-polymerase chain reaction (RT-PCR) method, using two pairs of overlapping primer.The first primer PCR products were grouped into Group-A, Group-B, Group-C and Group-D. The second primer PCR products were grouped into Group-a, Group-b, Group-c and Group-d. Oral glucose tolerant test (OGTT) was done to both groups.Blood samples were taken during fasting, at 1 hour and 2 hours after the oralglucose load, for blood glucose, and insulin level measurement. Results: There were seven pairs of the group combination including group A-a, A-c, A-d, C-a, C-c, C-dand D-c, and group C-c was the most commonly found among all subjects in this study (42.3%). Subjects with diabetes risk allele were 2.66 times more likely to express group C-ccompared to subjects without diabetes risk allele of SNPs in the TCF7L2 gene (95%CI 1.24–5.72 p=0.011). Mean response of insulin increment after oral glucose load among group C-c was higher compared with the non-group C-c, 84.63±41.01 vs. 62.02±38.09ng/ml (mean difference 22.61 95%CI 0.41–44.80 p=0.046). However, blood glucose level, fasting insulin, HOMA %B and HOMA IR were similar between the two groups. Conclusion: Variants of TCF7L2 gene expressed different mRNA isoformsin the peripheral blood.Compared to subjects without diabetes risk allele of SNPs in the TCF7L2 gene, subject with diabetes risk allele expressed dominant variant isoform mRNA 2.66 timeshigher.The higherresponse of insulin increment after oral glucosewas observedin the dominant variant which may reflectthe early condition of insulin resistant.Keywords: Diabetes, SNP TCF7L2, isoforms mRNA.

Abstract

ABSTRAK Obstructive Sleep Apnea (OSA) merupakan kelainan pada pernafasan yang memiliki gejala apnea dan hipopnea selama tidur. National Sleep Foundation memperkirakan 18 juta orang di dunia menderita OSA. Prevalensi tidak diketahui secara pasti karena gold standar diagnosis berupa pemeriksaan polisomnografi (PSG), tidak semua fasilitas kesehatan memiliki layanan PSG dan kurang dikenalinya gejala OSA. Beberapa faktor risiko penyebab OSA, salah satunya obesitas yang identik dengan aktivitas fisik rendah (dominan sedenter). Jenis pekerjaan pada umumnya mempengaruhi kecenderungan aktivitas fisik, salah satunya pedagang sering duduk lama. Tujuan dalam penelitian ini mencari ada/tidaknya korelasi antara obesitas serta aktivitas fisik dengan risiko kejadian OSA di pedagang di Pasar Seni Semarapura, Klungkung. Penelitian ini dilakukan dengan design analitik cross sectional menggunakan instrumen berupa kuisioner Berlin serta International Physical Activities Questionnaire-Short Form selama 3 bulan (Februari-April 2021) pada 75 responden dengan variabel bebas berupa obesitas dan aktivitas fisik serta variabel terikat adalah resiko menderita OSA. Penelitian dianalisis secara univariat dan bivariat. Diperoleh 14 responden berisiko tinggi menderita OSA. Selain itu, dalam penelitian ini ditemukan obesitas berkorelasi dengan risiko menderita OSA (p value = 0,036) sedangkan aktivitas fisik tidak memiliki korelasi dengan risiko menderita OSA (p value = 0,423). Kedua hal ini dapat dipengaruhi oleh faktor lain sesuai dengan item yang ditanyakan dalam kuisioner. Ada korelasi antara obesitas dengan resiko menderita OSA sedangkan aktivitas fisik tidak. Perlu dilakukan penelitian lebih lanjut pada faktor risiko jenis lainnya dengan jangkauan subjek penelitian yang lebih luas. Kata kunci : Resiko menderita OSA, Obesitas, Aktivitas Fisik.

Abstract

Diabetes melitus merupakan penyakit metabolik yang dapat menyebabkan perubahan struktur dan fungsi eritrosit. Perubahan ini berpengaruh terhadap perkembangan komplikasi kronik diabetes. Tujuan dari penelitian untuk mengetahui gambaran indeks eritrosit pasien diabetes melitus tipe 2 di RSUP Sanglah Denpasar periode tahun 2021-2022. Metode penelitian berjenis deskriptif retrospektif cross sectional. Penelitian menggunakan rekam pasien diabetes melitus tipe 2 Diabetic Center RSUP Sanglah. Sebanyak 272 rekam medis digunakan sebagai sampel. Gambaran indeks eritrosit pasien diabetes melitus tipe 2 menunjukan anemia ringan normositik normokromik. Nilai rerata hemoglobin pria 12.75 g/dl dan wanita 11.82 g/dl. Nilai rerata indeks eritrosit pasien yakni MCV 86.0 fL, MCH 28.2 pg, dan MCHC 32.8 g/dl. Anemia ringan mempunyai proporsi tertinggi dengan pria kelompok usia 56-60 tahun 18 sampel (6.6%) dan wanita 51-55 tahun 12 sampel (4.4%).
