Abstract

Product quality is critical for businesses to survive and thrive in the marketplace. Likewise with PT. Barata Indonesia (Persero), which always wants to provide good product quality for customer satisfaction, PT. Barata Indonesia (Persero) is a State-Owned Enterprise (BUMN) in the manufacturing sector, which has a business line in the foundry or foundry sector, one of which is the production of train components. The company's difficulty is that there are still problems in producing the S2HDC bogie set, such as break mold, porosity, slag, and crack. This study aimed to analyze the quality of the Bogie Set S2HD9C product using the six sigma method and propose improvements using Failure Mode and Effect Analysis (FMEA). The research results found that the percentage of defects that occurred during August 2022 - January 2023 was porosity of 40.38%, break mold of 30.77%, crack of 17.31%, and slag of 11.54%. To obtain an average DPMO of 14191 with a sigma value of 3.69. The causes of these defects are caused by human, material, machine, method, and environmental factors. From the FMEA analysis, the defect's cause is the highest RPN value of 392, with the proposed improvement to closely monitor the cleaning process and recheck the core box in every process that will use the mold.

Abstract

PT. INKA Multi Solusi is a subsidiary of PT. Industri Kereta Api is engaged in manufacturing services, manufacture, and trade of spare parts and products for land transportation and the manufacture of railway components according to customer specifications. The product currently being worked on is the type 50 FT flat carriage (UGL) product ordered by the KiwiRail company, Australia, with total production from November 2022 to April 2023 of 791 carriages with a total of 1582 headstock arrangements and a total of 408 units of defects. This study aims to determine product quality and provide suggestions for improving the quality of welding joints for UGL type 50 FT train products. This study uses the Six Sigma method and Failure Mode and Effect Analysis (FMEA). The study result shows that the average Sigma value based on total defects from November 2022 to April 2023 is 3.16, which means that the six sigma target is still unmet. Recommendations for improvements that need to be given are conducting briefings and training for welders so that they are more adept at using the welding machine, cleaning (blasting) the workpiece to remove residual welding impurities, and providing a memo regarding the results of the welding to be given to the next welder.

Abstract

General Background: Quality control is essential for ensuring competitiveness in manufacturing industries, particularly in footwear production. Specific Background: UD. XYZ experiences a defect rate of 12.85%, exceeding the company standard of 5%, indicating systemic issues in its production process. Knowledge Gap: Previous studies have utilized New Seven Tools and FMEA, yet have not sufficiently integrated both methods to produce comprehensive improvement recommendations tailored to the root causes of shoe defects. Aim: This study aims to identify defect-causing factors and propose corrective actions using combined New Seven Tools and Failure Mode and Effect Analysis (FMEA). Results: Analysis revealed 27 proposed improvements through the PDPC, with 25 deemed feasible, while FMEA identified untidy sewing as the most critical failure mode, with the highest RPN value of 336. Novelty: This study offers an integrated diagnostic–corrective framework that systematically links qualitative mapping tools with quantitative risk prioritization to strengthen quality improvement strategies. Implications: Findings provide actionable guidance for enhancing worker performance, machine calibration, material handling, and production methods, supporting sustained quality enhancement in the footwear industry. Highlights: Identifies key defect sources in shoe production using integrated qualitative and quantitative methods. Highlights untidy sewing as the most critical issue based on the highest RPN value (336). Provides feasible improvement actions, with 25 of 27 recommendations implementable for quality enhancement. Abstract

PT Margabumi Matraraya is a company engaged in the management and operation of the Surabaya-Gresik toll road. This company is responsible for ensuring that the toll road operations run smoothly in accordance with the established service standards. However, in the toll road traffic service process, inefficiencies are still found, such as long customer waiting times, miscommunication, repeated reporting, inefficient routes, less than optimal workforce performance, and excessive service. This study aims to apply the lean service concept to improve efficiency in the toll road service process and provide recommendations for improvements to reduce waste. The methods used include Value Stream Mapping (VSM), Value Stream Analysis Tools (VALSAT), and Root Cause Analysis (RCA). The results of the analysis using VSM showed that the initial service lead time was 265 minutes with non-value-added activities of 5.96%. After the implementation of the improvement recommendations, non-value-added activities were eliminated, so that the lead time was reduced to 228.2 minutes, or equivalent to a time reduction of 13.89%. The proposed improvement recommendations are optimization of communication systems, application of GPS technology for real-time location monitoring, and improvement of training for officers to maximize human resource potential.

Abstract

One of the sugar factories built by the Indonesian state as an effort to revitalize the national sugar industry through the use of modern technology is PT XYZ. However, this sugar factory is experiencing problems with the results of testing the production of white crystal sugar which has defects that are not in accordance with the quality requirements of white crystal sugar with SNI number 3140-3: 2020. Defects that occur include defects in sugar moisture content, sugar solution color, grain size, and sugar crystal uniformity. This study was conducted to determine the factors that can affect the occurrence of defects in the white crystal sugar production process at PT XYZ and how the proposed improvements can be implemented to control the quality of white crystal sugar products at PT XYZ. The methods used in this research are the Fault Tree Analysis (FTA) and Failure Mode and Effects Analysis (FMEA) methods. Based on calculations using the Fault Tree Analysis method, the calculated probability results for each type of defect, namely large grain defects, moisture content, color, and sugar crystal uniformity, are 0.000657%, 0.000889%, 0.001597%, and 0.000662%, respectively. The FMEA method obtained a large grain type defect with the highest RPN value of 120 (High). Large grain type defects cause white crystal sugar to not conform to market specifications and dissolve more slowly. In the second rank, there is a defect in water content with an RPN value of 112 (medium). Quality control should be carried out regularly and continuously so that it can be a reference for continuous improvement in the future.

Abstract

PT XYZ is a manufacturing company engaged in the production of Crude Palm Oil (CPO). In its production process, a significant amount of waste due to product defects is still found. This can be seen from the production parameters recorded from January to June 2024. Based on 137 samples taken over six months, 132 samples did not meet the company's established standards across four parameters. This study aims to determine the types of waste occurring in the CPO production process, assess the sigma level, provide waste reduction recommendations, and minimize non-value-added activities. The study uses the Lean Six Sigma method. It begins by identifying waste in the CPO production process, with the most dominant forms being defects, overproduction, and waiting. The sigma level of the CPO production process at PT XYZ was found to be 2.20, with a DPMO (Defects Per Million Opportunities) of 240,277.8, which falls into the "good" category based on the average performance of the palm oil industry in Indonesia. Further analysis was conducted to identify the root causes of defects using a fishbone diagram. Improvement proposals were developed using the Failure Mode and Effect Analysis (FMEA) tool. With the design of Process Activity Mapping (PAM) and Big Picture Mapping, the lead time was successfully reduced from 807.22 minutes to 728.88 minutes.

Abstract

General Background: Production systems frequently experience capacity imbalances that disrupt material flow and reduce throughput, particularly in small- and medium-scale manufacturing environments. Specific Background: UD Anugrah, a footwear manufacturer specializing in children’s shoes, faces recurring work-in-progress accumulation caused by unequal capacities among production workstations. Knowledge Gap: Prior studies have widely applied the Theory of Constraints, yet empirical applications integrating Drum-Buffer-Rope scheduling within the footwear manufacturing sector remain limited. Aims: This study aims to optimize production capacity at UD Anugrah by applying the Theory of Constraints integrated with the Drum-Buffer-Rope concept to identify, manage, and resolve system bottlenecks. Results: Sewing and Injection workstations were identified as primary constraints; optimization through linear programming, flow synchronization, and selective capacity elevation increased monthly throughput from IDR 88,166,810 to IDR 91,707,000, representing a 3.86% increase, while financial analysis yielded a positive Net Present Value of IDR 61,881,704 and a Payback Period of 3.41 years. Novelty: This research presents an integrated TOC–DBR application within a shoe manufacturing context, supported by quantitative throughput and investment feasibility analysis. Implications: The findings demonstrate that structured constraint management and DBR-based flow control provide a practical framework for improving production performance and investment decision-making in small- and medium-scale manufacturing enterprises. Highlights: Sewing and Injection stations were consistently identified as system constraints across multiple production periods. Linear programming supported optimal product mix allocation under limited workstation capacity. Financial evaluation confirmed that selective machine addition was economically feasible within the system lifespan. Abstract

General Background Pesticide manufacturing plays a critical role in supporting agricultural productivity, yet complex production systems frequently generate operational waste and product defects. Specific Background PT XYZ produces multiple pesticide variants, with powder pesticides showing the highest defect proportion during the October 2024–September 2025 period, indicating inefficiencies within the production process. Knowledge Gap Despite recurring defects and extended lead time, systematic waste identification and structured failure risk prioritization had not been comprehensively applied in this production context. Aims This study aimed to identify dominant waste types, evaluate process performance, and formulate improvement recommendations using Lean Six Sigma integrated with Failure Mode and Effect Analysis. Results The analysis identified defects, waiting, transportation, and environmental health and safety as dominant wastes. Lead time was reduced from 763.11 minutes to 681.38 minutes through the elimination of non-value-added activities. Process performance showed an average DPMO of 37,519.68 with a sigma level of 3.28, alongside an increase in Process Cycle Efficiency from 59.55% to 66.69%. FMEA results indicated the highest Risk Priority Numbers were associated with non-standard product weight and product clumping caused by operator inconsistency and suboptimal machine performance. Novelty This study presents an integrated application of Lean Six Sigma and FMEA to map waste sources and prioritize failure risks within a pesticide powder production system. Implications The findings provide structured improvement recommendations, including operator training, standardized machine settings, and routine maintenance, offering a data-driven reference for manufacturing process optimization in similar industrial settings. Highlights: Defect-related losses constituted the largest proportion of inefficiencies in the studied manufacturing flow. Quantitative performance metrics demonstrated measurable reductions in processing time and defect opportunity rates. Abstract

Untuk menjaga efektivitas biaya, perusahaan harus mempertimbangkan dengan cermat maskapai mana yang akan digunakan untuk perjalanan dinas. Untuk membantu perusahaan memilih maskapai terbaik berdasarkan faktor-faktor seperti biaya, kenyamanan, pelayanan, dan ketepatan waktu, penelitian ini bertujuan untuk mengembangkan sistem pendukung keputusan dengan menggunakan teknik TOPSIS. Informasi yang digunakan mencakup ketepatan waktu penerbangan, kenyamanan armada, tarif tiket, dan pelayanan dari berbagai maskapai. Perhitungan metode TOPSIS menunjukkan bahwa Maskapai B adalah pilihan terbaik, diikuti oleh Maskapai D dan Maskapai E. Hasil perhitungan nilai preferensi menunjukkan bahwa Maskapai B menduduki peringkat pertama dengan nilai preferensi tertinggi 0,6862, diikuti oleh Maskapai D yang berada di peringkat kedua dengan nilai preferensi 0,6623. Maskapai E berada di peringkat ketiga dengan nilai preferensi 0,4406, dan Maskapai F berada di peringkat keempat dengan nilai 0,4289. Maskapai C berada di peringkat kelima dengan nilai preferensi 0,4278, sedangkan Maskapai A menduduki peringkat terakhir dengan nilai preferensi 0,2295. Sistem ini dapat membantu perusahaan dalam membuat keputusan yang lebih tepat dan efektif dalam memilih maskapai terbaik untuk perjalanan dinas.

Abstract

This technical implementation study discusses the optimization of production employee shift scheduling at PT Japfa Comfeed Indonesia Tbk using the Simple Additive Weighting (SAW) and Integer Linear Programming (ILP) methods. The scheduling model applied considers performance factors, job satisfaction, work discipline, and work knowledge to form teams with balanced total values, thereby increasing work effectiveness and efficiency. The rule-based scheduling method shows more regular and consistent shift distribution results, with a "1 team 1 shift in 1 week" pattern that supports stable routines and facilitates employee adaptation. The web-based scheduling system was developed using HTML, CSS on the front-end, and PHP and MySQL on the back-end, which is equipped with security features such as authentication and authorization. The applied results successfully showed increased productivity and reduced miscommunication between teams. The use of Excel Solver and Lingo 20.0 helped optimize work schedules with more precise calculations. The applied results also made a significant contribution to human resource management, but the imbalance in workload between teams still needs to be improved. Further studies are recommended to explore machine learning-based methods, integrate the system with HRMS tools, and study the psychological and economic impacts of implementing this system.
