Abstract

BackgroundDiffuse astrocytic tumours (DAT) is a diffuse infiltrative astrocytoma tumor accompanied by molecular parameters in the form or absence ofIsocitrate Dehydrogenase (IDH) gene mutations. The DAT category without the examination of the IDH gene mutation is classified as NotOtherwise Specified (NOS). Ki-67 is a marker for DAT proliferation so that it can assess the degree of DAT. Diffuse astrocytic tumoursbehavior is influenced by proliferative ability and the presence of a tumor microenvironment. The tumor microenvironment PD-L1 factorinfluences DAT behavior and allows the development of target therapy in DAT cases. This study aimed to analyze the relationship betweenPD-L1 and Ki-67 imunoexpression in the histopathological degree of DAT.MethodsCross-sectional study of 30 paraffin blocks of DAT NOS cases from 2014-2018 in the Department of Anatomical Pathology RSUP dr. HasanSadikin, Bandung. The sample consisted of 15 cases of grade II, 7 cases of grade III and 8 cases of grade IV, carried outimmunohistochemical staining with GFAP, PD-L1 and Ki-67. Assessment of PD-L1 immunoexpression was carried out by the histoscoremethod. The Ki-67 assessment was assessed based on its distribution, which was <10% and ≥10%. PD-L1 and Ki-67 immunoexpressionwith histopathological degrees in DAT were statistically analyzed by Kolmogorof Smirnov test, however association betweenimmunoexpression of PD-L1 and Ki-67 were analyzed by chi-square test.ResultsThere was a significant relationship between PD-L1 immunoexpression and histopathological degree in DAT (p=0.005). There was asignificant relationship between Ki-67 immunoexpression and DAT histopathology degree and with PD-L1 (p=0.001 and p=0.010).ConclusionPD-L1 immunoexpression and proliferation rates affect the degree of DAT. Proliferation rate of DAT is influenced by PD-L1 so that in thisstudy, PD-L1 and Ki-67 can be used as a marker of prognosis predictors.

Abstract

Rhabdomyosarcoma (RMS) is a type of malignant soft tissue neoplasm that develops from mesenchymal cells of skeletal muscle origin. This condition comprises multiple subtypes with distinct histologic characteristics. While cases of RMS are uncommon overall, they are more frequently diagnosed in children, although adults can also be affected. Treatment options for RMS include radiotherapy, with the specific therapy and dosage determined according to the IRS Group classification system. This study was conducted by collecting secondary data of RMS patients from the medical records consecutively who had been diagnosed and received radiotherapy. The data resources were from Radiotherapy Department and Anatomical Pathology Department of RSUP Dr. Hasan Sadikin Bandung and RSUD Al-Ihsan Bandung, between 2017 and 2023. A total of 31 patients were documented. The age of patients ranging from 3 to 66 years old (mean=34.6), with each of age group (children, adult, elderly) consists of 10, 10, and 11 patients respectively. Based on histologic subtype, most cases of RMS recorded are embryonal subtype (n=10) followed by alveolar RMS and pleomorphic RMS with 6 patients each. The extremities were the most common location (n=13) followed by the head and neck region (n=10). After radiotherapy, 20 cases were accounted as radiosensitive and 11 cases were radioresistant. Thirty one patients with rhabdomyosarcoma have received radiotherapy at two referral hospitals in Bandung between 2017-2023. The tumors in pediatric and elderly patients were more sensitive to therapy. Pleomorphic RMS were the most sensitive subtype, and the larger a tumor, the more sensitive it can be.

Abstract

Background: Anogenital condyloma acuminata (CA) is a benign lesion caused by low-risk human papillomavirus (HPV). Although typically benign and non-dysplastic, a subset may undergo histopathological progression. Histopathology is essential not only for diagnosis but also for early detection of malignant transformation. In people living with HIV (PLHIV), immunosuppression may further modify the course of CA. Purpose: To analyze clinicopathologic and sexual behavioral factors associated with HIV status in patients with CA and to evaluate histopathological progression from a pathological perspective. Methods: A retrospective cross-sectional study included 224 patients with histopathologically confirmed CA at Dr. Hasan Sadikin General Hospital, Bandung, between 2023 and 2024. Sociodemographic, clinicopathologic, and sexual behavioral data were retrieved from medical records. Associations with HIV status were analyzed using chi-square or Fisher’s exact tests and multivariate logistic regression. Result: Among 224 patients, 87 (38.8%) were HIV-positive. Bisexual orientation (aOR: 12.52; 95% CI: 1.03–151.98), lesion duration >6 months (aOR: 6.14; 95% CI: 1.74–21.72), and internal lesion location (aOR: 0.17; 95% CI: 0.03–0.88) were independent predictors of HIV positivity. Histopathological progression was identified in 13 patients (5.8%), comprising eight low-grade squamous intraepithelial lesions, three high-grade squamous intraepithelial lesions, and two squamous cell carcinomas. It occured in both HIV-positive (n=3) and HIV-negative (n=10). Conclusion: HIV is highly prevalent among CA patients, with sexual and clinicopathologic factors as significant predictors. The identification of histopathological progression, including malignant transformation, highlights the crucial role of pathology and emphasizes the necessity for routine histopathological evaluation to detect malignant potential.
