Abstract

Background: Uterine smooth muscle tumor of uncertain malignant potential (STUMP) presents a diagnostic dilemma, especially in Southeast Asian populations, where delayed presentation often leads to advanced tumor burden. This study aimed to characterize the clinicopathological profile of STUMP in Indonesia using the 2021 WHO Classification and modified Stanford parameters. Methods: A descriptive study analyzed 37 STUMP cases diagnosed between 2023 and 2025 at a tertiary referral center in West Sumatra. A panel of pathologists re-evaluated archival slides for atypia, mitotic activity, necrosis, and growth patterns, establishing inter-observer reliability. Fisher’s Exact Test was employed to correlate tumor size with morphological markers of aggression. Results: The cohort demonstrated a distinct large-mass phenotype, with 54.1% of tumors exceeding 10 cm and a median diameter of 14.5 cm. Pathological review classified 56.8% of cases into WHO Group 4 (atypia with ambiguous mitosis). A statistically significant correlation was identified between tumor size >10 cm and the presence of infiltrative growth margins (81.1%; p=0.034). Conclusion: Indonesian STUMP cases are distinctively characterized by massive size and high rates of infiltrative growth, likely driven by prolonged natural history. The prevalence of ambiguous mitotic figures underscores the utility of the WHO Group 4 category in resource-limited settings. These findings advocate for aggressive surgical management and wider resection margins in large-mass variants to mitigate local recurrence risk.

Abstract

Aging is a complex biological process characterized by progressive functional decline of organs and tissues, largely driven by oxidative stress, chronic inflammation, and cellular senescence. Recent evidence suggests that age-related gut microbiota dysbiosis contributes significantly to inflammaging and oxidative damage, accelerating the aging process. Probiotics have emerged as potential gerobiotics due to their antioxidant, anti-inflammatory, and microbiota-modulating properties. This literature review aims to evaluate the role of probiotics in preventing and delaying the aging process through modulation of gut microbiota, oxidative stress reduction, and regulation of inflammatory. A narrative literature review was conducted using scientific publications from 2012 to 2023 obtained from NCBI (PubMed), ScienceDirect, and Google Scholar databases. Articles were selected based on relevance to probiotics, gut microbiota, oxidative stress, inflammation, and aging mechanisms. A total of 300 articles were identified from PubMed (n = 120), Science Direct (n = 95), and Google Scholar (n = 85). After removing duplicates, 235 articles were screened. 45 full text articles were assessed for eligibility, and 11 studies were included in the quantitative synthesis. The reviewed studies indicate that probiotics, particularly strains from the genera Lactobacillus, exert anti-aging effects by reducing reactive oxygen species (ROS), enhancing endogenous antioxidant enzymes (SOD, CAT, GPx), regulating inflammatory pathways (NF-κB, cytokines), activating Nrf2 signaling, maintaining intestinal barrier integrity, and restoring gut microbiota balance. Several probiotic strains were shown to delay telomere shortening, suppress cellular senescence markers, and improve metabolic. Probiotics play a significant role in preventing aging by modulating gut microbiota composition, reducing oxidative stress, and suppressing chronic inflammation. These findings support the potential application of probiotics as gerobiotics to promote healthy aging and extend health span.

Abstract

The increasing global elderly population has been accompanied by a rise in age-related health problems, particularly degenerative conditions such as cognitive decline. Physical activity is considered a modifiable factor that may play a protective role in maintaining cognitive health, as regular exercise is believed to delay cognitive deterioration in older adults. This study aimed to investigate the correlation between physical activity and cognitive function among elderly individuals living within the service area of Padang Kandis Public Health Center, located in Lima Puluh Kota Regency. This study employed an analytic observational design with a cross-sectional approach. Participants were selected using a consecutive sampling technique, resulting in 52 elderly individuals residing in the study area. Physical activity levels were assessed using the Global Physical Activity Questionnaire (GPAQ), while cognitive performance was evaluated using the Indonesian version of the Montreal Cognitive Assessment (MoCA-Ina). Bivariate analysis was performed using the chi-square test. The results showed that 23 respondents (44.23%) engaged in a moderate level of physical activity, and 27 respondents (51.92%) demonstrated normal cognitive function. Statistical analysis indicated a significant relationship between physical activity and cognitive function, with a p-value of 0.001 (p < 0.05). In conclusion, this study demonstrated a significant association between physical activity levels and cognitive performance in the elderly population served by the Padang Kandis Public Health Center.
