Abstract

Introduction: Work discipline is the obedient attitude of an employee towards the rules that apply in the workplace. Work discipline directly affects work productivity. Methods: This study aimed to investigate the correlation of work motivation and work discipline of the employee at Wiyung Sejahtera Hospital Surabaya. This study was an observational analytical research with cross sectional method, where each of the respondents was observed or interviewed for once. The samples used proportional random sampling with a sample size of 80 respondents who were hospital employees. Independent variable in this study was work motivation and a dependent variable was dicipline of employee. Data were analyzed by using spearman. Result: The respondents’ characteristics were mostly female (81.25%) with age criteria age 21-30 years old (80.0%). Meanwhile, the characterictic based on educational level suggested that more than half of respondents were diploma graduates(67.5%)  and have the most work periode for more than 3 years (33.75%). The majority of respondents had a moderate level of work motivation at 41.3% and the majority of respondents had a moderate level of work discipline at 42.5%. The results showed that there was correlation between work motivation and work discipline (p = 0.000). Conclusion: It can be concluded that there is a relationship between work motivation and work discipline of employee at Wiyung Sejahtera Hospital Surabaya. As the relation is very strong, an employee who has a high level of work motivation will have a high level of work discipline too and otherwise.  Keywords: work motivation, work discipline,Wiyung Sejahtera Hospital

Abstract

Background: Job dissatisfaction in healthcare facilities can increase medical errors. In order to reduce the medical errors, hospitals must focus on employee job satisfaction. In this case, both stress and burnout can lower job satisfaction. Aims: This research was conducted to analyze the internal factors that affect the level of job satisfaction of employees in health facilities. Methods: The method applied is the PRISMA framework, which consists of five stages, namely eligibility criteria, information sources, study selection, data collection process, and data items. Results: Systematic review was done on 34 quantitative studies about the factors that influence job satisfaction. Based on the review, the internal factors known are demographic and personal factors. In this case, demographic factors mostly do not affect employee job satisfaction, one of which is salary. Meanwhile, the most researched personal factor and the biggest influence is burnout. Conclusion: Based on the systematic review, it is known that the internal factors affecting employees’ job satisfaction include demographic and personal factors. The demographic factor that has a significant effect is salary, while the most researched individual factor is burnout. Working in the health sector requires caution so as not to cause medical errors. Health workers who experience burnout are at risk of causing medical errors. For that we need a strategy to overcome burnout so as to increase job satisfaction. Keywords: burnout, healthcare, internal factor, job satisfaction, PRISMA

Abstract

Introduction: Home delivery increases the chance of maternal death. Aims: The study examines suitable targets for developing policies to reduce home births. Methods: This cross-sectional study analyzed 15,357 mothers through stratification and multistage random sampling, including 13 independent variables: age, education, employment, marital, parity, insurance, knowledge of pregnancy danger signs, antenatal care (ANC), residence, the autonomy of health and family finance, household head sex, and wealth. We examined the data using binary logistic regression. Results: About 23.8% of mothers deliver at home. Older age, higher education, primiparous, insured, knowing the pregnancy danger signs, living in an urban area, and doing ANC ≥4 times were protective factors to not home delivery. Being employed, married, having a male household head, and being poor were risk factors for home delivery. Mothers without health autonomy are less likely to deliver at home than those with health autonomy. Mothers with family finance autonomy are 1.239 times more likely than those without to give home birth. Conclusion: The target to reduce home deliveries: young, low education, employed, married or divorced/widowed, having many children, uninsured, do not know pregnancy danger signs, doing ANC <4 times, living in a rural area, have no health autonomy, have no family finance autonomy, having a male household head, and poor. Abstract

Congenital hypothyroidism is one of the main causes of mental retardation which can be prevented. Research in 2020 found that the prevalence of congenital hypothyroidism in Indonesia was 1:1167. During the previous 18 years of implementation of the Congenital Hypothyroidism Screening Program (CHSP), it was found that only 4,6% of newborns. This study aims to conduct a literature review to identify problems and obtain information on improving the implementation of congenital hypothyroidism screening programs in Indonesia. This research was carried out between November-December 2021 and consists of a review of a range of research articles and journals which have been published within the last decade. Several studies have shown that during the implementation of the CHSP, several obstacles were found which hindered the effectiveness of the program, namely the lack of training for health workers, sources of financing which have not been fully borne by the center, accuracy of collection and sample delivery, and less than optimal management function in regulating the implementation of the program thus a more in-depth evaluation needed, especially in terms of economic benefits and the level of effectiveness of the screening program. Abstrak: Hipotiroid kongenital merupakan salah satu penyebab utama keterbelakangan mental yang dapat dicegah. Penelitian pada tahun 2020 menemukan bahwa prevalensi hipotiroid kongenital di Indonesia adalah 1:1167. Selama 18 tahun terakhir pelaksanaan Congenital Hypothyroidsm Screening Program (CHSP), ditemukan hanya 4,6% bayi baru lahir. Penelitian ini bertujuan untuk melakukan studi literatur untuk mengidentifikasi masalah dan memperoleh informasi dalam meningkatkan pelaksanaan program skrining hipotiroid kongenital di Indonesia. Penelitian ini dilakukan antara November-Desember 2021 dan terdiri dari review berbagai artikel penelitian dan jurnal yang telah diterbitkan dalam dekade terakhir. Beberapa penelitian menunjukkan bahwa selama pelaksanaan CHSP ditemukan beberapa kendala yang menghambat efektifitas program, yaitu kurangnya pelatihan bagi tenaga kesehatan, sumber pembiayaan yang belum sepenuhnya ditanggung oleh pusat, ketepatan pengumpulan dan penyampaian sampel, dan kurang optimalnya fungsi manajemen dalam mengatur pelaksanaan program sehingga diperlukan evaluasi yang lebih mendalam terutama dari segi manfaat ekonomi dan tingkat efektivitas program skrining hipotiroid kongenital.

Abstract

Hospitals and other healthcare facilities place a high priority on hand hygiene because pathogen-contaminated hands can spread infectious diseases known as healthcare-associated infections (HAIs). According to the findings of preliminary research that involved document tracing, the implementation of hand hygiene at private hospitals in Manado from January to July 2020 did not meet the stated standards for compliance indicators. This study aimed to analyze the influence of organizational, occupational, and individual factors on the level of compliance of nurses in Private Hospitals in Manado in performing hand hygiene practices. This study was an analytical observational study using a cross-sectional design. Data was collected using questionnaires and observations of 125 respondents from 14 treatment rooms at Private Hospital in Manado. Multiple linear regression was performed to analyze the data. The results of the multiple linear regression test showed that climate factors of safety, facility availability, information and training, supervision, and risk perception (p-value: 0.001; 0.017; 0.002; 0.019; and 0.022) were significant for nurses’ compliance in performing hand hygiene. The hospital management is expected to provide inspiration to increase nurse motivation, build communication-related to hand hygiene practices, and implement reward and punishment programs. Abstrak: Hand hygiene merupakan praktek yang penting di rumah sakit maupun fasilitas pelayanan kesehatan lainnya karena. tangan yang terkontaminasi patogen dapat menularkan penyakit infeksi yang disebut Healthcare-associated infections (HAIs). Hasil studi pendahuluan berupa telusur dokumen ditemukan bahwa capaian indikator kepatuhan pelaksanaan hand hygiene di RS Privat di Manado selama Januari hingga Juli 2020 belum mencapai standar yang telah ditetapkan. Penelitian ini bertujuan melakukan menganalisis pengaruh faktor organisasi, pekerjaan, dan individu terhadap tingkat kepatuhan perawat di RS Privat di Manado dalam melaksanakan praktek hand hygiene. Penelitian ini merupakan penelitian observasional analitik menggunakan desain cross-sectional. Pengumpulan data dilakukan melalui pengisian kuisioner dan observasi terhadap 125 responden dari 14 ruang perawatan di RS Privat di Manado. Analisis data dilakukan dengan uji regresi linear ganda. Hasil uji regresi linear ganda menunjukkan bahwa faktor iklim keselamatan, ketersediaan fasilitas, informasi dan pelatihan, supervisi, dan persepsi risiko (p-value: 0.001; 0.017; 0.002; 0.019; dan 0.022) signifikan terhadap kepatuhan perawat dalam melakukan hand hygiene. Pihak manajemen rumah sakit diharapkan dapat memberikan inspirasi untuk meningkatkan motivasi perawat dan membangun komunikasi terkait praktek hand hygiene, serta menerapkan program reward and punishment.

Abstract

Background: Eastern regions have a higher prevalence rate of stunted children than other regions in Indonesia. Objective: The study analyzes the factors related to stunted toddlers in Eastern Indonesia. Methods: The study employed secondary data from the 2017 Indonesia Nutritional Status Monitoring. We analyzed 24,103 toddlers as a sample. The study analyzed data using a multinomial logistic regression test. Results: The study found that toddlers living in urban areas had a 0.798 times less chance than toddlers residing in rural areas to experience severe stunted (AOR 0.798; 95% CI 0.719-0.885). A toddler with a married mother is 0.706 times less likely than a toddler with a divorced/widowed mother to experience severe stunted (AOR 0.706; 95% CI 0.547-0.911). The results indicate that the better a mother's education level, the less likely it is to have stunted and severely stunted toddlers. The study also found that toddler age is related to stunted and severe stunted toddlers. The study concluded four factors related to stunted toddlers in eastern Indonesia. The four factors were the type of residence, the mother's marital and education status, and the toddler's age. Conclusion: The study conclusion provides policy targets for policymakers to reduce stunted toddlers in eastern Indonesia, i.e., toddlers living in rural areas, toddlers with divorced/widowed mothers, and toddlers with poor education mothers.

Abstract

Introduction: The delivery of health services at mother and child hospitals involves the midwife's role. Midwives are more responsible for providing patient care when faced with high Bed Occupation Rate (BOR) situations and emergency cases. However, several factors can affect midwives' turnover rates in hospitals. Purpose: This study aimed to analyze the relationship between individual factors (age, marital status, education level, years of service, and employment status), work factors (workload, work environment, and work stress), and financial compensation on job satisfaction and intention to leave midwives as well as recommendations for reducing turnover rates. Methods: This study was conducted at the Trisna Medika Women and Children Hospital, Tulungagung, Indonesia, in April 2022. The questionnaires were distributed to 65 respondents. The research results were analyzed using the partial least squares (PLS)Structural Equation Model (SEM) method. Results: Individual factors had no significant effect on job satisfaction or intention to leave. The variables that directly affect the intention to leave (medium) are workload, financial compensation (strong), and job satisfaction (medium). Job satisfaction is directly affected (moderately) by financial compensation and work stress. The indirect effect shows a significant relationship between job stress and job satisfaction in the work environment. Conclusion: The direct effect of financial compensation on intention to leave was stronger than the effect of financial compensation on job satisfaction.

Abstract

Utilization of health services by pregnant and postpartum women can save women from the incidence of maternal death and severe complications during pregnancy and postpartum. Several factors that directly contribute to the utilization of health services are decision making and sources of information obtained by pregnant and postpartum women regarding the utilization of health services. This study aims to analyze gender equality in decision making on the use of health services in urban and rural areas of Indonesia. This study used a mixed-method approach from primary data and a literature review. The target in this article was pregnant women and postpartum mothers who were spread out in several health centers in Surabaya City and Bojonegoro Regency with a total of 175 respondents who were calculated using the proportion estimation formula. Data analysis used univariate and bivariate analysis. The results showed that the wife in terms of the number of frequencies dominated in making decisions on the use of health services (92.5%). In addition, when the decision maker is the husband, all postpartum mothers (100%) will take advantage of health services. Related to the source of information that many respondents chose in providing information related to pregnancy health for the use of health services, it was health workers (86.3%) compared to other media. The wife's decision is a determinant in making decisions about the use of health services. On the other hand, husband’s support as a decision maker can also influence his wife to take advantage of health services.

Abstract

Introduction: Maluku is a region characterized by an archipelago with approximately 1,340 islands and an area of 712,479.69 km2. Most of the site is water (92.4%), while the land area is only around 7.6%. Aims: This study aimed to analyze the determinant of hospital utilization in Maluku Province, Indonesia. Methods: This cross-sectional study surveyed 788 respondents. The variables examined included hospital utilization, age group, gender, education level, work type, marital status, health insurance, transportation cost, and time travel. The author used binary logistic regression in the final stage. Results: The results show that the 30-39 age group was 2.293 times more likely than the ≥ 50 age group to utilize the hospital (OR 2.293; 95% CI 1.177 – 4.466). Married people were more likely to use the hospital 1.764 times than those with the never-married category (OR 1.764; 95% CI 1.074 – 2.898). Christians were 1.599 times more likely than Muslims to utilize the hospital (OR 1.599; 95% CI 1.146 – 2.231). Conclusion: The study concluded that three variables are determinants of hospital utilization among people in Maluku Province, Indonesia. The three were age group, marital status, and religion. The study's results were significant for local policymakers to provide specific directions to accelerate the increase in hospital utility in Maluku Province in Indonesia.

Abstract

Background: Primary health care (PHC) is the closest institution known to people in disadvantaged areas. Aims: The study determines the policy target based on the demographic characteristics to increase PHC utilization in a disadvantaged area. Methods: The cross-sectional research explored 42,644 respondents. In addition to PHC utilization, the study examined eight independent variables: residence, gender, marital, age, wealth, insurance, education, and travel time. We employed a binary logistic regression in the last stage. Results: Live in urban areas had 1.967 more likely to utilize the PHC (95%CI 1.942-1.992). Males could be 1.412 higher than females (95%CI 1.402-1.421). All marital types had more chances than never married. All education was more likely than no education. The employed had 0.972 less likely than the unemployed (95%CI 0.965-0.980). All wealth groups had less possibility than the poorest. Those with government-run insurance were likelier than other types. The ≤10 minutes travel time was more likely than the >10 minutes. Conclusion: There were seven policy target characteristics to increase PHC use in disadvantaged areas in Indonesia: live in a rural area, female, never married, have no education, employed, not the poorest, don't have government-run insurance, and have a travel time of more than ten minutes.
