Abstract

Kader kesehatan memiliki peran penting dalam deteksi dini factor risiko komplikasi penyakit di masyarakat pertanian. Namun, kader kesehatan memiliki hambatan dalam penguasaan keterampilan tersebut, terutama keterampilan deteksi dini kaki diabetik. Hal ini inline dengan tingginya jumlah kasus risiko kaki diabetik di wilayah pertanian Jember. Oleh karena itu, peningkatan keterampilan melalui pemberdayaan kader kesehatan menjadi urgensi untuk ditangani. Tujuan dari Pendidikan kesehatan ini adalah meningkatkan keterampilan kader kesehatan di wilayah pertanian melalui pemanfaatan Technology-Based Diabetic Foot Detection. Tahapan pengabdian yang dilakukan meliputi identifikasi kebutuhan mitra, pembuatan rancangan program pengabdian, pelaksanaan program dengan pelatihan pemberdayaan pada kader, evaluasi kegiatan. Metode edukasi yang dilakukan meliputi, pemutaran video animasi Technology-Based Diabetic Foot Detection, demonstrasi, diskusi, dan evaluasi. Hasil kegiatan menunjukkan rata-rata nilai pengetahuan kader tentang deteksi dini kaki diabetik sebelum dan sesudah edukasi. Hasil nilai rata-rata pengetahuan dari kader kesehatan sebelum edukasi adalah 76.67 (pretest), sedangkan hasil nilai rata-rata sesudah edukasi menjadi 95 (postest). Peningkatan nilai rata-rata pengetahuan dari kader kesehatan menunjukkan bahwa edukasi dengan pemanfaatan video animasi Technology-Based Diabetic Foot Detection berpengaruh terhadap peserta. Dengan demikian, pemberian edukasi ini dapat dimanfaatkan untuk kader kesehatan dalam meningkatkan keterampilan mendeteksi adanya risiko kaki diabetik.

Abstract

Hiperurisemia merupakan keadaan peningkatan kadar asam urat >7 mg/dL pada laki-laki dan >6 mg/dL pada perempuan. Asam urat merupakan produk akhir dari degradasi purin. Purin didapatkan dari dalam tubuh (Endogen) dan dari luar tubuh (Eksogen). Purin eksogen didapatkan dari salah satu sumber nutrisi yaitu makronutrien. Karbohidrat, lemak, dan protein merupakan komponen dari makronutrien yang berperan besar dalam menghasilkan energi sebagai sumber tenaga bagi tubuh. Pola konsumsi makronutrien yang berlebihan dapat menyebabkan peningkatan kadar asam urat pada penderita obesitas dan DMT2. Penelitian ini merupakan penelitian cross sectional study dengan total sampel sebanyak 63 responden yang merupakan peserta Prolanis. Data diperoleh dari data sekunder departemen Ilmu Kesehatan Masyarakat (IKM) Fakultas Kedokteran Universitas Jember. Pengaruh pola konsumsi makronutrien terhadap kadar asam urat dianalisis menggunakan uji korelasi Pearson. Hasil uji analisis didapatkan nilai p value untuk karbohidrat, lemak, dan protein terhadap kadar asam urat 0,034; 0,042; 0,061. Pada penelitian ini disimpulkan bahwa konsumsi karbohidrat dan lemak memiliki hubungan yang signifikan terhadap peningkatan kadar asam urat pada penderita obesitas dan DMT2, sedangkan konsumsi protein tidak memiliki hubungan yang signifikan terhadap peningkatan kadar asam urat pada penderita obesitas dan DMT2.

Abstract

The COVID-19 pandemic condition presents its own difficulties in achieving this competency for students. Learning from home has several limitations so that the Government of Indonesia through the Indonesian Ministry of Education allows the school learning process offline or face-to-face. This requires readiness from schools in implementing health protocols in a disciplined manner to reduce the risk of transmission of covid 19. Supervision of the enforcement of health protocols will be very difficult if you only rely on health workers. The Hope Integrated Islamic Elementary School (SDIT Harum) Jember will now start limited face-to-face learning in order to fulfill the competence of students. The Faculty of Medicine and the Faculty of Public Health, University of Jember have a moral responsibility to provide education and health promotion activities to the surrounding community, one of which is basic education institutions. One solution that can be done is to form cadres who are tasked with enforcing the implementation of health protocols in a disciplined manner. The hope of this program is that this health protocol discipline enforcer cadre can become an extension of health workers in promoting the importance of health protocol discipline as the main pillar of preventing COVID 19 in limited face-to-face learning.

Abstract

Farm labor is one of the main livelihoods in Indonesia. Farmers' activities on agricultural land are never separated from the use of pesticides, although basically pesticides are not only harmful to plant pests but also pose a potential hazard to farm workers as pesticide users. Various types of pesticides circulating and used in Indonesia include the type of organophosphate. Jam because of its affordable price, organophosphates have good effectiveness in controlling plant pests. This ability is comparable to the dangers of organophosphates for health. Various studies have proven that organophosphate pesticides and other pesticides can cause both acute and chronic health problems. Health problems can range from mild to severe symptoms resulting in disability or death. Pesticides intoxication can actually be prevented. One of the prevention efforts that can be done is the use of good and correct Personal Protective Equipment (PPE). Farmers actually know about the potential dangers of pesticides and the importance of using PPE when spraying pesticides. However, most farm workers do not use standard PPE and do not know how to use the correct PPE. They choose to use simple PPE that they make themselves, for example using t-shirts as head coverings and face masks instead of using hats, goggles and masks as face and head protection. These measures are, of course, non-standard and cannot provide perfect protection. The community service that is carried out has the target of farm workers who live in Gayasan Village, Jenggawah District, Jember Regency. This community service activity uses counseling methods and training on the proper and correct use of PPE, accompanied by the implementation of free health checks and medication for farm laborers and the surrounding community. Measurement of the level of knowledge is measured using a questionnaire and then compared between before and after counseling. The results obtained are an increase in the knowledge of farm workers about the dangers of pesticides and the use of good and standard PPE.

Abstract

Background: Maternal Mortality Rates (MMR) and Infant Mortality Rates (IMR) are two of the indicators on the success of health programs in Indonesia. Jember has become the district with the highest rate of maternal and infant deaths throughout 2020-2021. Methods: This research assessed the contributing factors of MMR and IMR in Puskesmas Kencong, Jember Regency. This study uses a qualitative descriptive research design. Data in this study were taken by conducting interviews to fill out questionnaires to mothers who had given birth at least once and the Coordinating Midwife and Head Midwife of PONED (Basic Emergency Neonatal Obstetrics Services) at the Puskesmas Kencong. Then the data from the questionnaires and interviews will be processed and then explained in the narrative. Results:. Based on data from questionnaires filled out by 37 respondents, as well as questions posed to the midwife, Puskesmas Kencong has fulfilled the requirements needed as a PONED Health Center according to PONED Guidelines. Conclusion: The PONED Health Center at Kencong Health Center has a low prevalence of MMR and IMR.

Abstract

Background: Indonesia as an agricultural country has more than 33 million workers in the agriculture. Pesticides have an important role in agriculture because they function as powerful plant pest exterminators. However, its use is considered to cause various toxic effects, one of which is on the nervous system. This study aimed to evaluate the impact of pesticide exposure on cognitive impairment by assessing the butyrylcholinesterase biomarker in the blood of rats as experimental animals.Methods: This was a true experimental study conducted during September–November 2022 at the Pharmacology and Biochemistry Laboratory, Faculty of Medicine, University of Jember with a post-test only approach using Wistar rats which were divided into 1 control group and 4 treatment groups treated with chlorpyrifos 5 mg/kg orally. Serum butyrylcholinesterase enzyme levels were measured using the kinetic photometric method and then analyzed with a regression test in IBM SPSS® Statistics software.Results: The average value of butyrylcholinesterase was 920.23±145.27 (Control group); 904.48±91.6 (7 days after chlorpyrifos administration); 889.81±95.2 (14 days after chlorpyrifos administration); 457.84±69.5 (28 days after chlorpyrifos administration); 575.82±194.25 (56 days after chlorpyrifos administration). The results of the One-Way ANOVA test showed significant differences between groups as indicated by a significance test of 0.024 (p <0.05).Conclusions: There is a relationship between the duration of exposure to chlorpyrifos and butyrylcholinesterase levels in Wistar rats. This fact shows that the longer an organism is exposed to low doses of chlorpyrifos pesticides, the higher the level of neurological system toxicity that occurs in that organism.

Abstract

Prebiotik telah dikenal luas memiliki manfaat dalam kesehatan manusia. Sebagai prebiotik alternatif, resistant starch tipe 3 (RS3) memiliki beberapa kelebihan yakni tidak menyebabkan konstipasi maupun diare, RS3 juga menurunkan glukosa darah dan meningkatkan kadar Glucagon Like Peptide-1 (GLP-1) yang memiliki peran utama dalam mekanisme kontrol glukosa darah pada pasien dengan diabetes mellitus (DM). Salah satu bahan yang bisa digunakan sebagai sumber RS3 adalah Modified Cassava Flour (MOCAF). Tujuan dari penelitian ini adalah untuk menganalisis peran RS3 MOCAF sebagai prebiotik alternatif pada tikus diabetes mellitus. Penelitian ini menggunakan hewan coba tikus wistar jantan (Rattus novergicus) model diabetes dengan memberi injeksi streptozotocin (stz) sebanyak 35mg/KgBB secara intraperitoneal. Dilakukan pemeriksaan kadar glukosa darah pada awal penelitian dan setelah induksi stz untuk menentukan kondisi diabetes, dikatakan diabetes jika kadar glukosa darah lebih dari 200mg/dL. Diet RS3 diberikan sebanyak 20 gram/hari selama 4 minggu. Pemberian diet RS3 MOCAF mampu menurunkan kadar glukosa darah puasa dari 494 mg/dL menjadi107 mg/dLdan glukosa darah post prandial dari 526 mg/dL menjadi 96 mg/dL. Analisis mikrobiologi menunjukkan bahwa bakteri yang paling banyak tumbuh pada pemberian diet RS3 MOCAF adalah Lactobacillus Sp. RS3 MOCAF memiliki peran sebagai prebiotik dan mampu menurunkan kadar glukosa darah pada tikus model diabetes melitus.

Abstract

Ibu hamil ialah kelompok yang rentan terinfeksi COVID-19 akibat adanya perubahan fisiologis di masa kehamilan dan cenderung mempunyai kondisi klinis yang lebih buruk. Berdasarkan penelitian sebelumnya, ibu hamil dengan COVID-19 yang parah berisiko mengalami preeklamsia 5 kali lipat dibandingkan ibu hamil dengan COVID-19 tanpa gejala. Identifikasi hubungan infeksi COVID-19 dengan preeklamsia pada ibu hamil sangat penting dilakukan untuk membantu menentukan langkah penanganan yang tepat. Penelitian ini ditulis menggunakan metode literature review. Sumber pustaka atau data yang digunakan dalam penelitian ini didapatkan dari publikasi ilmiah baik internasional maupun nasional yang tercantum dalam Pubmed, Science Direct, dan Google Scholar dengan kata kunci “COVID-19, preeclampsia, SARS-CoV-2, angiotensin converting enzyme (ACE2), renin-angiotensin system (RAS)”. Review dilakukan terhadap 16 jurnal internasional dan 4 jurnal nasional. Infeksi COVID-19 berhubungan dengan kejadian preeklampsia. Risiko preeklamsia pada ibu hamil dengan COVID-19 meningkat sebanding dengan derajat keparahan COVID-19 yang diderita. COVID-19 dapat menyebabkan preeklamsia melalui beberapa mekanisme, yakni melalui disfungsi endotel, peningkatan kaskade koagulasi, dan peningkatan respon inflamasi. Infeksi COVID-19 dapat memicu terjadinya down-regulasi pada Renin-Angiotensin System (RAS) akibat pengikatan SARS-CoV-2 pada reseptor ACE2. Kondisi ini dapat mengakibatkan terjadinya disfungsi endotel yang nantinya menyebabkan preeklamsia. Infeksi COVID-19 dapat memicu terjadinya preeklamsia melalui peningkatan aktivitas kaskade koagulasi, yaitu perubahan pada PT (prothrombin time), BT (bleeding time), aPTT (activated Partial Thromboplastin Time), dan CT (clotting time). Infeksi COVID-19 juga diketahui dapat meningkatkan respon inflamasi yang ditandai adanya peningkatan nilai pada CRP (C-reactive protein), D-dimer, dan NLR (neutrophil lymphocyte ratio) yang pada akhirnya juga memicu terjadinya preeklamsia.

Abstract

Type 2 diabetes (T2D) is an urgent health issue that leads to one of the most significant cardiovascular complications worldwide, diabetic cardiomyopathy (DCM), which is associated with high morbidity and mortality. Chronic hyperglycemia and oxidative stress are major contributor for DCM. Analog rice based of MOCAF and corn starch is a functional food recommended as nutritional supplementation to replace staple foods in T2D patients. Analog rice has a high fiber content, high resistant starch and low glycemic index, which can help lower blood sugar levels and prevent DM complications. MOCAF in analog rice is also rich in phenolic compounds, which has potential effect of antioxidant. Metformin is the first line oral therapy for T2D. The present study investigates the potential effects of an analog rice diet in combination with metformin, for the development of DCM. Type 2 diabetic rats were induced by HFD/STZ method and was treated by analog rice and metformin for 4 weeks. Myocardial cell death measured by pyknotic nuclei percentage was analysed. The study result demonstrate that analog rice and metformin prevent the myocardial cell damage observed in type 2 diabetic animals, highlighting analog rice potential as adjuvant therapy, for treating DCM has no clinical significance than metformin therapy alone.

Abstract

Burn wounds are challenging to treat because they often heal slowly and are susceptible to complications. The inflammatory phase plays an essential role in the repair process. However, prolonged inflammation can delay tissue healing. The total leukocyte count is an important parameter to evaluate the process, as it reflects the balance between inflammation and the initiation of tissue regeneration. Oyster mushrooms (Pleurotus ostreatus) contain β-glucans and phenolic compounds with anti-inflammatory and antioxidant properties, which may help regulate leukocyte activity and promote wound healing. This study aimed to evaluate the effect of oyster mushroom powder on total leukocyte counts during burn wound healing in Rattus norvegicus. Twenty-seven healthy male rats aged 2-3 mo were randomly divided into 3 groups, with 9 rats in each group. The negative control group received aquadest, the positive control group received bovine serum albumin, and the treatment group received 10% oyster mushroom powder. All groups received identical topical wound management with 1% silver sulfadiazine ointment to prevent infection and maintain a moist wound environment during healing. Blood samples were collected on days 0, 4, 8, and 12 after burn induction, and leukocyte counts were measured. The results showed that the treatment group consistently had lower leukocyte counts compared with control groups. On day 0, the treatment group had significantly lower leukocyte levels compared with the negative control (p = 0.019). On day 8, leukocyte counts in the treatment group were also significantly lower than the positive control (p = 0.030). By day 12, all groups showed a decrease, but the treatment group demonstrated the most gradual and stable reduction over time. In conclusion, oyster mushroom powder reduces leukocyte levels and supported a more controlled inflammatory phase, allowing faster progression to proliferation and improved burn wound healing.
