Abstract

ABSTRAK Angka Kematian Ibu (AKI) menjadi sebuah tantangan di Indonesia. AKI di Indonesia belum dapat mencapai target yang telah ditetapkan oleh SDGs meskipun AKI di Indonesia terus mengalami penurunan. Kematian ibu terjadi setiap satu menit dalam sehari. Kematian ibu sering kali disebabkan oleh komplikasi obstetri. Salah satu penyebab komplikasi obstetri pada ibu adalah rendahnya cakupan deteksi dini risiko kehamilan yang dilakukan melalui asuhan antenatal. Tujuan dari literature review ini adalah untuk mengkaji efektivitas asuhan antenatal dalam mencegah komplikasi obstetri. Penelitian ini merupakan literature review dengan pendekatan narrative literature review. Peneliti melakukan pencarian dan pengumpulan terhadap artikel sumber rujukan melalui basis data online Google Scholar. Artikel dikumpulkan berdasarkan kriteria inklusi yang telah ditetapkan oleh peneliti. Ditemukan sebanyak 5 artikel sumber rujukan sebagai bahan penulisan literature review. Hasil kajian menjelaskan bahwa asuhan antenatal menjadi upaya preventif untuk mencegah kematian ibu sehingga asuhan antenatal menjadi strategi efektif untuk menekan AKI. Asuhan antenatal dapat mendeteksi dini komplikasi obstetri yang menjadi faktor risiko penyebab kematian ibu. Kesimpulan dari literature review ini adalah asuhan antenatal memiliki efektivitas tinggi dan kontribusi besar dalam mencegah kematian ibu. Disarankan untuk melakukan kolaborasi seluruh pilar safe motherhood sebagai upaya pencegahan komplikasi obstetri sebagai penyebab kematian ibu. Kata Kunci: Asuhan antenatal, kematian ibu, komplikasi obstetri  ABSTRACT The Maternal Mortality Rate (MMR) is a challenge in Indonesia. MMR in Indonesia has not been able to achieve the target set by the SDGs even though the MMR in Indonesia continues to decline. Maternal death occurs every one minute of the day. Maternal death is often caused by obstetric complications. One of the causes of obstetric complications in mothers is the low coverage of early detection of pregnancy risk carried out through antenatal care. The purpose of this literature review is to examine the effectiveness of antenatal care in preventing obstetric complications. This research is a literature review with a narrative literature review approach. Researchers search and collect reference source articles through the Google Scholar online database. Articles are collected based on inclusion criteria that have been set by the researchers. Found as many as 5 reference source articles as material for writing literature review. The results of the review explained that antenatal care is a preventive effort to prevent maternal death so that antenatal care becomes an effective strategy to suppress MMR. Antenatal care can early detect obstetric complications that are risk factors for the cause of maternal death. This literature review concludes that antenatal care has high effectiveness and a great contribution to preventing maternal mortality. It is recommended to collaborate on all pillars of safe motherhood to prevent obstetric complications as a cause of maternal death. Keywords: Antenatal care, maternal mortality, obstetric complication

Abstract

Latar Belakang : Pandemi Covid-19 di awal tahun 2020 meningkatkan permasalahan kesehatan reproduksi remaja. Hal ini dibuktikan dengan peningkatan kasus pernikahan dini yang disebabkan oleh premarital seks dan kehamilan tidak diinginkan remaja. Sekitar 49% dari 21 juta remaja perempuan usia 15-19 tahun di negara berkembang mengalami kehamilan tidak diinginkan. Oleh karena itu berbagai upaya dilakukan untuk mencegah permasalahan remaja tersebut mulai dari praktisi, akademisi maupun LSM. Berdasarkan penelitian sebelumnya diketahui bahwa hampir 100% remaja terpapar dengan edukasi kesehatan berupa penyuluhan. Namun sebagian besar remaja menyatakan bahwa metode ini kurang efektif dan belum sesuai dengan karakteristik remaja saat ini bahkan remaja cenderung bosan dengan metode ceramah.Tujuan : Menganalisis efektivitas metode edutainment sebagai metode pelatihan bagi peer educator Remaja remaja di era pandemi.Metode : Penelitian ini merupakan quasi eksperimental. Keterlibatan sasaran sangat berperan penting dalam keberhasilan kegiatan ini. Kegiatan ini diikuti oleh 50 siswa dari 2 sekolah SMP di Kabupaten Malang. Intervensi melalui aplikasi berbasis android, yaitu Aplikasi Konco SREGEP. Aplikasi ini dirancang dengan melibatkan remaja dan berbagai stakeholder. Aplikasi ini dapat diakses responden dan responden dapat mengakses 13 materi secara bertahap (narsumber dari berbagai stakeholder). Setiap level materi terdapat prepost test. Kegiatan akses materi diberikan waktu selama 1 bulan. Analisis yang digunakan adalah wilcoxon test.Hasil Penelitian : Semua remaja yang menjadi peserta pelatihan terlibat aktif dalam setiap aktivitas pelatihan edutainment yang dilakukan melalui blended learning. Hal ini dibuktikan dengan adanya diskusi dua arah dan peningkatan pengetahuan remaja tentang materi yang ada dalam fitur aplikasi yang berbasis android. Hasil penelitian ini juga menunjukkan adanya perbedaan secara signifikan tentang pengetahuan sebelum dan sesudah intervensi, yaitu p=0,000 α=0.05.Kesimpulan : Edutainment merupakan salah satu strategi promosi kesehatan remaja yang efektif di era pandemi. Untuk menjaga efektifitas edutainment, remaja perlu dilibatkan dari awal sesuai kapasitasnya dalam merancang metode promosi kesehatan yang selaras dengan karakteristik, kebutuhan dan kapasitas remaja saat ini. Partisipasi remaja akan meningkat jika remaja merasa memiliki program dan mempunyai power untuk keberhasilan implementasi program.Keyword : Edutainment, Promkes, Remaja, Pandemi, Good Health, Well-being

Abstract

Maternal mortality in Indonesia is known to be a concern to be able to achieve targets in accordance with the focus on SDGs and achieve adequate maternal health services in terms of access and quality. One of the urgencies for strategies to reduce maternal mortality is to map the factors associated with maternal mortality and identify the implementation that is carried out. This study aims to analyze the relationship between coverage of health services for pregnant women, delivery mothers, and postpartum women with maternal mortality in East Java, quantitative type research with a correlational approach conducted with secondary data, in the East Java Health Profile from 2019 to 2021. Variables in this study include maternal mortality coverage of health services for pregnant women, delivery mothers, and postpartum women (ANC for pregnant women K1, ANC for pregnant women K4, delivery assisted by health workers, delivery conducted at health services, KF 1, KF 2 and KF 3 postpartum women. The results of the study found that the variables correlated with maternal mortality in East Java from 2019 to 2021 coverage of health services for mothers during pregnancy, delivery mothers, and postpartum.

Abstract

Background: Cesarean section delivery should be chosen if there are certain medical indications. However, the trend of childbirth by cesarean section shows a high increase in Indonesia. Complications of childbirth after cesarean section are higher compared to normal childbirth, this condition can endanger the health and safety of the mother and baby. Aims: to determine the determinants of cesarean section decisions in Indonesia. Method: this study is a systematic review using PRISMA, a database search via Google Scholar, PubMed, and ScienceDirect, keywords are adjusted to the topic discussed. Results: 13 articles were reviewed that were relevant to the research topic. Determinants of cesarean section decisions are medical indication including age, parity, pregnancy complications (hypertension and pre-eclampsia), history of delivery (cesarean section history), and labor complications (premature rupture of membranes and fetal distress) and by choice (own request) including residence in urban areas and employment. Conclusion: Interventions such as education, counseling, and others using appropriate communication, information, and education media, cooperation, and collaboration with academics and nongovernment organizations are needed in carrying out interventions.

Abstract

Background: Peer education programs provided in schools are one method to help prevent drug abuse; however, the effectiveness of these programs relies, in part, on their delivery to the population in need. Therefore, the effectiveness of the program relies on the self-efficacy of peer educators. The peer education program helps students to practice their skills as peer educators in avoiding drug abuse. Aims: This study aimed to explore the students’ self-efficacy as peer educators after being a peer educator in the peer education program. Methods: In-depth semi-structured interviews were conducted with 45 student peer educators in Middle schools located in Surabaya, Indonesia. Results: Adolescents who acted as student peers perceived and experienced an improvement in their knowledge and ability to help their peers avoid drug abuse. Conclusion: The peer education program is perceived to improve students’ self-efficacy in counseling tasks, to learn analytical skills, and to maintain positive behavior.

Abstract

Based on the results of the 2021 Family Data Collection, it was revealed that Ngadiluwih District, Kediri Regency, East Java Province has a high unmet need case of 19.25%. This unmet need case is far below the target set by the RPJMN for 2019-2024. The village with the highest unmet need rate in Ngadiluwih District is Mangunrejo Village with a case of 16.89%. Various factors contribute to the unmet need rate, including socioeconomic conditions. The purpose of this study was to analyze the occupational status of the intention to use contraception in couples of childbearing age with unmet needs in Mangunrejo Village. The research was conducted with a cross-sectional approach through analytic observational methods in Mangunrejo Village in April-June 2022. The method used was interviewing and distributing questionnaires to respondents. The sample used couples of childbearing age who fit the inclusion criteria in Mangunrejo Village. As for the total of 12 respondents working and 12 respondents not working, as many as 61.3% of respondents not working have the intention to use contraception and as many as 47.8% of working respondents have the intention to use contraception. The results of the analysis showed that there was no significant relationship between the employment status of unmet need women with intention to use contraception (p=0.325). So it can be concluded that reproductive couples in Ngadiluwih District who are in unmet need condition, do not intend to use contraception who are all of those who are workers

Abstract

Background: The population growth rate in Indonesia from 2010-2020 was 1.25% per year. The rate of population growth must be accompanied by an increase in the quality of human life. Human quality of life begins from within the womb, so that preventive efforts can be undertaken. The Family Planning Program was implemented to overcome the problem of population density so that it becomes more controlled. However, in line with the existence of the family planning program, there are still incidents of unmet need for family planning that occur among couples of productive ages. Purpose: This study aims to undertake a cluster analysis to see which variables are the dominant reasons for couples of childbearing ages to have unmet needs. Methods: This research was conducted using the K-Means cluster analysis method, using secondary data in 25 sub-districts from the Banyuwangi Regency Social, Women's Empowerment and Family Planning Service. Results: Research showed that 3 clusters were formed, each cluster had a dominant incidence of unmet need. Cluster 1 was dominant in Drop Out incidents in 14 sub-districts, Cluster 2 was dominant in IAT incidents in 9 sub-districts, and Cluster 3 was dominant in TIAL incidents in 2 sub-districts. Conclusion: The implementation of cluster grouping can make it easier for officers to focus on reducing the number of unmet need incidents that occur among residents in each sub-district.

Abstract

Background: Menstruation is one of the signs that a woman is entering adolescence. Menarche is the first menstrual event in women which generally occurs at the age of 12 to 15 years. An increase in the number of adolescents who experience menarche before the age of 12 years occurs in several countries. Sexual behavior in adolescents that can be positive or negative can be the reason for the increase and development of hormones in the human body. Objectives: The study was conducted to determine the relationship between sexual behavior and the incidence of early menarche in adolescent girls in Indonesia in 2017. Methods: The study is a quantitative descriptive study with a cross sectional research design using secondary data obtained from the 2017 Indonesian Demographic and Health Survey (IDHS) conducted in 34 provinces in Indonesia starting on July 24 and ending on December 30, 2017. The sample in this study was 1,494 adolescent girls in Indonesia with the age of 15 years, unmarried, and already experiencing menstruation. The variables involved in this study were sexual behavior and the incidence of early menarche in adolescent girls in Indonesia in 2017. Data analysis in this study used bivariate analysis by displaying frequency distribution tables along with analysis tables with chi-square tests. Results: The majority of respondents (71.2%) in this study had high-risk sexual behavior, the incidence of early menarche in adolescents aged ≤13 years was 77.4% and the p-value <0.05 on the relationship between sexual behavior and the incidence of menarche in adolescent girls in Indonesia in 2017. Conclusions: This study shows that there is an association between sexual behavior and the incidence of early menarche in adolescent girls in Indonesia. Education and parental supervision of sexual relationships and behavior in adolescents need to be done to minimize the incidence of early menarche in adolescent girls in Indonesia.

Abstract

Background: Anaemia is a nutritional problem that mostly affects adolescent girls around the world and has become a public health problem in many countries. Iron supplementation is one of public health intervention used to prevent adolescents from iron deficiency anaemia. Various countries have widely applied this program to reduce and prevent anaemia in adolescent and pregnant women.Objectives: Therefore, this literature review will discuss the effects of iron supplementation program on adolescent girls.Materials and Methods: This study was a systematic literature review of eight articles. These articles were found from scientific journal articles from five databases (Proquest, Science direct, EbscoHost, Pubmed, SAGE).Results: Our study showed iron supplementation in adolescent girls increased haemoglobin status, serum ferritin and decreased anaemia prevalence.Conclusion: Initiatives for iron supplementation programs among adolescent girls are necessary to reduce iron deficiency anaemia.

Abstract

Background: Conventional health promotion media such as leaflets are considered not to meet the needs of patients for health information. Ineffective health promotion media can contribute to the occurrence of a health problem. The HEY Website is a health promotion media that is aligned with the current situation which is the digital era. Objectives: This study was conducted to analyze the increase in knowledge of high school students who have received edutainment interventions from the HEY Website. Methods: This research was quantitative research with a quasi-experimental research design in the form of peer educator training using the HEY website. The respondents of this study were 50 high school students at SMAN 1 Turen in Malang Regency with the inclusion criteria being grade X and XI and active in student organizations as they may bring better influence among peers. Results: There was an increase in knowledge, as shown by the average pretest score of 64.5. After the training, the post-test score showed an average number of 86.2. Statistical analysis also showed significant results in knowledge differences before and after the intervention (P=0,000). Conclusion: The HEY website as a digital-based adolescent health promotion media proved significant in increasing student knowledge about adolescent health.
