Abstract

Hemodialysis therapy that is carried out for a long time can cause physical stress in the form of fatigue and have an impact on the patient’s quality of life. Giving intradialytic exercise intervention is believed to reduce the level of fatigue. The purpose of this study was to determine the effect of intradialytic exercise on the level of fatigue in chronic kidney failure patients undergoing hemodialysis. This study used a systematic literature review method based on data from Google Schoolar, Garuda, Science Direct, and the Directory of Open Access Journal (DOAJ), by including key words are fatigue, chronic kidney failure, hemodialysisi, and intradialytic exercise. Article were selected through screening according to inclusion criteria, including publications within the last 10 years (2011 – 2022), original articles, Indonesian and English, descriptive analytic study types, and quasi experiments. The results show that Intradialytic exercises applied include breathing exercise, Range of Motion (ROM), slow deep breathing, Progressive Muscle Relaxation (PMR), Pursed Lip Breathing and Physical Exercise. Based on the 9 articles found that intradialytic exercise has an effect on the level of fatigue. The average p-value <0.05, the average post-test fatigue score is smaller than the pre=test, the smaller the average value fatigue score then the level of fatigue decreases. In conclusion, Intradialytic exercise affects the fatigue level of chronic kidney failure patients undergoing hemodialysis. Suggestions are given to HD nurses to be able to implement an intradialytic exercise intervention in patients undergoing hemodialysis to reduce fatigue.

Abstract

Introduction: Diabetic foot ulcers are a complication that often occurs in people with diabetes mellitus. This complication begins with a decrease in foot sensitivity which leads to neuropathy, resulting in frequent trauma to the feet. Decreased foot sensitivity in the form of decreased sensation of heat, cold, and pain is a dangerous complaint because it has a very high risk of causing foot ulcers or neuropathic foot ulcers. Objective: This study aimed to determine the relationship between the level of foot sensitivity and the incidence of dibetic foot ulcers in diabetes mellitus patients. Method: The type of research is a descriptive correlation with a cross-sectional approach. The population, all DM pstients at Internal Medicine Clinic 1 and 2 Indramayu District Hospital. The sample was 48 people taken by accidental sampling. Data collection tools used observation sheets and 10 G monofilament, while data analysis techniques used the Chi-square test. Result: The research results obtained from 44 respondents showed that 22 respondents (%) experienced neuropathy. Of these, 21 (95.5%) had ulcers. Meanwhile, of the 26 respondents who did not have neuropathy, 21 (80.8%) did not have ulcers. The results of further analysis obtained a p-value of 0.000 and an OR value of 88.2. Conclusion: The conclusion from this study is that there is a relationship between the level of sensitivity and the incidence of diabetic foot ulcers. DM patients who experience neuropathy are 88.2 times more likely to develop ulcers than those who do not experience neuropathy. Suggestions are given to nurses to continue making efforts to prevent diabetic foot ulcers by checking the sensitivity of the patient's feet regularly, at least once a year.

Abstract

Introduction. Hemodialysis therapy is a kidney replacement therapy that functions to remove metabolic waste resulting from kidney damage. The hemodialysis dose is adjusted to the patient's needs. One of the factors that influence the achievement of adequacy is quick blood flow (Qb). The higher the Qb, the more adequacy will be achieved, but it is not yet known how much Qb speed influences the achievement of hemodialysis adequacy. This research aims to determine the relationship between quick blood flow and hemodialysis adequacy. Method. This research uses descriptive analytical methods with a cross-sectional approach. The population in this study was 133 patients undergoing hemodialysis at the Indramayu District Hospital and a sample of 44 respondents using a purposive sampling technique. This research instrument uses an observation sheet and data is analyzed using the chi-square test. Result. The average quick of blood for patients was 270.57 ml/minute. A total of 24 (54.5%) respondents had Qb ≤ 270.57 ml/minute and 24 (54.5%) respondents achieved adequacy. The results of further analysis obtained a p-value of 0.029 (α < 0.05), meaning that there was a relationship between quick blood flow and hemodialysis adequacy in chronic kidney failure patients in the Hemodialysis Room at Indramayu District Hospital. Conclusion. Quick of blood is related to hemodialysis adequacy. Suggestions are aimed at nurses to adjust Qb according to the patient's dose, namely a minimum of 270 ml/minute so that adequacy is achieved.

Abstract

Diabetes mellitus (DM) is a global health issue with a rising prevalence. One effective intervention for managing blood glucose levels is diabetic foot exercises, which enhance blood circulation and insulin sensitivity. The purpose of this study is to determine the effect of diabetic foot exercises on blood glucose levels. This study employed a quasi-experimental design with a control group pretest-posttest approach. The sample consisted of 30 respondents divided into intervention and control groups. The intervention group performed diabetic foot exercises twice a week for four weeks. Data were analyzed using the Independent T-test. The intervention group showed a significant decrease in mean blood glucose levels, from 245.40 mg/dL to 204.93 mg/dL, with a difference of 40.47 mg/dL. Conversely, the control group experienced an increase in mean blood glucose levels, from 258.46 mg/dL to 305.80 mg/dL, with a difference of 47.34 mg/dL. Statistical analysis indicated a significant difference between the intervention and control groups, with a p-value of 0.000 (p < 0.05). Diabetic foot exercises effectively lower blood glucose levels in type 2 DM patients. This intervention can serve as an additional therapy for diabetes management, particularly for patients at risk of diabetic neuropathy. Education and widespread implementation of diabetic foot exercises are recommended as an accessible and efficient diabetes control strategy.

Abstract

Hemodialysis is the most preferred renal replacement therapy for patients with chronic kidney failure worldwide. The indicator of the success of hemodialysis therapy is the fulfillment of the adequacy of the hemodialysis dose as measured by Kt/V. Many factors affect the adequacy of hemodialysis, including internal factors and external factors of the patient. This study aimed to analyze the factors that affect the adequacy of hemodialysis in patients undergoing hemodialysis at Indramayu Hospital. Research design using a cross-sectional approach. The sample in this study was 44 respondents who were taken based on purposive sampling. The result showed that the average Kt/V was 1.71 with the lowest Kt/V value being 0.76 and the highest Kt/V being 2.33. Of 44 respondents, 24 (54.5 %) found adequacy achieved, and 20 (45.5 %) respondents did not achieve adequacy. Factors related to hemodialysis adequacy are gender (p=0.000; OR=4.0), dry weight (p=0.006; OR=7.286), dialyzer reuse (p=0.005; OR=8.000), and quick of blood (p=0.38; OR=4.5). Suggestions for hemodialysis nurses to be able to evaluate, especially the use of dialyzer reuse no more than 4 times, and maintain a quick blood speed of at least 250 ml/minute to achieve adequacy.
