Abstract

Background: Clinical leadership among nurse managers plays a vital role in ensuring the quality and effectiveness of hospital services. Nurse managers are responsible for integrating professional competence, managerial skills, and interpersonal collaboration to enhance patient outcomes and team performance. However, in Indonesia, there remains a lack of standardized and contextually relevant instruments to measure clinical leadership competencies. Objectives: This study aimed to develop and validate a clinical leadership competency assessment instrument specifically tailored to the Indonesian hospital context and to evaluate its feasibility for practical application in nursing management. Methods: The study employed a research and development design conducted in Tasikmalaya City. The sample consisted of 38 nurse managers selected through accidental sampling. The research process consisted of several stages: instrument design, expert validation, revision, and limited testing. Content validity was examined using the Content Validity Index to ensure conceptual and contextual relevance, while item validity and reliability were analyzed through appropriate statistical methods to ensure measurement accuracy and internal consistency. Results: The developed instrument included 49 items across five core dimensions: communication, collaboration, decision-making, team empowerment, and change management. Validation results showed a CVI of 0.86 (>0.80), and high reliability (Cronbach’s Alpha 0.950). Non-valid items were removed, resulting in a final instrument that is valid, reliable, and contextually appropriate for use by nurse managers. Conclusions: The instrument is valid, reliable, and feasible for assessing nurse managers’ clinical leadership competencies. Beyond its psychometric soundness, it provides a strategic framework to strengthen leadership accountability, improve team coordination, and enhance hospital service quality. The instrument can also serve as a valuable tool in hospital management and nursing education to guide professional development and promote evidence-based leadership practices

Abstract

Introduction: Chronic kidney disease causes problems with fluid regulation, requiring hemodialysis patients to adhere to strict fluid intake restrictions. However, excessive thirst is often the main complaint that contributes to non-adherence to fluid restriction and increases interdialytic weight gain. Therefore, effective, safe, and easily applicable non-pharmacological interventions are needed to help reduce thirst. This study aimed to determine the effect of administering ice cubes on thirst levels in chronic kidney disease patients undergoing hemodialysis. Methods: The study employed a quasi-experimental design with one group pretest–posttest, involving 60 respondents selected through consecutive sampling. Thirst levels were measured using the Visual Analog Scale (VAS) before and after the intervention of sucking 10 ml of ice cubes for one hour. Data were analyzed using the Paired T-Test. Results: The findings showed a significant decrease in thirst levels, from a mean of 4.45 (moderate–high category) to 1.01 (very low category) after the intervention, with a p-value of 0.00 (p < 0.05). These results indicate that ice cubes are effective in reducing thirst both clinically and statistically. Conclusion: Ice-cube therapy is an effective non-pharmacological intervention to reduce thirst levels in hemodialysis patients. Recommendation: Health professionals are encouraged to apply this intervention as part of fluid management and patient comfort strategies, and to develop standardized SOPs to ensure safe and effective implementation.

Abstract

Hemodialysis therapy that is carried out for a long time can cause physical stress in the form of fatigue and have an impact on the patient’s quality of life. Giving intradialytic exercise intervention is believed to reduce the level of fatigue. The purpose of this study was to determine the effect of intradialytic exercise on the level of fatigue in chronic kidney failure patients undergoing hemodialysis. This study used a systematic literature review method based on data from Google Schoolar, Garuda, Science Direct, and the Directory of Open Access Journal (DOAJ), by including key words are fatigue, chronic kidney failure, hemodialysisi, and intradialytic exercise. Article were selected through screening according to inclusion criteria, including publications within the last 10 years (2011 – 2022), original articles, Indonesian and English, descriptive analytic study types, and quasi experiments. The results show that Intradialytic exercises applied include breathing exercise, Range of Motion (ROM), slow deep breathing, Progressive Muscle Relaxation (PMR), Pursed Lip Breathing and Physical Exercise. Based on the 9 articles found that intradialytic exercise has an effect on the level of fatigue. The average p-value <0.05, the average post-test fatigue score is smaller than the pre=test, the smaller the average value fatigue score then the level of fatigue decreases. In conclusion, Intradialytic exercise affects the fatigue level of chronic kidney failure patients undergoing hemodialysis. Suggestions are given to HD nurses to be able to implement an intradialytic exercise intervention in patients undergoing hemodialysis to reduce fatigue.
