Abstract

This study examines the political participation of the Medan City community to the Medan mayor elections. One indicator of the success of the elections, can be seen from the community participation in the exercise of the right of choice. However, voters ' participation in Medan city elections since reform to date, tends to have a low level of participation. The phenomenon, quite indicating that the community of the city of Medan has no orientation in using the right to select the regional head election. Therefore, the estimative participation of voters in the city elections in Medan year 2020 interesting to learn, to be a reference and reflection material in the press of the number of abstainers in Medan. This research is done by a qualitative method of descriptive. Data collection techniques using secondary data in the form of journals, documents, newspapers and primary data through in-depth interviews to the informant that originated from the agency of elections, political and social organisations. Data analysis using the Miles and Huberman techniques are: data reduction, data display and draw conclusions. The results of the study showed the community participation in Medan is dominated by participation in voting and total apathy. In addition, the factors that determine the level of participation in the upcoming elections 2020 are; Government, the party's role in political recruitment and the update of voters data.

Abstract

Tuberculosis (TB) remains one of the most significant public health issues globally, with high morbidity and mortality rates. The City of Palembang, Indonesia, has been identified as a high-burden area for TB, which poses challenges in achieving the national and global elimination targets by 2030. This study explores the roles of collaborative governance and public value in enhancing patient adherence to TB treatment in Palembang. Using qualitative research methods, including in-depth interviews and focus group discussions (FGD) with TB patients, healthcare providers, and stakeholders, this research aims to understand how collaborative governance structures and patients' perceptions of public value influence treatment adherence. The study finds that effective collaboration between government agencies, healthcare facilities, and non-governmental organizations (NGOs) plays a critical role in improving TB service delivery and treatment adherence. However, challenges remain in the areas of data sharing, coordination, and resource allocation, which hinder the full potential of collaborative governance. Furthermore, the research reveals that patients' adherence to treatment is significantly influenced by their perception of public value, such as feeling respected, having access to equitable healthcare, and trusting the healthcare system. Patients who feel valued and treated with dignity are more likely to adhere to their treatment regimen, while stigma and discrimination remain significant barriers to adherence.

Abstract

Background: Propofol is a widely used anesthetic induction agent, yet post-injection pain is a frequent adverse effect. Common strategies to reduce this pain include venous occlusion following lidocaine administration and the admixture of lidocaine with propofol. This study investigates whether lidocaine pretreatment with tourniquet application provides superior pain relief compared to the admixture method. Method: We conducted a single-center, randomized controlled, single-blind trial involving patients scheduled for elective surgery. Participants were randomly assigned to receive either lidocaine pretreatment with tourniquet application (n=15) or a lidocaine-propofol admixture (n=15). The primary outcomes were pain scores and C-reactive protein (CRP) levels, while secondary outcomes included heart rate and blood pressure measurements. Result: 30 patients underwent elective surgery in Dr. Moewardi General Hospital between October and November 2023. Post propofol injection, increments in pain severity were observed in both treatment groups. However, lower pain severity was observed in the tourniquet handling group.  After injection, moderate and severe CRP levels were recorded in the lidocaine-propofol admixture (33.3% and 6.7%, respectively), whereas no moderate or severe CRP levels were discovered in the tourniquet handling group. The tourniquet handling group experienced less discomfort after propofol injection (3.139 ± 0.743 vs. 4.27±0.704, p=0.001). Lower CRP levels were evaluated in the experimental group (0.279 ± 0.121 to 1.68 ± 2.49, p=0.002). Conclusion: Lidocaine pretreatment with tourniquet occlusion or one minute before delivering propofol as an anesthetic induction agent was more effective than the lidocaine-propofol admixture in relieving pain after propofol injections.

Abstract

Keadaan kegawatdaruratan adalah suatu keadaan dimana korban akan mengalami kecacatan atau bahkan kematian, bila tidak mendapatkan pertolongan dengan segera. Salah satu jenis masalah kegawatdaruratan yang dapat menimbulkan kematian mendadak biasanya ditemui diakibatkan oleh henti jantung (cardiac arrest). Penanggulangan kegawatdaruratan merupakan aspek yang penting dan tidak terbatas pada tenaga medis terlatih. Nyeri adalah sensasi yang sangat tidak menyenangkan dan sangat individual yang tidak dapat dibagi dengan orang lain. Siswa Sekolah Menengah Atas (SMA) adalah pelajar yang menduduki masa pendidikan formal sebelum memasuki bangku perkuliahan. Masih minimalnya pengetahuan siswa SMA akan manajemen nyeri menyebabkan seringkali penanganan dan tindaklanjut mengenai keluhan nyeri tidak adekuat. Hal ini yang menjadi dasar dilakukannya kegiatan ini sehingga siswa SMA dapat lebih teredukasi dan dapat menangani kondisi nyeri dengan baik serta memberikan edukasi berlanjut terutama kepada masyarakat. Departemen Anestesiologi dan Terapi Intensif Rumah Sakit Umum Daerah Dr. Moewardi bermaksud mengadakan pelatihan tentang Bantuan Hidup Dasar (BHD) bagi masyarakat dalam lingkup wilayah SMA untuk melakukan BHD pada korban henti jantung dan memberikan penyuluhan terkait manajemen nyeri dengan harapan agar para peserta lebih teredukasi mengenai kondisi nyeri dan bagaimana cara menanganinya. An emergency is a situation where the victim will experience disability or even death if they do not receive immediate help. One type of emergency problem that can cause sudden death most commonly occurs due to cardiac arrest (cardiac arrest). Emergency management is essential and not limited to trained medical personnel. Pain is a very unpleasant and highly individual sensation that cannot be shared with other people. High School (SMA) students are students who complete a period of formal education before entering college. High school students lack knowledge about pain management, so treatment and follow-up regarding pain complaints are often inadequate. This is the basis for carrying out this activity so that high school students can be more educated, handle pain conditions well, and provide ongoing education, especially to the community. Department of Anesthesiology and Intensive Therapy, Dr. Regional General Hospital. Moewardi intends to hold training on Basic Life Support (BHD) for the community within the SMA area to carry out BHD for cardiac arrest victims and provide counseling regarding pain management with the hope that the public will be more educated about pain conditions and how to handle them.
