Abstract

Background: One of the causes of increasing antimicrobial resistance is health workers' lack of knowledge about antibiotics, thus causing irrational behavior in antibiotic stewardship.Objectives: This study aimed to analyze the knowledge and behavior among health workers about antibiotic stewardship and their relationship at a private hospital in Pontianak.Methods: This study was a descriptive observational study with a cross-sectional design. Data were collected using consecutive sampling of health workers involved in antibiotic prescribing using a validated and reliable questionnaire. The data were analyzed using Spearman Rank correlation to determine the relationship between knowledge level and behavior.Results: 39 respondents participated, including nine general doctors, 17 specialists, and 13 pharmacists. Most health workers were 26–35 years old (43.59%) and had worked for 1–5 years (48.72%). All respondents had a good knowledge (100%) of the knowledge topic of the etiology, control, and effect of antibiotic resistance. However, only 76.92% of respondents had good behavior, and the rest had moderate behavior (23.08%) on the behavioral topics in the performance of antimicrobial resistance control programs, controlling antibiotic prescribing, performance preventing the spread of resistant microbes, and good antibiotic prescribing practice. The knowledge had a significant relationship towards behavior (p<0.05) with a correlation coefficient (r)=0.568.Conclusion: Most health workers in a private hospital had good knowledge and behavior about antibiotic stewardship, and there was a moderately strong and positive relationship between them.

Abstract

Kasus Demam berdarah dengue (DBD) meningkat   di Tahun 2023. Kurangnya kesadaran warga terkait bahaya dan cara pencegahannya diduga menjadi penyebab angka DBD belum menurun. Permasalahan DBD juga terdapat di Desa Jeruju Besar dan peningkatan terhadap kesadaran masyarakat untuk mempelajari dan melindungi lingkungan diperlukan. Tujuan kegiatan ini adalah untuk menganalisis peningkatan pengetahuan masyarakat mengenai DBD melalui metode edukasi berbasis kasus. Metode yang dilakukan berupa pre-experimental one group pre test-post test design, dengan memberikan 5 soal sebelum dan sesudah pemberian penyuluhan dengan memberikan contoh nyata kasus DBD yang terjadi untuk ditangani. Peserta kegiatan sebanyak 23 orang yang merupakan kader PKK desa Jeruju Besar yang diberikan pretest, edukasi dengan memberikan kasus terkait DBD, postest dan kuesioner evaluasi kepuasan peserta. Hasil yang diperoleh adalah terdapat peningkatan pengetahuan peserta yang signifikan (p=0.002; p0.005) sebesar 24,71% dengan rata-rata nilai pretest dan postest masing-masing sebesar 73,91 dan 92,17. Nilai Asymp.Sig (2-tailed) setelah pretest dan posttest dengan uji Wilcoxon menunjukkan pengaruh bermakna pada peserta setelah menerima edukasi penanganan DBD dengan metode berbasis kasus. Kesimpulan yang dapat diambil adalah terdapat peningkatan pengetahuan yang bermakna terhadap pengetahuan masyarakat Desa Jeruju Besar mengenai penanganan DBD dengan metode edukasi berbasis kasus.

Abstract

The number of people with chronic diseases in Indonesia, especially hypertension and diabetes mellitus, continues to increase, including in the Pontianak City area, West Kalimantan. Alianyang Community Health Center, Pontianak City, is one of the basic health facilities that implements the Chronic Disease Management Program. The problem experienced by patients with chronic diseases such as hypertension and diabetes mellitus is a decrease in quality of life due to the disease they suffer. One of the causes of a decrease in a person's quality of life is a lack of knowledge and motivation in implementing a healthy lifestyle. Health promotion in the form of education on healthy lifestyles needs to be provided directly to patients and to prolanist cadres of the Alianyang Health Center. The targets of this activity are patients and prolanist cadres of the Alianyang Health Center. The activity aims to provide information to patients and prolanist cadres regarding efforts to improve the quality of life of hypertension and diabetes mellitus patients. The method used is health promotion through educational videos and leaflets. The results showed that there were changes before and after the delivery of health promotion, namely that the average participant experienced an increase in knowledge between the pre-test and post-test, namely 63.33 and 79.17 respectively. Evaluation of activities through questionnaires showed that participants felt the material presented was very interesting and participants gained new knowledge through the educational media provided.

Abstract

The prevalence of dyspepsia in Sungai Kakap Village is ranked third largest based on data from the Sungai Kakap Community Health Center, Kab. Kubu Raya with an incidence of 1,636 cases in 2023. One of the problems experienced by dyspepsia patients is inappropriate self-medication or self-medication. This Community Service Activity (PKM) is aimed at PKK members and Posyandu cadres because they are part of the community and are also an extension of health workers in helping solve problems experienced by dyspepsia patients. The method used in this PKM is case-based and educational videos. This PKM activity consists of several activities including preparation of activities; orientation and coordination of activities; selecting the target audience for educational media creation activities; providing pre-tests and cases at the beginning of activities; providing educational materials; discussion; post-test; evaluation of activity implementation; and data analysis. The output of this community service is publication in local electronic media in West Kalimantan (Suara Pemred/Tanjungpura Times), publication of scientific articles in the Sinta 4 accredited journal " PengabdianMu ", Copyright of educational videos and videos of implementation of activities on the FK UNTAN YouTube channel. The overall results obtained were an increase in knowledge of 27.19% with an average pre-test score (60.6) and post-test score (77.08). According to the evaluation of activities through questionnaires, participants considered the material presented to be very interesting, and the learning media provided was considered to provide new knowledge.

Abstract

Allicin is one of the compounds contained in garlic which previous research predicted could be obtained using PPARG (Peroxisome Proliferatr Activated Receptor Gamma). However, short research requires further analysis, namely molecular dynamics. This research aims to analyze the interaction of allicin binding to PPARG used molecular dynamics. The dynamic molecular method used Gromac 2023 software with the chemical structure of allicin obtained from Pubchem data, while PPARG data was obtained from RCSB website. The results obtained from this study indicate that allicin interacts with similar amino acids as pioglitazone. However, its energy affinity is not as great as pioglitazoe. Based on the results of this research, it shows that allicin is predicted to be able to interact with PPARG in a molecular dynamic mannerKeywords: Allicin, PPARG, Molecular dynamics.

Abstract

Irrational use of antibiotics is currently still a major health problem in the world. Worldwide around 50% of medicines are sold, distributed, and given to patients inappropriately. Irrational use of antibiotics can trigger antibiotic resistance. Knowledge and behavior regarding antibiotic stewardship are important in preventing antibiotic resistance. This cross-sectional research was conducted to determine the relationship between knowledge and behavior regarding antibiotic stewardship among healthcare workers. This study's 58 respondents were general practitioners/dentists, specialist doctors/dentists, and pharmacists. The instrument used in this study was a valid and reliable questionnaire. The data analysis used was univariate analysis and Spearman analysis with IBM SPSS Statistics 26 and significance set as p0.05. In this study, the results showed that the majority of respondents worked as specialist doctors/dentists (53.44%), aged in the range 26-35 years (36.20%), of the type female (58.62%), have work experience for more than 10 years (39.65%) and all of the surveyed respondents have good knowledge and behavior (100%). Spearman’s rank analysis showed no significant relationship between knowledge and antibiotic stewardship behavior (p0.05). The high level of expertise of respondents does not significantly affect the level of behavior because it can influence several factors, both internal and external. Further study is needed to identify other factors influencing antibiotic stewardship and rational use of medicine.

Abstract

The increasing Antimicrobial Resistance (AMR) drives the need of new antibacteria as drug candidate to eradicate resistant bacteria such as ESBL (Extended Spectrum Beta-Lactamase) producing Escherichia coli. Extract and fraction of Sapindus rarak fruit contains numerous saponin compounds, which exert potential antibacterial effect by binding to the bacterial cell membrane. The purpose of this research is to analyze and evaluate Sapindus rarak fruit extract and fraction antibacterial effects through in silico and in vitro approach. Molecular docking is conducted to identified compounds contained in Sapindus rarak fruit against Penicillin Binding Protein (PDB ID: 6NTZ) using Autodock Tools. Antibacterial testing of Sapindus rarak fruit extract and fraction is also conducted with disc diffusion and microdilution. In silico docking study resulted in potential activity from raraoside A and rarasaponin V, which showed affinity tinggi through their binding energy whilst interacting with the active site of the receptor protein. No inhibition zone was detected on extract and fraction, yet lowest minimum inhibition concentration (MIC) was detected at 2.5×104μg/mL on water and n-hexane fraction Sapindus rarak fruit ethanol extract. This weak antibacterial effect tends to happen to crude extract than to compound isolates. Hence, Sapindus rarak fruit fraction exhibits weak antibacterial effect on ESBL producing E. coli, but certain isolates might be potential to be further researched. Sapindus rarak fruit isolates such as raraoside A and rarasaponin V is potential to solve antimicrobial resistance by acting as a plant based antibiotic.

Abstract

Red ginger (Zingiber officinale Roscoe var. rubrum) and angkak (Oryza sativa) are used in traditional chinese medicine to improve body health. Both are known to have immunostimulatory activity that increases phagocytic activity of macrophage. This study aims to determine the phagocytic activity of the combination extract of red ginger and angkak using carbon clearance method. Combination of red ginger and angkak (7:3) was kinetically macerated in ethanol 96%. Twenty white mice were divided into 5 groups, such as control group (CMC-Na 0.5%), positive control (Levamisole 2.5 mg/kgBB), and three experiment group that was given red ginger extract (105 mg/kgBB), angkak extract (105 mg/kgBB), and combination extract of red ginger-angkak (150 mg/kgBB). All treatments were given orally for sevendays, consecutively. The phagocytosis index (IF) value was determined on day 8 by injecting carbon intravenously and immunostimulant activity was shown if IF>1. The highest IF value of three extracts was shown by combination extract (1.2489±0.0261) compared to red ginger and angkak extract, mentioned 1.2030±0.0448 and 1.0537±0.0254, respectively. Moreover, the IF values of combination extract and red ginger were significantly (p<0.05) different compared to the control group of CMC-Na (IF=1). However, it is significantly (p>0.05) lower compared to the levamisole group (1.5187±0.0534). It could be concluded that the combined extract of red ginger-angkak showed the greatest phagocytic activity against male white mice compared to extract of red ginger and angkak itself.

Abstract

Self-medication has become a serous problem among the public due to a lack of knowledge and behavior about self-medication. The percentage of West Kalimantan people practicing self-medication during the last month in 2022 was 86.81%. The purpose of this study was to analyze the level of knowledge and behavior of community self-medication in tiong keranjik village, belimbing hulu sub-district, melawi district and the relationship between the two. This research method is a descriptive cross sectional non-experimental research. The sampling technique used purposive sampling with a questionnaire measuring instrument that had passed ethical review and tested validity and reliability on 30 respondents. Data processing was carried out with the spearman rank correlation statistical test. The results of the study of 100 respondents showed that the level of knowledge of community self-medication behavior is included in the good category by 25%, moderate by 57% and less by 18%. The results of the spearman rank correlation test between knowledge and self-medication behavior showed a significance value of 0.000 with a correlations coefficient of 0.651, and a positive direction. It can be concluded that the majority of people have sufficient knowledge with appropriate behavior and there is a significant relationship between the level of knowledge and self-medication behavior in the community of tiong keranjik village with a strong and unidirectional correlation
