Abstract

Dispute claims for Covid-19 patient financing become a problem amidst the pandemic that could impact the sustainability of health service cash flow in several provinces in Indonesia. This research aims to explore the handling and dispute claims of Covid-19 patients in nine provinces in Indonesia. Also, this is qualitative research using a case study approach. Informants came from primary and secondary health services. Data analysis was carried out by means of triangulation of sources between the informants. The results from this research indicate that several provinces still have difficulties using the online application for claim handling based on the experience that has occurred. Other obstacles include claim documents that failed to upload, unreadable documents, and residents without a national identity number or NIK. These are what made most of the dispute claims occur due to the incomplete claim files and the criteria for guaranteed patients who were not eligible. One of the efforts the hospital took was improving the hospital management information system to minimize dispute claims. This research concludes that dispute claims are still vulnerable, so increasing the capacity and information system of health services, commitment, coordination, and regular socialization from the payer side is needed to prevent the dispute claims of Covid-19 patients.Abstrak:Dispute klaim pembiayaan pasien Covid-19 menjadi permasalahan di tengah pandemi yang dapat berdampak pada keberlangsungan arus kas pelayanan kesehatan di beberapa provinsi di Indonesia. Penelitian ini bertujuan untuk mengeksplorasi penanganan dan dispute klaim pasien Covid-19 di sembilan provinsi di Indonesia. Penelitian ini juga merupakan penelitian kualitatif dengan pendekatan studi kasus. Informan berasal dari pelayanan kesehatan primer dan sekunder. Analisis data dilakukan dengan cara triangulasi sumber antarinforman. Hasil dari penelitian ini menunjukkan bahwa beberapa provinsi masih kesulitan menggunakan aplikasi daring untuk penanganan klaim berdasarkan pengalaman yang telah terjadi. Kendala lainnya antara lain dokumen klaim yang gagal diunggah, dokumen yang tidak terbaca, dan penduduk yang tidak memiliki nomor identitas nasional atau Nomor Induk Kependudukan. Hal inilah yang membuat sebagian besar dispute klaim terjadi karena berkas klaim yang tidak lengkap dan kriteria jaminan pasien yang tidak memenuhi syarat. Salah satu upaya yang dilakukan pihak rumah sakit adalah dengan meningkatkan sistem informasi manajemen rumah sakit untuk meminimalisasi dispute klaim. Penelitian ini menyimpulkan bahwa dispute klaim masih terjadi sehingga peningkatan kapasitas dan sistem informasi pelayanan kesehatan, komitmen, koordinasi, dan sosialisasi secara berkala dari pihak pembayar diperlukan untuk mencegah terjadinya dispute klaim pasien Covid-19.

Abstract

Diarrhea is a condition where there is increased defecation due to an infection. Children can be said to have diarrhea if the volume of stool is measured at 10ml/kg per day. According to the World Health Organization (WHO) 2020, the benefit of washing hands is to keep hands clean, not only as an effective defense against the spread of infections and serious diseases. The aim of the research was to determine the relationship between hand washing and the incidence of diarrhea in children in Cibinong District. This type of research is quantitative research. The research design used is correlational research with a controlling approach. The sampling technique uses total sampling. The number of samples in this research is 30 medical records of children suffering from diarrhea at the Cirimekar Community Health Center. In this research, the Chi-Square statistical test is used. . The results show that the relationship between hand washing behavior and the incidence of diarrhea shows that the P-Value is 0.001 <0.05, so it can be concluded that there is a significant relationship between hand washing behavior and the incidence of diarrhea in children in the Cirimekar Health Center Working Area. The conclusion of this research is that there is a relationship between hand washing behavior and the incidence of diarrhea in children in the Cirimekar Health Center Working Area in 2023.

Abstract

Kasus Penyakit Antraks di Kabupaten Merauke diketahui pada tanggal 23 april 2024, yang diawali dengan adanya kasus kematian misterius hewan ternak berupa sapi dan babi di daerah pemukiman transmigrasi (di Distrik Tanah Miring- Kabupaten Merauke) yang semula dicurigai berasal dari gigitan nyamuk karena didapati oleh para peternak adanya kasus kematian sapi setelah digigit oleh nyamuk yang berukuran lebih besar dari nyamuk pada umumnya. Setelah Beberapa hari kemudian dilakukan pemeriksaan sampel pada babi yang mati dan ditemukan adanya bakteri bersifat zoonosis. Tujuan  Penelitian untuk mengkaji langkah-langkah yang harus ditempuh Pemerintah Daerah setempat agar dapat mempercepat pengendalian dan pencegahan kasus antraks pada Manusia. Metode penelitian yang digunakan adalah kualitatif melalui pengambilan data sekunder dan primer dari berbagai pihak serta studi literatur dari penelitian sebelumnya, sedangkan manfaat dari penelitian ini diharapkan dapat memberikan kontribusi kepada Pemerintah Daerah dan warga masyarakat untuk lebih waspada dan terhindar dari kasus antraks sehingga  tidak terjadi Kejadian Luar Biasa Penyakit Antraks Pada Manusia.  Untuk mendapatkan data terkait penelitian dilakukan wawancara dengan instrument pertanyaan yang disesuaikan dengan pengertian variable. Informan dalam penelitian ini antara lain Pejabat Otoriter Veteriner Provinsi Papua Selatan (POV PPS),. Gambaran kewaspadaan dini sebagai salah satu bentuk respons kesiapsiagaan, deteksi dini kemungkinan terjadinya Kejadian Luar Biasa (KLB) Penyakit Antraks pada manusia di Kabupaten Merauke belum sepenuhnya sesuai dengan petunjuk teknis yang dikeluarkan Kemenkes. Berdasarkan pengolahan data dan identifikasi masalah terkait pencegahan Antraks di Kabupaten Merauke, maka perlu dibuat suatu strategis prioritas yang harus diperhatikan sebagai langkah antisipasi yang terbagi menjadi strategi jangka panjang dan jangka pendek.

Abstract

Kasus Demam Berdah Dengue (DBD) termasuk penyakit yang mengancam kesehatan global dan kasusnya berfluktuasi setiap tahun di Kabupaten Seruyan. Penyebaran kasus hampir menyebar di seluruh wilayah. Tujuan penelitian ini adalah melakukan pemetaan kasus dengan analisis spasial dan menemukan upaya pengendalian vektor yang tepat untuk menurunkan kasus DBD. Jenis penelitian yang digunakan adalah mixed method, gabungan kuantitatif dan kualitatif. Metode kuantitatif yang digunakan adalah observasional analitik dengan desain cross sectional. Analisis spasial dilakukan dengan Sistem Informasi Geografi (SIG) menggunakan software ArcGis Pro Versi 3 dan analisis regresi untuk mengetahui hubungan antara kepadatan penduduk dan angka bebas jentik (ABJ) terhadap kejadian DBD. Metode kualitatif yang digunakan adalah observasi dan wawancara mendalam. Pola spasial kasus DBD di Kabupaten Seruyan yaitu berkelompok (cluster). Pola sebaran cluster ini menunjukkan bahwa lingkungan di Kabupaten Seruyan berpotensi terjadi penularan setempat yang mengelompok. Semakin padat penduduk semakin tinggi pula kasus DBD di Kabupaten Seruyan. Terdapat pengaruh signifikan kepadatan penduduk dengan kejadian Demam Berdarah Dengue (DBD) di Kabupaten Seruyan dengan nilai p 0,002 > 0,05. Tidak terdapat pengaruh signifikan antara Angka Bebas Jentik (ABJ) dengan kejadian Demam Berdarah Dengue (DBD) di Kabupaten Seruyan dengan nilai p 0,149 < 0,05. Belum optimalnya manajemen pengendalian vektor di Kabupaten Seruyan menjadikan salah satu penyebab tingginya kasus DBD didaerah ini. Perlu diterbitkan Peraturan Daerah Kabupaten Seruyan tentang pengendalian penyakit Demam Berdarah Dengue (DBD) untuk meningkatakan peran serta semua pihak serta kewaspadaan dini terhadap kejadian DBD.

Abstract

SISRUTE, an application that connects patient data between health facilities, is faced with obstacles such as data incompatibility and dependence on internet stability. Even though it started in 2020 in North Bengkulu, only 22.7% of health facilities have adopted it, especially community health centers. Implementation obstacles include staff, IT infrastructure, commitment of referring hospitals, patient conditions, and slow response.Usability testingneeded to improve health referral services. The aim of this research is to analyze the evaluation of the use of the Integrated Referral System (SISRUTE) inmedical facilityNorth Bengkulu Regency in 2023. This research is a mix methods research. The research instrument was a questionnaire which was analyzed using univariate analysis, bivariate analysis and multivariate analysis as well as observation and in-depth interviews with informants. Research analysis shows that perceived usefulness, ease of use, and intensity of use have a significant effect on attitudes and use of SISRUTE in hospitals. Attitude towards use is proven to mediate the relationship between perceived usefulness, ease of use, and intensity of use with SISRUTE use. Perception of usefulness has the greatest influence on the use of SISRUTE. Factors such as management support in the form of monitoring and evaluation of use, staff training, health infrastructure, government support, and financial issues influence the acceptance and use of SISRUTE in North Bengkulu Regency. Evaluation of the use of SISRUTE that needs to be carried out is increasing the efficiency of patient referrals, user satisfaction, and identifying areas of improvement such as monitoring and evaluating use.

Abstract

In achieving a reduction in the Infant Mortality Rate, Regional Governments have a very important role in policy programs in the health sector, especially maternal and child health. The SPM achievements related to the Infant Mortality Rate in Seruyan Regency have not yet reached the SPM target in accordance with Minister of Health Regulation number 4 of 2019 concerning minimum service standards in the health sector. This research aims to analyze health service programs in an effort to accelerate the reduction of infant mortality rates in Seruyan Regency in 2023. The research method is qualitative, involving 7 groups of informants selected based on the capacity and knowledge of the resource persons related to health services in reducing infant mortality rates in Seruyan Regency. . Data analysis used in data processing uses the Milles and Huberman method and Fishbone Diagram (fishbone analysis). Data validation in this research uses source triangulation and method triangulation. The results of the research found that health services in order to accelerate the reduction of infant mortality rates in Seruyan Regency in 2023 have not been effective, it is known that the implementation of regulations related to maternal and child health services has not been optimal, has not been efficient, there is no budget allocation for the newborn health service program in 2022, has not been responsive because the integrated K6 antenatal care (ANC) service at the Community Health Center is not yet optimal, the competence and distribution of health workers, the maternal neonatal emergency referral service is not yet optimal, there are still early pregnancies, and the socio-cultural culture of indigenous peoples who still give birth at home and are assisted by village shamans.

Abstract

Amidst the rising numbers of smokers in Indonesia and the increasing burden of smoking-related diseases on the country, the Smoking Cessation Services (SCS) still fail to operate optimally. The aim of this research is to develop a more effective strategy for the SCS model based on the challenges encountered during its implementation. The research method employed qualitative study with a case study approach. The research location was in Depok City, chosen as a national exemplary of SCS. Research period from February to May 2024. Data collection was conducted through in-depth interviews and observations at Community Health Centers who involved in SCS including health professionals, health promotors, and managers. The findings indicate that the SCS is not yet optimal due to insufficiently trained staff, low patient turnout, and failure to achieve the target quit rate. The services still focus on counseling and have not yet incorporated pharmacotherapy. The research concludes that short-term alternative strategies include follow-up calls to patients, establishing telecommunication groups with patients, and reaching out to new patients through schools, while long-term strategies such as program integration and building a telemedicine information system could be implemented to enhance patient visits.

Abstract

Merauke Regional Referral Hospital in South Papua reported 5,482 visits of OAP patients who do not have health insurance during 2022 with an total cost of IDR 8,859,983,341, - Of course, this will be a problem for Merauke Regional Hospital if there is no adequate budget support to serve the community, especially OAP who have not been registered as JKN participants. Therefore, this study was conducted to analyze the financing of health services for Indigenous Papuans (OAP) without health insurance at Merauke Regional Hospital so that it can provide input or recommendations for regional leaders and the Regional Government Budget Team as well as the South Papua Provincial Health, Population Control and Family Planning Office in determining policies related to budget support for Merauke Regional Hospital for health service financing for unsecured OAP health. This research is a type of qualitative research with a case study approach, where data collection is carried out through document review and interviews. Informants were selected purposively and data were analyzed using Miles and Huberman's interactive model. The results of this study obtained that the financing of OAP health services without health insurance at the RSUD during 2022 experienced a "deficit" of Rp 4,356,283,611, - Therefore it is necessary to support the budget through a policy of increasing the allocation of the Special Autonomy Fund at the Merauke Regional Hospital by the Merauke Regional Government Budget Team. The results of this study obtained that the financing of OAP health services without health insurance at the RSUD during 2022 experienced a "deficit" of Rp 4,356,283,611, - Therefore it is necessary to support the budget through a policy of increasing the allocation of the Special Autonomy Fund at the Merauke Regional Hospital by the Merauke Regional Government Budget Team.

Abstract

Target universal kepesertaan Jaminan Kesehatan Nasional (JKN) belum terpenuhi sedangkan kader posyandu belum banyak dilibatkan untuk membantu proses edukasi kepada masyarakat. Penelitian ini bertujuan untuk menganalisis kesiapan kader posyandu dalam melakukan kegiatan edukasi seputar program JKN kepada masyarakat. Jenis penelitian ini adalah kualitatif dengan pendekatan studi kasus. Informan penelitian yaitu kader posyandu. Analisis data dilakukan dengan mengulas hasil wawancara mendalam antar informan kader posyandu. Hasil penelitian menunjukkan bahwa edukasi terkait program JKN kepada masyarakat terbatas pada obrolan, di sela-sela kegiatan posyandu, diantara rapat RT dan pemanfaatan grup telekomunikasi. Dukungan yang diberikan dari Puskesmas masih terbatas dan kurangnya pelatihan informasi seputar program JKN kepada kader Posyandu. Penelitian ini menyimpulkan bahwa Belum ada kegiatan terencana, formal dan rutin yang diselenggarakan oleh kader untuk melakukan edukasi program JKN kepada masyarakat. Puskesmas bekerjasama dengan BPJS Kesehatan sebaiknya memberikan pelatihan rutin kepada kader posyandu mengenai informasi seputar program JKN agar edukasi yang disampaikan oleh kader kepada masyarakat semakin komprehensif.

Abstract

Complaints from patients are often used as an indicator of the quality of health services. The causes of complaints from inpatients are related to factors that cause dissatisfaction or discomfort experienced by patients during their hospitalization. This study aims to see the causes of patient complaints about inpatient services. In addition, researchers also examine the relationship between several variables and complaints and analyze further to determine the main factors causing complaints from patients about inpatient services. This study uses a sequential explanatory approach. The study design is cross-sectional for inpatients at Hospital X who are willing to be respondents and provide their perceptions of inpatient services. The factors of inpatient services that are assessed are facilities, human resources, cleanliness, officers' responsiveness, and information availability. Bivariate and multivariate analyses show that these factors have a significant relationship with the level of patient complaints. Patients with a length of stay of more than four days, non-surgical patients, and those who give poor ratings to human resources, facilities, cleanliness, and responsiveness are more likely to complain about hospital services. Hospitals need to implement strategic steps to improve various aspects, such as reducing complaints and improving the quality of service. One effort can be made to provide a spreadsheet link or Google Drive link containing a flow for accelerating complaint handling. Another effort is to implement a comprehensive improvement strategy to improve the patient experience during treatment.
