Abstract

Highlights: The most sought-after medical career choices and institutions were specialist doctors and hospitals. The most sought-after medical career choices were obstetrics and gynecology specialists.   Abstract Introduction: Government policies during the coronavirus disease (COVID-19) pandemic have limited the clinical rotation learning system of the Medical Faculty at Tanjungpura University. This study examined the description of medical career choices during the COVID-19 pandemic for students of the Medical Profession Study Program, Faculty of Medicine, Tanjungpura University. Methods: This was a descriptive study using a quantitative method in the form of a questionnaire. A total of 138 students were selected through the purposive sampling technique (n=138). The variables were then measured using a medical career choice questionnaire. The obtained data were further processed using the International Business Machines Corporation (IBM) Statistical Package for Social Sciences (SPSS) for Windows version 23.0 and Microsoft Excel 2013. Results: Study results showed that from 138 samples (n=138), students chose careers as general practitioners (male=17.5%; female=16%), specialist doctors (male=70%; female=73%), lecturers (female=2%), structural doctors (male=5%; female=6%), and non-medical careers (male=7.5%; female=2%). Regarding their preferred workplace, students chose the government hospital (male=40%; female=47%), private hospital (male=37%; female=32%), public health center (male=7%; female=9%), private clinic (male=5%; female=4%), state-owned clinic (male=3%, female=1%), military (male=5%, female=2%), ministry (male=3%; female=1%), educational institution (female=3%), and police (female=1%). Conclusion: The most sought-after medical career choices and institutions were obstetrics and gynecology specialists and hospitals. Factors that influenced the selection of a medical career were personal and professional characteristics, employment conditions, personal life, preclinical and clinical rotation phase, demands of postgraduate education, and family roles. Conducting further research to determine the factors that influence the choice of a medical career by using mixed-method research or qualitative interviewing is recommended.

Abstract

Highlights: Most students had very high academic motivation and achievement in the very satisfactory category. No correlation between academic motivation and achievement in undergraduate students.   Abstract Introduction: Academic motivation is said to be correlated to academic achievement. The coronavirus disease (COVID-19) pandemic has led to a distance learning policy allowing students to continue their education. Distance learning has several impacts that can result in students' academic motivation. Therefore, it affects academic achievement. This study examined the relationship between academic motivation and achievement in undergraduate students of the Medical Study Program, Faculty of Medicine, Tanjungpura University, during the COVID-19 Pandemic. Methods: This was an observational analytic study with a cross-sectional approach. A total of 169 students (n=169) were selected through a stratified random sampling technique. Data was processed using the International Business Machines Corporation (IBM) Statistical Package for Social Sciences (SPSS) version 24.0. The variables were measured using the Academic Motivation Scale (AMS)—Short Indonesian Language Version questionnaire and the student's Grade Point Average (GPA). The Spearman Rank correlation test showed a significance value of 0.096 (Sig>0.05; r=0.128). Results: The results showed that most of the samples had very high academic motivation (71.6%) and academic achievement in the very satisfactory category (51.5%). Spearman Rank correlation test showed a significance value of 0.096 (Sig>0.05; r=0.128). Conclusion: There was no correlation between academic motivation and achievement in undergraduate students of the Medical Program, Faculty of Medicine, Tanjungpura University, during the COVID-19 pandemic.

Abstract

Background: The theory that gluteomorphin and casomorphin can act as opiate-like peptides has an effect like opiates by binding to specific receptors in the central nervous system, which triggers the implementation of a casein-free gluten-free diet (GFCF). The binding of gluten and casein derivates in these specific receptors leads to increased activity of the endogenous opioid system associated with symptoms of autism spectrum disorder. Objectives: To describe the application of a casein-free gluten-free diet in children with autism spectrum disorder (ASD). Materials and Methods: We conducted this research with cross-sectional design quantitative research using total sampling (38 respondents, all female, mother) in July-August 2021 with a food frequency questionnaire (FFQ). Results: The result revealed that the average age of ASD children is 10.47 years, weight 35.87 kilograms, height 141.45 centimeters, and mostly boys (68%). Most parents (86.8%) didn’t adopt a GFCF diet. FFQ score mostly amounting to >150 (57.9%). Conclusion: ASD children in SLB Autis Kalbar are given foods with gluten and casein with diverse variations enough that children have an FFQ score >150, and their parents didn’t implement GFCF diet.

Abstract

Background: Breast cancer has a high prevalence rate, with diagnosis often occurring at an advanced stage (stage III-IV), particularly in developing countries. Tumor-infiltrating lymphocytes (TILs) are a prognostic factor that can be assessed based on histopathological features in relation to response to therapy. Furthermore, histopathological grading, tumor size, and lymph node metastasis are predictive factors for breast cancer prognosis. This study aimed to investigate the relationship between TIL and other predictive factors, histopathological grading, tumor size, and axillary lymph node metastasis.Methods: This research is an observational analytic study with a cross-sectional approach. This research was approached by observing the 55 tissue slides of hematoxylin-eosin (HE) in breast cancer patients that met the inclusion and exclusion criteria. The inclusion criteria included breast cancer patients who underwent histopathological examination at the Anatomical Pathology Laboratory of Dr. Soedarso Hospital Pontianak in the period January until December 2021 with invasive carcinoma of no special type and had undergone Modified Radical Mastectomy (MRM) surgery. The exclusion criteria were patients with breast cancer recurrence and double primary cancer cases, and no data on tumor size was obtained on the results of histopathological examination. The data collected was then statistically analyzed with the Spearman test. Results: Breast cancer in this study was mostly diagnosed as grade III, T4, and N2. The results of the Spearman's rho correlation test showed that there was no correlation between TIL and the histopathological grading (p = 0.231; r = 0.164 ), tumor size (p = 0.507; r = -0.091), and axillary lymph node metastases (p = 0.842; r = -0.028).Conclusions: There is no statistically significant correlation between TIL and the grade, tumor size, and axillary lymph node metastases in breast cancer at the anatomical pathology laboratory of Soedarso Hospital Pontianak.

Abstract

Latar belakang: Tanaman Kesum (Polygonum minus Huds.) merupakan tanaman endemik daerah Kalimantan Barat mengandung flavonoid yang dipercaya dapat berperan sebagai antiinflamasi. Tujuan: Mengetahui efek ekstrak etanol daun Kesum terhadap jumlah neutrofil, monosit, dan limfosit tikus putih jantan galur Wistar yang diinduksi karagenin. Metode: Desain penelitian true experimental dengan complete randomized design menggunakan 30 tikus yang dibagi menjadi 5 kelompok. Kelompok kontrol negatif diberi CMC 0,5 mg/kgBB, kelompok kontrol positif diberi natrium diklofenak 2,7 mg/kgBB, kelompok perlakuan 1 diberi ekstrak etanol daun Kesum 4,332 mg/200gBB, kelompok perlakuan 2 diberi ekstrak etanol daun Kesum 8,664 mg/200gBB, dan kelompok perlakuan 3 diberi ekstrak etanol 17,328 mg/200gBB. Karagenin 1% diinduksi pada daerah subplantar. Pengambilan darah, pewarnaan, dan hitung jenis leukosit pada jam 0, 4, 8, dan 12 untuk neutrofil dan monosit, pada jam 0, 12, 24, dan 48 untuk limfosit. Analisis data menggunakan IBM SPSS v20.0 dengan uji General Linear Model dilanjutkan Post Hoc Bonferroni. Hasil: Pada kelompok perlakuan 3 terdapat perbedaan bermakna dalam hal jumlah neutrofil (p<0,05) pada jam ke-4 pada perhitungan apus darah tepi. Simpulan: Ekstrak etanol daun Kesum (Polygonum minus Huds.) memiliki efek terhadap jumlah neutrofil. Background: Kesum plant (Polygonum minus Huds.) is West Borneo endemic plant that contains flavonoid believed to have anti-inflammatory activities. Objective: To investigate the effects of ethanol extract of Kesum leaves on white Wistar rat neutrophils, monocytes and lymphocytes count induced by carrageenan. Method: True experiment with complete randomized design on 30 rats divided into 5 groups. Negative control group is given CMC 0.5%; positive control is given 2.7 mg/200 mgBW diclofenac sodium; treatment-1 4.322 mg/200 mgBW; treatment-2 8.644 mg/200 mgBW, and treatment-3 17.328 mg/200 mgBW. Carrageenan 1% was induced on sub-plantar area. Blood count and differential leucocytes count were done at 0, 4, 8, and 12 for neutrophils and monocytes, 0, 12, 24, and 48 hours for lymphocytes. Data were analysed using IBM SPSS version 20.0 with General Linear Model test followed by Post Hoc Bonferroni. Results: In treatment-3 there is a significant difference (p<0.05) of neutrophyl count in fourth hour. Ethanol extracts of Kesum leaves have an effect on neutrophyl count.

Abstract

Background: Breast cancer has been identified as the most commonly diagnosed cancer in Indonesia, contributing to the highest incidence rate in 2020. The clinical characteristics of cancer patients in Indonesia may vary depending on the region, and social and cultural factors can also play a role in these differences. West Kalimantan, a province in Indonesia, is known for its diverse population comprising three significant ethnic groups: Malay, Dayak, and Chinese. This study aims to describe the characteristics of breast cancer patients in West Kalimantan.Method: This study is a descriptive study conducted on patients from 2019 to 2022. The study included 143 breast cancer patients who met the inclusion and exclusion criteria. All medical records were reviewed and analyzed to obtain the variable data; they were age, gender, origin, ethnicity, education, clinical manifestation, stage of breast cancer, tumor size, lymph node metastasis, distant metastases, lymphangioinvasion, histopathology type, and grading.Results: Breast cancer patients for the 2019 to 2022 period totaled 341 patients. Breast cancer has the highest number of cases in 2022, 148 patients. In this study, 143 patients from 2019 to 2022 met the inclusion criteria. Breast cancer was found from age 30 with a peak at the age of 44-50 years, and all patients were women. The highest percentage of patients had completed primary education (28%). Most patients came from Pontianak city (37.1%), with the most ethnic groups being Malays (73.5%). The most common clinical symptom was a painless lump (97.9). Most patients came at stage IIIb (29.4%). Pathological characteristics are tumor size T4 (42%), N1 lymph node metastasis (30.1%), 86.4% of patients had no distant metastasis. A total of 77.6% of patients had negative lymphangioinvasion. Based on histopathology, the type of invasive ductal carcinoma ranks the highest (79%) and had grade 3 breast cancer (47.6%).Conclusion: Breast cancer cases in Soedarso Hospital have increased every year. The majority of breast cancer patients at Soedarso Hospital from 2019 to 2022 were in advanced stages and the type of invasive ductal carcinoma with grade 3.

Abstract

Highlights: The e-learning readiness measurements were positive, and the level of anxiety was found to be relatively low. Many factors can affect both aspects of e-learning readiness and anxiety among medical students.   Abstract Introduction: The Coronavirus Disease (COVID-19) pandemic has had a significant impact on all sectors, including education. The change from conventional learning systems to e-learning during the pandemic could affect students' mental health, such as the emergence of anxiety. Therefore, this study focused on the relationship between e-learning readiness and anxiety levels, a topic that has not been previously studied in Indonesia. Methods: This was an observational analytic study with a cross-sectional approach. A total of 132 preclinical medical students were chosen through a stratified random sampling technique. Variables were measured using the e-learning readiness (ELR) and Zung Self-Rating Anxiety Scale (SAS) questionnaires. Bivariate analysis using Spearman's rho correlation test was performed using the International Business Machines Corporation (IBM) Statistical Package for Social Sciences (SPSS) for Windows version 24.0, with a p<0.05 considered statistically significant. Results: The results showed that the sample was ready to run e-learning but needed minor improvement (55.3%) and had low anxiety levels (87.1%). Spearman's rho correlation test showed no significance, with p=0.545 (p>0.05; r=-0.053). Conclusion: There was no statistically significant relationship between e-learning readiness and anxiety levels of preclinical medical students during the COVID-19 pandemic. These results are crucial for evaluating the ongoing implementation of e-learning as a form of innovation in medical education, even after the COVID-19 pandemic has ended.

Abstract

Menurut pemeringkatan secara global di dunia, kanker kolorektal paling sering dijumpai pada laki-laki di urutan keempat dan pada wanita di urutan ketiga. Beberapa faktor klinikopatologik diketahui berperan sebagai indikator prognosis kanker ini, di antaranya usia, jenis kelamin, dan derajat diferensiasi histologis. Penelitian ini mengevaluasi hubungan antara usia dan jenis kelamin dengan derajat diferensiasi kanker kolorektal berdasarkan data dari Laboratorium Patologi Anatomi RSUD Dr. Soedarso. Desain penelitian menggunakan pendekatan observasional analitik dengan metode potong lintang. Data diperoleh dari 207 pasien yang dipilih melalui teknik total sampling dan analisis dengan uji Chi Square. Hasil penelitian menunjukkan nilai p untuk hubungan usia dengan derajat diferensiasi sebesar 0,124 dan hubungan jenis kelamin dengan derajat diferensiasi sebesar 0,366. Dengan demikian, tidak terdapat hubungan bermakna antara usia maupun jenis kelamin dengan derajat diferensiasi kanker kolorektal pada laboratorium tersebut.

Abstract

AKI tercatat sebesar 305 per 100.000 kelahiran hidup. Sedangkan AKB sebesar 40 per 1.000 kelahiran. Pada tingkat layanan kesehatan primer, beberapa layanan fasilitas kesehatan, termasuk kesehatan ibu dan anak, tercatat terhenti selama pandemi COVID-19. Penelitian ini bertujuan untuk mengetahui hubungan persepsi kualitas pelayanan antenatal dengan kepuasan ibu hamil pada masa pandemi COVID-19. Penelitian ini menggunakan metode observasional analitik melalui pendekatan cross-sectional. Populasi penelitian yaitu seluruh ibu hamil yang memeriksakan kehamilannya di Puskesmas Gang Sehat Kota Pontianak pada tahun 2021. Sampel dipilih dari ibu yang melakukan kunjungan ANC dari bulan Agustus hingga Desember 2021 dengan sampel 90 orang yang diambil dengan teknik konsekutif. Analisis bivariat menggunakan uji Chi-Square. Hasil analisis menghasilkan nilai p-value untuk dimensi persepsi kualitas pelayanan dengan kepuasan p<0,05. Terdapat hubungan yang signifikan antara persepsi kualitas pelayanan keandalan, daya tanggap, bukti nyata, jaminan, dan empati dengan kepuasan ibu hamil pada masa pandemi COVID-19 di Puskesmas Gang Sehat Kota Pontianak.

Abstract

Latar Belakang: Kanker kolorektal termasuk dalam keganasan terbanyak di dunia. Berbagai faktor turut mempengaruhi prognosis dari kanker salah satunya sistem imun. Sistem imun di sekitar tumor memiliki pengaruh terhadap progesivitas sel kanker. Salah satu sistem imun yang dapat diamati adalah Tumor-infiltrating lymphocytes (TIL). Tujuan: Penelitian ini bertujuan untuk membuktikan adanya hubungan antara TIL dengan berbagai parameter klinikopatologi pada kanker kolorektal. Metode : Penelitian ini menggunakan desain potong lintang. Subjek penelitian berupa 38 sampel blok parafin dan slide hematoksilin-eosin (HE) serta data klinikopatologi yang berasal dari sediaan operasi kanker kolorektal pada Laboratorium Patologi Anatomi RSUD Dr. Soedarso periode Januari – Desember 2020. Penilaian jumlah TIL dibagi menjadi tiga kelompok yaitu ringan (0-10%), sedang (11-59%), dan berat (60-100%). Hasil: Hasil penelitian menunjukkan bahwa jumlah TIL terbanyak adalah TIL berat yaitu sebanyak 16 kasus (42,1%). Hasil uji korelasi Spearman menunjukkan hubungan antara jumlah TIL dengan derajat diferensiasi, stadium Dukes dan stadium p TNM dengan nilai p<0,005. Tidak terdapat hubungan antara jumlah TIL dengan ukuran tumor, status metastasis kelenjar getah bening, status metastasis jauh, dan lokasi tumor. Kesimpulan: Terdapat hubungan antara jumlah TIL dengan derajat keganasan, stadium Dukes dan stadium pTNM yang merupakan faktor-faktor prognosis pada kanker kolorektal.
