Abstract

Background: Ventilator-associated Pneumonia (VAP) is a pneumonia infection after 48 hours of mechanical ventilation. According to WHO in 2018, the incidence of VAP in mechanically ventilated patients is around 22.8% in the world and accounts for 86% of nosocomial infections with the global incidence of VAP increasing between 9-27%. Even though it is standardized, the number of nurses compliance with ventilator bundles is low. Aim: to determine the factors that increase nurse compliance in implementing the ventilator bundle. Method: The research was conducted in June-September 2024. The research used a literature review spanning the years 2020-2024. The search used the PubMed and Google Scholar databases with the PRISMA search flow. Results: the increase in nurse compliance in implementing the ventilator bundle had an average of 22.56%. Some of the research locations studied were in various hospitals around the world, such as Brazil, Hungary, Argentina and India. The dominant research design or design in the study is cross-sectional. In the study, it was found that nurses' knowledge and education based on a combination of gamification, debriefing and simulation methods related to the ventilator bundle were internal factors. Apart from that, monitoring the ventilator bundle checklist is an environmental characteristic of external factors. Conclusion: internal factors that increase nurse compliance are ventilator bundle knowledge and education, while external factors are environmental characteristics in the form of a ventilator bundle checklist, so they need to be implemented to achieve compliance according to standards.

Abstract

Anemia affects 32% of Indonesian teenagers, which means that 3–4 out of 10 of them have the condition. Adolescent anemia will develop into pregnant anemia if it is not treated, which will affect the results for the mother and the newborn and could raise the chance of stunting. Before iron is distributed and stored as reserves (ferritin), hepcidin is a crucial hormone that controls systemic iron homeostasis. A test of hepcidin can distinguish between iron deficiency anemia and anemia of chronic disease. The purpose of this study is to examine the variations in hepcidin levels between teenage girls who are anemic and those who are not. This study uses a cross-sectional comparative approach and is of the observational analytical kind. Female students in their late teens attending Baiturrahmah University in Padang made up the study's sample. Requirements for inclusion include not having a history of diabetes mellitus, kidney illness, hemoglobinopathy, or smoking. Using a sequential sampling procedure, 36 individuals were selected, 18 of whom had anemia and the remaining 18 of whom did not. Hepcidin levels are derived from the outcomes of the ELISA method used to measure the serum hepcidin-25. The statistical test used is the Mann-Whitney test because the data is not normally distributed. The results showed that the average hepcidin level in the anemia group was lower than the hepcidin level in the non-anemia group (0,43±0,1 ng/ml vs 0,46±0,09 ng/ml. Statistical tests showed that there was no significant difference between hepcidin levels in the anemia group and the non-anemia group (p=0,43). The conclusion was that there was no significant relationship between hepcidin levels and anemia in adolescents. It is recommended that further research be conducted on hepcidin and other biomarkers that affect the action of hepcidin and the need to consider hepcidin testing in diagnosing anemia.

Abstract

Background: The field of dentistry prioritises and pays attention to patient safety because unsafe health services in the dental field result in an increase in the potential for morbidity and mortality as well as a financial burden on the health service system. In connection with health services in hospitals, errors have occurred due to the characteristics of service errors in dental and oral hospitals. Hospital staff themselves often need to correct things, including mistakes in discipline, communication, and technical errors. Patient safety is the concept of patients who are in health services so that they can achieve the expected impact. Objective: To measure the characteristics of service errors with patient safety policies at the Dental and Oral Hospital of Muhammadiyah University Semarang. Methods: This research is a quantitative descriptive study with a cross-sectional research design. A total of 68 samples were taken from dentists, young dentists and staff. Univariate analysis was assessed based on frequency distribution. Results: The highest positive response results were in the dimensions of the assessment of patient safety policies and services 95.59% were included in the very good/strong category, which means that the level of patient safety policies and benefits at RSGM Unimus was already supportive and in the assessment of internal cooperation (in units) had the lowest positive response, namely 71.64%, can be categorized as moderate so that it still needs to be improved in internal collaboration at RSGM Unimus. Conclusion: Overall, the characteristics of service errors and patient safety policies at the Muhammadiyah University Semarang Dental and Oral Hospital are good, especially in the assessment dimension of patient and service safety policies.

Abstract

Electronic prescribing systems are considered capable of facilitating the communication process related to pharmaceutical administration and services, as well as helping to translate prescriptions from doctors with pharmacists, patients, and other related medical personnel, to reduce the chances of medication errors occurring at the prescribing stage. This research article was conducted to determine the effect between the use of an electronic prescription system and the incidence of medication errors in the hospital. The research method used was a Systematic review study design conducted from May to June 2023. The Systematic review was based on data sources ScienceDirect, DOAJ, and Google Scholar with the keywords “electronic prescriptions” AND “medication errors” AND “patient safety”. From the extraction based on the inclusion and exclusion criteria, 9 selected articles were obtained. The results of the Systematic review showed that there was a positive effect of the use of electronic prescriptions on the incidence of medication errors in the hospital. So it can be concluded that the use of electronic prescriptions can reduce the incidence of medication errors in hospitals and improve the quality of health services.

Abstract

The Healthy Indonesia Program with a Family Approach (PIS-PK) integrates program implementation through an approach of 6 main components in strengthening the health system (Six building blocks), namely health service efforts, availability of health workers, health information systems, access to the availability of essential medicines, financing and government leadership. Indonesia Health Program with Family Approach (PIS-PK) is a program that focuses on strengthening basic quality health through a family approach. However, its implementation is not easy because it requires strong motivation and adequate infrastructure. The purpose of the research was to determine the integration of the PIS-PK program in the management of Sidomulyo Community Health Center, Pekanbaru City. Method: The research used a qualitative research design with phenomenological approach. Participants consist of 7 people with interviews conducted since August 2023. Results of the research planned input stage P1 were limited because of a lack of human resources, inconsistent data input and entry and unclear sources of funding plans, difficult process for implementation of P2 intersectoral such as adjusting schedules with stakeholders and meeting target families because they were rarely at home and P3 monitoring, control and assessment was limited infrastructure and facilities such as a small number of computers, frequent interruptions of applications, and inadequate interventions. It is hoped that the health community centre to add human resources, and infrastructure and establish partnerships to complete the PIS-PK roadmap.

Abstract

In Indonesia, 29.9% of stunting cases are undiagnosed, and this figure increases to 70% worldwide. Babies can benefit from various forms of stimulation, including massage. This study aimed to evaluate the efficacy of video tutorials on infant massage compared to traditional methods in teaching appropriate care to new mothers. This research uses a quasi-experimental design with a quantitative approach. The design uses two treatment groups instead of a control group. In each intervention group, the research sample consisted of 33 mothers whose babies were aged between 1 and 12 months. The sampling process uses probability sampling. To analyze data using univariate methods, we used the Wilcoxon test to determine differences in knowledge and skills and to determine differences in intervention effectiveness, we used the Mann-Whitney test. The mother's level of knowledge before and after the intervention was shown by the results of the Wilcoxon test. The skills acquired as well as the knowledge and skills of the video group showed a p-value of 0.000 < 0.05 after the video intervention. Therefore, in the group that underwent the demonstration, the p-value for knowledge was 0.002 < 0.05 and for skills was 0.000 < 0.05. This means that the results of knowledge and skills in these groups are different. Neither the demonstration method nor the baby massage video method differed significantly in effectiveness in terms of maternal education, according to the Mann-Whitney test (P value = 0.201). The skills test results for both interventions were statistically significant (P = 0.000), indicating that the video education method had a different impact on mothers' skills compared to the demonstration method. The impact of the demonstration is that the mother's knowledge increases and the mother can provide baby massage correctly.

Abstract

The change of status of 50 Puskesmas to BLUD in Jember Regency, where each Puskesmas has a different background, some being internally strong and others weak, but on the other hand, there are standard requirements that must be achieved as a consequence of the Puskesmas' status change to BLUD, thus requiring strategies from the Puskesmas to achieve this goal. The purpose of this research is to analyze the strategy of UPTD Puskesmas with BLUD status in Jember Regency using the SPACE matrix approach. This research is a descriptive quantitative study. Data processing and analysis were conducted using quantitative analysis, with data collection done cross-sectionally. The sample size of 4 Puskesmas was selected using purposive sampling. Data collection techniques included questionnaires, validated through focus group discussions (FGD), and partly using existing secondary data at the Puskesmas. The analysis of the SPACE matrix shows that Puskesmas with weak internal strength are in the competitive quadrant, while Puskesmas with strong internal strength are in the aggressive quadrant. The organizational effectiveness, measured by aspects of financial performance and service delivery, shows good indicators. These results are influenced by each Puskesmas' strategy, thus requiring the implementation of clear, measurable, and achievable strategies according to an organization's capabilities

Abstract

Community health centers are pivotal in delivering healthcare services, and it is crucial for them to continually enhance employee performance. The maximum employee headcount of Teluk Lingga Community Health Centre is reflected in the 2021 Community Satisfaction Index (CSI) results as follows: 81.63% in 2021, 81.69% in July 2022, and 81.61% in January 2023. It has yet to attain 100%. This study aims to investigate the factors influencing employee performance. The independent variables identified include organizational climate, work discipline, reward system, employee competence, and physical environment, while the dependent variable is defined as employee performance. An analytical research design employing a cross-sectional approach was utilised. The sample for this study comprises a total of 69 employees from Community health centers in Teluk Lingga. The data analysis used is a chi-square test and multiple logistic regression method. The results of the study are that the influential factors are organisational climate, work discipline, reward system, and employee abilities, which are not related to the physical environment. It is recommended that the provision of appropriate monetary and non-monetary rewards enhances employee performance. Leaders and management should be encouraged to ensure equitable rewards for all employees.

Abstract

Puskesmas Dlingo has initiated Basic Occupational Health Services (BOHS) in the Mangunan forest tourism village since 2019. To develop the BOHS, the program manager of the BOHS at the health centre is to carry out mentoring functions during the initiation of formation, coaching tasks after the UKK post is formed, and monitor and evaluate the implementation of the UKK post. Since its establishment, this UKK post has never been assessed for its activity. The purpose of this study was to assess BOHS in Mangunan forest tourism. This study is qualitative research with a case study approach. The informants consisted of nine people: the BOHS program manager and the head of the Puskesmas Dlingo, workers/cadres, the BOHS supervisor from the Bantul Regency Health Office, and the tourism manager.  All informants were selected purposively. Data were collected using in-depth interview methods and analyzed thematically. The components to assess are the number of cadres, integrated health service activities, promotive and preventive activities, BOHS facilities, recording and reporting, and revolving funds. The study found that all components indicate the BOHS is currently less active and tends to be inactive. Puskesmas policies are needed through budgets, necessary training in occupational health empowerment, and networking.

Abstract

Convergence is an approach to delivering interventions that are coordinated, integrated, and jointly delivered to prevent stunting for priority targets. The implementation of stunting convergence in Rokan Hulu Regency began in 2018. This study aims to analyze the implementation of the stunting convergence program in Rokan Hulu Regency in 2021. This is a qualitative study with a rapid assessment procedure research design.The research informants consisted of the main informants, namely from Bappeda, the Health Office, the Head of the Stunting Locus Health Center, and the Stunting Locus Village Head. Supporting informants are DP3AKB, Department of Public Works, Department of Education, Department of Food Security, and Department of Fisheries and Marine Affairs. Data analysis was carried out by transcribing, analyzing, and forming a matrix of research results, then triangulating the data. The results showed that the human resources in charge of monitoring nutrition at the Stunting Locus Health Center were not all trained. The budget for stunting convergence was from the APBD and the APBN and was considered to have been right on target and in accordance with the budgeting needs given. Facilities and infrastructure tend to be inadequate to implement the stunting convergence program. In the implementation of 8 stunting convergence actions, it was found that every action had been carried out, even though there were constraints in human resources, internet network availability, data availability, and suitability. It was recommended to the Rokan Hulu Government to increase the capacity of human resource managers, complete facilities and infrastructure, and improve coordination between managers of the stunting convergence program.
