Abstract

Immunization programs include efforts to reduce morbidity, mortality and mortality rates in infants and toddlers. This program is carried out for diseases that can be prevented by immunization such as tuberculosis, diphtheria, tetanus, hepatitis B, polio, measles, rubella, inflammation of the lining of the brain and pneumonia. Based on the Health Profile of the Sungai Salak Health Center in 2021, the achievement of complete basic immunization is 13.4% of the target of 95%. The purpose of the study was to analyze the Implementation of the Complete Basic Immunization Program (IDL) in the Working Area of the Sungai Salak Health Center, Indragiri Hilir Regency. This type of research is qualitative, descriptive, with a case study design. The research location is in the Working Area of the Sungai Salak Health Center, Indragiri Hilir Regency. The research time is August 2023. There were 6 informants for this research, consisting of 1 Main Informant, 2 Key, and 3 Supporting Informants. Data collection was conducted through in-depth interviews, observations and document review. Data validity is done by triangulation of sources, methods, and data. The results of this study found that human resources were still lacking because there were no logistics management personnel, the distance to immunization services was difficult, the low knowledge of mothers and families on the importance of immunization, and there was no monitoring so that the reporting of Complete Basic Immunization coverage was gaped. It is expected that the Head of the Puskesmas will form an immunization program energy team, in collaboration with Cross Sectoral to run the My Home My Vilage (MYMV) Program. Keywords: Complete Basic Immunization, Sungai Salak Health Center, and Immunization Program

Abstract

The World Health Organization (WHO) 2021 stated that tuberculosis is still one of the main world health problems where tuberculosis is the 13th leading cause of death in the world and in 2020 tuberculosis became the infectious disease that caused the most deaths in the world after COVID-19. The National Strategy for Tuberculosis Prevention in Indonesia 2020-2024 explained that the scope of finding tuberculosis patients has not reached the target. Based on the Health Profile of the Salak Health Center in 2021, the achievement of the Pulmonary Tuberculosis program (Incidince Rate) was 21 suspected patients or 10.6% of the target of 198 suspected patients. The purpose of the study was to analyze the low number of passive and active case finding, and analyze community empowerment towards the implementation of the Pulmonary Tuberculosis Program in the Working Area of the Sungai Salak Community Health Center. This type of research is qualitative, descriptive, with a case study design. The research location is in the Working Area of the Sungai Salak community health center, Indragiri Hilir Regency. The research period is January to August 2023. In-depth interview research informants there were 5 people consisting of 3 main informants (tuberculosis Program Holders at Puskesmas, Analysts, Doctors) and 2 Supporting In-Informants (Head of Puskesmas and Tuberculosis Program Holders at the Health Office), while the Focus Group Discussion informants there were 6 people consisting of village heads, RT leaders, RW leaders, community leaders, PMOs, tuberculosis cadres. The form and flow of passive examination of tuberculosis patients are in accordance with the SOP. The obstacle is that sometimes patients who come for treatment to the puskesmas find it difficult to remove their sputum at that time. Active case discovery has not been in accordance with Standard Operating Procedures (Screening and Household Contact of tuberculosis patients) because nothing has been done. It is expected that the role of tuberculosis officers can ensure that tuberculosis cases are reported regularly through the tuberculosis program information system. Keywords : Community Empowerment, Passive Case Discovery, Active Case Discovery, Sungai Salak Community Health Center, and Tuberculosis Program

Abstract

This research aims to analyze the achievement of Universal Health Coverage (UHC) from the perspective of the Policy Analysis Triangle by examining the interconnection of actors, content, context, and process. The study, conducted in the Pelalawan Regency from June to August 2023, employs qualitative research methods with 5 primary informants and 3 supporting informants. The findings reveal that each informant's role aligns with their respective fields, indicating sufficient budgetary allocations. However, not all patients receive counseling about JKN. Regarding content, JKN covers all promotive, preventive, curative, and rehabilitative services. Differences exist in the services provided to general patients and JKN participants, with non-cut-off services functioning well. In the context aspect, all community members are obligated to be covered by JKN, and local governments can budget for JKN financing in accordance with Presidential Regulation number 83 of 2018. Both infectious and non-communicable diseases are covered by JKN, and there is no service discrimination at the first-level health facilities. Regarding the process, the UHC planning initiated in March 2022 was implemented by October 2022. Key drivers for UHC include the integration of (Integrated Social Welfare Data ) DTKS data into JKN and continuous socialization and advocacy efforts.
