Abstract

Working in confined spaces has high risks, such as inhaling hazardous substances, trapped, falling, falling objects, fires, and explosions. One work sector that uses a lot of confined spaces in its production process is the Steam Power Plant. In Indonesia, Paiton Steam Power Plant is one of the largest steam power plants, supplying electricity to the islands of Java and Bali. To reduce the risk of accident in confined spaces, the Indonesia government, through the Ministry of Manpower, has established the latest regulations on confined spaces, which must be implemented in all companies that use confined spaces. This research aims to evaluate the current procedure and regulation about confined spaces in one of the companies that operated Paiton Steam Power Plant with the latest regulations by the Indonesia Ministry of Manpower. By using descriptive qualitative with literature study instrument on documents, observations, and interviews with safety specialist and safety officers. The result is within 56 checklist point evaluation based on Indonesia Regulation, there are 7 points minor inconsistencies, and 1 point is major inconsistencies. The company has implemented the regulation really well and just need to do some adjustment in the future.

Abstract

The intention to quit smoking among adolescents plays a crucial role in the success of smoking cessation behavior. According to the Theory of Planned Behavior (TPB), this intention is influenced by three main domains: attitude, subjective norm, and perceived behavioral control. This study aims to describe the intention to quit smoking among male students at SMAN 4 Kota Kupang. A quantitative descriptive design was used, with a total sampling technique involving 46 respondents. The findings showed that 80.4% were classified as light smokers; 39.1% reported a desire to smoke after meals; 67.4% started smoking after the age of 13; 37% smoked in private spaces; and 50% indicated curiosity as the primary reason for smoking. Based on univariate analysis, 63% of respondents had a strong intention to quit smoking; 87% showed a positive attitude toward quitting due to perceived health, economic, and social benefits; 67.4% reported a positive subjective norm supported by family encouragement and social expectations; and 52.2% had strong perceived behavioral control, indicating their ability to refrain from smoking in stressful situations.

Abstract

Anesthesia services are an important component of medical services in surgical procedures and other invasive procedures that aim to reduce or eliminate pain during the procedure. The success of anesthesia is not only triggered by the anesthetic technique used, but the patient's clinical condition also has an influence. Specialist anesthetists and anesthetists play an important role in determining patient suitability, adjusting the appropriate anesthesia technique, and monitoring during and after the procedure. Hypertension is a chronic disease that requires special attention in anesthesia services because of a higher risk of blood pressure fluctuations, organ perfusion disorders, and cardiovascular events such as stroke and myocardial infarction. This research aims to examine the anesthesia service policy at Pindad Turen Hospital and analyze the obstacles experienced by medical personnel in its implementation. The results of this study show that there is a main problem in implementing anesthesia service policies for hypertensive patients in hospitals, namely that there is no standardization of policies or SOPs related to determining the suitability of hypertensive patients to undergo anesthesia and there are differences in perception between specialist anesthetists and anesthetists, especially regarding safe blood pressure limits and recording the results of pre-anesthesia evaluations. From the problems found, a situation analysis was carried out using fishbone, USG, and SWOT diagrams, and priority problems were found, namely the absence of appropriate blood pressure standards for hypertensive patients and the absence of a special SOP for anesthesia services for hypertensive patients. These problems have the potential to affect patient safety during anesthesia. The intervention was carried out through outreach and training to health workers and it was proven to increase knowledge and uniformity of perception of medical staff in determining the suitability of hypertensive patients as evidenced by an increase in evaluation results from 2.8 (pretest) to 5 (posttest).

Abstract

Early Warning Scoring (EWS) is a clinical instrument designed to detect early deterioration in a patient's medical condition by monitoring vital signs such as pulse, blood pressure, and respiratory frequency. This early detection allows health workers to carry out medical interventions quickly and precisely, thereby increasing the chances of patient safety. However, the implementation of EWS at Pindad Turen Hospital is still not running optimally. The main problem found was the lack of integration in the assessment and recording of EWS scores by health workers, which resulted in delays in clinical decision-making. This study aims to analyze the level of EWS utilization in the hospital through outreach and training activities. Before the training, a pretest was carried out, and after the training, a posttest was carried out to measure the increase in knowledge of health workers. The results show that there is no uniform standard for writing EWS scores. As a form of intervention, the supervision function is strengthened as cues to action whose role is to encourage and guide health workers in the correct implementation of EWS. Through the active involvement of supervision, it is hoped that there will be increased consistency and accuracy in the assessment and recording of EWS scores by nurses in each service unit.

Abstract

Enhancing primary health care quality is crucial, particularly when it comes to managing chronic conditions like diabetes mellitus (DM). The purpose of this study is to investigate how patients with diabetes mellitus at the Kartasura Health Center perceive the quality of their services and how satisfied they are with such services. With a cross-sectional design and a quantitative methodology, this study included 50 Prolanis DM subjects chosen by total sampling. The research instrument refers to the five dimensions of SERVQUAL and KKP-2017, which have been tested for validity and reliability. Data were analyzed univariate and bivariate using chi-square tests. The results show that most patients had a good perception of the quality of service, as well as a high level of satisfaction (90%). All aspects of service quality, including tangibles, assurance, responsiveness, empathy, and reliability, were found to be significantly correlated with patient satisfaction (p<0.05). These findings suggest that the better the perception of service quality, the higher the patient satisfaction. In conclusion, the optimal dimension of service quality, both from physical and interpersonal aspects, plays an important role in increasing DM patient satisfaction. Therefore, improving the quality of patient experience-based services needs to be a strategic focus in primary health services.

Abstract

One of the chronic diseases whose incidence rate continues to increase, especially in Sukoharjo Regency, which shows a high number of cases at the Kartasura Health Center is Diabetes Mellitus (DM). This disease requires optimal attention and long-term treatment, where the quality of health services plays an important role in its success. This study was conducted to analyze the relationship between service quality and the trust of patients with diabetes mellitus at the Kartasura Health Center. The method applied in this study was a quantitative approach with a cross-sectional study design, using a total sampling technique, where data were collected through questionnaires distributed to 50 diabetes mellitus prolanis patients who attended the Puskesmas in three consecutive months. The results of the analysis showed a significant positive relationship between service quality and patient trust with a moderate level of relationship strength (p-value0.044; r=0.287), indicating that the better the quality of services provided, the higher the level of patient trust in the health facility. This study confirms the importance of improving the quality of health center services to build patient trust which in turn can support patient adherence to recommended care and treatment.
