Abstract

Gel handsanitizer wich is sell in the market today is mostly alcohol-based. Repeated useas a handsanitizer can cause dryness and irritation skin. An option is, we can substitute an alcoholinto lemongrass oil (Cymbopogon nardus). There are three steps of the test, a preliminary test of oilconcentration, a test of the physical properties of the gel preparation and a modifi ed replication test.Based on the preliminary test, citronella essential oil can kill Escherichia coli bacteria at a concentrationof 5% and reduce Staphylococus aureus at 10% of concentration. The test of spread for gel in thephysical test is between 5.5-6 cm with pH 7-9 and the adhesion is 1 second.The modifi ed replica testshowed a signifi cant decrease in the number of bacterial colonies at each concentration formula ofthe three test groups. This signifi cance indicates the inhibition of bacteria caused by the use of handsanitizing gels. Based on the organoleptic test and antibacterial test of hand sanitizer gel a chosenconcentration is 10%. This concenrtration was liked by the public and could inhibit bacterial growth.

Abstract

The World Health Organization declares Diabetes Mellitus and Hypertension as one of the health emergencies. Adherence to medication is an important factor in controlling this disease. Research is needed to find out the combination of counseling and treatment reminder aids will aff ect adherence and clinical outcomes. Pretest-posttest design was conducted in February 2019 - May 2019. Sample population was Prolanis patients at Puskesmas Kembaran I, Purwokerto Timur II and Sumbang I. The inclusion criteria were 66 patients with type 2 diabetes and 72 patients with hypertension. The determination of the pretest-posttest group was by simple random sampling. The adherence instrument used MARS, counseling and medication reminder tools according to pharmaceutical counseling guidelines. The respondents were women, low education, normal BMI, didn’t work, combination of Diabetes medication and single medication for Hypertension. The pretest group was categorized as non-adherent, but experienced an increase after the intervention. The interventions given also had an eff ect on improving medication adherence and controlling the clinical outcome of type 2 DM patients and hypertensive patients at the Puskesmas (p-value 0,000 <0.05). The combination of pharmacist counseling interventions with medication reminders is more eff ective in increasing medication adherence in patients with type 2 Diabetes mellitus and Hypertension.

Abstract

Malaria infection is still become a serious and complex health problem in the world. The diffi culty in treating malaria is caused by the resistance of the malaria parasite to synthetic drugs. One alternative to prevent resistance is to use herbal medicines such as the Ashitaba plant (Angelica keiskei [Miq.] Koidz). This study aims to determine the activity of Ashitaba stem ethanol extract as an antimalarial to the Plasmodium falciparum parasite strain 3D7. Ashitaba stem extract was tested for chemical compound content and antimalarial activity against Plasmodium falciparum strain 3D7 with concentrations of 100, 10, 1, 0.1 and 0.01 μg / ml. The test results for the chemical compounds of Ashitaba stem extract were positive for fl avonoids and chalcone compounds. Ashitaba stem extract with a concentration of 100, 10, 1, 0.1 and 0.01 μg / ml had an average resistance value of 67.75%, 46.61%, 32.56%, 19.60%, and 7.56%. The antimalarial activity test results of Ashitaba stem ethanol extract had an IC50 value of 11.07 μg / ml. The antimalarial activity of Ashitaba stem ethanol extract is included in the category of good antimalarial activity because the IC50 value falls within the range of 10-50 μg / ml.

Abstract

Acne is an inflammatory condition that was caused by Propionibacterium acnes. Sunfl owerseeds (Helianthus annuus L.) with terpenoids and linoleic acid could prevent and treat acne. In theprevious study, sunfl ower seed oil had a Minimum Inhibitory Concentration (MIC) value for P. acnesbacteria ≥1.5% with a Diameter Inhibitory Zone 15.27 mm which was classifi ed as a strong antibacterialagent. This study was intended to develope cream formula (F1-F4) using variations of cetyl alcoholconcentrations of 8, 10, 12% and blanko respectively. The creams were formulated by heating the waterphase and the oil phase separately at 70˚C, then stirred to form the cream. The prepared formulations wereevaluated for organoleptic, homogeneity, type of cream, viscosity and rheology, spread ability, globulesize, centrifugation, and pH. The organoleptic results showed that the cream was white, odorless, softin texture and homogeneous with the type of oil in water (O/W) cream. The viscosity test showed thatthe viscosity of F1-F3 formulas were 490.29; 3886.29; 22510.67 cP and F4 of 111.04 cP with plasticthixotropic fl ow properties. Formula F1-F4 had a spread ability of 8.9; 5,4; 4,4; 7.3 cm2, the globulesizes were 137.78; 133,33; 151.11; 94.44 μm, did not show phase separation during centrifugation andthe pH of the F1-F3 formula were 6.3 and F4 was 5.93. It may be concluded that the evaluation of thephysical and chemical quality of sunfl ower seed oil cream are within the acceptable standard.

Abstract

Community-acquired pneumonia is an acute inflammation of the pulmonary parenchyma caused by bacterial infection. . The main therapy for community-acquired pneumonia is the use of antibiotics. Inappropriate antibiotics can lead to bacterial resistance. The study aims to determine Appropriateness of antibiotics prescribing using the method Gyssens with clinical improvement. This study is a retrospective analytic cohort descriptive. Data was collected retrospectively based on medical records of inpatients at one of the hospital in Yogyakarta for the January-December 2019 period based. The accuracy of empiric antibiotic prescription using the Gyssens method. The clinical improvements observed were leukocyte count, temperature, respiratory rate. This study was analyzed using the chi square method. The results showed 41 patients met the inclusion and exclusion criteria, male gender (51.2%), age> 60 years 68.3%, the average length of stay was five days. Ceftriaksone is a commonly used antibiotic. Evaluation prescribing appropriateness based on the Gyssens method of 52 antibiotic regimens. appropriate antibiotic prescribing (13.5%) and inappropriate (86.5%). The correlation between appropriateness of antibiotics prescribing with clinical improvement leukocytes count, respiratory rate and temperature did not significant (p> 0.05). There was no correlation between the accurasy of antibiotics prescribing based on the Gyssens method with clinical improvement.

Abstract

Health services provided to the public in the implementation of the National Health Insurance (NHI) includepromotional, preventive, curative and rehabilitative. In supporting the implementation of NHI, it is necessaryto make eff orts to ensure the availability, aff ordability and accessibility of medicines through the application of the National Formulary (Fornas) as a reference in health services in all health facilities. The purpose of this study was to determine the percentage of prescription drugs in accordance with the National Formulary at Sawah Besar Hospital in 2018. The research design descriptive analysis and data collection was carried out retrospectively in the form of prescribing drugs and from the Hospital Management Information System and Patient Medical Record data. This research was conducted with a sample size of around 19.690 recipes and a length of 6 months of research from July 2018 to December 2018. From the results of the study pattern of prescription writing against the suitability of the National Formulary was 71.22%. Compatibility of 75% antihyperlipidemic drugs with atorvastatin as the drug with the most non-conformity of restrictions. The conformity of antihypertension drugs 54.11% with Valsatran as one of the inconsistencies in restriction and insulin drugs, 70.03% the antihyperglycemia drug restriction inconsistencies.

Abstract

The mortality rate for Community-Acquired Pneumonia (CAP) in Indonesia is in the fourth rankand the top ten diseases each year. The main therapeutic principle of CAP is the administration of antibiotics.Improper antibiotic usage can aff ect the success of therapy. The pattern of antibiotics used in hospitals is usually based on clinical and empirical experience, not based on the germ pattern and the sensitivity of antibiotics. The study aims to describe the suitability of empiric antibiotics against the antibiogram profi le and treatment outcomes in inpatients with CAP at PKU Muhammadiyah Gamping Hospital. This study was conducted under the descriptive-analytic observational study design with a retrospective cohort design where the data source came from secondary data, namely the medical records of community pneumonia patients in 2019 and hospital antibiogram data. The data were collected by using the total sampling method. There were 79 community pneumonia patients. The results of the empirical antibiotic suitability test results on antibiograms and clinical outcomes showed that there was no signifi cant relationship between the suitability of empirical antibiotics to the clinical outcome of leukocytes (p = 0.550; RR = 0.725; 95% CI = 0.252-2.086), clinical outcome temperature (p= 0.545; RR = 0.576; 95% CI = 0.049-6.747) and clinical outcome length of stay (p = 0.631; RR = 0.767; CI95= 0.258-2.275). The conclutions there was no signifi cant relationship between the appropriateness of empirical using of antibiotics and antibiogram profi le include leukocyte, temperature and length of stay of the patients (p>0.05).

Abstract

Hyperuricemia is a metabolic disorder that is often found in the community, and requires long-termtreatment. To determine the effi cacy and safety of the hyperuricemia herbs, clinical studies have been carried out on the effi cacy and safety of tablet preparations compared to boiled of the hyperuricemia herbs. The clinical study was conducted with an open label research design, randomized clinical study and parallel design. The study involved 55 subjects who had met the inclusion and exclusion criteria. Subjects were randomized and divided into groups of tablets or boiled with intervention for 28 days. The tablet group took tablets with a dose of 3x2 tablets per day and the boiled group drank infusion with a dose of 3 times a glass per day. Blood uric acid was examined at D0, D14 and D28. SGOT, SGPT, urea, and creatinine were examined at D0 and D28. The tablet reduced uric acid (7.74 mg/dL to 6.53 mg/dL) and the boiled reduced uric acid (7.93 mg/dL to 7.09 mg/dL). The administration of tablet preparations eliminates the clinical symptoms of swollen/pain of leg, swollen/pain of hand, stiff ness, pain and other symptoms at the deff erent time than boiled administration. The administration of both preparations for 28 days in the subjects did not increase the value of SGOT, SGPT, urea and creatinine and no symptoms of serious side eff ects were found.

Abstract

Pharmacy is a facility for pharmaceutical services conducted by pharmacists. One of pharmaceuticalservices performed at a pharmacy is swamedication services. The purpose of this study was to determine thelevel of satisfaction of swamedication patients in drug information services in several pharmacies in the EastSurabaya Coastal Area, based on fi ve dimensions of service quality, namely reliability, responsiveness, assurance, empathy and tangibles. This cross sectional study using a validated questionnaire instrument. The research sample is swamedication patients who meet the inclusion criteria. Swamedication patients were selected nonrandomly by consecutive sampling method. Data retrieval was carried out in October-December 2018. Data from the fi ve dimensions of service quality were analyzed by the SERVQUAL method. There were 100 patients who performed swamedication services that met the inclusion criteria. The results showed that the average gap between expectation and performance is -0.36 with the largest gap value indicated on the reliability dimension of -0.51. At overall satisfaction level, the results are 89%. The conclusion is that the level of expectations of swamedication patients is higher than the level of performance of drug information services provided by the pharmacy with a good level of patient satisfaction.

Abstract

Fluconazole (FLU), an oral antifungal widely used in the treatment of vaginitis andcandidiasis, is known to have low bioavailability due to its low solubility. The purpose of this studywas to prepare and characterize co-crystal fl uconazole-resoscinol (FLU-RES). The preparation ofco-crystal was performed by grinding together the equimolar mixture of FLU-RES which is drippedwith a few ethanol. Powder X-ray diff raction, diff erential scanning calorimetry (DSC), and polarizedmicroscopy methods were performed to characterize the formation of FLU-RES co-crystal. Relevantphysicochemical properties include solubility tests in water and dissolution tests in pH 1.2; 4.5 and6.8 buff er solution. The powder X-ray diff ractogram of FLU-RES milled result showed the presenceof new peaks and loss of the main peaks of FLU and RES. The characterization of grinding result byDSC and polarized microscopy methods also showed the co-crystal formation between FLU and RES.The solubility of FLU-RES co-crystal in water is solubility two folds more than pure FLU, while itsdissolution rate is 1.67-1.72 times faster than pure FLU.
