Abstract

Low back pain (LBP) is a widespread issue that affects many people globally, causing significant limitations in daily activities and work. LBP imposes a considerable economic burden due to healthcare costs and lost productivity. This review explores platelet-rich plasma (PRP) therapy, a promising treatment because of its ability to regenerate tissues. PRP contains growth factors that stimulate cell activity and collagen production, which can significantly reduce pain and enhance mobility and quality of life for LBP sufferers. PRP's capacity to aid in tissue repair and decrease inflammation suggests it could be beneficial for these various causes. Compelling clinical studies have demonstrated that PRP can dramatically reduce pain and disability, offering patients renewed hope and improved outcomes. Additionally, PRP therapy is minimally invasive with rare and mild side effects, underscoring its safety and efficacy. This review underscores the urgent need for further research to refine PRP treatment protocols and confirm long-term benefits. Large-scale randomized controlled trials are essential to develop standardized guidelines and ensure consistent, positive results across diverse patient populations. PRP therapy represents a revolutionary approach to managing LBP, promising to address the root causes and deliver lasting relief rather than merely masking symptoms. The future of LBP treatment could be significantly improved through the widespread adoption of PRP therapy.

Abstract

Introduction and importance Pott’s disease is an extra-pulmonary manifestation of TB described in 1779 by Percivall Pott which is the most common form of skeletal TB particularly in children. The manifestation leads to bony destruction, characterized by back pain, fever, anorexia, night sweats, weight loss, and paraparesis, in very low progression. Case presentation A 3-year-old boy with alternating walk pattern and pain in his lower leg when walking more than 100 meters and walks slightly bent over. A bony protrusion slightly palpated in thoracic area, and Mantoux test results were positive. A thorax X-ray revealed a paravertebral mass, and an MRI showed multiple level vertebral body disintegration on thoracic 7 and thoracic 8 with a paravertebral abscess in the thoracic area. The patient is receiving conservative treatment, including TB drugs, thoracolumbar orthosis, and calcium Discussion It is important to recognize early symptoms of spinal tuberculosis in children to prevent the deterioration of neurological status and kyphotic deformity. In these cases, slight alteration of walking pattern without concomitant symptoms and significant laboratory findings give us significant changes on MRI. Early diagnosis might give favoured results on conservative management Conclusions The most prevalent type of skeletal tuberculosis is Pott's disease, and incidence in youngsters varies. Majority of patients presented with anorexia, fever, night sweats, weight loss, rachialgia, and paraplegia. Early discovery of the disease's mild signs may help to stop its progression. Abstract

Background. This study aims to see the relationship between the use of N95 masks and the autonomic responses of individuals with anxiety disorders. Methods. This study is an analytic experimental study on 140 respondents. Assessment of anxiety disorders was carried out by completing the DASS 42 questionnaire and psychiatric interviews by a psychiatristt. Autonomic responses are measured by measuring heart rate variability (HRV). Short-term root mean square of successive differences (RMSSD) HRV parameters were measured using the Polar H10. HRV was measured when the respondent was not wearing a mask, when wearing an N95 mask and when wearing an N95 mask with a valve that had been opened (sham). Results. HRV in respondents with anxiety disorders had a significantly lower value than respondents without anxiety disorders. Both at baseline (median: 16.33 vs 23.35, p=0.004), when using an N95 (median: 16.47 vs 22.01, p=0.008) and when using a sham N95 (median: 16.94 vs 22.24, p=0.005). However, there was no relationship between the use of N95 masks and HRV in respondents with anxiety disorders (RMSSD baseline vs N95: median 16.33 vs 16.47, p=0.979). Conclusion. There is no relationship between the use of N95 masks and HRV in individuals with anxiety disorders. Keywords: anxiety disorders; heart rate variability; N95 mask.

Abstract

Acute cardiac Infarction (AMI) is a disorder characterised by the necrosis of cardiomyocytes resulting from a disparity between cardiac demand and oxygen delivery. The primary aetiology of IMA is thrombotic occlusion resulting from a ruptured atherosclerotic plaque. IMA is categorised into two types: STEMI and NSTEMI. In STEMI patients, entire occlusion results in increased severity, namely transmural infarction affecting all layers of the heart wall, in contrast to NSTEMI patients, who have partial occlusion. Myocardial infarction elicits an inflammatory response to eliminate necrotic cardiomyocytes and residual matrix debris, facilitating scar tissue formation and the commencement of the healing process. During inflammation, there is a change in leukocyte values characterised by an increase in neutrophil count, indicative of an initial inflammatory response, followed by a drop in lymphocyte count, suggesting a stress response, ultimately culminating in an elevated Neutrophil Lymphocyte Ratio (NLR).

Abstract

Cholelithiasis is a common condition in gastroenterology that affects 10% to 15% of adults. It is commonly known as gallstones and results from the precipitation of substances in the bile. Meanwhile, prostatic hyperplasia is an enlargement of the prostate gland that can cause various urinary tract symptoms, including bladder outlet obstruction and dysuria, which significantly affect the patient's quality of life. This case reports a 54-year-old man who was admitted to the hospital with complaints of shortness of breath and swollen legs 1 day before admission. After 2 days of hospitalization, the patient felt pain in the right abdomen. So an abdominal ultrasound examination was performed and found thickening of the wall and a picture of stones in the gallbladder and an enlarged prostate. Actions taken include a cholecystectomy surgery for gallstones in the gallbladder and observation for the development of symptoms of prostate enlargement. The patient's prognosis was good, indicating a good outcome or positive recovery.

Abstract

Nephrolithiasis, or kidney stones, is a condition that can cause severe pain and serious complications if not properly managed. This case report discusses a 64-year-old female patient presenting with a chief complaint of stabbing pain in the lower back radiating to the stomach for the past month. Ultrasonographic examination revealed bilateral kidney stones. This report aims to describe the disease progression, diagnostic evaluation, and management of a patient with bilateral nephrolithiasis. Abstract

Streptococcus suis (S. suis) is a significant zoonotic pathogen that can cause severe infections in humans, primarily manifesting as meningitis, septicemia, and endocarditis. This review focuses on the pathophysiology of S. suis meningitis, emphasizing the bacterium's mechanisms for crossing the blood-brain barrier (BBB) and eliciting a robust inflammatory response. The review highlights the virulence factors, including collagenase-like protease and capsular polysaccharides, that facilitate the pathogen's evasion of the immune system and contribute to the severity of the disease. Notably, the prevalence of S. suis infections remains a critical public health concern, particularly in regions with high pig consumption, where occupational exposure increases the risk of human disease. The manuscript also evaluates current diagnostic techniques, comparing traditional methods, such as cerebrospinal fluid culture, with advanced molecular diagnostics, including polymerase chain reaction (PCR) and next-generation sequencing (NGS). These newer methods demonstrate enhanced sensitivity and specificity, crucial for timely and accurate diagnosis. Furthermore, emerging innovations in diagnostic technology, including point-of-care testing and biosensors, hold promise for rapid identification of S. suis, potentially mitigating morbidity associated with the disease. This comprehensive review highlights the urgent need for enhanced diagnostic capabilities and public health strategies to reduce the burden of S. suis meningitis, promoting interdisciplinary collaboration to improve disease prevention and management.

Abstract

Human sperm undergo physiological and biochemical changes when present in the female reproductive tract, known as capacitation. The release of cholesterol from the plasma membrane, which increases membrane fluidity and permeability to bicarbonate and calcium ions, plasma membrane hyperpolarization, protein phosphorylation changes, protein kinase activity, increased bicarbonate (HCO3-), intracellular pH, Ca2+ concentration, and cyclic adenosine monophosphate (cAMP) levels are all biochemical changes connected with the capacitation process. Sperm capacitation can be induced by various factors, one of which is Reactive Oxygen Species (ROS). Physiologically, spermatozoa produce ROS as a byproduct of aerobic respiration metabolism. Due to cytoplasmic insufficiency, human sperm structurally have few intracellular antioxidant enzymes. Therefore, sperm are more vulnerable to their own generated ROS. Interestingly, in large quantities, ROS can cause oxidative stress that may lead sperm towards apoptosis; however, if present in adequate amounts, it can induce the capacitation signaling pathway in sperm. The main focus of this paper is to discuss the role of ROS in inducing capacitation signaling pathway in human sperm.

Abstract

Introduction: Osteoarthritis (OA) is a degenerative joint disease that causes pain, inflammation, and impairment in physical functions. Antioxidants, which neutralize free radicals, have shown potential in modulating inflammatory responses associated with OA. Vitamin E can suppress IL-1β-induced toxicity of chondrocytes, preserving cartilage integrity. However, further investigation is needed to determine their efficacy in diverse populations and conditions, such as osteoarthritis (OA). Methods: The search was conducted across five databases: PubMed, Cochrane, Epistemonikos, Scopus, and ProQuest, as well as EBSCO, up to January 2025. Eligibility criteria included data availability in a compatible language, inclusion of intervention studies, involvement of animal or human samples with knee osteoarthritis, utilization of antioxidants for efficacy outcomes, and consideration of at least the interleukin-1 beta parameter. Results: The study thoroughly evaluated 13 studies, removing duplicates and conducting an initial screening of abstracts, and assessed their demographics and clinical characteristics. Six studies were evaluated using the RoB 2.0 tool for randomized clinical trials, and seven studies were assessed using the SYRCLE tool for animal trials. All studies have a low risk of bias. Conclusion: Antioxidants are effective in treating knee osteoarthritis (OA) compared to a placebo, but dosage is crucial for optimal results. Further safety studies are needed.

Abstract

Dapsone hypersensitivity is a form of drug allergic reaction involving multiple organs. This article reports the case of a man who experienced dapsone hypersensitivity after undergoing treatment for multibacillary leprosy with the dapsone regimen. The patient was diagnosed with dapsone hypersensitivity based on the presence of a skin eruption in the form of erythroderma and involvement of various internal organs. Hypersensitivity to dapsone is potentially fatal. Limited facilities for risk factor screening become a challenge in preventing the incidence of dapsone hypersensitivity.
