Abstract

Background Placenta accrete remains a problem in developing countries. The incidence rate has increased over the last 30 years accompanied by the risk of maternal death. Bleeding is still the main cause of death. This rising incidence rate has increased Sectio Caesaria (SC) operations, resulting in greater treatment costs. This is due to its cost management as it requires intensive care and massive use of blood transfusions. This study aims to describe the characteristics, management, and maternal outcomes of placenta accrete spectrum cases. Methods A Cross-sectional study was conducted at Moewardi Hospital Surakarta between January 2016 and December 2021. Sample selection was based on medical records with a suspected diagnosis of PAS. A total of 121 cases of placenta accrete spectrum had been confirmed by anatomical pathology results were analyzed Assessments were carried out on maternal characteristics, risk factors, MAP score, intraoperative management, and postoperative care. Statistical analysis was carried out with a 95% confidence interval (CI), chi-square test, and odds ratio assessment using SPSS edition 23. Results Hysterectomy has more morbidity and maternal complications than conservative management in blood loss, massive transfusion, iatrogenic incident, and a long stay in the Intensive care unit. Conclusions The high rate of morbidity in the management of the placenta accrete spectrum requires comprehensive management to reduce maternal mortality.

Abstract

Background Most countries around the world suffered from the high-level transmission COVID-19. Thus, the government enforced a policy for the citizens to work from home. These restrictions impact the mental health of the citizens, and the prevalence of stress increases. Some of the factors that affect stress are magnesium intake and physical fitness. The study found that magnesium levels and fitness are correlated with stress levels. But some studies got different results. Thus, learning more about the relationship between magnesium level, fitness, and stress level is necessary. Methods This is a cross-sectional design study with consecutive non-random sampling methods. This study involving online drivers was held in Kampus B, Fakultas Kedokteran Universitas Trisakti, in September 2022. The inclusion criteria are aged 15-64 years, willing to sign the informed consent, can communicate well, while the exclusion criteria are in the therapy of anti-depressant or anti-anxiety drugs, and has a history of heart attack within one month. Results Based on the data, it can be concluded that most respondents are male, from the middle-aged group, and suffer from obesity, and about 50% of the respondents suffer from high blood pressure. Most respondents have normal blood-magnesium levels and medium levels of stress, while all the respondents have poor or very poor physical fitness. On the Chi-square test between age, sex, body mass index, blood pressure, physical fitness and stress level, the level of significance consecutively are 0.645; 0.208; 0.364; 0.451 and 1.000. While on the Fisher test, the significance level between blood magnesium level and stress level is 0.099. Conclusions Based on the bivariate test, it can be concluded that there is no significant relationship between age, sex, body mass index, blood pressure, physical fitness and blood magnesium level with the stress level.

Abstract

The maternal mortality rate in Indonesia is still high. According to the Indonesian Household Health Survey (SKRT) in 2001 found that bleeding  in pregnancy contributes as the main cause of maternal mortality. Antepartum hemorrhage can be caused by placental abruption (PA) and placenta previa. This bleeding condition is an emergency case because it threatens the lives of both mother and  fetus (mother-fetal dyad). Placental abruption is usually accompanied by pain due to continuous uterine contractions. In this case, although concealed hematoma exists in almost 80% of PA cases, the mother shows no sign or symptom even though the fetus is severely at risk. This condition can cause delayed management that leads to mother and fetal mortality, known as asymptomatic placental abruption.

Abstract

Background The aging process results in physiological changes in the body, such as decreased muscle strength and impaired balance, so that the risk of falls, injury and dependency increases in the elderly. Changes in the nutritional status of the elderly who tend to be predominantly underweight also occur due to the aging process and have an impact on the muscle strength of the elderly in the long term. This study aims to determine the relationship between BMI and hand grip strength in the elderly at PSTW Sabai Nan Aluih Sicincin. Methods This study is an analytical and secondary research with a cross-sectional design, involving 62 elderly individuals from PSTW Sabai Nan Aluih Sicincin as the population. All population members meeting the inclusion criteria and excluding outliers were considered as the research sample. The inclusion criteria encompassed complete data such as BMI, handgrip strength, age, weight, knee height, and gender. Two outlier samples were excluded based on the exclusion criteria. Bivariate data analysis utilized the Chi-square statistical test. Results The research findings indicate that most elderly women at PSTW Sabai Nan Aluih Sicincin have normal to overweight BMI, while elderly men have underweight BMI. Elderly women at PSTW Sabai Nan Aluih Sicincin exhibit abnormal handgrip strength, whereas elderly men have normal handgrip strength. Furthermore, there is no significant relationship (p=0.676) between BMI and handgrip strength values among the elderly at PSTW Sabai Nan Aluih Sicincin. Conclusions This study concluded that there was no significant relationship between BMI and hand grip strength scores in the elderly at PSTW Sabai Nan Aluih Sicincin.

Abstract

Depression and obesity are two disorders that profoundly impact worldwide health. This is primarily because of their high occurrence, which is linked to both illness and death. Obesity increases the likelihood of developing depression and vice versa. The susceptibility to depression in individuals with obesity implies the existence of linked mechanisms between both conditions, although the molecular processes involved are not well comprehended. We describe a 28-year-old man who sought help from a dietitian or nutrition specialist to address weight gain that occurred after he began taking medication for his major depressive disorder (MDD) three years ago. This case study aims to address the challenges of treating obesity in patients with major depressive disorder (MDD) treatment.

Abstract

Mycobacterium other than tuberculosis (MOTT) is an environmental bacterium that can be an opportunistic pathogen. These bacteria are resistant to various types of disinfectants and antibiotics because they have the characteristics of thick cell wall peptidoglycan that are rich in lipids and mycolic acid. There are now over a hundred MOTT species, some of which are known to infect people with immune system disorders such as chronic obstructive pulmonary disease (COPD), cystic fibrosis (CF), people with a history of tuberculosis (TB), HIV infection, or diabetes mellitus, but can also infect individuals with good immune systems. This type of mycobacterium can also cause nosocomial infections because it can contaminate hospital water as well as medical devices such as bronchoscopes, endoscopes, and dialysis fluids. Infections in humans originate from environmental exposure and spread through ingestion or inhalation. The clinical manifestations of MOTT infection can be pulmonary and extrapulmonary infections, including skin, soft tissue, the gastrointestinal system, bones, and joints, and disseminated with symptoms that are difficult to distinguish from a Mycobacterium tuberculosis infection. Therefore, it is necessary to conduct supporting examinations, in particular microbiological examinations, to detect and identify the species of MOTT and then determine the appropriate therapeutic management. The types of microbiological examination that can be performed are microscopic examination with acid-fast staining, culture, identification with biochemical tests, molecular tests, and immunodiagnostic tests.

Abstract

Background Diabetes Mellitus (DM) is a disorder of the body's metabolism with the main characteristic of which is increased blood glucose levels caused by interference of insulin secretion and action. One of the efforts to prevent complication is to check and control blood glucose levels in DM patients. To monitor blood glucose levels requires non-pharmacological management, such as eating arrangements and exercise activities. This study aim was to assess the association between eating arrangements and exercise activities with blood sugar control in adult diabetic patients. Methods Study desain was an observational analytic with a cross-sectional approach. Study was conducted at Grogol Petamburan Health Center, in September-November 2022, as many as 63 diabetes patients aged 18-60 years were selected consecutively by taking into inclusion and exclusion criteria. Data were collected using the 24 hours food recalls, Baecke Habitual Physical Activity Questionnaire and measuring non-fasting blood sugar levels. Statistical analysis using Chi – Square test with a significance level of p <0.05. Results Age of the subjects ranged from 41-50 years as many as 47.6%  and 69.8% were women, as many as 50.8% had a normal BMI.  Subjects who did not make meal arrangements as many as 63.5% and 50.8% were categorized as active in sports activities. Subjects with uncontrolled GDS levels were 52.4%. The Chi-Square test to assess the association between eating regulation and exercise activity with blood sugar control in adult diabetic patients showed p = 0.009 and p = 0.000. Conclusions Dietary arrangements and exercise activities were associated with blood sugar control in adult diabetic patients.

Abstract

Untreated congenital hypothyroidism (CH) leads to intellectual disability. Newborn screening (NBS) for CH should be done on all infants. Prompt diagnosis by NBS leads to early and adequate treatment outcomes in very normal neurocognitive outcomes in adulthood. Newborn screening (NBS) for congenital hypothyroidism (CH) is one of the major achievements in preventive medicine. Most neonates born with CH have a normal appearance and no detectable physical signs. Blood spot thyroid stimulating hormone (TSH), thyroxine (T4), or both can be used for CH screening. The latter is more sensitive but not cost-effective, so screening by TSH or T4 is used in various programs worldwide. TSH screening is more specific in the diagnosis of CH. T4 screening is more sensitive in detecting newborns with rare hypothalamic-pituitary-hypothyroidism, but it’s less specific with a high frequency of false positives especially in low birth weight and premature infants. NBS alone is not sufficient to prevent adverse outcomes from CH in a pediatric population. In addition to NBS, the management of CH requires timely confirmation of the diagnosis and accurate interpretation of thyroid function testing, effective treatment, and consistent follow-up. Doctors need to consider hypothyroidism in the face of clinical symptoms, even if NBS thyroid test results are normal. When clinical symptoms and signs of hypothyroidism appear (such as large posterior fontanelles, large tongue, umbilical hernia, prolonged jaundice, constipation, lethargy, and/or hypothermia), measurement of serum thyroid hormone and free thyroxine is indicated, regardless of NBS results. So all these babies should be treated as having CH during the first 3 years of life, taking into account the risk of mental retardation. Re-evaluation after 3 years is required in such patients.
